
Replace secure special care units with one ‘centre of excellence', Tusla told

should replace its three special care detention units with a single 'centre of excellence campus' as part of a bid to overcome severe challenges within the area, an external review group has advised.
The centre, as well as improvements to staff pay and conditions, are among 30 recommendations made in the review, which was commissioned by the Tusla child and family agency amid 'critical' bed shortages in the special care facilities.

Due to a lack of staff, just 14 of 26 special care beds in the three facilities across the State's three special care units are open, a statistic that has been repeatedly criticised by judges. The placement shortage has left children deemed to be at serious risk of harm living in unregulated emergency accommodation for a time.

Rather than maintaining the three existing special care centres, the review group recommends a single campus featuring units with 'graduated security levels'.
The proposed centre of excellence should be a 'state-of-the-art' purpose-built facility, supported by a multidisciplinary staff team, shared services and research capacity, it said.
Noting challenges with each existing centre, it said none have the physical infrastructure to support a centre-of-excellence model. There are significant limitations around accommodation and staff capacity.
The three existing centres, Ballydowd and Crannóg Nua in Dublin and Coovagh House in Limerick, require ongoing and 'considerable' investment, the review group said, noting that in Ballydowd, some bedroom windows cannot be opened to access fresh air.
In Crannóg Nua, it noted children are admitted through an enclosed garage, which it said is 'concerning'.
'Put plain and simple, the current build environment supplying special care beds is not fit for purpose,' the review states.
Separately, it found Tusla's special care provision has become 'unfocused' and has 'drifted' from its intended purpose as a short-term intervention.
Instead, it has become a 'de facto placement option', in which some children have been detained for up to two years.
This shift is largely due to insufficient onward placements and a lack of community-based resources for discharge-ready children, it found.
An earlier practice whereby an onward placement would be identified as part of the application process 'appears to no longer be functioning', it said.

A lack of suitable onward placements alongside insufficient staffing, which emerged as a 'critical challenge', is resulting in under-occupancy, with just under half of the available beds not in use.
The review notes that while 168 staff have been hired since 2021, 174 staff have left the service in the same period.
Exposure to violence, harassment and aggression were identified as key issues affecting staff who leave the service.
Rolling and targeted recruitment campaigns have not yielded meaningful numbers of eligible applicants, 'and there is no indication that this situation will change in the short to medium term', the review notes.
Staff absenteeism, meanwhile, is 'significantly high' at 20 per cent in both 2022 and 2023, with the review group noting that usual anticipated levels of absenteeism in the public services range from three per cent to five per cent.

The review group recommended aligning remuneration with comparable roles at the Oberstown Children Detention Campus to address recruitment and retention issues. It noted that pay scales for those working in the youth detention and remand facility are 'substantially higher'.
Other recommendations to target staffing issues include more psychological support, the use of 'safety pods' and a review of the terms of employment and rosters to make the role more attractive.
The report presents 30 recommendations across capacity, governance, staffing, care models and funding. It proposed implementation over three years.
Although this is an 'ambitious' timeline, the review group said it is 'necessary to transition from crisis management to systematic service delivery'.
'Success depends on sustained Government commitment, adequate resources and effective inter-agency collaboration to ensure vulnerable children receive appropriate support,' the report says.

One year later, work on epilepsy medication inquiry is yet to begin
Work has yet to begin on an inquiry into the prescribing of epilepsy medicine, a year after a chairperson for the probe was appointed — on a 12-month contract. Affected families want to know what has caused the delay to the inquiry beginning its work in full. The role of chairperson was advertised early last year, and when the inquiry was publicly launched and the chairwoman confirmed in June 2024, it was understood that a certain amount of time was needed to lay the groundwork. At the time, then health minister Stephen Donnelly said work would begin 'within weeks'. This led families to understand the chairwoman, barrister Brid O'Flaherty, would be supported to begin work quickly. Recruitment notices advised a chairperson would 'sign a contract for services for a period of 12 months with the possibility of extending by a further six months'. Asked whether this contract is being extended now, a department spokeswoman said they cannot comment on individual contracts, adding: However, once the inquiry formally commences, a time commitment of between 12 and 18 months is envisaged for the chair to conduct an independent, fair, and rigorous process. Ms O' Flaherty has met patient group the Organisation for Anticonvulsant Syndromes (OACS) Ireland for initial discussions. However they do not know when her wider work with individual families can start. Solicitor for the group, Ciara McPhillips, partner at Michael Boylan LLP, said the department's stance raises more questions than it answers. 'The questions now point back to the department, did they appoint the chair prematurely before they had their groundworks done, or has some unforeseen problem arisen,' she said. The department previously cited work on data regulations as one reason for the delays. However Ms McPhillips said OACS Ireland were told in October 2023 this work had already started. The families have requested a meeting with minister Jennifer Carroll MacNeill but are yet to hear when this might happen. They have questions. Autistic man (23) ‘trapped' in psychiatric unit for five years as HSE fails to secure accommodation
A 23-year-old autistic man has been in a psychiatric unit for the last five years as the Health Service Executive (HSE) has been unable to source an appropriate care facility for him, his mother has claimed. Robert Brady, from Walkinstown, Dublin 12, has been 'trapped in a psychiatric unit for years with no end in sight', Anne Dillon said. His family is not permitted to visit him at the moment and are only allowed to receive phone calls from him inside the facility, Ms Dillon said, leaving them feeling 'crushed, devastated and broken'. Mr Brady was diagnosed with autism spectrum disorder in 2019 while under the care of the Child and Adolescent Mental Health Services at the Linn Dara inpatient facility in Ballyfermot, Dublin 10. Ireland saw dramatic improvement in mortality in 20-year period, data shows
Ireland experienced a dramatic improvement in mortality across the 20-year period from 1985 to 2005, according to data from the Central Statistics Office (CSO). Heart attacks were the main cause of death in nearly two out of every 10 deaths for men and women in Ireland in 1985, and that rate reduced to just one out of every 10 deaths in 2005. The crude death rate per 100,000 of population stood at a 'very high level' at 938.2 in 1985 and had reduced by 4.5 per cent to 895.8 in 1995, the CSO figures show. The following 10-year period witnessed a further 23.7 per cent reduction in the rate, where it stood at 683.6 deaths per 100,000 of population in 2005. READ MORE From 1985 to 2005, there was a 60 per cent decrease in the mortality rate of children aged between five and nine years, while the mortality rate of those aged 15 to 19 years increased by 12.7 per cent. Over the 20-year period, crude mortality rates in infants and children up to nine years of age saw the largest decreases, while rates in those aged 15-24 years increased. Heart attacks were the number one cause of death for both males and females in 1985. Nearly one out of every four deaths for males that year was due to a heart attack, while by 2005, this had decreased to one out of every 10 deaths for males. For females, deaths due to heart attacks accounted for just under two out of every 10 deaths in 1985 and by 2005, this had fallen to one out of every 10 deaths. While deaths due to heart attacks reduced over the 20-year period, other forms of chronic ischaemic heart disease, which can include deaths due to coronary atherosclerosis and aneurysms, saw an increase in proportion of all deaths.
