CDSCO notifies draft standard evaluation protocols for issuing IVD licenses

New Delhi: India's medical device regulator, CDSCO in collaboration with the Indian Council of Medical Research (ICMR) has issued draft standard protocols for evaluating In-Vitro Diagnostics (IVDs) devices quality and performance, to be used for issuing licenses.
The protocols are meant to be followed by the country's
IVD manufacturers testing labs
and the regulator has sought stakeholders' comments over the notified draft by August 25.
'These protocols are now being placed in the public domain for comments from relevant stakeholders. This window of opportunity will close on 25th August 2025, and, once finalized, there will be minimal scope for change in these documents,' the notice reads.
The 180-page document details the standard performance evaluation and field evaluation protocol for 15 types of IVDs including Dengue, Malaria, Influenza virus, Nipah virus, Chandipura virus, SARS-CoV-2 molecular detection differentiation assay, among several others.
In Vitro Diagnostics (IVDs) are medical devices used to test samples such as blood, urine, or tissue outside the human body, to diagnose diseases, conditions, or infections, and to monitor a person's health based on predefined bio markers.
Under the Medical Device Rules of 2017, 'Licensure of In-Vitro Diagnostics (IVDs) requires a detailed evaluation protocol for the
performance evaluation of IVDs
to evaluate their quality and performance.'
The evaluation protocol helps to test the device safety, sensitivity, and accuracy, to validate the device clinical and analytical performance, required for issuing the product license, thereby allowing its usage in the country.
In India the ICMR develops evaluation protocols to standardize the process of assessing the performance of diagnostic kits, particularly In Vitro Diagnostics (IVDs) and the CDSCO oversees the evaluation and enforcement of the protocols.

New Delhi: Over six in every one thousand deliveries in India in 2020 could have been a stillbirth, with higher rates among urban mothers compared to rural, according to an analysis of data from national surveys and the civil registration system. Researchers, including those from the All India Institute of Medical Sciences, Gorakhpur, and the Indian Council of Medical Research, New Delhi, also identified that stillbirth hotspots are located primarily in northern and central India. Chandigarh, Jammu and Kashmir, and Rajasthan in north India recorded the highest rates of stillbirth, the findings published in The Lancet Regional Health Southeast Asia journal show. The team analysed data gathered from the fifth round of the National Family Health Survey (NFHS) and the 2020 report of the civil registration system, which is managed by the Ministry of Home Affairs and records vital events, including births, deaths and stillbirths across India. "The nationwide stillbirth rate (SBR) in 2020 was 6.548 per 1,000 total births (female: 6.54; male: 6.63). Stillbirth rates were higher among urban mothers compared to rural mothers," the authors wrote. A higher rate of stillbirth was also found in districts where pregnant women were anaemic (deficient in iron) and underweight -- factors known to increase risk of adverse birth outcomes, including stillbirth. Hygienic menstrual practices and caesarean (C-section) deliveries -- in which a baby is delivered through a surgical procedure -- were associated with a lower rate of stillbirth, especially in Telangana, Karnataka, and Tamil Nadu. The results were in line with data from NFHS-5, according to which the prevalence of C-section deliveries was about 45 per cent in south India in 2019-2020, the authors said. Evidence from states including Assam, Meghalaya, Maharashtra, Uttar Pradesh, Himachal Pradesh and Chhattisgarh showed that receiving at least four pre-delivery check-ups and adhering to iron and folic acid supplementation during pregnancy can significantly reduce the risk of stillbirth, the team said. Overall, higher rates of stillbirths were found in regions where pregnant women were anaemic, more deliveries were occurring in public health facilities and a larger fraction of women were having a lower wealth status, they said. However, there were no disparities in sex-specific rates of stillbirth across the country's districts, even though a higher rate of stillbirths seen among male foetuses may point to a higher biological vulnerability, the researchers said.

From Independence to Innovation: How India and Genomics Grew Together

The 1940s saw the beginning of two remarkable stories. First is India's independence in 1947, and second, the pioneering science of genomics in 1944. Over the past eight decades, India's development and the evolution of genomics have reflected each other's spirit, deeply rooted in developing indigenous capabilities, driven by innovation, and inspired by the vision of a better India. Although DNA was discovered in the latter half of the 19th century, when Friedrich Miescher isolated the hereditary material that was later understood to be DNA, the attempt to decode it was made only in 1944 – when Oswald Avery, Colin MacLeod, and Maclyn McCarty demonstrated its role as carrier of genetic information, laying the foundation for the genomic era. The 1950s & 60s witnessed India's investment in building expertise, with the establishment of CSIR, ICMR, and the IITs. Similarly, on the other side, expertise in genomics was evolving, with Rosalind Franklin's X-ray images showing the DNA structure, Watson and Crick deciphering this structure, and early tools for reading it beginning to emerge. During the 1960s and 70s as India launched its space program, expanded higher education, and began investing in biotechnology, on the genomics front, Frederick Sanger, Allan Maxam, and Walter Gilbert developed methods to sequence DNA and genomics witnessed the birth of recombinant DNA technology giving way to the development of the first drug, human insulin, in 1982. The 1990s brought global integration for India. India's market was unlocked because of Economic liberalisation, unleashing new opportunities in IT, healthcare, and biotechnology. It was also a turning point in the genomics revolution with the launch of the Human Genome Project , aided by automated DNA sequencing and the mapping of disease-related genes. The project was aimed at identifying all the genes in human DNA and determining the sequences of the 3 billion base pairs that make up that DNA. The project took 13 years to complete. Its results opened doors to a new era of precision medicine and targeted disease management. In parallel, the dawn of the millennium also ushered in India as a global services hub. For Indian healthcare, this was also the era of several breakthroughs in clinical research and generics. The last couple of decades have been defined by leadership and innovation. India's digital revolution and the start-up boom showed the world its ability to lead in both scale and speed. In genomics, new technologies such as CRISPR genome editing and next-generation sequencing (NGS) have brought rapid improvements in throughput and cost, making NGS mainstream for research and clinical applications. Global quality genetic diagnostic tests, which can identify the root cause of complex diseases, are now available in India at affordable costs. High-end genetic tests developed in India and validated and backed by credible research stand testimony to India's scientific excellence, innovation, and talent, while also reiterating the country's commitment to its Make in India initiative. Population-scale projects like the Genome India Project aimed at sequencing one million genomes, a national effort by the Department of Biotechnology (DBT) to build a comprehensive reference genome for India, reflect India's contribution to advancing genomics, strengthening precision medicine and ensuring that genomics led early intervention and disease management are accessible to India's diverse and dynamic landscape. As we celebrate the 79th year of independent India, this reflection on India's remarkable journey in parallel with the journey of the source code of our life, our DNA, highlights not just the freedom of governance, but the freedom to have healthier, longer, and better lives. Dr Ajay Nair, Sr Consultant, Internal Medicine, Narayana Hospital, Jaipur shares all you need to know: Understanding Dengue and Its Carrier Dengue fever is caused by the bite of mosquitoes carrying the dengue virus (DENV) and is transmitted primarily by Aedes aegypti and, to a lesser extent, Aedes albopictus mosquitoes. These vectors are well-adapted to urban environments, thriving in stagnant water around homes, offices, and public spaces. Once infected, the mosquito can remain a carrier for its lifetime, transmitting the virus to humans every time it bites. Unlike malaria, which is spread by Anopheles mosquitoes active at night, Aedes mosquitoes cannot be linked to a single time frame, as their biting patterns vary. This often causes confusion about the timing of their bites. The Feeding Habits: Science Over Assumptions The peak activity of the Aedes aegypti mosquito has been recorded in the early morning and late afternoon, as documented in multiple entomological studies, including those published by the World Health Organization (WHO) and the National Vector Borne Disease Control Programme of India. The highest activity typically occurs for about two hours after sunrise and several hours before sunset. However, their bites do not occur only in the morning. While early daylight hours see the most concentrated activity, Aedes mosquitoes can bite throughout the day, particularly in shaded indoor areas with low light. Dengue cases typically rise during the monsoon, when breeding sites multiply and mosquito activity remains high almost throughout the day until evening. This flexibility increases the chances of virus transmission, especially in densely populated areas. Clearing the Misconception The belief that dengue mosquitoes bite only in the morning likely stems from early observations showing higher bite counts after sunrise. While this is statistically accurate as a trend, it is not the whole picture. People are still at risk during other active hours, particularly in the late afternoon and early evening. Prevention: Beyond the Clock Since the Aedes mosquito's activity is not limited to one specific time of day, dengue prevention must be a continuous effort. Medical recommendations from Narayana Health Hospital include: Controlling the surroundings Make it a habit to empty stagnant water from containers such as flower pots, buckets, utensils in the sink, discarded tyres, and open storage tanks. Frequently change water in plant trays and bird baths, and always cover stored water. Remember, Aedes mosquitoes breed in clean water too. Installing physical barriers Use mesh screens for doors and windows, wear long-sleeved clothing, and keep doors closed during peak mosquito activity. Using repellents Apply mosquito repellent creams or sprays on exposed skin every few hours during daylight, not just in the morning. Alternatively, use mosquito nets. Community action Participate in neighbourhood clean-up drives to remove puddles and water collected in discarded items. Fumigation can also help reduce mosquito populations. Prevention during travel When visiting dengue-hit areas or places experiencing heavy rains, ensure you are protected from bites regardless of the time of day. Why Awareness Matters Millions worldwide suffer from the effects of dengue fever, with India bearing a significant share of the global burden. Misunderstanding mosquito behaviour can lead to carelessness, inadvertently increasing infection rates. The idea that dengue mosquitoes bite exclusively in the morning oversimplifies their complex behaviour. While Aedes aegypti activity may peak just after sunrise and before sunset, bites can occur at other times too. Public health protection relies not on a specific 'danger hour," but on vigilance from dawn to dusk. By consistently following preventive measures, individuals and communities can significantly reduce transmission.
