EMA Recommends Romvimza for Tenosynovial Giant Cell Tumor

The active substance of Romvimza, vimseltinib, is an oral switch-control TKI designed to selectively inhibit the colony-stimulating factor 1 receptor (CSF1R), a key driver in the pathology of TGCT. By blocking CSF1R signaling, the drug targets the overgrowth of synovial cells and inflammatory components that drive tumor development and joint deterioration. The CHMP's recommendation was based on results from the phase 3 MOTION study, which enrolled adults with symptomatic, unresectable TGCT. In the study, the objective response rate at week 25 was 40% for patients treated with vimseltinib compared with 0% for patients receiving placebo, as measured by RECIST criteria. Patients also reported clinically meaningful improvements in stiffness, range of motion, physical function, and quality of life. Romvimza will be available as 14 mg, 20 mg, and 30 mg capsules. The drug's most common side effects include increased liver enzymes, periorbital edema, increased cholesterol, rash, increased creatinine, decreased neutrophils, fatigue, face edema, pruritus, peripheral edema, and hypertension. The CHMP concluded that the benefits of Romvimza outweighed the risks in this patient population, fulfilling an unmet medical need in those for whom surgery is not a viable option. (Headquarters: Osaka, Japan; President and COO: Toichi Takino; 'Ono'), today announced that the European Medicines Agency's (EMA) Committee for Medicinal Products for Human Use (CHMP) has adopted a positive opinion recommending the approval of vimseltinib for the treatment of adult patients with symptomatic tenosynovial giant cell tumor (TGCT) associated with clinically relevant physical function deterioration and in whom surgical options have been exhausted or would induce unacceptable morbidity or disability. The positive CHMP opinion is a scientific recommendation for marketing authorization and one of the final steps before the European Commission (EC), which has the authority to approve medicines in the European Union (EU), issues a decision on Deciphera's marketing authorization application (MAA) for vimseltinib. This decision is expected in the second quarter of the fiscal year ending March 31, 2026. 'CHMP's positive opinion is an important milestone for the TGCT community in the European Union (EU), where there are currently no approved treatments for TGCT, and for vimseltinib, which is now one step closer to potential EU regulatory approval,' said Ryota Udagawa, President and Chief Executive Officer of Deciphera. The positive CHMP opinion is supported by compelling efficacy and safety results from the pivotal Phase 3 MOTION study of vimseltinib in patients with TGCT not amenable to surgery with no prior anti-CSF1/CSF1R therapy (prior therapy with imatinib or nilotinib allowed), compared to placebo, as well as the Phase 1/2 study of vimseltinib. In MOTION, vimseltinib demonstrated a statistically significant and clinically meaningful objective response rate (ORR) at Week 25 in the intent-to-treat (ITT) population, as assessed by blinded independent radiologic review (BIRR) per Response Evaluation Criteria in Solid Tumors version 1.1 (RECIST v1.1), versus placebo (40% in vimseltinib arm vs 0% in placebo arm, p <0.0001). The primary endpoint was supported by statistically significant and clinically meaningful improvements in active range of motion, patient-reported physical functioning, and patient-reported pain observed in the vimseltinib arm compared to the placebo arm at week 25. The safety profile of vimseltinib is manageable and consistent with results previously disclosed in the Phase 1/2 clinical trial. About MANEUVERThe pivotal Phase 3 MANEUVER study is a three-part, randomized, double-blind, placebo-controlled study to assess the efficacy and safety of pimicotinib in patients with TGCT who require systemic therapy and have not received prior anti-CSF-1/CSF-1R therapy. The study is being conducted by Abbisko Therapeutics in China (n=45), Europe (n=28), and the US and Canada (n=21). In the double-blind Part 1, 94 patients were randomized 2:1 to receive either 50 mg QD of pimicotinib (n=63) or placebo (n=31) for 24 weeks. The primary endpoint is objective response rate (ORR) at week 25, as measured by Response Evaluation Criteria in Solid Tumors (RECIST) version 1.1 by blinded independent central review in the intent-to-treat (ITT) population. Secondary endpoints include tumor volume score, active range of motion, stiffness by Numeric Rating Scale (NRS), pain by Brief Pain Inventory (BPI), and physical function measured by Patient-Reported Outcomes Measurement Information System (PROMIS). After the double-blind Part 1, eligible patients could continue to the open-label Part 2 for up to 24 weeks of dosing, results of which are expected in mid-2025.
