Health Care CFOs Treating New External Pain Points: Survey

The vast majority of health care finance leaders now view external forces as their biggest challenges to growth. That represents a distinct change from recent years when internal concerns, such as workforce issues, cost reduction, and cybersecurity, consistently ranked among their top five priorities. Nearly three-quarters (73%) of CFOs also report concerns about revenue growth and operating profitability, a sharp contrast from late last year, when most health care leaders anticipated rising revenue and improved profitability this year. Those findings, among others, emerged from the sixth annual U.S. Health Care CFO Survey, conducted by the Deloitte Center for Health Solutions. The survey drew respondents from 64 U.S. health care finance leaders, divided evenly between those from large health systems and those employed by large health plans. In recent years, health care industry CFOs have faced a succession of challenges, including labor shortages, inflation spikes, and shifts in government-run health care programs. They are now less focused on urgently cutting expenses and more on pursuing growth in an increasingly complex industry. What worries these CFOs about the changing environment?In the survey, 84% of respondents say they are concernedabout potential regulatory and policy changes, macroeconomic uncertainty, and disruptions related to tariffs and supply chains. Potential new tariffs on imports like prescription drugs, medical equipment, and other medical supplies could increase costs, disrupt procurement processes, and complicate supplier negotiations. In terms of regulatory issues, health plan finance leaders identified changes to drug-pricing policies, like modifications to the Inflation Reduction Act that capped Medicare's annual out-of-pocket costs at $2,000, as the regulatory changes most likely to impact their organizations. Both health system and health plan finance leaders ranked Medicaid reform as the second most impactful regulatory change. Health system CFOs expressed concerns about lower reimbursement rates and potential uncompensated care due to eligibility reductions or budget caps. External Environmental Impact Last year's U.S. Health Care CFO survey highlighted the benefits of adopting a holistic approach to margin improvement. That strategy involves focusing collectively on levers across strategic growth and revenue improvement, cost reduction, and efficient capital deployment—rather than addressing each one separately. Our analysis found that some levers, often considered lower priority by CFOs, could offer better opportunities for financial transformation. This year, many finance leaders seem to have increased their focus on these high-impact levers, but most have not yet derived impact from them. Only 28% of health system respondents and 25% of health plan respondents who engaged in M&A efforts say those deals exerted a strong impact. And while more than 90% of the respondents say they were focused on alliances and partnerships, only one-third say they had seen a strong impact from those deals. Optimizing operating models can lead to cost savings. Outsourcing noncore services can reduce operating expenses, offering scalability, minimizing infrastructure spending, and enhancing efficiency. Yet among organizations that outsource, just 22% of health systems and 31% of health plans characterize it as having a strong impact. New Interventions To help them navigate a dynamic economic and regulatory environment, health plan and health system respondents report that they are prioritizing technology investments to improve data quality, reporting capabilities, and scenario planning. However, only one-third of respondents from health plans and health systems say that their efforts to modernize core technologies such as electronic health records, enterprise resource planning systems, and data modernization efforts have led to a strong impact. Data modernization, which improves accessibility and security, remains a fundamental aspect of modernizing and integrating core and transformative technologies. Finance leaders have opportunities to invest in data modernization to improve care delivery, streamline operations, and achieve better returns. That said, 53% of health systems report strong impacts from transformative technologies like generative AI and cloud, while 72% of health plans report only moderate-to-no impact. As organizations consider adopting these technologies, especially generative AI, it is important to take a holistic, institutional approach and address data, consumer, and workforce considerations to realize meaningful impact. What Else CFOs Can Do The COVID-19 pandemic likely taught health care finance leaders the importance of agility and building tech-enabled financial planning capabilities. In today's changing macroeconomic environment, here's how such leaders can apply those lessons: As stewards and operators of both stability and agility, health care CFOs play a vital role in maintaining financial health and driving long-term growth. Alight Reports Second Quarter 2025 Results

CHICAGO--(BUSINESS WIRE)--Alight, Inc. (NYSE: ALIT), a leading cloud-based human capital and technology-enabled services provider, today reported results for the second quarter ended June 30, 2025. 'Our underlying business operations continued to strengthen during the second quarter,' said CEO Dave Guilmette. 'We are making important strategic progress to accelerate our client management and delivery capabilities through automation, artificial intelligence, innovation and partnerships. These initiatives are helping our clients realize improved return on investment from their benefits solutions and driving continued strength in client retention.' Presentation of Results Second Quarter 2025 Highlights (all comparisons are relative to second quarter 2024) Revenue decreased 1.9% to $528 million Gross profit of $176 million and gross profit margin of 33.3%, compared to $167 million and 31.0%, respectively, and adjusted gross profit of $205 million and adjusted gross profit margin of 38.8%, compared to $196 million and 36.4%, respectively Net loss of $1,073 million compared to net loss of $4 million, primarily driven by the $983 million non-cash goodwill impairment charge related to our Health Solutions reporting unit Adjusted EBITDA improved to $127 million from $105 million Diluted loss per share of $2.03 compared to diluted loss per share of $0.01, and adjusted diluted earnings per share of $0.10 compared to $0.05 per share New wins or expanded relationships with companies including Thermo Fisher Scientific, Highmark Health, Reinsurance Group of America, Incorporated (RGA) and Trinity Industries Repurchased $20 million of common stock under existing share repurchase program Declared and paid a $0.04 per share dividend Second Quarter 2025 Results Revenue decreased 1.9% to $528 million, as compared to $538 million in the prior year period. The change was primarily due to lower project revenue and net commercial activity. Recurring revenues were 93.2% of total revenue. Gross profit was $176 million, or 33.3% of revenue, compared to $167 million, or 31.0% of revenue in the prior year period. The change in gross profit was primarily due to productivity savings. Selling, general and administrative expenses improved $16 million when compared to the prior year period. This was due to lower professional fees incurred related to the sale and separation of the Payroll & Professional Services business and a reduction in compensation expenses primarily related to non-cash share-based awards. During the quarter, the Company recognized a non-cash goodwill impairment of $983 million relating to the Health Solutions reporting unit after evaluating macroeconomic and industry conditions, and the market valuation of the Company. This non-cash charge does not impact future operations. Interest expense of $22 million improved $11 million from the prior year period. Interest expense benefited from the repricing of the 2028 term loan and the $740 million debt pay down in the third quarter of 2024. The Company's loss from continuing operations before income tax was $1,076 million compared to a loss from continuing operations before income tax of $2 million in the prior year period. This was primarily attributable to the non-cash goodwill impairment charge related to our Health Solutions reporting unit, non-operating fair value remeasurements of financial instruments and the tax receivable agreement, partially offset by lower selling, general and administrative expenses, lower interest expense as a result of the debt pay down and other income recorded in conjunction with the transition services agreement entered into with the purchaser of the divested Payroll & Professional Services business. Balance Sheet Highlights As of June 30, 2025, the Company's cash and cash equivalents balance was $227 million, total debt was $2,015 million and total debt net of cash and cash equivalents was $1,788 million.
