Continued Enrollment Increases for the Individual Health Insurance Market by Mark Farrah Associates

MCMURRAY, Pa.--(BUSINESS WIRE)--In its latest Business Strategy brief, Mark Farrah Associates (MFA) assessed short- and long-term individual health insurance enrollment trends. According to financial statements filed by insurers, and with estimates by MFA, enrollment in individual medical plans, both on and off the exchange, totaled approximately 26.8 million as of March 31, 2025. This is an increase of 2.3 million members from a year ago. The individual segment has been profitable, overall, for health plans since 2017. However, since 2021, premium growth has, once again, been outpaced by medical expense growth, leading to increasing medical loss ratios (MLRs). MLRs for 1Q25 experienced an average aggregated traditional MLR of 80.4%, up from 78.9%, at premiums per member per month (PMPM) of $566 and claims PMPM of $455. Mark Farrah Associates Reported Individual and Small Group Health Insurance Segment Profitability

MCMURRAY, Pa.--(BUSINESS WIRE)--Mark Farrah Associates (MFA), examined trends in membership, claims incurred PMPM (per member per month), adjusted premiums PMPM, and MLRs (Medical Loss Ratios) for the Individual and Small Group comprehensive medical/health insurance markets and segment leaders. Based on an analysis of the National Association of Insurance Commissioners (NAIC) Supplemental Health Care Exhibit (SHCE), MFA also analyzed the profitability of both health insurance segments for the last five years. The Individual and Small Group business segments were profitable for insurers in 2024 where both experienced higher MLRs between 2023 and 2024. The Individual segment experienced enrollment growth of 21%, to 21.5 million members for 2024. Premiums PMPM increased 1.8%, to a segment average of $559 PMPM. The average medical claims PMPM of $493 in 2024 was an increase of 2.3% from 2023. The Small Group segment continues to lose membership, down another 7% to 7.9 million in 2024. Average claims incurred PMPM increased from $495 in 2023 to $537 in 2024, an 8.4% increase. Blue Cross Blue Shield (BCBS) association members, parent organizations, and their subsidiaries insured about 37.5% of the total health insurance market. In the latest Healthcare Business Strategy report, Mark Farrah Associates (MFA), examined health insurance enrollment changes in key segments for Blue Cross Blue Shield (BCBS) and Non-BCBS companies between 2020 and 2024. Some highlights include: Individual enrollment for BCBS companies increased 36.6% over the last 5 years, and non-BCBS companies reported a 106.1% increase during the same period. Fully insured Group enrollment, which includes the Federal Employees Health Benefits Program (FEHBP), continued to experience decreases throughout the 5-year timeframe for both Blues and non-Blues plans. During the 5-year period, Medicare Advantage (MA) enrollment experienced a 32.8% increase for BCBS plans, and non-BCBS MA enrollment saw a 32.3% increase.
