Army to review training procedures after UTM Palapes cadet's death
KUALA LUMPUR: The army will review its training procedures to address any shortcomings and prevent a recurrence following the death of a Universiti Teknologi Malaysia (UTM) Reserve Officer Training Unit (Palapes) cadet. Army chief General Tan Sri Muhammad Hafizuddeain Jantan said the army regretted what had happened to 22-year-old Syamsul Haris Shamsudin and is taking the necessary steps to investigate the cause. "We will look into the training procedures to identify any weaknesses and areas for improvement in order to prevent a recurrence. "The army takes the training of its personnel seriously, whether at the individual or collective level. All syllabuses and standard operating procedures are developed to achieve the desired training objectives and end state. "One of the main considerations in training is safety. It remains a top priority," he told the New Straits Times. On July 28, Syamsul Haris died during Palapes UTM training at the Army Training Centre (Pulada) Combat Inoculation Range in Ulu Tiram, Johor. His mother, Ummu Haiman Bee Daulatgun, has called for a probe, claiming the cause of death was unclear and questionable. The single mother said her observations during the post-mortem revealed severe injuries on her son's body that were inconsistent with the initial explanation that his death was due to a seizure. On Saturday, Defence Minister Datuk Seri Mohamed Khaled Nordin had said that a task force comprising representatives from three ministries has been formed to investigate the death. The task force includes representatives from the Defence Ministry, the Home Ministry, and the Higher Education Ministry. Khaled added that the armed forces have also set up its own committee to determine the cause of death of Syamsul. On Friday, Kota Tinggi police chief Superintendent Yusof Othman said police were investigating the cadet's death. Syamsul had joined the Palapes UTM training last Saturday and that his mother received a call last Monday that he had died. Syamsul has since been buried.

SUNGAI SIPUT: The investigation into last Monday's death of a Universiti Teknologi Malaysia (UTM) Reserve Officer Training Unit (PALAPES) cadet during training will be conducted comprehensively to ensure justice for all parties involved, including the victim's family, the university, and higher education institutions. Deputy Defence Minister Adly Zahari said the ministry is committed to reviewing all aspects of the incident, particularly issues relating to participant safety and health. 'All relevant factors will be scrutinised to identify areas for improvement and ensure there will be no negligence in the future,' he told reporters after opening the Sungai Siput Veteran Desa MADANI (VDM) programme here today. Earlier, Defence Minister Datuk Seri Mohamed Khaled Nordin announced that the ministry had established a special committee to investigate the circumstances surrounding the cadet's death, which occurred during training at the Ulu Tiram Army Combat Training Centre (PULADA). Meanwhile, Adly said the ministry would also fully cooperate with the police in the official investigation being conducted. He emphasised that the ministry takes the matter seriously and will not tolerate any form of misconduct, including bullying, injuries caused by negligence, or any criminal elements, if proven to have occurred. 'The Ministry of Defence will not compromise on any form of bullying if it is found to have taken place. 'Anyone found guilty of an offence will face disciplinary action from the ministry and, at the same time, be subject to legal action by the relevant authorities,' he said. Adly also stressed the ministry's commitment to prioritising participants' health in all military training programmes, highlighting the importance of transparent and accurate health assessments. 'The health status of every participant is crucial, and we must ensure no medical information is concealed. Military training is physically demanding, and that's why thorough health screenings are necessary. 'For instance, in the former National Service Training Programme (PLKN), participants who did not meet the health criteria were sent home. We do not want individuals with health concerns to be put at unnecessary risk,' he explained. - Bernama

Defence Ministry sets up committee to investigate trainee's death
JOHOR BARU: The Defence Ministry has established a committee to investigate the death of a Reserve Officer Training Unit (Palapes) trainee at the Army Combat Training Centre (Pulada) in Ulu Tiram. Defence Minister Datuk Seri Mohamed Khaled Nordin said the investigation committee includes representatives from the Higher Education Ministry and the Home Ministry. "I was informed that a post-mortem has already been conducted, but we are still waiting for the results. "At the Malaysian Armed Forces level, we have established a committee to investigate the cause and circumstances of the victim's death," he said at a press conference at the Pasir Gudang Umno division meeting. In a statement on Friday (Aug 1), Kota Tinggi OCPD Supt Yusof Othman said they have launched an investigation into the death of the 22-year-old Universiti Teknologi Malaysia (UTM) Palapes trainee. He said initial investigations revealed that the victim suddenly became disoriented while undergoing Combat Inoculation Training at Pulada in Kota Tinggi on Monday (July 28). 'Upon noticing his condition, medical officers from the military clinic rushed him to Kota Tinggi Hospital for further treatment. 'He was later pronounced dead while receiving treatment at the hospital's emergency zone,' said Supt Yusof in the statement. He said the victim's body was brought to the Forensic Medicine Department at Hospital Sultan Ismail for a post-mortem examination. In an earlier press conference in Perak, the victim's mother reportedly urged the authorities to investigate her son's death. She had also lodged a police report alleging possible foul play after discovering injuries on his body.
