
How your genes interact with your environment changes your disease risk − new research counts the ways
Sitting in my doctor's examination room, I was surprised when she told me, 'Genetics don't really matter for chronic disease.' Rather, she continued, 'A person's lifestyle, what they eat, and how much they exercise, determine whether they get heart disease.'
As a researcher who studies the genetics of disease, I don't fully disagree – lifestyle factors play a large role in determining who gets a disease and who doesn't. But they are far from the entire story. Since scientists mapped out the human genome in 2003, researchers have learned that genetics also play a large role in a person's disease risk.
Studies that focus on estimating disease heritability – that is, how much genetic differences explain differences in disease risk – usually attribute a substantial fraction of disease variation to genetics. Mutations across the entire genome seem to play a role in diseases such as Type 2 diabetes, which is about 17% heritable, and schizophrenia, which is about 80% heritable. In contrast to diseases such as Tay-Sachs or cystic fibrosis, where mutations in a single gene cause a disease, chronic diseases tend to be polygenic, meaning they're influenced by multiple mutations at many genes across the whole genome.
Every complex disease has both genetic and environmental risk factors. Most researchers study these factors separately because of technical challenges and a lack of large, uniform datasets. Although some have devised techniques to overcome these challenges, they haven't yet been applied to a comprehensive set of diseases and environmental exposures.
In our recently published research, my colleague Alkes Price and I developed tools to leverage newly available datasets to quantify the joint effects that genetic and environmental risk factors have on the biology underlying disease.
Aspirin, genetics and colon cancer
To illustrate the effect gene-environment interactions have on disease, let's consider the example of aspirin use and colon cancer.
In 2001, researchers at the Fred Hutchinson Cancer Research Center were studying how regularly taking aspirin decreased the risk of colon cancer. They wondered whether genetic mutations that slowed down how quickly the body broke down aspirin – meaning aspirin levels in the body would stay high longer – might increase the drug's protective effect against colon cancer. They were right: Only patients with slow aspirin metabolism had a decreased risk of colon cancer, indicating that the effectiveness of a drug can depend on a person's genetics.
This raises the question of how genetics and different combinations of environmental exposures, such as the medications a patient is taking, can affect a person's disease risk and how effective a treatment will be for them. How many cases of genetic variations directly influencing a drug's effectiveness are there?
The gene-environment interaction of colon cancer and aspirin is unusual. It involves a mutation at a single location in the genome that has a big effect on colon cancer risk. The past 25 years of human genetics have shown researchers that these sorts of large-effect mutations are rare.
For example, an analysis found that the median effect of a genetic variant on height is only 0.14 millimeters. Instead, there are usually hundreds of variations that each have small but cumulative effects on a person's disease risk, making them hard to find.
How could researchers detect these small gene-environment interactions across hundreds of spots in the genome?
Polygenic gene-environment interactions
We started by looking for cases where genetic variants across the genome showed different effects on a person's biology in different environments. Rather than trying to detect the small effects of each genetic variant one at a time, we aggregated data across the entire genome to turn these small individual effects into a large, genome-wide effect.
Using data from the UK Biobank – a large database containing genetic and health data from about 500,000 people – we estimated the influence of millions of genetic variants on 33 complex traits and diseases, such as height and asthma. We grouped people based on environmental exposures such as air pollution, cigarette smoking and dietary patterns. Finally, we developed statistical tests to study how the effects of genetics on disease risk and biomarker levels varied with these exposures.
We found three types of gene-environment interactions.
First, we found 19 pairs of complex traits and environmental exposures that are influenced by genetic variants across the genome. For example, the effect of genetics on white blood cell levels in the body differed between smokers and nonsmokers. When we compared the effects of genetic mutations between the two groups, the strength of gene-environment interaction suggested that smoking changes the way genetics influence white blood cell counts.
Second, we looked for cases where the heritability of a trait varies depending on the environment. In other words, rather than some genetic variants having different effects in different environments, all of them are made stronger in some environments. For example, we found that the heritability of body mass index – the ratio of weight to height – increased by 5% for the most active people. This means genetics plays a larger role in BMI the more active you are. We found 28 such trait-environment pairs, including HDL cholesterol levels and alcohol consumption, as well as neuroticism and self-reported sleeplessness.
Third, we looked for a type of gene-environment interaction called proportional or joint amplification. Here, genetic effects grow with increased environmental exposures, and vice versa. This results in a relatively equal balance of genetic and environmental effects on a trait. For example, as self-reported time spent watching television increased, both genetic and environmental variance increased for a person's waist-to-hip ratio. This likely reflects the influence of other behaviors related to time spent watching television, such as decreased physical exercise. We found 15 such trait-environment pairs, including lung capacity and smoking, and glucose levels and alcohol consumption.
We also looked for cases where biological sex, instead of environmental exposures, influenced interactions with genes. Previous work had shown evidence of these gene-by-sexinteractions, and we found additional examples of the effects of biological sex on all three types of gene-environment interactions. For example, we found that neuroticism had genetic effects that varied across sex.
Finally, we also found that multiple types of gene-environment interactions can affect the same trait. For example, the effects of genetics on systolic blood pressure varied by sex, indicating that some genetic variants have different effects in men and women.
New gene-environment models
How do we make sense of these distinct types of gene-environment interactions? We argue that they can help researchers better understand the underlying biological mechanisms that lead from genetic and environmental risks to disease, and how genetic variation leads to differences in disease risk between people.
Genes related to the same function work together in a unit called a pathway. For example, we can say that genes involved in making heme – the component of red blood cells that carries oxygen – are collectively part of the heme synthesis pathway. The resulting amounts of heme circulating in the body influence other biological processes, including ones that could lead to the development of anemia and cancer. Our model suggests that environmental exposures modify different parts of these pathways, which may explain why we saw different types of gene-environment interactions.
In the future, these findings could lead to treatments that are more personalised based on a person's genome. For example, clinicians might one day be able to tell whether someone is more likely to decrease their risk of heart disease by taking weight loss drugs or by exercising.
Our results show how studying gene-environment interactions can tell researchers not only about which genetic and environmental factors increase your risk of disease, but also what goes wrong in the body where.

Pune: The final phase of the 1990s marked a turning point in Maharashtra's political history. For the first time, the state saw a Shiv Sena-BJP coalition in power, while the Congress faced a major setback with one of its strongest leaders walking out over differences with the high command. That leader was Sharad Pawar. In his rebellion, he took along a band of young leaders from across the state, each rooted in different regions and sectors like education, agriculture, and cooperatives. Among them was a relatively young but prominent face: Ajit Pawar, his nephew. Though not senior in age or hierarchy, Ajit commanded attention for both his lineage and his prompt, unorthodox style of leadership. Little did the 40-year-old leader know that decades later, he would not only challenge his uncle's authority but also lay claim to the very party Sharad Pawar had founded, with the same symbol, and backed by many of the senior leaders who once rebelled with his uncle. Fast forward to June 10, 2025. Ajit Pawar's NCP (AP) is nearly two years old. Yet, he too will celebrate the 26th foundation day of the Nationalist Congress Party-thanks to the Election Commission of India's recognition of his faction as the official NCP. The contrasting scenes at Bal Gandharva Rang Mandir and Chhatrapati Shivaji Maharaj Sports Complex in Pune reflect the mood and political positioning of both factions in the state. At Bal Gandharva in Shivajinagar, not more than 50 people - mostly local party workers - were seen quietly preparing for the big day. A small mandap was set up, and modest arrangements were in place by the afternoon. "It is a very happy moment, but also one of curiosity. The party has been in power and opposition. In the next four months, we have local body elections coming up. Pawar saheb will give a good message for the party and the MVA," said NCP (SP) Pune chief Prashant Jagtap. Ten kilometres away, the preparations were grander at the Chhatrapati Shivaji Maharaj Sports Complex. Pink banners covered the venue. Over 100 workers were busy with final touches, under the supervision of 35-year-old Parth Pawar, the elder son of Ajit Pawar. "Tomorrow, the party is celebrating 26 years. Our leader, Ajit Pawar, will be addressing us. Sunil Tatkare and Parth Pawar contributed to this programme. Fifty thousand people from across the state are expected," said Anand Paranjpe, chief spokesperson of the NCP (AP). The Big Question What makes this year's celebration nationally significant is the buzz around a possible merger between the two factions of the NCP. Sharad Pawar has delegated the decision to his daughter, Supriya Sule, while the Ajit Pawar faction is awaiting an official proposal. "If Pawar had to go with the BJP, he would have gone in 2014. Even in 2019, there was an attempt. If we had to go, we would have gone with Ajit Dada earlier. Sharad Pawar has spent more time in the opposition than in power-he has never aligned with the BJP. People like me are sure it's not going to happen. No discussion about party merger has happened, or will happen," said Mr Jagtap. On the other hand, the Ajit Pawar faction - fresh off a victory in the Vidhan Sabha - is optimistic about a flood of new entries. "After tomorrow's program, there are going to be a lot of additions. A lot of people want to work with Ajit Dada. Many will be on the ground. A different message will be given by our leader-things will become clear soon. Many are ready to join us," said Suraj Chavan, NCP (AP) spokesperson. The Upcoming Civic Polls Beyond the merger rumours, the Foundation Day also kicks off preparations for the upcoming local body elections, long overdue in the state. Pune, in particular, remains a stronghold for both factions. "The 'Ladki Bahin Yojana' isn't reaching people, inflation is up -we're paying more, but getting the same money back. It's been two months since anyone received funds. Petrol prices, unemployment, high school fees, and expensive bus tickets are all issues we'll be taking up," said Dilshad Akhtar from NCP (SP). The Ajit Pawar faction, too, sees this event as a stepping stone toward civic poll strategies. "Directions for the upcoming elections will be given by the party chief," said Anand Paranjpe.

Directive 8020 is straight up a playable version of The Thing
It's no secret that developer Supermassive Games is inspired by horror movies. The studio's brand of choose your own adventure horror games has always pulled ideas from classic slashers and adapted those ideas into original stories. With Directive 8020, the latest entry in its long running Dark Pictures anthology, that inspiration is clearer than ever. I played a quick slice of it at Summer Game Fest and I'm happy to report that Supermassive has basically just made John Carpenter's The Thing, but playable. That isn't apparent right away when my demo begins. I start in the middle of a dramatic conversation between two of its main characters who set up the sci-fi world and express some concerns with an operation to Mars. For the purposes of the demo, it's basically just a way for Supermassive to show off how strong its cinematic chops have gotten. The character models approach hyperrealism, and the acting doesn't feel far off from a Hollywood movie. After that quick introduction, things rapidly heat up. One of those characters winds up facing down his doppelganger, leading to a thrilling little fight scene. While those bits have some minimal interaction, I get to do a lot more when a doppelganger suddenly mutates into an enormous flesh monster pulled straight from a John Carpenter movie. I'm tossed into a straight-up stealth horror sequence where I need to get past a patrolling monster and sneak my way to a ladder. I can use a scanner to track its position through walls. Though it's a bit of a standard stealth hiding sequence, it's a remarkably tense one. I'm legitimately terrified of getting caught, likely because the monster that's chasing me is just so grotesque with its human face hanging from its hulking body. I almost get to the end undetected, but it spots me at the last moment. I make a run for the ladder and grab it with a second to spare. It's another true movie moment. Recommended Videos Even with all these tells, I don't fully pick up on how much Supermassive is making The Thing in space until the next cutscene. The ship's crew has a potential doppelganger contained in a cell and must decide if he's friend or foe. I'm given the choice to either shoot him dead or spare him. Naturally, I shoot the sucker dead. That's where I learn about the biggest way that Directive 8020 is evolving the Dark Pictures formula. As soon as I make my choice, a pop up appears on screen that asks me if I want to rewind. I'm no longer stuck with my bad decision. When it happens, a developer from the Supermassive team come over to me and explains exactly how that works. By opening a menu, players can now see exactly how a chapter's story tree will branch. It doesn't show what the choices are, but it shows how many permeations there are from the jump. In my case, there were three ways my encounter could have gone. It turned out that the guy I shot was legitimately a human (whoops), but certain story choices earlier on could lead to a mimic being there instead — hence three possible outcomes. And don't worry: If you're a purist, there will be a mode that disallows you from using rewinds, so you have to live with your bad choices. All of this seems like a smart evolution for a formula that Supermassive Games has nearly perfected at this point. The few choices I had to make felt impactful and I like having a little more classic horror gameplay in that bit of stealth. More than anything, I just like running away from gross body horror monsters that would make David Cronenberg proud. I'm ready to be grossed out. Directive 8020 launches on October 2 for PS5, Xbox Series X/S, and PC.
