
Brain Changes Linked to Dementia Found in Ex-Rugby Players
The findings provide the first prospective evidence of physical brain and blood abnormalities in this group.
Previous research had already shown that elite rugby players face a higher risk of neurodegenerative diseases later in life. The studies examined links between repeated head impacts in rugby and conditions such as Alzheimer's disease and chronic traumatic encephalopathy (CTE).
Post-mortem examinations of former players have shown neuropathologies consistent with repetitive brain trauma.
Traumatic brain injury, already a known risk factor for neurodegeneration, contributes to an estimated 3%-15% of dementia cases in the general population.
While recent advances in fluid and imaging biomarkers have transformed dementia diagnosis, these techniques have not been systematically applied to rugby players previously exposed to multiple head impacts.
Study Cohort and Methods
The research involved 200 ex-professional rugby players aged 30-61 (median age, 44), all of whom had self-referred with brain health concerns but had no dementia diagnosis at baseline. At least one previous concussion while playing was reported by 193 (96.5%) of the former players, with a median of seven concussions.
The rugby group was compared with 33 age- and sex-matched healthy controls with no evidence of previous head trauma or dementia onset.
Participants were 90% male. The median rugby career lasted 10.5 years, with 63% playing as forwards and 37% as backs.
Mental Health and Behavioural Symptoms
The former players scored higher on self-rated scales of depression, anxiety, and post-concussion symptoms than those in the control group, though not on sleep quality.
These symptoms, along with behaviour ratings of executive dysfunction and neuropsychiatric symptom severity, were more prevalent among individuals who had experienced a greater number of self-reported concussions. However, this was unrelated to the number of years played, or position of play.
Despite frequent subjective memory complaints, the performance of players in cognitive testing did not differ significantly from that of the control group. However, 24 former players, particularly those who had played as forwards and those who had reported more concussions, met the research criteria for CTE syndrome based on neurobehavioural disturbance. This was determined with low provisional levels of certainty: 21 were classed as 'suggestive.' three as 'possible.' and none as 'probable/definite.' Seven of the 24 had cognitive impairment, 12 had neurobehavioral dysregulation, and five had both.
Imaging Findings
3T MRI imaging showed the presence of cavum septum pellucidum in 24% of players, compared with 12% of controls. This was more common in those who had experienced more concussions. They also showed reduced volumes in the frontal and cingulate cortices, with reduced white matter and lower hippocampal volume associated with longer career durations.
Only 4.6% showed trauma-associated white matter changes on diffusion tensor imaging.
Elevated Blood Biomarkers
Using ultrasensitive digital enzyme-linked immunosorbent assays, researchers analysed fluid biomarkers associated with neurodegeneration.
Key findings included:
Phospho-tau217 levels were 17.6% higher in former players
23.1% had elevated phospho-tau217
9.0% had raised plasma neurofilament light
While levels were lower than in late-onset Alzheimer's patients, players with elevated markers had more severe neuropsychiatric symptoms, including depression and anxiety.
Frontal brain volumes correlated negatively with neurofilament light, and hippocampal volumes correlated negatively with phospho-tau217.
The findings were published simultaneously in two papers in the journal Brain .
Professor David Sharp, director of the UK Dementia Research Institute Centre for Care Research & Technology at ICL, who co-led the work, said: 'We didn't see any cases of early dementia in this group of former players, which is reassuring. However, the changes in blood biomarkers and brain imaging abnormalities show some long-term effects of repeated head impacts on the brain.'
The studies are set to continue for a further 4 years.
Calls for Action on Player Safety
'Nearly half of dementia cases are linked to known health and lifestyle risk factors, including traumatic brain injury from contact sports like rugby,' said Dr Jacqui Hanley, head of research at Alzheimer's Research UK. While not much is known about how such injuries cause changes to the brain, 'deepening our understanding could ultimately help lower dementia risk for professional sportspeople.'
Hanley called for stronger efforts to reduce head injury in contact sports, stating: 'Reducing traumatic brain injury in contact sports is critical to help prevent brain damage and minimise dementia risk for the players.'
The Alzheimer's Society echoed the concern, noting that professional rugby players face approximately twice the risk of dementia. They called for accurate data on injury patterns and their long-term effects. However, they also stressed that physical activity remains one of the most effective ways to reduce dementia risk.
The Dementia Trust has warned that repeated tackles, scrums, and collisions can contribute to CTE among rugby players and noted a rise in early-onset dementia among retired professional players.
In 2023, a group of 260 former professionals launched a lawsuit against World Rugby, the Welsh Rugby Union, and the Rugby Football Union. They alleged negligence in failing to protect players from the risk of neurodegenerative diseases.

LANSING, Mich. (WLNS) — With the Fourth of July approaching quickly, it's important to remember that fireworks celebrations are not for everyone. This includes pets, veterans and those living with dementia. The Alzheimer's Foundation of America (AFA) created the 'F.O.U.R.' steps to creating a dementia-friendly Fourth of July. 'Fireworks and loud noises can create challenges for individuals living with dementia, which is why families need to make thoughtful, proactive adjustments to help their loved ones with dementia celebrate and enjoy Independence Day,' said Jennifer Reeder, LCSW, SIFI, senior director of educational and social services at AFA. 'Being prepared and adaptable are the best ways caregivers can create a dementia-friendly 4th of July for their loved ones.' If someone is a war veteran, AFA says the noises that come from fireworks can be triggering and mistaken for gunshots or bombs. Being inside at times when they are likely to hear fireworks is a good precaution. 'Even if that person is inside, they still may be in an area where fireworks are going off,' Chris Schneider, director of media relations at AFA said. 'That can be leaving the air conditioning on, getting a white noise machine or playing soothing, familiar music. All of that can help muffle some of those sounds.' It can also be helpful to check in on your loved one throughout the night if they live with you. Large crowds can be overwhelming for someone living with dementia. If you are hosting a gathering, limit guests to a few family members and close friends. AFA says name tags could be beneficial. 'Structure the celebration around the person's routine,' Schneider said. 'Routine disruptions can be troubling for someone living with dementia. If they normally eat at a certain time, build that into the day. If they usually take a nap at a certain time, build that into the day.' The AFA says to revise how you celebrate. The organization mentions that living with dementia shouldn't limit how someone celebrates Independence Day. Caregivers could make patriotic decorations and crafts, or look through a family album with pictures of previous Fourth of July celebrations. Both are dementia-friendly activities. Enrolment concludes in Belite Bio's trial of Tinlarebant for geographic atrophy
Belite Bio has completed enrolment in the 24-month PHOENIX Phase III trial assessing the tolerability and safety of oral Tinlarebant in individuals with geographic atrophy (GA) in dry age-related macular degeneration (AMD). This multicentre, randomised, placebo-controlled, double-masked, global trial is crucial in evaluating the therapy's potential to minimise the atrophic lesion growth rate in these patients. It has enrolled 500 participants across various sites in the US, the UK, the Czech Republic, France, Switzerland, Taiwan, Australia, and China. Tinlarebant, a one-daily tablet, is being developed as an early intervention to preserve retinal tissue health and integrity in those with Stargardt disease type 1 (STGD1) and GA. At present, there are no treatments approved by the Food and Drug Administration (FDA) for STGD1 and no approved oral treatments for GA. The tablet has received fast track, rare paediatric disease, and breakthrough therapy designations in the US, orphan drug status in the US, Japan, and Europe, and Sakigake designation in Japan for STGD1. Tinlarebant is also being evaluated in a Phase III trial (DRAGON) and a Phase II/III trial (DRAGON II) for adolescent STGD1 individuals, in addition to the PHOENIX trial for GA subjects. Belite Bio CEO and chairman Dr Tom Lin said: 'Completing enrolment in the PHOENIX trial marks an important milestone for Belite Bio as we advance our lead candidate, Tinlarebant, for the treatment of geographic atrophy. Ministers face backbench calls to widen access to top tier of sickness benefits
Ministers are facing Labour backbench calls to widen access to their proposed top tier of sickness benefits. Labour backbencher Graeme Downie has proposed a welfare reform Bill amendment, so universal credit claimants with Parkinson's or multiple sclerosis who cannot work do not face repeated medical assessments to receive a payout. If MPs back his amendment, patients with 'evolving' needs who cannot work could also qualify for a higher rate of benefits. The Government's Bill has already cleared its first Commons hurdle at second reading, after work and pensions minister Sir Stephen Timms vowed not to restrict eligibility for the personal independence payment (Pip), with any changes coming in only after a review of the benefit. To meet his promise, ministers have had to table amendments to their own draft new law, to remove one of its seven clauses, which MPs will debate next Wednesday. Universal credit claimants with Parkinson's 'are already possibly struggling financially', Mr Downie told the PA news agency ahead of the debate. He added: 'The cost of living with a condition like Parkinson's can be very high. 'You may well require or need additional support.' The Dunfermline and Dollar MP said patients who struggle with their motor control might buy pre-chopped vegetables or chicken. 'Those things are expensive, so if you're already on universal credit and you're struggling, being able to do that significantly impacts your health, it significantly impacts your ability to live properly,' he continued. As part of the Government's reforms, the Department for Work and Pensions has proposed a new 'severe conditions criteria' for universal credit. Claimants in this category will be entitled to a higher rate of the benefit, and will not be routinely reassessed to receive money. To qualify, claimants must have limited capability for work or work-related activity (LCWRA) and symptoms which 'constantly' apply. Mr Downie's amendment would expand these criteria to claimants with 'a fluctuating condition'. It would cover 'conditions like Parkinson's but also multiple sclerosis, ME (myalgic encephalomyelitis), long Covid and a whole range of other conditions where, you know, in the morning things could be really good and in the afternoon things could be really bad, and even hour by hour things could change', he said. 'I felt it was necessary to table an amendment to really probe what the Government's position is on this, and ensuring that people with Parkinson's and conditions like that are not excluded from even applying and being considered.' Mr Downie's proposal has backing from 23 cross-party MPs. Juliet Tizzard, external relations director at Parkinson's UK, said: 'Criteria in the Bill say that a new claimant for the universal credit health payment will have to be 'constantly' unable to perform certain activities to qualify. 'This doesn't work for people with Parkinson's, whose symptoms change throughout the day. 'People with Parkinson's and other fluctuating conditions like multiple sclerosis will be effectively excluded from getting all the financial support they need. 'The Government has responded to our call and withdrawn the damaging restrictions to Pip. 'Now, they must do the same with the universal credit health element. The health of many people with Parkinson's is in their hands.'
