
How a Painless Patient Fooled Every Monitor
A case report by Thomas French, MD, research fellow in the Department of Cardiothoracic Surgery, Royal Infirmary of Edinburgh, Edinburgh, England, documented a rare case of congenital pain insensitivity.
The Patient and His History
The patient presented for an elective endoscopic ultrasound-guided biopsy (EBUS) of a lesion in his right middle bronchus and video-assisted thoracoscopic lung resection for two nodules in his right lung.
He had undergone excision and adjuvant radical radiotherapy of a pT3 N0 M0 sarcoma on the right side of his neck 4 years earlier. Forty-six months later, a chest x-ray revealed a 9 mm nodule in the apical segment of his right lung.
His past medical history was notable for CIP with anosmia and a host of related hospital presentations, including poorly healing ulcers, scoliosis, Staphylococcus discitis , and infected soft tissue injuries.
On one occasion, he presented to the emergency department after feeling a painless 'popping' sensation in his left arm and was diagnosed with a biceps tendon rupture. Imaging revealed widespread skeletal degeneration.
His condition had first come to his attention after scalding himself while working as a plumber. He had not noticed the injury until later. Subsequent genetic sequencing led to the diagnosis of CIP.
He had not required analgesia following his previous surgeries but had experienced autonomic responses to pain, characterised by tachycardia, hypertension, and pyrexia.
Findings and Diagnosis
During EBUS, a lesion was observed at the bifurcation of the right middle and lower lobes. A VATS wedge resection of the right upper and lower lobes was performed, and three tissue samples — from the lesion, the upper lobe, and the lower lobe — were sent for histopathology. Intraoperatively, the patient's blood pressure, pulse, and respiratory rate remained stable, with no autonomic responses to surgical stimuli.
He underwent EBUS of the lesion located at the bifurcation of his right middle and right lower lobes and VATS wedge resections of the right upper lobe and right lower lobe. Three samples were sent for histopathologic analysis: biopsied lesion, upper lobe wedge, and lower lobe wedge.
The patient displayed no autonomic response to the primary incision while under general anaesthesia: his blood pressure, pulse rate, and respiratory rate remained constant throughout the entire operation, from the induction of anaesthesia onward.
Post-operatively, the patient was started on an oxycodone patient-controlled analgesia (PCA) regime. He exhibited intermittent tachycardia, hypertension, and low-grade pyrexia in the immediate post-operative period, which resolved following administration of an opioid bolus. The PCA was stopped at 8 hours post-operatively, as he was feeling nauseous after each bolus without any analgesic benefit.
As with the intravenous opioid, oral opioids and oral paracetamol in the immediate post-operative period successfully resolved the patient's tachycardia, blood pressure elevation, and pyrexia. His chest drain was removed one day after surgery, and he was discharged on the second post-operative day.
Four days after the surgery, the patient was readmitted because of fever, cough, and some small-volume purulent discharge from his drain site. A repeat chest x-ray was unremarkable, and a wound swab was negative. He was empirically treated with 24 hours of intravenous antibiotics and given a short, 4-day oral course to complete at home. The team felt that a lower threshold for treating infection would be appropriate, given his diagnosis of CIP. He was discharged one day after readmission and made a good recovery.
Histopathology of the lesion biopsy at the bifurcation of the right middle and lower lobe bronchus was consistent with metastatic synovial carcinoma. The same finding was reported for a nodule contained within the right upper lobe wedge. The histopathology of the lower lobe wedge was unremarkable.
He was referred back to the sarcoma multidisciplinary team and underwent radiotherapy for the lower lobe nodule.
Discussion
'CIP encompasses a wide range of overlapping and poorly understood phenotypes. Patient analgesic requirements are likely to vary significantly from patient to patient, but opioids can be useful in controlling the autonomic response to painful stimuli,' the authors wrote.

Dogs Able to Sniff Out Parkinson's Before Symptoms Appear
Dogs can be trained to identify individuals with Parkinson's disease (PD) by sniffing out a disease-specific odor in skin swabs with up to 98% specificity and 80% sensitivity, a recent study suggested. The findings may help inform key biomarkers for PD before symptoms appear. There is currently no single diagnostic test for PD, and reported estimates show roughly a quarter of patients are misdiagnosed. 'Sensitivity levels of 70% and 80% are well above chance, and I believe that dogs could help us to develop a quick noninvasive, and cost-effective method to identify patients with Parkinson's disease,' lead investigator Nicola Rooney, associate professor at Bristol Veterinary School at the University of Bristol, Bristol, England, said in a news release. The study was published online on July 14 in the Journal of Parkinson's Disease . The Nose Knows Previous studies have shown that dogs can detect disease-specific odors in human breath or urine in patients with cancer, diabetes, and even posttraumatic stress disorder. This study adds to the growing evidence they can also identify PD. Seborrheic dermatitis and increased sebum production are premotor signs of PD. Previous research published last year by a nonprofit Parkinson's organization based in Washington showed that various breeds of dogs can be trained to recognize sebum unique to PD patients. This led the UK-based researchers to suspect sebum could be used as an indicator of PD. For this double-blind, randomized controlled trial, investigators collected dry skin swabs from 130 patients with PD and 175 individuals without the condition. Of these, 205 swabs were used for training dogs from the organization Medical Detection Dogs. For double-blind testing, 40 samples were from patients who didn't use drugs for PD (42.5% women; mean age, 69.6 years), and 60 samples were from individuals without PD. After 38-53 weeks of training, a Golden Retriever named Bumper and a Black Labrador named Peanut were shown samples on four stands in a line. In each line, there was either one positive sample or all controls. Both the trainer and experimenter were blind to the location of positive or negative samples. Computer software confirmed whether the dog's response was correct. If the dog was unsure, samples were retested in new lines until a clear decision was made. Dogs were rewarded for correctly identifying a positive or negative sample. The dogs showed high specificity by correctly ignoring 90% and 98.3% of swabs from the control group and high sensitivity by correctly identifying 70% and 80% of the swabs from patients with PD ( P < .0001). Of note, the Labrador had 15 weeks more training than the Golden Retriever and achieved the higher of the two results. 'While we do not envisage dogs being diagnostic, they could potentially, with refinements, help to validate methods of clinical utility…and aid rapid screening and diagnosis,' the investigators wrote.

MP attends Warrington support group meeting for those with bipolar
A SUPPORT group has marked nearly a decade of helping people. Charlotte Nichols, MP for Warrington North, attended the Warrington Bipolar UK Support Group (WBSG) to mark its nine and a half years of supporting individuals with bipolar disorder. The visit followed recent discussions around Personal Independence Payment (PIP) and gave Ms Nichols the chance to take part in a typical group meeting. (Image: Supplied) Ms Nichols said: "Thank you to the Bipolar UK support group in Warrington for inviting me to join their meeting tonight, and for everyone's openness in speaking about their experiences of the condition and how it impacts their lives. "The conversation was really wide-ranging, with people with a variety of different backgrounds, and in a really safe, supportive and welcoming environment. "I was shocked to learn tonight that it takes on average 9.5 years to get a correct diagnosis of bipolar and there is a misdiagnosis an average of 3.5 times." The group was founded in 2016 by former mayor of Warrington Geoff Settle, who used funds from his mayoral charities. Mr Settle said: "I became a facilitator because I wanted to share my experiences and help others. "When new members join, they often say that they have never talked to another bipolar person and that no one understands how they feel. "Some break down crying when they tell their story but when it's time to leave they usually depart with a smile on their face." He has taken courses to better support members, including suicide prevention training with Warrington-based charity Papyrus. Mr Settle said: "Suicide is the elephant in the room. "Mental health has always been low on the list of priorities and services, so I need to be prepared." Farmer stepped over dad's body to feed calves
A farmer who struggled with depression after his parents died within two years of each other has urged others to seek help. George Atwell, from North Somerset, said things "got too much" after he had to step over his dad's body to go to work in 2016, and his mum then died in 2018. "My father passed away in the farmhouse, in the kitchen," he said. "We had calves out in the shed that needed to be fed; I was walking over my father to go to feed the cattle. There's no other industry that would be doing that." Mr Atwell said he wanted others to know "it's OK not to be OK" after he was supported by the Farming Community Network (FCN), which sent a farmer to help him daily for two months. Data from the Farm Safety Foundation suggests 95% of young farmers believe mental health is the biggest issue facing the industry. "The industry faces many stress factors, which are placing increasing pressure on workers and putting them at greater risk of mental ill health," the foundation said. "These include extended amounts of time working in isolation, a blurring between work and home life, and financial uncertainty." Despite the heartbreak he had experienced, he said he needed to get on with farming, which is "full of problems" to deal with, such as "the weather, animals dying, making animals better". Mr Atwell said it had been reaching the point where he was unable to sleep. "I was trying to do everything, be 'the big I am'," he said. "I was terrified to leave the farm, I had hot sweats." Mr Atwell's wife could see something was not right and he went to the doctor for medication to help him with his mental health. "I didn't feel normal but, thanks to the support of the farming community, the local farmer that was coming to see me everyday, I managed to turn it around," Mr Atwell said. "It's OK not to be OK and hopefully others can take something from what I'm saying."
