"You're doing two full-time, emotional jobs" – 3 women share their 'sandwich carer' experiences
With ageing populations globally, the 'Sandwich generation' is growing rapidly. It's estimated that 1.4 million adults in the UK and 11 million in the US, the majority of whom are women aged 35-64, spend many hours a week juggling the needs of family members at both ends of the age spectrum.
Balancing these dual caregiving roles alongside work responsibilities and other commitments can create immense pressure, with many reporting feeling stressed, low on money and short on time.
The mental health impact is also significant, with the Office for National Statistics finding 31% of sandwich carers in the UK report symptoms of depression or anxiety (vs 24% in the general population) and 19% diagnosed with depression (vs 13% among all adults).
Different setups
Rachael Piltch-Loeb, 35, is a Professor of Public Health who lives in New York with her husband and three young children. She recently wrote a book titled The Millennial Caregiver, sharing her experience of assuming a caregiving role for her father, who was diagnosed with early-onset Alzheimer's at 58 in the same year her first son, now five, was born.
'It was one of the starkest contrasts to see the development of a child and the decline of a parent concurrently. My siblings and I took turns supporting my dad and offering respite to my mum, who was also working full time.
"We each assumed different roles based on our own personal and professional commitments. Given my career experience, I took on the research and management of care decisions.'
For Rachael, there were specific challenges in just entering the early stages of motherhood herself whilst taking on looking after a parent.
Dementia specialist Victoria Lyons, 50, who works at Dementia UK, is currently caring for her mother-in-law as well as her children aged 12 and 15.
'Like many, I had my children later in life, so my mother-in-law is now 90. Having been a widow since 1977, she was diagnosed with dementia in lockdown and, whilst she still lives alone, we are just two minutes' walk away.
"My husband, Andrew and I spend well over 30 hours a week with her, as well as both working. The longest we ever leave her is two to three hours.'
Sarah Gates, 49, has also taken on the role of a sandwich carer, supporting her granny, father and mother alongside looking after her teenage children for eight years.
She left her full-time lecturing job to be able to regularly visit her parents and grandmother (who lived 2.5 hours away) and still be hands-on at home with her teens.
Sarah tag-teams on visits with her brother, David, but when her father died five years ago, she turned to Age UK, which helped organise a carer for her mum.
'Mum was hugely resistant and angry about receiving any help. Her 'PA' was originally introduced to help her with 'admin and typing'. Once she was accepted as a friend, her visits became part of the routine.
"Now the team can cook meals, offer personal care and take Mum to appointments and out for a coffee too. Unfortunately, this amazing service comes at a cost, so we need to sell her house to fund getting her into a home near me.'
Deeply challenging
Despite Victoria's training as a dementia specialist, Admiral Nurse, her caregiving role at home puts significant strain on the family. She and her husband need to balance the school runs and kids' commitments while also getting her mother-in-law to a care centre three days a week, preparing her meals and seeing to her personal care.
'It's harder than I ever thought it would be, and we don't always get things right," she says.
"My husband and I both try to keep a handle on our stress levels and minimise the impact on the children, but I know at times they both feel like it is 'nanny first.'
"The experience has given the kids a greater understanding of what care means as well as an awareness of dementia.
"Spending so much time with my mother-in-law, there are moments of joy and great laughter – Nanny has a very funny sense of humour and listening to her and the boys cracking up and playing together is a real bonus.'
Look after yourself too
Rachael has some advice for others faced with so many life transitions – starting families, building careers, trying to afford housing, then adding caregiving into the mix suddenly.
'First, lower the bar," advises Rachael. "You're doing two full-time, emotional jobs, so let go of the idea that you must show up 100% for everyone every day. It's ok if some days your toddler gets screen time, and your dad gets a frozen meal.
"Second, accept help in whatever form it shows up, even if it's imperfect. And third, protect time for yourself. You can't pour from an empty cup – and I say that not as an inspirational quote but as a survival strategy.'

Practical advice
Victoria recommends devices such as a Ring doorbell (to prevent wandering, reduce anxiety, and provide a sense of security) and a dementia clock in addition to lots of noticeboards, pictures and signage as helpful practical steps.
Sarah found obtaining Power of Attorney (a legal document that grants a carer the authority to act on someone's behalf in legal, health and financial matters) a vital early step.
'We obtained two types of Power of Attorney (PoA) – one for health and one for finance, which were crucial in helping us look after Mum, Dad and my Granny (who died last year).
"When it comes to health, PoA enables you to call the GP to get test results or to make decisions regarding end-of-life care when the time comes.
"With finance, it meant being able to pay bills, budget for carers, order groceries online and help get cash for Mum when she'd forgotten her PIN number.'
Getting help
'Caring for elderly parents, grandparents, and teenagers means you must expect the unexpected. Meeting everyone's basic needs is the easier part," Sarah reflects.
"The hard part is the emotional fallout. Finding time to reassure a parent with dementia who feels lost and anxious alongside a teen who has had a horrible A-level exam and is upset and panicking is full on.
"There's an underlying feeling of anxiety as you try to work out what's best to support your loved ones with such different demands, but I've become better at delegating, getting help and asking people for advice.'
The constant balancing act of sandwich caring is highly pressurised, so seeking out guidance and help is essential. A growing network of services offers emotional support and practical input, as well as connecting communities of fellow carers and giving moments of peace amidst the pressure.

Protocol flexibility and navigating the US geopolitical landscape in oncology clinical trials
Industry experts have provided insight into how oncology clinical trials can be designed for success from the outset and rendered as patient-centric as possible. At the Clinical Trials in Oncology (CTO) East Coast conference, which took place between 8 and 9 July in Boston, US, key themes under consideration included strategies to account for the shifting geopolitical landscape in the US and beyond, budgeting effectively for an oncology clinical trial, trial design best practices, and augmenting trials with technology to shorten timelines and reduce costs. Accounting for the current geopolitical landscape With economic challenges, such as the Trump administration's imposition of tariffs, biotechs are facing mounting capital challenges that influence clinical trial activity. During a panel on the evolving oncology trials landscape in the US, speakers reflected on how to navigate this shifting landscape effectively. AstraZeneca's executive director of oncology regulatory strategy Shaily Arora advised keeping the global development strategy in mind, noting that it may be helpful to work with CROs that have specialisation in European regulation, given that regulation such as the EU's clinical trials regulation (CTR) is quite different in comparison to US regulatory requirements. Beth Chamblin, senior director, project management, late phase oncology at IQVIA Biotech, highlighted that trust, flexibility, and early integration have become foundational, and that policy advocacy is becoming increasingly more important with respect to navigating changes. The panellists also agreed that regional diversification was of rising importance in planning, for example, flexible trial designs, so that if something does change [in the US], there is a backup option to continue a clinical trial, pivoting if required. The importance of flexible trial protocol design During an interview with Clinical Trials Arena, NextPoint Therapeutics' chief medical officer Leena Gandhi stressed how flexible trial designs can shorten study timelines and decrease costs. A key aspect Gandhi homed in on was being aware when moving 'operationally slowly' through dose escalation in Phase I, positing this as one area that can stymy a trial's movement into Phase III. 'It's challenging from both a patient and clinician point of view to put patients on sub-therapeutic doses,' Gandhi says. 'There's a tendency to say, let's wait and see, especially if there's a large dose range, to get into those higher doses.' 'The more a trial's design focuses on using biology to minimise the dose levels that are chosen, the faster we can at least move through the dose evaluation, which represents one of the biggest challenges of spending too much time in Phase I.' However, Gandhi added that learning as much as possible during Phase I can serve to expedite oncology clinical trials, potentially making it possible to 'skip' a Phase II trial and progress to Phase III sooner. She commented: 'The more solidly we can understand both the right dose as well as the right patient selection strategy before Phase III, the bigger the impact will be, the bigger the difference in outcomes between the control arm in a randomised setting, and the faster you'll get through that Phase III trial, because you'll have a body of robust data before getting there.' While advocating for the creation of adaptable trial designs that allow for flexibility during trials for increased efficiency, Gandhi pointed out that being too flexible can also be a pitfall. Gandhi explains: 'You can spend a lot of time writing vague language in a protocol and then have it not be approved by the regulatory authorities because it's too vague. And at some level, you can't predict everything you're going to learn in a Phase I trial, either. You can't anticipate all the different things that you might need to do. You have to be able to respond to the data. 'One thing I hope we're moving towards, from a regulatory environment perspective, is getting faster and better at amending trials if we need to, so we don't lose time, because it's hard to know everything that you might run into ahead of time.' Patient-centricity is key Speakers also focused on how clinical trials can be made more amenable for patients. The prevailing theme was that patient centricity is key, and that success in oncology trials, given the gravitas of patients living with cancer, foremostly needs to meet patients where they are. To ensure patient retention in trials and mitigate drop-out rates, various speakers advised that trials should be designed with geographic location and patient convenience as the utmost considerations. Meanwhile, in certain cases and to the extent possible, attendees were advised to augment their oncology clinical trials with technology such as AI and remote patient monitoring tools to hasten and simplify measurement protocols where possible. Finally, with trial design, attendees were advised to plan to ensure there are no surprises later in the study to maximise the success of a trial with as high a participation rate as possible. Other sessions, including one led by Kelly Schlemm, vice-president and head of corporate affairs at Servier Pharmaceuticals, outlined the best approaches towards raising community awareness of oncology trials to boost participation. Schlemm's advice included leaning on patient advocacy groups to raise clinical trial awareness – a matter of particular importance for isolated or remote rural communities where there has historically been a scarcity of clinical trials. AstraZeneca's Shaily Arora reflected on the US Food and Drug Administration's (FDA) priority review programmes, such as the breakthrough therapy and fast track designation programmes, and how they can streamline developmental timelines, leading to faster drug approvals and thereby meeting patient needs earlier. In an interview with Clinical Trials Arena, Arora notes that there is a 'threshold of information' required for companies to show that they are offering something better than what is currently available when seeking approval for such programmes. Arora advises: 'Having a long-term plan, so engaging with the healthcare authorities early on to devise the strategy for your early phase clinical trials, utilising and optimising early phase trials to leverage the data that you need to help successful late phase trials, will be very beneficial. 'These designations can give you the opportunity to have that open dialogue and frequent communication with the health authority. That could be one way to streamline the development and design studies that are fit for purpose.' Hopes around AI Another key theme of the conference was the potential of AI to expedite trials, gain fresh insights, or hasten development timelines. During a panel session featuring speakers including Rebecca Jacob, project lead of clinical and global study management at Sanofi, panel members expressed their hopes that AI would over time play an increased role in managing data and quality, selecting the most suitable patients for trials, and reaching patients in remote rural areas by analysing their electronic healthcare records (EHR) to gauge trial participation suitability. However, panellists agreed that when thinking about AI, companies should evaluate what's most challenging for them and what may adequately be remedied with AI, rather than 'going with the trend' and implementing AI without due consideration. The speakers concurred that AI deployment should be centred around building efficiencies to allow everyone in an organisation to perform their jobs better or faster, rather than thinking about AI as a means of replacing the human workforce. Southampton Hospital Radio has launched a new mobile app. The volunteer-run station, which has been broadcasting for more than 70 years, will now be able to reach listeners beyond the hospital through the free app. Listeners can tune in from anywhere, whether at home, in the community, or on the go. Hong Kong Men Prioritise Tech Over Health, New Study Reveals
AXA Global Healthcare study highlights worrying gaps in how men prioritise their mental and physical health HONG KONG SAR - Media OutReach Newswire - 22 July 2025 - Men in Hong Kong are significantly more likely to prioritise repairing a device or resolving a banking issue than seeking help for potential signs of serious illness, a new study reveals. The research, commissioned by AXA Global Healthcare, highlights a concerning pattern in how men respond to health issues - particularly when it comes to early action and preventative care. While almost all respondents (95%) said they would fix a broken laptop within a week, only a third (33%) would act as quickly if they noticed potential red flags in their own health - such as unexpected changes in weight, bowel habits or a suspicious mole. Even symptoms like unusual fatigue (just 27%) and high levels of stress (24%) failed to prompt swift action in many cases, despite being possible early signs of more serious conditions. In contrast, men in Hong Kong reported far greater urgency when dealing with money, work commitments or vehicles. An overwhelming majority said they'd act urgently if they lost their mobile phone (99%) or bank card (98%), while 96% said they'd prioritise responding to a text or call from their boss. Personal health concerns also took a back seat to pet welfare, with 72% saying they'd respond quickly if their pet seemed unwell. By comparison, only 64% said they'd see a health professional at all if something felt wrong. Instead, 45% said they'd consult the internet first, and around 10% would turn to AI or a health app for answers. We urgently need to normalise a more proactive approach to wellbeing - not just wait for something to go wrong.'
