Set 282 to win, 70 more than had been scored in any of the first three innings at Lord's, Markram and captain Temba Bavuma put on 147 to break the back of the chase.
Bavuma fell early on Saturday morning for 66 and, though Markram was dismissed six runs short of the victory target, his 136 was the crucial innings and allowed David Bedingham and Kyle Verreynne to finish the job.
It ends a series of agonising near misses for the five-time World Cup semi-finalists and 2024 T20 World Cup runners-up and marks the Proteas' first global trophy since 1998, when they won the Wills International Cup – the precursor to the Champions Trophy – and the Commonwealth Games.
Markram told 5 Live Sports Extra: 'It's been a very long time coming, got close in the past couple of years and not been able to get over the line.
'The first one was always going to be the toughest one, the group of guys have been playing some really good cricket but it never really feels worth it until you win something.
'Because of us getting over the line today, a lot of emotions are coming out of some of the boys and they're just really proud of what they've achieved.'
Bavuma had battled through a hamstring injury on Friday evening as he and Markram piled on the runs.
He lasted only three overs of the fourth morning before a tame edge off rival captain Pat Cummins was taken by wicketkeeper Alex Carey.
Tristan Stubbs dug in, contributing eight to a stand of 24 with Markram lasting nearly an hour before he was bowled by Mitchell Starc.
Bedingham settled South Africa's nerves in partnership with Markram as the pair put on 35 in 10 patient overs.
Markram's brilliant innings finally came to an end when he whipped Josh Hazlewood into the leg side but was superbly taken low to his right by Travis Head.
The fielder's lack of celebration suggested that the game was up and – though there was a final moment of drama when Verreynne gloved Starc through to Carey but was given not out, with Australia's reviews spent – the winning run came two balls later.

And in world football terms, they don't come much bigger than this in general. But only a few dare take on the task of managing the national team, given its storied history and reputation in the game with four World Cups and two European Championships. That challenge now falls on former Gers star Gennaro Gattuso, who has been appointed as the new manager of Italy. Gattuso, 47, takes over from Luciano Spalletti who just a few days ago announced his OWN sacking. Spalletti's time was up after a 3-0 defeat to Norway but he still took charge of the national side's next game against Moldova. Those results leave the Azzurri sitting third of the five teams in their World Cup qualifying group. Gattuso, who recently left Hajduk Split after narrowly missing out on the Croatian title, is understood to have accepted the role as first reported by Fabrizio Romano. He was given a heartfelt send off from Split after winning 20 of 43 games, drawing 14 and losing nine. Hajduk finished just two points behind eventual champions Rijeka. Gattuso started his managerial career at Swiss club Sion but was sacked after 12 games. He later coached Palermo, OFI Crete and Pisa before landing the manager's role at AC Milan, where he had spent most of his playing career. He was appointed boss at Napoli later that year and won the Coppa Italia in his first season there. After being let go by Napoli in 2021, Gattuso returned to management with Valencia the following year but quit after six months and then had a similarly short spell with Marseille in which he was sacked after five months. Gattuso signed for Rangers in 1997, as part of an influx of Italians including Lorenzo Amoruso, Marco Negri and Sergio Porrini (Swede Jonas Thern also joined that season from Roma). He was just 19 when signed by then-boss Walter Smith but he fell out of favour under Dick Advocaat at Ibrox and was moved from his usual midfield position to right-back before being sold to Salernitana for £4m.
