ECG Challenge: Palpitation Episodes in a COPD Patient

A 70-year-old man presents with a chronic obstructive pulmonary disease (COPD) exacerbation. He reports shortness of breath and palpitations. He has no known heart disease but has experienced palpitations in the past that were short-lived and did not require therapy.

The correct diagnosis is atrial fibrillation with Ashman's phenomenon (Figure 2).

Discussion
The rhythm is irregularly irregular, with no organized P waves. The average rate is 174 beats/min. The QRS complex duration (0.08 sec) and morphology are normal.
There are only three supraventricular rhythms that are irregularly irregular:
Sinus arrhythmia (one P wave morphology and stable PR interval)
Multifocal atrial rhythm (also called wandering atrial pacemaker) with a rate < 100 beats/min and multifocal atrial tachycardia with a rate > 100 beats/min (≥ 3 different P wave morphologies and PR intervals without any predominant P wave morphology)
Atrial fibrillation in which there are no organized P waves
Therefore, this is atrial fibrillation.
Noted are several QRS complexes that have an increased duration (0.12 sec) with a right bundle branch block (RBBB) morphology with an RSR' morphology in lead V1 (←) and a broad terminal S wave in lead V5 (→).
These complexes are not runs of NSVT, because they have a typical RBBB morphology and their intervals are also irregular. They are not the result of rate-related aberration, because there are other RR intervals as short or even shorter than these aberrated QRS complexes that are not associated with aberration. However, preceding the aberrated complexes is a long (┌┐)-short RR interval (└┘). This is the Ashman's phenomenon which is not caused by an abnormality in His-Purkinje conduction but rather by normal rate-related changes in His-Purkinje refractoriness or time for repolarization.
When the heart rate is slow (long RR interval), His-Purkinje refractoriness or time for repolarization prolongs whereas when the heart rate is fast (short RR interval), His-Purkinje refractoriness or time for repolarization shortens. This change in refractoriness or repolarization with heart rate is what causes the QT interval to change with heart rate, ie the QT interval is shorter at a faster heart rate and longer with a slower heart rate. When there is an abrupt change in heart rate from slow (long RR interval) to fast (short RR interval), His-Purkinje refractoriness does not adapt or change immediately and hence one or several QRS complexes are conducted with aberration. Most commonly the aberration is an RBBB, likely because the refractoriness of the right bundle is slightly longer than that of the left bundle.

HHS Journal Ban Won't Stop Corruption — It'll Make It Worse

Robert F. Kennedy Jr has threatened to bar federal scientists from publishing in top medical journals. This move risks backfiring on two major fronts. First, it will only accelerate private industry's sway over the scientific record. Second, launching new, government-run journals will demand vast resources and years of effort — and still won't earn the credibility of established publications. With nearly five decades in medical and scientific writing, editing, and publishing — across nonprofit and commercial organizations, legacy print and digital platforms, and both subscription-based and open-access models — I write from experience. To see the flaws in Kennedy's proposal, we need to understand what works and what doesn't in science publishing. Primary, peer-reviewed medical/scientific literature has evolved and thrived in a culture of self-criticism, through letters columns, corrections, retractions, and open debate. The New England Journal of Medicine (NEJM) , The Lancet , and JAMA remain the gold standards in medical publishing because of their rigorous peer review, global reach, and editorial independence from government or corporate influence. Here's where RFK Jr's main objection with the current system seems to lie. The Secretary has portrayed medical journals as hopelessly corrupted by industry. Extensive firewalls, guidelines, and rules have been established to govern the relationship of industry to medical journals. They rest largely on honest disclosure with authors, editors, and readers paying attention. Cracks in those barriers are not unknown. But the solution lies in strengthening these firewalls, not sidelining them. A ban on government employees from submitting to NEJM , The Lancet , JAMA, and other top-tier titles will deliver more power — not less — to pharmaceutical, device, and biotech companies to set the scientific agenda. Far from reducing 'corruption,' such a misguided policy would magnify the role of the very stakeholders RFK Jr decries. And if federal grant support diminishes, the research that is published will become increasingly supported by industry, compounding the mistake. The notion of creating new government-owned medical journals from scratch is not an absurd idea. But Kennedy's illusion of fast-tracking NIH-affiliated "preeminent journals" that stamp federal‐funded work as unquestionably legitimate is a gargantuan endeavor. Building editorial boards, peer‐review standards, submission platforms, indexation in PubMed, and marketing to researchers worldwide takes years of work from countless individuals and would cost a substantial amount of money. Even then, a journal's reputation rests on trust and perceived independence. Readers judge not only the science but also the integrity of the editor–owner relationship. The hazard is that the owner (the government) would have to be trusted by the readers, or no one would bother reading these publications. A government 'house organ' would likely be viewed skeptically if the federal government can withdraw or prohibit publications at will. Banning federal scientists from submitting to journals the administration doesn't like does not cleanse the literature of industry influence — it deepens those ties. And while government-run journals might one day exist, they won't arrive fully baked, credible, or conflict-free. Better to invest in the proven mechanisms of editorial independence, enhanced peer review, and clearer disclosure than in a rushed, state-controlled alternative destined to struggle for trust and impact. If RFK Jr wants a better list of reforms, here's what I suggest: Take on predatory publishers and their fake journals, fake authors, and fabricated institutions and references — a threat that existed even before generative chat powered by artificial intelligence (AI). Take aim at rapacious mainstream publishers, whose excess profit margins and subscription price gouging represent a financial drain on researchers, readers, and academic libraries. Crack down on excessively large author fees to have an article considered/reviewed/published. Promote the publication of reproducibility studies. Raise the alarm about the use of AI in peer view and the creation of manuscripts — including the data in them. These steps aren't as sexy as proclaiming publishing bans for government scientist or launching new journals on whose mastheads you can put your own name. But they have the virtues of solving real problems and not making existing problems worse — which, as a physician, seems like something I've heard before somewhere …

Stone later added that his ailments, "haven't stopped me from hearing music, but they have stopped me from making it." What is COPD is it something you should worry about when it comes to your own health? Read on to learn more about COPD. COPD is a progressive lung disease that causes airways to become swollen and blocked. It's an umbrella term used to describe various lung conditions, including chronic bronchitis and emphysema, according to COPD Canada. In chronic bronchitis, the airways in your lungs become inflamed and narrowed. This means eventually, you will develop more mucus, leading to a persistent cough and difficulty breathing. For emphysema, the disease develops over time and involves the gradual damage of tiny air sacs in your lungs called alveoli. The damage eventually causes these air sacs to rupture, reducing the surface area that lets oxygen move through the bloodstream. The most common symptoms of COPD are difficulty breathing, chronic cough (sometimes with mucus) and feeling tired, according to the World Health Organization (WHO). The Canadian Lung Association added a person with COPD might also present some of the following symptoms: Feeling short of breath Lung infections (like the flu or pneumonia) that may last longer than they would in others Wheezing Losing weight without trying These symptoms can get worse quickly and may be called flare-ups. "These usually last for a few days and often require additional medicine," WHO stated. COPD develops over time, often as a result of a combination of risk factors, according to the WHO. These may include: Smoking or being exposed to second-hand smoke Exposure to dusts, fumes or chemicals in the workplace Indoor air pollution from sources like burning fuel for heat or coal to cook Asthma in childhood Early life events, including poor growth in utero, prematurity or frequent respiratory infections A rare genetic condition called alpha-1 antitrypsin deficiency The Canadian Lung Association indicated adults over age 40 who smoke or previously smoked may be at risk of COPD. "It is important to speak to a healthcare provider if you are at risk of COPD, even if you don't have symptoms or have only mild symptoms," the organization stated. "COPD is a progressive disease, meaning it gets worse over time." COPD is severely underdiagnosed, according to the health charity. That's because people living with the disease may not show any symptoms, or they link their symptoms to factors like age, a common respiratory infection or a "smoker's cough." Moreover, some people who have stopped smoking may believe they can no longer develop COPD. Historically, COPD was associated with older men who had a history of smoking. While the number of men and women who die of COPD in Canada each year is roughly equal, recent research has indicated COPD is likely underdiagnosed in women. The Canadian Lung Association shared women might also experience more severe symptoms and have earlier disease onset. Finally, over the past two decades, there has been a rising number of COPD hospitalizations for both women in the 40 to 64 age group as well as those older than 65. That's in addition to a decreasing number of hospitalizations in men. There are more than two million Canadians impacted by the disease, and a further one million likely living with the condition unknowingly. COPD is also the second-leading cause of hospitalization in Canada, behind only childbirth. Globally, the WHO has indicated COPD is the third-leading cause of death, causing more than three million deaths in 2019 alone. That year, there were more than 212 million prevalent cases of COPD reported worldwide. There is no cure for COPD. Still, there are ways to improve the condition and possibly slow its progression. Since the majority of COPD cases are related to using cigarettes, it's best to never smoke — or stop smoking now. Moreover, you should speak to your supervisor about protection if you work around occupational exposure to chemical fumes, dusts or anything other elements that may increase your risk for COPD.
