NHS chief accuses BMA of ‘extortion'

The head of the NHS attacked the British Medical Association (BMA) for demanding 'extortionate pay' amid a strike row.
Sir Jim Mackey, the NHS chief executive, criticised the doctors' union in a message to NHS hospital trust leaders.
Junior – now resident – doctors have entered the last of a five-day strike that began on Friday.
The BMA has rejected 18 emergency requests by NHS hospitals for striking doctors to cross the picket line and help.
Hospitals are able to make emergency requests for help to striking doctors – called 'derogations' – to help stop patients coming to harm, such as in emergency departments and cancer care.
However, Sir Jim said that of the 18 rejected requests, the BMA would only support half of these 'if extortionate pay rates were offered to striking doctors'.
In a message seen by The Telegraph, he said they had worked to 'overhaul the process for patient safety mitigations' with nine requests approved by Sunday night.
But he added: 'Unfortunately, despite all requests being made and verified by senior medics, 18 have been rejected by the BMA, with half of those rejections saying the BMA would support only if extortionate pay rates were offered to striking doctors.'
He encouraged NHS bosses to keep putting them forward where they were needed.
The exemptions are requested by senior NHS medics in exceptional circumstances to protect patient safety and must be agreed on a case-by-case basis by BMA leaders, including chairman, Dr Tom Dolphin.
As of Sunday evening, there had been 47 requests for 125 doctors, according to the BMA. Some are pending or were withdrawn.
The resident doctors are demanding a 29 per cent pay rise to return them to 2008 levels of pay.
They have been awarded a 28.9 per cent pay rise since Labour came into power, including a 5.4 per cent rise for this financial year.
Nurses, who are expected to overwhelmingly vote to reject their 3.6 per cent pay rise, could also ballot to take industrial action later this year.
One emergency request rejected by the BMA was for a resident doctor to carry out biopsies on men with suspected prostate cancer at Milton Keynes Hospital.
The doctor subsequently volunteered to return to work and was praised by Wes Streeting, the Health Secretary, for their 'bravery'. He said it 'ensured these men got the care they deserve'.
Of the requests granted by the BMA, one was for a doctor to return to work on a neonatal ward caring for new born babies in Nottingham City Hospital.
The BMA said it was launching a separate, related dispute with the Government over 'training bottlenecks'.
A survey of 1,053 resident doctors finishing their foundation training found that 52 per cent had not secured a job for next month.
After two years of foundation training, resident doctors begin speciality training to become consultants or GPs.
The BMA said this year there had been more than 30,000 doctors applying for just 10,000 posts.
Dr Ross Nieuwoudt and Dr Melissa Ryan, BMA resident doctors committee co-chairs said:'It's absurd that in a country where the Government says bringing down NHS waiting lists is one of its top priorities, not only is it not prepared to restore doctors' pay, but it also won't provide jobs for doctors ready, willing and capable to progress in their careers.
'Commitments from the Government to address this don't go far enough or are too vague to convince us that they understand the gravity of the situation, so we're making clear that, alongside pay, we are entering a dispute and demanding action so that no UK-trained, capable, doctor is left underemployed in the NHS.'
It comes as health workers represented by the trade union, Unite, voted to reject the 3.6 per cent pay award for this financial year.
Some 89 per cent rejected the deal, with the union saying it was prepared to take strike action.
It represents a range of NHS staff including paramedics, healthcare assistants, and cleaners.
Sharon Graham, Unite general secretary, said: 'When it came into power this Government was clear that the NHS was broken. But staff are still leaving in droves and morale is still at an all-time low. The NHS can't be repaired while the Government continues to erode pay and drastically cut NHS budgets.'
Health workers in the GMB union have also rejected the pay deal while the Royal College of Nursing is due to announce a ballot of its members this week.

A new report into the case of a five-year-old boy who died a week after he was sent home from a hospital emergency department has found his mother's instinct was "repeatedly not addressed across services". Yusuf Mahmud Nazir died on 23 November 2022, eight days after he was seen at Rotherham Hospital and sent home with antibiotics. A report into Yusuf's case in October 2023, by independent consultants and published by NHS South Yorkshire, found his care was appropriate and "an admission was not clinically required", but this was rejected by his family. Yusuf's uncle Zaheer Ahmed has always said they were told "there are no beds and not enough doctors" in the emergency department, and that Yusuf should have been admitted and given intravenous antibiotics in Rotherham. However, a new report published on Thursday by NHS England, and led by former general secretary of the Royal College of Nursing Peter Carter, said in its conclusions: "Our primary finding is that the parental concerns, particularly the mother's instinct that her child was unwell, were repeatedly not addressed across services. "A reliance on clinical metrics over caregiver insight caused distress for the family. "This led to a lack of shared decision-making and there was limited evidence of collaborative discussions with Yusuf's family around clinical decisions, leading to a sense of exclusion and reduced trust in care plans."

Retirement, often hailed as a gateway to freedom and new possibilities, frequently conceals a less-discussed reality: a period fraught with emotional challenges and profound uncertainty. The abrupt cessation of a structured working life can prove a particularly difficult adjustment for many individuals. This often-overlooked pattern is something BACP senior accredited psychotherapist and counsellor Lina Mookerjee, from Praxis Therapy, has consistently observed among her older clients. ' One of the biggest things that I have observed is that we are fed an image of retirement, but it's not actually the same for everybody,' reflects Mookerjee. 'There's often assumptions about how wonderful it's going to be and I think that encountering the reality can sometimes be a shock.' However, with the right mindset and preparation, retirement can be a fulfilling and healthy chapter. Here, Mookerjee shares her top tips for navigating this life-changing period with confidence… Create a healthy routine 'Having routine and structure to your days means that you commit to not just the immediacy of the day, but also long-term health in your diet, exercise, social contact and rest,' says Mookerjee. 'Maintaining health and wellbeing in older life becomes far more precarious and you have to be even more disciplined because things start to go wrong. A rhythm to your day also offers reassurance and comfort.' Find your purpose 'Going to work gives you a purpose. Often that purpose is to provide for your family, or the company's mission may help you feel like you're part of something,' acknowledges Mookerjee. 'During retirement, it's then down to you as an individual to take that on. It's a time of major reevaluation and can cause an existential crisis about our own existence. A sense of confusion and chaos can ensue, so in terms of mental health, it can be very destabilising.' Mookerjee encourages individuals in this situation to consider what else is truly important to them and to explore what brings them happiness beyond work. 'Take action and think about what you need to reevaluate,' advises the psychotherapist. 'Ask yourself, what's important to me? Is it charity work? Is it giving to the homeless? It's about finding worth, as worth and purpose are connected.' Consider volunteering 'I was in my local charity shop at the weekend and the women volunteering in there were in their late 80s and they were so busy,' says Mookerjee. 'But, they love it because it helps them feel wanted and needed. So, volunteering can be really powerful.' Make an effort to maintain existing connections 'During retirement people often move from an office that's busy to then silence,' highlights Mookerjee. 'As humans, we exist more healthily with people, so it's all about making the effort and being your own personal assistant when it comes to social plans. 'Try to get into a routine where you meet up with your former colleagues or mates on a weekly or monthly basis. Make a commitment to meet up at a social club or at a coffee shop, wherever is familiar.' Make new connections 'Make attempts to make new contacts,' recommends Mookerjee. 'Join a new club, volunteer at a charity shop or join an activity in a library. 'My local library offers all sorts of activities such as backgammon, Scrabble, Sudoku etc. It gets people out and about to meet, and it's familiar. It gives you something to look forward to, to get up and get dressed for.' Try gardening ' Gardening can be incredibly restorative,' says Mookerjee. 'I would definitely encourage everyone to get themselves three or four pots and to start planting. It's incredibly therapeutic and healing and gives you something to look after.' Go for a walk everyday 'Get up and go walk for 20 minutes at least every day, twice a day, on a familiar route,' recommends Mookerjee. 'Movement is vital, especially in the sunlight so that you get some vitamin D.' Stimulate your brain 'Retirement is an opportunity for trying new things which stimulate the brain,' says Mookerjee. 'My husband has retired and has started playing chess online against people all around the world. Playing games likes Sudoku and chess are great ways to stimulate the mind.' Celebrate your achievements 'Choose one or two small projects and celebrate every accomplishment you make because that starts to build your tank of confidence,' says Mookerjee. Shift your expectations 'Change your expectations and do things that are within your limits and respect the fact that we become more limited physically as we get older,' says Mookerjee. 'Don't stop being active, but work within your limitations. Our bodies start to change and become more frail and fragile, but your sense of self changes in retirement and can make you become stronger and wiser.' Allow yourself to rest 'Retirement is also a time to rest and recuperate and to let your body heal and to give it attention,' says Mookerjee. 'The trajectory that you've been on with work and stress is only going to lead to burnout, so allow yourself time to rest.'

Dear Richard, My mother is in a care home near us in south London, having lived in the country for many years. She is partially paralysed after a stroke; not classed as having dementia, but her memory is not good and she can seem disorientated. We visit as often as we can and her room is full of pictures of family and bits of her needlepoint and things from home. There's a roster of care workers who look after her, some of whom clearly adore her. It took her a while to get used to the indignity of 'personal care' but she does now speak fondly of at least some of them. However, she has recently complained to us about rudeness and roughness on the part of three of the carers, and asked that these individuals not look after her in future. Naming no names, we have asked the director whether residents can request specific carers – and she made it clear that that would not really be feasible. Our only option would therefore be to escalate Mum's complaints and name the carers involved. Of course, we want Mum to have the best possible experience of this inevitably trying stage of life. We also want to respect her experience and not give her the third degree. But the fact is that she is quite vague about what it was about the three carers she didn't like. It's also quite striking to us that they are all of west African heritage, with a way of talking that will be very unfamiliar to a white woman who has spent most of her life in rural Wiltshire. Without in any way wishing to accuse her of being prejudiced we wonder whether there is a sensitive way of handling this that doesn't involve subjecting three hard-working care professionals to possible disciplinary proceedings, with no guarantee that their replacements, if they were replaced, would be any more congenial to Mum. Any advice would be most welcome. – G&R, London Dear G&R, You need to prioritise in what I accept is a potentially difficult and sensitive situation. What is most important here? Answer: your mother. If she is unhappy with specific carers, while being more than comfortable with others, then you – and the care home – have a duty to investigate the reasons why. Start by gently finding out as much as you can about the specific problem. I believe you should be absolutely colour-blind in this issue. The only thing that matters is the truth. Are these three carers treating your mother unkindly? Yes or no? You must not be afraid of 'escalating' this, whether to the home's management, your local authority or even the Care Quality Commission. You don't have to accuse anyone directly of anything. You merely have to ask that your mother's concerns be properly and professionally looked into. That's in everyone's interests, surely? At the very least, your mother will have had a fair hearing and the truth – one hopes – will have been established. There may be some specific explanation for certain incidents to do with overwork or clashing rotas or somesuch. There may indeed be misunderstandings at play, and perhaps everyone can learn from the episode. But if your mother remains unhappy – even if these specific carers are exonerated – you may have to consider moving her to another care home. Let's hope it doesn't come to that.
