
When developing countries band together, lifesaving drugs become cheaper and easier to buy − with trade-offs

Procuring lifesaving drugs is a daunting challenge in many low- and middle-income countries. Essential treatments are often neither available nor affordable in these nations, even decades after the drugs entered the market.
Prospective buyers from these countries face a patent thicket, where a single drug may be covered by hundreds of patents. This makes it costly and legally difficult to secure licensing rights for manufacturing.
These buyers also face a complex and often fragile supply chain. Many major pharmaceutical firms have little incentive to sell their products in unprofitable markets. Quality assurance adds another layer of complexity, with substandard and counterfeit drugs widespread in many of these countries.
Organizations such as the United Nations-backed Medicines Patent Pool have effectively increased the supply of generic versions of patented drugs. But the problems go beyond patents or manufacturing – how medicines are bought are also crucially important. Buyers for low- and middle-income countries are often health ministries and community organizations on tight budgets that have to negotiate with sellers that may have substantial market power and far more experience.
We are economists who study how to increase access to drugs across the globe. Our research found that while pooling orders for essential medicines can help drive down costs and ensure a steady supply to low- and middle-income countries, there are trade-offs that require flexibility and early planning to address.
Understanding these trade-offs can help countries better prepare for future health emergencies and treat chronic conditions.
Pooled procurement reduces drug costs
One strategy low-income countries are increasingly adopting to improve treatment access is 'pooled procurement.' That's when multiple buyers coordinate purchases to strengthen their collective bargaining power and reduce prices for essential medicines. For example, pooling can help buyers meet the minimum batch size requirements some suppliers impose that countries purchasing individually may not satisfy.
Countries typically rely on four models for pooled drug procurement:
-- One method, called decentralized procurement, involves buyers purchasing directly from manufacturers.
-- Another method, called international pooled procurement, involves going through international institutions such as the Global Fund's Pooled Procurement Mechanism or the United Nations.
-- Countries may also purchase prescription drugs through their own central medical stores, which are government-run or semi-autonomous agencies that procure, store and distribute medicines on behalf of national health systems. This method is called centralized domestic procurement.
-- Finally, countries can also go through independent nonprofits, foundations, nongovernmental organizations and private wholesalers.
We wanted to understand how different procurement methods affect the cost of and time it takes to deliver drugs for HIV/AIDS, malaria and tuberculosis, because those three infectious diseases account for a large share of deaths and cases worldwide. So we analyzed over 39,000 drug procurement transactions across 106 countries between 2007 and 2017 that were funded by the Global Fund, the largest multilateral funder of HIV/AIDS programs worldwide.
We found that pooled procurement through international institutions reduced prices by 13% to 20% compared with directly buying from drug manufacturers. Smaller buyers and those purchasing drugs produced by only a small number of manufacturers saw the greatest savings. In comparison, purchasing through domestic pooling offered less consistent savings, with larger buyers seeing greater price advantages.
The Global Fund and the United Nations were especially effective at lowering the prices of older, off-patent drugs.
Trade-offs with pooled procurements
Cost savings from pooled drug procurement may come with trade-offs.
While the Global Fund reduced unexpected delivery delays by 28%, it required buyers to place orders much earlier. This results in longer anticipated procurement lead time between ordering and delivery – an average of 114 days more than that of direct purchases. In contrast, domestic pooled procurement shortened lead times by over a month.
Our results suggest a core tension: Pooled procurement improves prices and reliability but can reduce flexibility. Organizations that facilitate pooled procurement tend to prioritize medicines that can be bought at high volume, limiting the availability of other types of drugs. Additionally, the longer lead times may not be suitable for emergency situations.
With the spread of COVID-19, several large armed conflicts and tariff wars, governments have become increasingly aware of the fragility of the global supply chain. Some countries, such as Kenya, have sought to reduce their dependence on international pooling since 2005 by investing in domestic procurement.
But a shift toward domestic self-sufficiency is a slow and difficult process due to challenges with quality assurance and large-scale manufacturing. It may also weaken international pooled systems, which rely on broad participation to negotiate better terms with suppliers.
Interestingly, we found little evidence that international pooled procurement influences pricing for the U.S. President's Emergency Plan for AIDS Relief, a major purchaser of HIV treatments for developing countries. PEPFAR-eligible products do not appear to benefit more from international pooled procurement than noneligible ones.
However, domestic procurement institutions were able to secure lower prices for PEPFAR-eligible products. This suggests that the presence of a large donor such as PEPFAR can cut costs, particularly when countries manage procurement internally.
USAID cuts and global drug access
While international organizations such as the Medicines Patent Pool and the Global Fund can address upstream barriers such as patents and procurement in the global drug supply chain, other institutions are essential for ensuring that medicines actually reach patients.
The U.S. Agency for International Development had played a significant role in delivering HIV treatment abroad through PEPFAR. The Trump administration's decision in February 2025 to cut over 90% of USAID's foreign aid contracts amounted to a US$60 billion reduction in overall U.S. assistance globally. An estimated hundreds of thousands of deaths are already happening, and millions more will likely die.
The World Health Organization warned that eight countries, including Haiti, Kenya, Nigeria and Ukraine, could soon run out of HIV treatments due to these aid cuts. In South Africa, HIV services have already been scaled back, with reports of mass layoffs of health workers and HIV clinic closures. These downstream cracks can undercut the gains from efforts to make procuring drugs more accessible if the drugs can't reach patients.
Because HIV, tuberculosis and malaria often share the same treatment infrastructure – including drug procurement and distribution networks, laboratory systems, data collection, health workers and community-based services – disruption in the management of one disease can ripple across the others. Researchers have warned of a broader unraveling of progress across these infectious diseases, describing the fallout as a potential 'bloodbath' in the global HIV response.
Research shows that supporting access to treatments around the world doesn't just save lives abroad. It also helps prevent the next global health crisis from reaching America's doorstep.

80 Years After WW2, Japan Again Finds Itself in Uncharted Waters
The country's postwar history was a remarkable story of national rebirth. Now the assumptions that underpinned the last 80 years of policy are being stressed to the limit. August is a month of remembrance for Japan. On August 6, 1945, Japan became the first (and so far only) country in the world that has ever been struck by an atomic bomb. This was quickly followed by a second atomic bombing on August 9. Then August 15 marks the date of Japan's unconditional surrender to the Allied Forces, officially ending World War II. This year marks the 80th anniversary of the war and destruction. Japan's postwar history was a remarkable story of a nation's rebirth and reconstruction. Having vowed to strive to 'occupy an honorable place in international community' in its Constitution – and prohibiting itself from resorting to war as a means to settle disagreements with other countries – Japan has become one of the staunchest supporters of the international order, institutions, and norms that were created after World War II. Placing its bilateral alliance with the United States at the core of its foreign policy, Japan has striven to fulfill the role of 'a rule-promotor, a commons' guardian and an effective partner and ally to the U.S. and other democracies,' as Prime Minister Shinzo Abe put it in 2013. Indeed, Japan's postwar foreign policy embodies its tireless – and continuing – efforts in this endeavor. Despite persistent economic stagnation, Japan is the third largest financial contributor to the United Nations, only after the United States and China. It also is one of the top providers of Official Development Assistance. Both distinctions are fitting for a country that remains one of the most powerful economies in the world. Also, as the only country in the world that experienced the catastrophic damage of the atomic bomb, Japan has consistently been among the leading voices to push for nuclear disarmament. Prime Minister Ishiba Shigeru reaffirmed Japan's commitment to ultimately achieve a 'world without nuclear weapons' when he spoke at the peace memorial ceremonies in Hiroshima and Nagasaki earlier this month. Looking at Japan's postwar foreign policy, the Japan-U.S. alliance has undergone the greatest transformation since the two countries signed their original mutual security treaty in 1951, which was later revised into its current form in 1960. What once was a lopsided alliance between the United States as 'protector' and Japan as the 'protected,' in exchange for hosting a U.S. military presence, has evolved into a much more equitable partnership. So far, the alliance has demonstrated a remarkable resilience, adapting to the changing reality of geostrategic environment from the Cold War to the post-Cold War through the 21st century. However, as Japan reflects on the last eight decades, it also finds itself at a crossroad, both domestically and externally. Domestically, the so-called '1955 system,' which consistently provided stable political leadership under the dominant rule of the Liberal Democratic Party (LDP), has been showing signs of atrophy in recent years. The LDP's undeniable decline following its electoral setbacks in three consecutive elections, coupled with a proliferation of small parties (none of which enjoys enough voter support to replace the LDP as a new governing force), strongly suggests that Japan will enter a period of muddling through with weak political leadership with frequent leadership changes. Meanwhile, Japan's economy continues to struggle, as inflation outpaces the rise in people's wages. With a declining population and a society that is aging at an accelerating rate, Japan is also struggling to deal with enduring issues such as immigration, diversity, and gender equality. The political, economic, and societal uncertainties at home could not have hit Japan at a worse time. Today, the country finds itself having to navigate uncharted waters as the international order undergoes tectonic shifts. With the U.S. abdicating its noblesse oblige with Trump administration's 'America First' policy and its tariff war against allies, partners, and foes alike, the resilience of the post-World War II liberal international order and the strength of multinational institutions are increasingly being questioned. The deteriorating environment in the Middle East and protracted war between Russia and Ukraine has been accompanied by increasingly close strategic alignment among North Korea, China, Russia, and Iran. Add to that declining confidence in the postwar multinational framework, and these factors all point to a inevitable reality: anchoring its foreign policy in a robust Japan-U.S. alliance no longer sufficiently serves Tokyo's interest. In the years immediately followed World War II, Japan's policy was led by the Yoshida Doctrine, focusing on economic development by relying on the Japan-U.S. alliance for security. As the world searched for a post-9/11 adjustment to the international order, the late Abe Shinzo offered visions for Japan such as the 'Proactive Contribution to International Peace' and 'Free and Open Indo-Pacific.' Will Ishiba be able to offer a forward-looking vision that can guide Japan through domestic and international uncertainty at this important time of reflection for Japan? His ultimate political fate may depend on it.

TICAD Draft Outlines African Development Strategies;Japanese Technologies to Have Key Role in Cooperation
Leaders of Japan and African countries aim to support economic growth in Africa through private-sector initiatives using Japan's cutting-edge technologies, according to a draft document to be adopted at the Ninth Tokyo International Conference on African Development (TICAD 9) to be held on Aug. 20-22 in Yokohama. The draft document, the Yokohama Declaration, obtained by The Yomiuri Shimbun, also states the importance of the 'rule of law' for sustainable growth on the continent. TICAD 9 will be cohosted by the Japanese government, the United Nations, the African Union Commission and others, and will be chaired by Prime Minister Shigeru Ishiba. TICAD 8 was held in Tunisia in 2022, and this will be the first time for the conference to be held in Japan since 2019. The draft proposes cooperation measures in three key areas — economy, peace and stability, and society — aimed at strengthening relations between Japan and Africa, where population growth is expected to continue and economic growth is anticipated. In the field of economy, the draft proposes the establishment of 'Africa-Japan Innovation Hubs' to promote digitization and technological cooperation. Japan will support Africa's economic growth by leveraging its strengths in robotics and artificial intelligence. The draft also calls for the establishment of a governance system aimed at realizing 'safe, secure and trustworthy AI.' In regard to procurement of development funds in African countries, the draft emphasizes the need for reforms, such as improving transparency. 'The high cost of capital, largely due to inadequate credit ratings … [is] diverting Africa's resources away from financing development,' the draft says. The background to this is that China has provided huge loans to African countries for the purpose of securing resources, resulting in many of these countries falling into debt and facing the risk of default. The draft also says, 'Increasing trade protectionism is limiting Africa's access to global markets.' This appears to be in response to tariff measures taken by the administration of U.S. President Donald Trump. The draft says, 'We aim to accelerate efforts to … integrate African countries into global supply chains through a free, open, and fair trade and investment environment.' In the field of peace and stability, the draft emphasizes that 'democracy and the rule of law are foundational pillars.' It also states the view that strategic cooperation between Japan and the African Union, which conducts peacekeeping activities in Somalia, should be strengthened to improve security in Africa. In the field of society, the draft mentions plans to promote the manufacturing of medicines and vaccines in Africa. It also proposes the use of Japanese satellite data for drought and flood countermeasures.

Extra tariffs that have already been collected since the new rate came into effect at 1:01 p.m. on Thursday will be reimbursed. Akazawa apologized for what he called a 'regrettable error.' The two countries reportedly reached a consensus on July 22, with a 15% baseline import tariff rate imposed on Japan. However, the verbal accord was never put into a signed document. This caused confusion in Tokyo, despite Akazawa insisting there were 'no discrepancies' in their understanding of the agreement. Yet, according to the Trump administration, the additional 15% tariff was to be imposed on all Japanese imports, whereas the Japanese government insisted it applied only to certain products. Hiroshima Mayor Warns of the Growing Acceptance of Nuclear Weapons on 80th Anniversary of Atomic Bombing Speaking at a ceremony to mark the 80th anniversary of the atomic bombing of Hiroshima on Wednesday, the city's mayor, Kazumi Matsui, warned of the growing acceptance of nuclear weapons. Referring to the conflicts in Ukraine and the Middle East, he said , 'These developments flagrantly disregard the lessons the international community should have learned from the tragedies of history. They threaten to topple the peace-building frameworks so many have worked so hard to construct.' Matsui also urged the Japanese government to ratify the Treaty on the Prohibition of Nuclear Weapons. It has been signed by over 70 countries. Japan has, thus far, refused to do so because it 'supports the retention and potential use of US nuclear weapons on its behalf.' Earlier this year, the government refused to attend an anti-nuclear meeting at the United Nations as an observer state. Terumi Tanaka, co-chair of the Nobel laureate organization, Nihon Hidankyo, called the decision 'pathetic and disappointing.' Fire Breaks Out at Minatomirai Fireworks Festival A fireworks launch system is believed to have gone out of control in Yokohama's Minatomirai district on Monday, causing two out of eight barges carrying the explosive devices to go up in flames. Five crew members on one of the flat-bottomed boats subsequently jumped into the water to escape the blaze. Fortunately, they were all rescued, though one of them was hospitalized with minor injuries. There were no people on the other barge that caught fire. The blaze broke out at around 7:50 p.m. Event organizers attempted to halt the launch system, but the fireworks continued to go off. According to the Japan Coast Guard, the fire was extinguished around 15 hours after it started. The organizing committee of the festival held a press conference on Wednesday. 'We offer our sincerest apologies for causing concern and trouble to local residents and related parties,' said committee chair, Hiroyuki Sudo. Japanese Population Shrinks by 900,000 as Foreign Residents See Record Rise According to figures released by the Internal Affairs Ministry on Wednesday, the population of Japanese nationals fell to just over 120.65 million in 2024. That's around 908,000 fewer than the previous year, a record drop. At 0.75%, it's also the largest rate of decline since record-keeping began 57 years ago. A total of 46 prefectures saw a decrease in numbers. The only place that didn't was Tokyo, which saw an increase of 0.13%. The population of Japanese residents peaked at 27 million in 2009. Since then, it has been declining year-on-year and is forecast to fall below 120 million in 2026. The total population for 2024 was a little over 124.33 million, down 0.44% (554,485) from a year earlier. As for the number of foreigners living in Japan, the number rose by more than 350,000 to 3.77 million. It's the largest increase since the ministry started keeping records in 2013. Pretty Chuu Idol Alleges She Was Sexually Assaulted by Producer's Boyfriend A member of the recently formed idol group Pretty Chuu allegedly filed a police report after claiming she was sexually assaulted and secretly filmed by Taichi Saito, the boyfriend of the group's producer, Yurinya. The news was first leaked on X last Friday by a YouTuber who goes by the name of Korekore. He posted that 19-year-old Shuna Amamiya alleges she was 'secretly undressed and filmed by Taichi' and was impregnated after being 'assaulted while sleeping.' Yurinya, an influencer with more than 430,00 followers on Instagram, responded to the allegations with a statement on Friday, denouncing Saito's actions. Pretty Chuu uploaded an apology letter on its official X page the following day. 'Due to the inappropriate behavior of Taichi Saito, one of the group's managers, we have decided to remove him from Pretty Chuu's management team,' it read. 'Shuna Amamiya has requested to leave Pretty Chuu, and we have accepted this request.' Miyu Yamashita Wins Women's Open for First Major Title Miyu Yamashita turned 24 on Saturday, but the real celebration began a day later as she became the third Japanese golfer to win the Women's Open, following in the footsteps of Ayako Okamoto and Hinako Shibuno. Holding a three-shot lead through the midway point of the tournament, Yamashita saw her advantage narrowed to a single shot going into the final day of action at Royal Porthcawl in the UK. A two-under round of 70, though, was enough to seal the victory. Approaching the final hole, Yamashita knew a bogey would win her the tournament. Despite hitting the rough with her first two shots, she held her nerve with a nice chip to the green. She then tapped in for par to finish on 11-under, two shots ahead of Great Britain's Charley Hull and Japan's Minami Katsu. It secured Yamashita her first major trophy. She's the fourth Japanese golfer to win an LPGA major title over the last two seasons.
