
New Data on Postoperative Delirium a ‘Call to Action'
An estimated one in 30 patients in the study experienced postoperative delirium, with high levels of variation in incidence rates among hospitals.
Researchers said the findings are a 'call to action' to health providers to proactively manage the risks of postoperative delirium.
'This study is not just about identifying a problem, but it's also about giving people a sense of the scope of the problem. When you do that, it kind of energizes people to say okay, let's go out and see if we can do something about this,' principal investigator Laurent Glance, MD, professor and vice-chair for research in the Department of Anesthesiology at the University of Rochester School of Medicine (URMC), Rochester, New York, told Medscape Medical News .
The study is the largest to date to examine the clinical implications of postoperative delirium on a national scale and was published online on July 8 in JAMA Network Open .
Acute Brain Failure
Postoperative delirium was once thought to be a minor and temporary complication. However, that has changed over the past decade.
The American Geriatrics Society, which describes postoperative delirium as 'acute brain failure,' recommends the condition be treated as a medical emergency that requires rapid diagnosis and treatment. The cost of managing the condition runs roughly $26 billion to $42 billion in the US each year, and recent studies have linked postoperative delirium to the development of dementia in older adults.
For this study, investigators used Medicare claims data from 2017 to 2020 of more than 5.5 million adults over the age of 65 years (mean age, 74.7; 57.2% women). Postoperative delirium was identified based on ICD-10 codes.
Postoperative delirium was identified in 3.6% of cases. Patients with delirium were more likely to be male and have comorbidities, including cerebrovascular disease, heart failure, and dementia.
Compared to patients without delirium, those with the condition were significantly more likely to experience death or major complications (adjusted odds ratio [aOR], 3.47), non-home discharges (aOR, 3.96) and 30-day mortality (aOR, 2.77; P < .001 for all).
Findings were similar even after adjusting for hospital characteristics and excluded the patients admitted in 2020 during the height of the COVID-19 pandemic.
'Opportunity for Improvement'
Investigators also found a nearly threefold variation in the rates of postoperative delirium among the 3169 hospitals included in the study.
'When you see that level of variation, what it tells you is that there is an opportunity for improvement,' Glance said.
In fact, researchers noted that prior work found that postoperative delirium is preventable in up to 40% of cases.
'[Postoperative delirium] is not something that's guaranteed because you're older or because you have multiple medical problems,' lead author Heather Lander, MD and assistant professor in the Department of Anesthesiology and Perioperative Medicine at URMC, Rochester, New York, told Medscape Medical News .
'This is something that we as a healthcare system can identify early,' she added.
Variability among institutions could also be due to differences in coding accuracy and delirium screening, instead of reflecting true differences in hospital performance, the investigators wrote.
Delirium Often Underreported
The incidence of postoperative delirium reported in the study is markedly lower than previously reported rates of roughly 20% for noncardiac surgery patients. The researchers noted that incidence of postoperative delirium is known to be underreported in Medicare claims data.
Postoperative delirium can be miscoded as other conditions for financial incentives or go undetected by clinicians, particularly for hypoactive delirium where patients may not show typical symptoms of restlessness or disorientation.
Commenting on the finding for Medscape Medical News , Donna Fick, PhD, distinguished professor at Pennsylvania State University College of Nursing, University Park, Pennsylvania, noted that the miscoding of postoperative delirium might underrepresent affected patients in the study.
'When you use ICD-10 codes, claims or administrative data to examine this problem, you're dependent on clinicians coding the delirium, and we know that that's grossly under representative,' said Fick, who was not part of the study.
Given the poor health outcomes identified and extrapolated onto a likely higher number of patients affected, this is a 'really important study' to represent delirium impacts on a national scale, Fick added.
The authors also did a good job of controlling for important variables, including economic and social determinants of health as well as frailty and comorbidity, she said.
Reducing Delirium Rates
Investigators also found that patients undergoing surgery in hospitals with a higher rate of postoperative delirium had 1.5-fold higher odds of experiencing it compared to hospitals with lower rates.
However, 'the inference from that finding has to be very carefully interpreted,' Esther Oh, MD, PhD, professor and associate director of the Johns Hopkins Memory and Alzheimer's Treatment Center, Baltimore, told Medscape Medical News.
Some hospitals that are referral-based will take sicker patients compared to smaller hospitals, and those that have a delirium screening policy will find more delirium as a direct result, Oh noted.
Both Oh and Fick supported several low-cost, nonpharmacologic interventions to reduce delirium risk that Glance and colleagues recommended in the study, including avoiding sleep disturbance, sensory support and enhanced screening for at-risk patients.
'It would be our hope eventually that postoperative delirium will be one of the quality metrics that will be used by CMS to evaluate hospital quality,' Glance said.
In doing so, public reporting will enhance transparency, awareness and accountability of this issue and hospitals might up their efforts to reduce the rates of postoperative delirium, he added.

Crew-10 astronauts to depart ISS: How they set stage for ‘stuck' Starliner crew to leave
In mid-March, four spacefarers arrived at the International Space Station on a mission that at any other time would have been relatively routine and unremarkable. NASA astronauts Nichole Ayers and Anne McClain were joined by Japanese astronaut Takuya Onishi and Russian cosmonaut Kirill Peskov on a mission known as Crew-10 that took on far more significance than most of the regular ventures jointly carried out by SpaceX and NASA. As expected, awaiting the Crew-10 contingent at the orbital outpost were months of scientific experiments tailored to be conducted in microgravity. Crucially, though, the mission also attracted plenty of headlines and fanfare as it cemented its place in spaceflight history for its role in ending the infamous Starliner saga. The Crew-9 team may have arrived in September on a spacecraft with room for the two astronauts who crewed the doomed Boeing Starliner to hitch a ride home, but it was the arrival of the Crew-10 astronauts at the space station that set the stage for Butch Wilmore and Suni Williams to make their long-awaited homecoming. Now that the Crew-10 astronaut are soon due to depart the space station more than four months later, here's everything to know about the mission and why it made headlines. Remembering the Boeing Starliner: Look back at mission's biggest moments What was the Crew-10 mission? Astronauts relieve 'stuck' Starliner crew The March 15 arrival of Crew-10 astronauts at the space station made it possible for astronauts Wilmore and Williams, who arrived in June on the doomed Starliner, to finally depart. The mission got off the ground a day earlier from NASA's Kennedy Space Center. The U.S. space agency had been working toward a February liftoff before announcing in December 2024 that the mission had been pushed to late March to give SpaceX more time to prepare a new Dragon capsule. The launch date was then moved up to mid-March – most likely because of pressure from President Donald Trump and SpaceX founder Elon Musk – when NASA decided to instead use a previously flown Dragon. The Dragon docked at the orbital outpost after a 28-hour journey, allowing the crew to exit the vehicle and enter the space station through its Harmony module. Once aboard, the four Crew-10 spacefarers officially greeted the Expedition 72 crew members, including the astronauts who flew aboard the Starliner. What happened with the Boeing Starliner? Selected for the inaugural crewed flight of the Boeing Starliner, Wilmore and Williams became fixtures of the news cycle when the vehicle they flew to the space station in June 2024 encountered a series of technical failures. NASA and Boeing ultimately decided that the troubled Starliner capsule wasn't safe enough to crew and would instead undock and return to Earth without them. On Sept. 28, 2024, NASA launched the SpaceX Crew-9 mission as planned, but with one crucial change: Just two astronauts – Nick Hague of NASA and Russian cosmonaut Aleksandr Gorbunov – headed to the space station on a Dragon, leaving two empty seats on their vehicle reserved for Wilmore and Williams. NASA opted to keep Williams and Wilmore at the station a few extra months rather than launch an emergency mission to return them to Earth and leave the station understaffed. Williams and Wilmore eventually departed the space station with the Crew-9 team and safely landed March 17 off the Florida coast after the arrival of the Crew-10 mission. When will SpaceX launch Crew-11 astronauts on NASA mission? Now, the astronauts of the Crew-10 mission are set to return to Earth themselves after their own replacements arrive. The Crew-11 mission is scheduled to launch no earlier than 12:09 p.m. ET Thursday, July 31, from near Cape Canaveral, Florida, according to NASA. As the name suggests, Crew-11 is NASA and SpaceX's 11th science expedition to the International Space Station. The missions, most of which last about six months, are contracted under NASA's commercial crew program. McClain, Ayers, Onishi and Peskov will depart a few days later on the same Dragon capsule that flew them to the space station. Robotic crab claw ‘Wavy Dave' challenges real crabs to fight — then loses badly
Like many people around the world, animal behavior researcher Joe Wilde took up a new hobby during the COVID-19 pandemic. Wilde — a scientist with the Centre for Research in Animal Behavior (CRAB) at the University of Exeter at the time — spent his time learning about 3D printing. The researcher was looking at freely available 3D print scans of fiddler crabs when he got an idea. What if he could print a crab's claw and put it in front of real crabs to see how they reacted? Wilde considered it a 'pipedream,' according to an Aug. 5 news release from the University of Exeter, but soon, that dream became a reality. Fiddler crabs, or Afruca tangeri, is a species found in mudflats and sandbanks, often in underground burrows, according to a study published Aug. 6 in the peer-reviewed journal Proceedings of the Royal Society B. The crabs defend their burrows during mating season, researchers said, and males of the species will wave their large claws to attract mates. Previous research shows female fiddler crabs prefer males with larger claws and faster waves, so Wilde and his team wondered what would happen if those real crabs were forced to compete with an artificial replacement that was not only bigger but also faster, according to the study. Between May and July 2022, Wilde and his team created 'Wavy Dave,' an artificial fiddler crab with a robotic claw that could wave back and forth. It was placed in the Ria Formosa Natural Park in the crab's natural habitat of southeastern Portugal along the mudflats and salt marshes. 'We know many animals adjust their sexual displays if rivals are nearby, but less is known about how they react to the actual displays themselves,' Wilde said in the release. 'If you own a shop and your rivals start selling things really cheaply, you might have to change how you run your business.' Researchers said the female crabs caught on quickly, recognizing that Wavy Dave wasn't the same as the other male crabs from which they were used to getting attention. 'The females realized he was a bit odd,' Wilde said. The males, however, didn't quite notice their efforts were in vain. The study found males were more likely to spend more time out of their burrow when Wavy Dave was in action, according to the study, though they didn't appear to change the pace at which they waved. Some males were also less likely to engage with Wavy Dave if their claw was smaller than the robots, possibly because they believed it was a fight they couldn't win, according to the release. Other male crabs, on the other hand, were braver. 'One male broke Wavy Dave by pulling off his claw,' Wilde said. 'We had to abandon that trial and reboot the robot.' The research team suggests some of the males may have been 'eavesdropping' on Wavy Dave when they came out of their burrow but chose not to speed up their waves, according to the study. Seeing Wavy Dave would suggest a female was in the area, but until they saw the female themselves, they saved their waving energy. 'Our findings reveal the subtle ways in which these crabs adjust their behavior to compete in a dynamic environment, investing more in signalling when it is likely to be most profitable,' Wilde said. Ria Formosa Natural Park is on the southern coast of Portugal, along the Atlantic Ocean. The research team includes Wilde, Safi K. Darden, Jordan D. A. Hart, Michael N. Weiss, Samuel Ellis and Tim W. Fawcett.
