A starch- and sucrose-reduced diet ranks the highest for decreasing global irritable bowel syndrome (IBS) symptoms, whereas a low fermentable oligosaccharides, disaccharides, monosaccharides, and polyols (FODMAP) diet offers benefits in reducing global IBS symptoms, abdominal pain, and abdominal bloating.
METHODOLOGY:
Patients with IBS often report food-related symptoms and seek dietary interventions to help manage their condition.
Researchers conducted a systematic review and network meta-analysis of randomized controlled trials to evaluate the efficacy of dietary interventions for IBS symptoms.
Eligible trials compared an active dietary intervention involving multiple food changes with either a control intervention (such as a habitual diet, sham diet, high FODMAP diet, or other dietary advice) or another active intervention also requiring multiple food changes
Outcomes included the proportion of patients with no improvement in global IBS symptoms, abdominal pain severity, bloating/distension severity, or bowel habits.

Among dietary interventions superior to habitual diet, a starch- and sucrose-reduced diet ranked first in reducing global IBS symptoms in two trials (relative risk for no improvement [RR], 0.41; P -score = .84), and a low FODMAP diet ranked fourth across 24 trials (RR, 0.51; P -score = .71).

Given the variability in IBS pathophysiology that can exist across patients, different diets could provide different benefits, and offering multiple evidence-based options might prove valuable,' an expert wrote in an accompanying comment.

LIMITATIONS:
Most trials had relatively short durations, limiting their ability to measure efficacy for a chronic condition like IBS. Only two trials examined all three stages of the FODMAP diet (ie, restriction, reintroduction, and personalization). Although IBS is primarily managed in primary care, only four trials were conducted partially in this setting.

Below, we break down six toilet-related red flags that experts say you should never ignore. And no, it's not just about your bowel movements; your wee, urgency levels, and even smells all matter, too. Whether you're suddenly constipated, dealing with constant diarrhoea, or noticing your stool looks different (narrower, paler or greasy), changes that last longer than a few weeks are worth investigating. The NHS advises that ongoing shifts in bowel habits could be linked to anything from diet and stress to Irritable Bowel Syndrome (IBS), coeliac disease or inflammatory bowel disease (IBD). In some cases, persistent symptoms could be a sign of bowel cancer, so don't brush them off. When to see your GP: If your usual routine has changed for more than two to three weeks, especially if it's paired with stomach pain, fatigue, or weight loss. It might be bright red or dark and tarry; either way, it's not something to ignore. Bright red blood can come from piles or small tears, but darker blood might mean there's bleeding higher up in the digestive system. When to see your GP: If you see blood in your poo. You may be offered a stool test, or in some cases, a referral for further checks. If anything feels amiss and suddenly starts to sting, burn or feel uncomfortable, you might assume it's a urinary tract infection (UTI). And often, that's true. But it can also be a sign of kidney stones or bladder issues. Blood in your urine should also be taken seriously, even if it only happens once. When to see your GP: If you have pain while urinating, see blood or feel the urge to go far more often than usual. Needing the loo more often than usual (especially at night), feeling like you can't wait, or leaking a little when you cough, sneeze or laugh could signal an overactive bladder, prostate issues (in men), or pelvic floor dysfunction. These symptoms might feel embarrassing, but they're generally common and manageable with the right support. When to see your GP: If bladder leaks or urgency interfere with your daily life, or if you notice a sudden change in how often you need to go. A little mucus in your stool isn't always a worry, but frequent slimy stools can be a sign of infection or inflammation in your gut. When to see your GP: If mucus appears regularly, especially if it comes with bloating, pain or a change in bowel movements. If going to the toilet has become uncomfortable, painful, or feels like hard work, that's your body waving a red flag. Regular straining can cause or worsen hemorrhoids, but it might also signal bowel issues or even neurological problems affecting your pelvic floor. When to see your GP: If you're straining often, feel like you're not fully emptying your bowels, or notice pain during or after a bowel movement. If you're unsure, remember that everyone has their own baseline of what's 'normal'. However, there are a few simple signs to watch for as a guide. According to the Bladder and Bowel Health Service, healthy bowel movements should be soft, smooth and easy to pass; ideally at least three times a week. You should also be able to urinate without pain or discomfort. If you also notice you're often bloated, tired, straining, or notice blood or mucus, it's time to speak to your GP. Changes might be harmless, but they're always worth checking. This program is called Healthy Living for your Brain and Body, and it will be on June 27, from 10am-11:30am at the Onslow County Public Library in Swansboro. At this event, you will get to learn how certain modifiable risk factors and health behaviors can help reduce the risk of cognitive decline. There will also be a cooking demonstration and tasting from the Onslow NC Cooperative Extension program MED instead of MEDs, which was designed to address healthy eating and educate attendees on how eating a Mediterranean diet can help protect them from chronic illnesses and potentially decrease the need for medications. 'The brain changes that cause Alzheimer's are thought to begin 20 years or more before symptoms start, which suggests that there may be a substantial window of time in which to intervene in the progression of the disease,' Executive Director, Alzheimer's Association – Eastern North Carolina Chapter, Christine John-Fuller, said. No disciplinary sanction for doctor's ‘grave' failures in care of Martha Mills

A senior doctor will face no disciplinary penalty for failings in his care of a teenager who died days later from sepsis. Professor Richard Thompson did not refer Martha Mills, 13, to intensive care despite her displaying several high-risk indicators of sepsis. The on-call consultant also chose not to return to London's King's College Hospital to assess her in person as her condition deteriorated. A Medical Practitioners Tribunal Service (MPTS) panel sitting in Manchester had ruled those omissions were misconduct, which they described as 'particularly grave', and found his fitness to practise was impaired. However, on Wednesday the tribunal decided there were 'exceptional circumstances' which justified taking no further action against the world-renowned paediatric liver specialist. Martha had been an inpatient on the hospital's Rays of Sunshine Ward after she suffered a serious injury to her pancreas when she slipped while riding a bike on a family holiday in Wales in July 2021. Weeks later she experienced a fever and increased heart rate, followed by more spikes in her temperature before the consultant hepatologist saw Martha on his morning ward round on Sunday August 29. Prof Thompson left the hospital at 3pm, but was phoned at home two hours later by a trainee doctor, who gave an update on Martha's condition. Medical records showed she had deteriorated over the course of the afternoon, and into the early evening, with a drop in her blood pressure, the appearance of a new rash and increases in heart rate, respiratory rate and body temperature. Tribunal chairman Robin Ince noted that by 5pm there were 'several high-risk indicators' as set out in the Nice guidelines relating to sepsis. The duty registrar called Prof Thompson again at 8.30pm because of ongoing concerns over Martha's fever, but she was kept on the ward despite the continued presence of moderate to high-risk indicators and the absence of meaningful clinical improvement. Martha collapsed on August 30 and was moved to intensive care before she was transferred to London's Great Ormond Street Hospital, where she died in the early hours of August 31. Announcing its conclusions on Wednesday, Mr Ince said: 'Professor Thompson has done everything possible to address his failings. 'The tribunal considered that the best way to repair any harm caused by his failings would be for him to continue to provide his specialist expertise at home and abroad. 'To now – some four years after the index event – remove Professor Thompson from practice, even for a short period of time, for one single lapse of judgment in an otherwise exemplary career would, in the tribunal's view, be akin to punishment which is not the role of the MPTS.' Among the 'exceptional circumstances' cited were that there was no allegation or evidence that Prof Thompson either caused or contributed to Martha's death. There were also systemic failings regarding how the ward functioned at the time with regard to referrals to the paediatric intensive care unit, said the tribunal. Mr Ince said: 'A sufficiently clear message has already been sent to the profession and to the public – that even such an experienced doctor as Professor Thompson could still make serious errors of clinical judgment for which he will be called to account. 'The public would be aware that this finding would remain a stain on Professor Thompson's reputation for the rest of his life.' At a 2022 inquest into her death, a coroner ruled that Martha would most likely have survived if doctors had identified the warning signs and transferred her to intensive care earlier. Martha's mother, Merope Mills, an editor at The Guardian, said she and her husband, Paul Laity, raised concerns about Martha's deteriorating health a number of times but these were not acted on. The couple later successfully campaigned for Martha's Rule to give patients, families and carers the chance to easily request a second opinion from a senior doctor in the same hospital in the event of a suspected deterioration or serious concern. Giving evidence, Prof Thompson told the MPTS hearing that he no longer provided in-patient care because he began to 'doubt my own judgment' after the tragic events. He said he felt 'deep remorse' for Martha's death but did not believe he made any errors in her case, as he denied all the allegations brought by the General Medical Council (GMC). The tribunal heard he had since completed a training course relating to the management of sepsis and a deteriorating child in paediatric care. His barrister, Ben Rich, said Prof Thompson has been a dedicated doctor and specialist for nearly 40 years and had never previously been investigated by a regulator. He said he had a reputation as a 'hard-working and outstanding clinician and researcher, who has an international reputation as one of the leading paediatric liver specialists in the world'. Mr Rich urged the tribunal members to impose an order of conditions involving supervision on Prof Thompson's registration, but the panel disagreed and said such a measure would be 'unnecessary and artificial', as they opted to take no further action. Christopher Rose, for the GMC, said that Prof Thompson should be suspended to send a message to the wider public and the wider profession, given the seriousness of the failings found.
