Alarming trends show that colon – or bowel – cancer is increasing in younger people. If the rise continues, colorectal cancer is projected to become the leading cause of cancer-related deaths among young adults globally by 2030.
Until recently, the reasons for this surge were largely unclear or unknown. Now research points to a surprising suspect: gut bacteria.
A recent study reveals that exposure during childhood or adolescence to a toxin produced by certain strains of E coli, whose growth is encouraged by highly processed diets, may lay the groundwork for aggressive bowel cancers decades later. This discovery could help explain why people under 50 are at the heart of one of the fastest-growing cancer epidemics of our time.
Colon cancer is currently the second biggest cause of cancer death, yet only one in three cases are diagnosed in the earliest stages. Often symptomless in its early forms, colon cancer typically begins as polyps and can take ten to 15 years to develop. This slow progression makes regular screening crucial, especially because many patients experience no early warning signs.
For the new research, an international team analysed the complete DNA sequences of 981 colorectal cancer tumours from patients across 11 countries. They discovered striking geographic patterns in the mutations that lead to cancer.
Two specific mutational signatures – SBS88 and ID18 – stood out for their association with colibactin, a DNA-damaging toxin produced by some E coli strains. These bacterial 'fingerprints' were 3.3 times more common in patients diagnosed before age 40 than in those over 70. Significantly, these mutations appear early in tumour development, suggesting the damage may occur years – even decades – before cancer is diagnosed.

Colibactin doesn't cause random DNA damage. The study found it tends to target the APC gene, a vital tumour suppressor that normally controls cell growth.
In colibactin-positive cancers, about 25% of APC mutations bore the toxin's unique signature. This direct hit to the body's internal 'brake system' could explain why these cancers appear earlier in life.
Molecular analysis indicated that colibactin-associated mutations often emerge within the first ten years of life. While this suggests the toxin may silently colonise children's guts and initiate cancerous changes early, it's important to note that this remains a theory; the study didn't directly examine children or young adults.
Still, the research maps out a microbial pattern of cancer risk. These gut bacteria are not the same as those that cause food poisoning – they often live within us and perform beneficial roles.
But their composition can vary widely by region. Countries including Argentina, Brazil, and Russia – where colorectal cancer rates are climbing – showed higher levels of colibactin-related mutations.
This may reflect regional differences in gut microbiomes influenced by diet (particularly ultraprocessed foods), antibiotic use and environmental factors. In contrast, Japan and South Korea, where rates are historically high but stable, showed different mutational patterns, suggesting other causes may dominate there.
Perhaps the most provocative finding relates to when this bacterial damage occurs. Unlike lifestyle risks that build up over decades, colibactin seems to strike during a narrow window – when the microbiome is still forming in childhood or early adulthood.
Potential triggers could include repeated antibiotic use that disrupts healthy gut bacteria, highly processed diets that favour E. coli growth and urban living that reduces exposure to diverse microbial environments.
These findings may also point to new prevention strategies. Screening programs could focus on younger adults carrying these high-risk bacterial strains, using stool tests to detect colibactin genes.
Diets high in fibre and low in processed foods might promote a healthier gut microbiome, potentially suppressing harmful bacteria. The research also adds weight to calls for lowering colorectal cancer screening ages worldwide, since many early-onset cases go undetected under current guidelines.
While this study is a major step forward, many questions remain. Why do some people carry colibactin-producing bacteria but never develop cancer? How do modern lifestyle factors amplify – or mitigate – these microbial risks? What we do know is that cancer results from the complex interplay between our genes and our environment – including the microscopic world within us.
As researchers continue to connect the dots, one thing is clear: the colorectal cancer epidemic of the 21st century may have begun with silent microbial battles in our guts, decades before diagnosis. This emerging view of cancer not just as a genetic or lifestyle disease, but also as a microbial one – could fundamentally reshape how we think about prevention for future generations.

Dangerously high levels of a bacteria that can cause vomiting, diarrhea and in rare cases, death, were detected in a public park near the University of Cincinnati. Researchers discovered E. coli in both dry and wet soil samples from Burnet Woods, a 90-acre public park that extends into Corryville, CUF and Clifton. E. coli levels ranged between 18 to over 600 times the U.S. Environment Protection Agency's limit for allowing swimming in the Ohio River, said Bob Hyland, an associate professor-educator in the School of Environment and Sustainability at UC, who oversaw the student-led research. On June 8, Cincy VegFest, a plant-based food festival, was hosted in Burnet Woods not far from where the E. coli was found. And three days later, on June 11, Cincinnati Parks staff cordoned off the area the researchers sampled, just a few dozen yards from parents dropping their kids off at summer camp and college students going on jogs. "Based upon what we've found over the last week or two," said Hyland, "we're concerned that this could be a public health risk that has not been taken seriously enough.' Sewer pipes that run under the wooded area of the park have led to periodic overflows during heavy rains, bringing bacteria such as E. coli, which is normally found in human feces, to the surface. But the fact that his team was able to find E. coli in the absence of a sewer overflow is especially worrying, said Hyland, who said he wants to know the reason why there's so much E. coli in the park. In a joint emailed statement issued with Cincinnati Parks, Deb Leonard, the spokesperson for the Metropolitan Sewer District of Greater Cincinnati, said the "limited" sampling "confirms" the presence of E. coli in the park. The city has been in contact with the University of Cincinnati "to better understand the situation," said Leonard, and test the area. "The city manager's office, MSD, parks, and health will take immediate steps to conduct additional tests and investigate the source of potential contamination within the surrounding park," she said. While the soil and standing water samples were compared to the EPA's limits for recreational water, which would not apply to the forested area, Leonard said, "the city encourages the public to continue to exercise caution when exploring the area and follow directions on park signage." E. coli is a group of bacteria that usually lives in the gut of healthy people and animals. Normally, the kind in your gut doesn't harm you, but when ingested, some strains of the bacteria can make you sick with a gastrointestinal illness, pneumonia or a urinary tract infection. The U.S. EPA considers E. coli an indicator organism, or a way for officials monitoring water quality to identify fecal contamination of freshwater and the presence of disease-causing bacteria and viruses. Symptoms of an E. coli infection include abdominal pain, dehydration, watery diarrhea, vomiting and a fever. And though they're rare, certain strains of E. coli can be life-threatening, according to Dr. Andrew Beck, a pediatrician at Cincinnati Children's, due to the toxins they release – usually identifiable by bloody diarrhea. Common ways of contracting E. coli include eating infected food, such as undercooked ground beef or unpasteurized milk, or from contact with another person, known as the "oral-fecal" route, where the bacteria travels from the feces of one person to the mouth of another, usually due to someone failing to wash their hands properly. Beck's research shows that children may be contracting gastrointestinal illnesses from sewer overflows. In 2017, he co-authored a study that found that sewer overflows in Cincinnati were associated with a 16% increase in Cincinnati Children's emergency department visits for gastrointestinal illnesses among children who lived within 500 meters of the sewer. Researchers' primary hypothesis for how the children contracted the GI illnesses was that they were "playing in contaminated water" and dirtying their hands, said Beck. Student researchers first found E. coli in Burnet Woods in the fall of 2024, according to Hyland. Similarly to the preliminary research released in June, they found elevated levels of E. coli despite there being no overflow of the sewer pipes. "We were perplexed," said Hyland. "What we now are feeling more confident about, but want to do more research to build our confidence is that the pipe is probably compromised." Hyland believes the pipes are leaking sewage to the surface of the park because of their aging infrastructure and the presence of sinkholes in Burnet Woods, which can be a sign that piping is damaged. The Enquirer confirmed that there are at least two sinkholes in the area near where the E. coli was found. Leonard confirmed that some pipe segments running underneath Burnet Woods were installed in the late 1800s, while newer segments were installed in the 1990s. "All the segments are inspected regularly and are in good condition with no structural issues," she said. Government records and neighborhood meeting notes indicate that residents living near Burnet Woods were affected by a sewage overflow more than once in the past year. Burnet Woods saw a sewage overflow once this year and twice in 2024 during heavy rains, according to Leonard. When that happens, she said that Cincinnati Parks, in collaboration with the Metropolitan Sewer District, installs signage notifying park visitors of the potential contamination. The meeting minutes for the Corryville Community Council, a membership organization for residents of the neighborhood, mention sewer overflow or the smell of sewage at least four times between January 2024 and now. The most recent finding of E. coli in Burnet Woods follows a sewage leak at Winton Lake in May, which resulted in officials suspending recreational activities at the lake. Could Kids' Gut Bacteria Trigger Young Colorectal Cancer?

According to a study published in Nature by an international team led by the University of California, San Diego, childhood exposure to the bacterial toxin colibactin could drive early-onset colorectal cancer (CRC). The first author is Marcos Díaz-Gay, PhD, head of the new Digital Genomics Group at the Spanish National Cancer Research Center (CNIO) in Madrid, Spain. CRC is considered a disease associated with aging; however, its incidence in adults younger than 50 years has approximately doubled each decade over the past 20 years in various countries. If current trends persist, it could become the leading cause of cancer-related deaths among young adults by 2030. Childhood Exposure Díaz-Gay told the Medscape Spanish edition that it remains unclear what factors over the past two decades have driven the rise in CRC among younger adults. 'Our study is one of the first to address this question, and it offers a partial answer. It supports the hypothesis that colibactin exposure contributes to this increase; however, the finding is correlative rather than causal. We need further evidence to prove causality,' said Díaz-Gay. He noted that although diet and lifestyle have changed over the past 20 years, 'but the changes occurred earlier, since these people are currently between 20 and 50 years old. With respect to colibactin, we are talking about exposures in the first 10 years of life,' he added. He emphasised that the study does not claim that colibactin is the cause of all cases in young patients, nor does it exclude its role in older adults. 'It is true that we see it more frequently in people under 50, but we also observe it in older people; in fact, 10% of those over 50 also have these mutations,' said Díaz-Gay. Mutational Patterns Colibactin is a toxin produced by certain Escherichia coli strains that damages the cellular DNA. The study showed that exposure to this toxin during early childhood imprints a characteristic genetic signature on the DNA of colon epithelial cells. 'The mutational pattern that colibactin imprints on the genome was already known,' Díaz-Gay explained. In 2020, Hans Clevers and Spanish collaborators conducted a study using organoids, in which a specific mutational profile induced by the toxin was observed. Michael R. Stratton's group subsequently identified the same profile in normal colon cells, noting its emergence and enrichment during childhood, when tumours are at their earliest developmental stages and are clearly enriched at that time. 'We confirmed that they are also present in early tumour stages,' said Díaz-Gay. The novel finding of this study was that the mutational patterns associated with colibactin were more frequent in patients younger than 50 years. 'We identified this internationally in nearly 1000 sequenced patients,' he said. Mutation Burden The investigators analysed 981 CRC genomes from 11 countries. The results showed that colibactin leaves specific DNA mutation patterns, which are true mutational signatures. These signatures are 3.3 times more frequent in adults diagnosed before the age of 40 years than in those diagnosed after the age of 70 years. Moreover, the overall colibactin signatures are especially prevalent in countries with high rates of early-onset CRC. The study found no significant differences in cancers with microsatellite instability but observed variations in the mutation burden and signatures in 802 microsatellite-stable cases. Multiple signatures, most of which have unknown aetiology, showed variable prevalence in Argentina, Brazil, Colombia, Russia, and Thailand, indicating geographically diverse levels of mutagenic exposure. The SBS88 and ID18 signatures, caused by the mutagen colibactin, showed higher mutation loads in countries with higher CRC incidence. SBS88 and ID18 were also enriched in early-onset CRC, being 3.3 times more common in those diagnosed before the age of 40 years and appearing in the earliest stages of tumour development. Colibactin exposure is linked to driver mutations in the tumour suppressor gene APC ; the mutational signature ID18 accounts for approximately 25% of APC insertions or deletions in colibactin-positive tumours. Conceptual Shift 'These mutation patterns are a kind of historical record in the genome,' said Ludmil Alexandrov, PhD, professor in the Shu Chien-Gene Lay Department of Bioengineering and the Department of Cellular and Molecular Medicine at University of California, San Diego, who is also a member of University of California, San Diego Moores Cancer Center and deputy director of Sanford Stem Cell Fitness and Space Medicine Center, San Diego, and lead author of the study, in a CNIO press release. 'If someone acquires one of these driver mutations at age 10,' Alexandrov explained, 'they could be decades ahead of the curve in developing colorectal cancer and develop it at age 40 instead of 60.' Alexandrov also pointed out the conceptual shift these results represent; many cancers can be traced to environmental or microbial exposures in the first years of life, long before diagnosis. 'This changes the way we think about cancer. It is not just about what happens in adulthood but also in the first decade of life, even in the first few years.' 'Until now, we had no satisfactory explanation for the increased incidence of CRC in young patients.' Various hypotheses have been proposed, but without conclusive data. 'The newly published study offers a solid and interesting hypothesis to explain this phenomenon and suggests a potential preventive strategy,' Andrés Muñoz, MD, a medical oncologist at Gregorio Marañón University Hospital in Madrid, Spain, told Medscape Spanish edition. On whether it would be appropriate to implement screening protocols to detect these cases in young people, Muñoz said: 'Before any screening or therapeutic protocol is introduced, these highly relevant findings must be validated and replicated in other studies and patient cohorts.' He added that genetic analyses to detect these mutations are currently only available at the research level, making their clinical application difficult. Díaz-Gay explained: 'We perform whole genome sequencing of tumour biopsies and blood cells as normal tissue to identify tumour mutations, but this is not applicable in clinical practice, where specific genes relevant to a given tumour are analysed.' He noted a desire to detect these mutations non-invasively via liquid biopsy or stool analysis rather than genetic testing but said that the effort is at an early stage. Geographic Variation The study found that SBS88 and ID18 were prevalent in cancers from specific countries — namely Argentina, Brazil, Colombia, Russia, and Thailand — suggesting that local environmental exposure may contribute to cancer, although the factors remain unknown. Spain was not included in this part of the study design, which focused on molecular differences among those countries, 'but ultimately the primary objective became recognising differences between age groups,' Díaz-Gay said. Although country-specific differences were found, the overall similarities in these tumours were considerable: 'There are specific genetic signatures, though not for colibactin, but in the mutational processes driving tumours. We have seen very concrete signatures in Argentina and Colombia; in these cases, though we could not determine the source, they could be related to microbial environmental exposures or other toxins.' On other little-known environmental carcinogens, Díaz-Gay said that none stand out for the colon at present but that mutation pattern analysis has yielded major discoveries in recent years: 'New environmental factors causing kidney cancer mutations have been identified — for example, aristolochic acid, a highly mutagenic carcinogen linked to traditional medicine plants in countries such as Taiwan,' he explained. He added that some high-risk cancer compounds are not associated with mutations, 'which is notable and the subject of much current research.' Future Research The authors outlined the key questions they would like to address in later phases of this research: How are children exposed to colibactin-producing bacteria, and how can that exposure be prevented? Do specific diets and lifestyles promote colibactin production? How can carriers of these mutations be identified? Trials are underway to assess whether probiotics can eliminate harmful strains and develop stool-based tests for the early detection of colibactin-associated mutations. Díaz-Gay has filed a European patent application with application number EP25305077.7. Conflicts of interest for the other authors are detailed in the original publication.
