Noah Doko rubs absent-mindedly at a scar slashed across his forearm. The former rebel fighter sits amid the crumbling remains of the former Panguna village in Bougainville, beneath the hills that were the hideout and battleground of his youth.
As a teenager, he was shot by the Papua New Guinea defence force: ostensibly by his own military, by a fellow citizen. In the aftermath of the Bougainville civil war, he was elected to parliament to represent veterans.
Now, he contemplates the reanimation of a mine he and his comrades started a war to shut down.
Abandoned three decades ago by owners Rio Tinto, the unremediated Panguna mine remains less a scar carved through the middle of Bougainville than an open wound that has never been allowed to heal.
Beneath the rusting, twisted hulks of the mine's gargantuan buildings, there are tiny signs of movement: dozens of families, including their children, in the grim, desperate work of digging by hand for flecks of precious metal.
But here, in the depths of this poisoned pit, lies the paradox of Panguna. The reviled foreign project that once tore this island apart, that buried villages under rubble and turned rivers a toxic blue, holds hope today for Bougainville's unity, prosperity and freedom.
'Panguna mine should be reopened,' Doko says, pointing towards the vast crater before him. 'The economy for running the independent nation of Bougainville is right there.'
He is unworried by the world's superpowers circling his tiny island. '[Our] independence should be backed up by the wealth, by the resources we have on Bougainville,' he says.
Doko might not live to see the day, but he says Bougainville's independence is irrepressible.
'No one can take it from us … I'm still standing up for it today, to make sure that when I go, I know that my children and my children's children, they will see independence. They will see prosperity and freedom.'
The newest nation on Earth has a due date.
'We are now at the final hurdle,' the president of Bougainville, former resistance fighter Ishmael Toroama, told the prime minister of Papua New Guinea earlier this year.
'My people have set 1st September 2027 as the date for independence to take effect.'
Toroama was a lionised Bougainville Revolutionary Army commander – famed for his heroics against the better-equipped PNG military. In the 21st century, few potential heads of state have fired a bow and arrow in combat.
Still less comfortable in a suit than fatigues, Toroama has spent the past two decades recasting himself as peacemaker and negotiator rather than fighter. Over five years as president he has travelled the world, quietly pressing what he calls his people's demand for independence.
Bougainville, a group of islands off Papua New Guinea to the north of Australia, has been variously controlled by Britain, Germany, Japan, Australia and PNG.
It declared independence in 1975 as the Republic of North Solomons, but was absorbed into the newly independent PNG weeks later.
Its close ethnic ties and proximity to the Solomons archipelago means it has always sat uneasily with Port Moresby's rule.
That discontent came to a violent head in 1988 after a long-running dispute over the lucrative Panguna copper and gold mine.
The mine, carved from a mountain in central Bougainville, was critically important to PNG, at one point generating more than 45% of the country's national export revenue.
Multinational Rio Tinto and other international investors earned nearly US$2bn from the mine over 17 years of operation. But less than 1% of the mine's profits went to Bougainvilleans, whose homes and lands had been destroyed by it.
Under the leadership of a former mine worker, Francis Ona, disaffected Bougainvilleans ran a sabotage campaign which forced the mine's closure in 1989.
The PNG Government responded by sending in police, then soldiers, to restore law and order. Bougainville was blockaded and the island descended into a brutal civil war that ran for a decade, and saw as many as 20,000 people die.
PNG later hired international mercenaries to end the conflict (the notorious Sandline affair), before a peace agreement was signed in 2001.
In a 2019 referendum, 97.7% of Bougainvilleans voted in favour of independence. Many argue that reopening Panguna mine will be key to the island's economic future as an independent nation.
In 2019, Bougainvilleans voted 97.7% in favour of political independence from PNG. But the overwhelming mandate Toroama carries for independence is freighted, too, with pressure to deliver it.
Toroama says the only way an independent Bougainville can be economically sustainable is to reopen the Panguna mine.
'We closed the mine because of oppression: today the mine is the key to our economic growth and settling all unresolved issues from the war,' he told the Guardian.
'For Bougainville, everything now is intertwined: you cannot separate politics from the economy.'
In the minds of the majority on Bougainville, independence and Panguna mine – specifically who owns it and who benefits from it – have been made inseparable by 'The Crisis' that seized the island a generation ago.
Opened in 1972, Rio Tinto's Panguna was once one of the world's most lucrative copper and goldmines, at one point accounting for 45% of all of PNG's exports. But less than 1% of its vast profits went to the Bougainville people, and Panguna's landowners say the mine left them only with political division, violence and environmental degradation.
In 1989, amid fomenting community anger at the environmental damage and the inequitable division of the mine's profits, landowners forced Panguna closed, blowing up its power lines and sabotaging operations.
To restart the foreign-owned mine, the PNG government sent in troops against its own citizens, sparking a decade-long civil war that led to the deaths of as many as 20,000 people – in conflict, but also from starvation and illness – many of them women and children forced to flee to the dense jungles of Bougainville's mountains.
A peace settlement was brokered in 2001, part of which granted the island a degree of autonomy from Port Moresby and which promised the independence referendum so resoundingly answered.
Today, independence is everywhere in Bougainville. The rhetoric of 'sovereignty' and 'freedom' features in everyday conversation. What is less clear is exactly what that independence looks like, and how it could work.
The principle of self-determination has been embedded over generations of having it denied, but the mechanics of nationhood are less clear.
There are pressing, immediate concerns for Bougainville's autonomous government: in simply keeping the lights on (blackouts are a daily occurrence), in healthcare (on some estimates, just 10 doctors serve an island of more than 300,000 people) and in basic infrastructure (the sole trans-island road is a dusty, rutted track regularly washed away by rain).
The fundamentals of running a country are not yet in place. Bougainville is slowly assuming political powers from Port Moresby: it has established a tax office, courts and other institutions of state. But economic roadmaps and draft constitutions remain that – maps and drafts – and Toroama's government yearns for broader international recognition and support.
Nation-building is slow work. Presidential elections later this year will be a test, less of candidates' commitment to independence than of Toroama's rapid, resolute timeline.
A senior government official, speaking on condition of anonymity, told the Guardian Bougainville's independence was 'inevitable' but that the 2027 timeline was potentially counter-productive.
'We are nowhere near [ready]. We don't want to give birth to a state on life support. We want a sovereign nation that is healthy, that is viable.'
PNG's parliament is seen by many on Bougainville as stalling progress towards independence, refusing to ratify the referendum result. The PNG prime minister, James Marape, says Bougainville is economically unready – it generates only 5% of its own budget, he told parliament – and still has too many guns at large to contemplate independence.
Toroama has conceded 'the recent surge of gun-related violence … cannot go unnoticed'. Some former fighters did choose to hide weapons rather than surrender them. But the island's streets are still far safer than those of Port Moresby.
Outsiders have long been Bougainville's problem.
From the Frenchman who named the island after himself to a succession of colonial masters, foreign domination has built a fiercely independent streak in Bougainville's politics and identity.
But outside interest in Bougainville - particularly its resources – is undiminished, despite its insurrectionist history. One simple driver is the sheer lucrative potential of the island.
There remains 5.3m metric tons of copper and more than 19m ounces of gold still unmined in Panguna, worth in excess of $80bn on current prices. Around 5% of the world's copper reserves, and 1% of its gold, are still buried in what remains of the mountain. And Panguna is not alone: core samples taken from adjacent mountain ranges have returned staggering concentrations of precious metals.
There's gold (and much else) in these hills – minerals vital to the 21st century economy. Potentially billions to be made.
'China and America are going head to head in this place,' a senior Bougainville government official says. 'There is competition for Bougainville because people can see the potential here. We have many resources that the world needs.'
There are fears a newly independent Bougainville will emerge as a fragile state, vulnerable to manipulation in a fiercely contested Pacific. Observers have raised concerns that amid America's withdrawal from global influence, China will seek to establish itself as the dominant development partner, with all of its attendant access.
Already, the state-owned China Railway Construction Group is extending the runway at the sleepy airport at Aropa, just outside Bougainville's largest city, Arawa. Construction – funded by the Asian Development Bank but branded as the Chinese company's project – quietly commenced this year.
Bougainville has known destructive foreign interference – Rio Tinto and its former subsidiary Bougainville Copper Limited are still despised for their exploitation and environmental vandalism – but there is no inherent belief here that Chinese investment will necessarily be damaging for the island or the region's geopolitics.
The overwhelming desire here is for development partners that will benefit the island itself rather than exploit its resources for enrichment offshore.
The deputy president and economic development minister, Patrick Nisira, said concerns over Chinese development were fuelled by 'western media sources which clouds independent and informed decision-making'. Since Panguna shut, he says, Bougainville's traditional allies had largely abandoned it.
'The dearth of options from traditional development partners over the last 20 years, a rapidly shortening timeframe for self-governance of Bougainville to 2027, and now Chinese companies' approaches for partnerships in these very areas, make such opportunities worth considering for the benefit of the people.'
In the US, the conservative Heritage Foundation – whose Project 2025 document has guided much of Donald Trump's agenda – is seized of Bougainville's geostrategic significance.
'The possibility of an independent Bougainville … provides the US an opportunity to be a proactive, rather than reactive, partner,' its briefing states.
'The US must take advantage of this opportunity, or risk ceding to Beijing a first-mover advantage in a vital corner of the Indo–Pacific, where the balance of power and influence could easily tip in China's favour.'
But the US's concern for Bougainville has been inconsistent.
Toroama has previously offered Bougainville as a site for a US base. In 2023, he visited America seeking the chance to press Bougainville's claim. But a promised meeting with a senior state department official simply never eventuated. After waiting in Washington DC for the phone to ring, he returned to Bougainville chastened, and wary.
The next year Toroama visited Guangzhou: Chinese companies have discussed with him reanimating Panguna, and he says, openly, he is prepared to play 'the Chinese card'.
But some in Bougainville are concerned that, in rushing to reopen the mine, the island risks repeating the mistakes of the past.
Theonila Matbob, a parliamentarian whose constituency covers Panguna, argues remediating the mine's environmental damage must take priority over restarting operations.
'It is the most important, fundamental step that needs to be taken before we talk about extraction. We need to understand how to heal our past first, and then be able to move on. It's going to take years.'
Matbob's home sits in the valley where the polluted Jaba River flows. Her father was killed in The Crisis, and she sees her children's generation suffer still from the mine's legacy.
Thousands of alluvial miners have set up crude camps in the abandoned pit of Panguna. The work is dangerous and dirty: tailing mountains collapse regularly; some miners use mercury to separate the tiny gold flakes from the silt; children work in the pit daily.
They would quit tomorrow, Matbob says, if they had somewhere else to go.
'It is not happening because the basics are not there: food is not there, water is not there, shelter is not there. Where can they easily go into the forest? They've got no forest left.
'So every single day they will have to bury themselves amid all these rocks and … the toxins from the soil … killing people. No one cares about that because it's survival that they're fighting for.'
Matbob has been a key figure in trying to bring Rio Tinto back to Panguna (the miner has not set foot on the island since 1989) to take responsibility for the mine's damage through an independent legacy impact assessment.
In December, that assessment reported the abandoned mine remains an acute threat to people's lives – from landslides, levee collapses and floods. The river delta will stay polluted for a century.
Rio Tinto has signed an MOU with the Bougainville government to discuss potential remedy of the Panguna site, but there's not yet a dollar figure committed. 'We recognise the gravity of the impacts identified and accept the findings,' Rio said in a statement.
Revolution casts a long shadow in Bougainville.
Decades since the end of the conflict, slain fighters are still being exhumed from mass graves and reburied in their villages.
17 May is Remembrance Day, when the island's war dead are honoured. Much of Bougainville's young population is too young to have lived through The Crisis, but its pain and privations, and its ideals, are kept alive by the valedictories that carry with them a note of caution.
No one on this island is preaching a return to violence.
Maria Tovivia watches over the bodies of 14 young men in the village of Sipotovai in the island's north. Thirty-one years after they were killed by members of the PNG defence force, their remains have been exhumed from a mass grave and are being reburied.
Hundreds follow the coffins, each draped in the bright blue flag of Bougainville, up the hill.
The politics of 21st century Bougainville draws a straight line from their deaths a generation ago to the island's nascent nationhood.
'The sacrifice by these brave men set the foundation for peace and unity on Bougainville and it set us on our current course to political independence,' president Toroama tells the congregation.
Tovivia believes independence is inevitable. But there will be no fighting. The people of Bougainville have known conflict: they want its opposite, she says.
'We want a peaceful way to independence. No more fighting. No more fighting.'

Former Texas Gov. Rick Perry has become an unexpected advocate for the illegal psychedelic drug ibogaine. The New York Times on Tuesday went on a trip with Perry, who twice ran for the White House and served as President Donald Trump 's Secretary of Energy during the Republican's first term. Perry recalled traveling to Tijuana, Mexico in September 2023 and spending 12 hours hallucinating that he was flying through space, seeing hieroglyphics and escaping the devil. 'I woke up very clearheaded, with this very warm feeling in my body,' Perry told the paper. 'I was as calm and as happy as I'd been in memory.' During his time serving as Texas governor, Perry knew that many veterans from the Iraq and Afghanistan wars were in trouble, but he first heard about ibogaine through a Navy SEAL he had hired in 2017 during his time running the Energy Department. The SEAL, Morgan Luttrell, told Perry he was taking time off to travel to Mexico to be treated with ibogaine after continuing to suffer from psychiatric issues post-deployment. Perry, a staunch conservative, told Luttrell - now a Republican U.S. congressman from Texas - to be careful. Cut to 2025 and Perry is convinced of its efficacy. 'It has literally given people their lives back,' he told The Times. In Perry's own case, after suffering from three severe concussions during his youth - and struggling with anxiety during his political career - the former Texas governor took an MRI before and after his ibogaine experience. Beforehand, his 73-year-old brain showed evidence of atrophy, not uncommon for someone of his age. Six months after his trip, in March 2024, the MRI showed the atrophy had vanished. 'You have the brain of a 40-year-old,' Dr. Charles Gordon, a Texas neurosurgeon and psychedelic skeptic, told Perry, The Times said. Since then, Perry has co-founded the nonprofit American for Ibogaine, as the drug remains a Schedule 1 drug in the United States under the 1970 Controlled Substances Act. He has called widening its use 'my life's mission,' he told the paper. He's found a sort-of across-the-aisle bedfellow in former Sen. Kyrsten Sinema, an Arizona Democrat turned independent, who left Congress earlier this year. Sinema told The Times that Perry was 'one of the few very conservative individuals in this space, with high credibility among what I'd call a nontraditional audience for psychedelic medicine.' Earlier this year, Perry successfully got Texas to put dollars into research on how ibogaine could help veterans. Sinema's Arizona became the second state to make that move. But at the national level, Perry still faces several hurdles. The Republican-led Congress already voted down Luttrell's amendment that would have allocated $10 million to the Pentagon to conduct clinical trials on ibogaine. Traditional conservatives also aren't on board. Family Research Council President Tony Perkins told The Times that while he hadn't done enough research to speak on ibogaine specifically, his conservative Christian organization has 'expressed opposition to the use of psychedelics because of longstanding spiritual, theological and ethical concerns about mind-altering drugs that open individuals up to mystical or transcendent experiences that are not in line with orthodox teaching of scripture.' And Perry still finds himself in the doghouse with Trump - as the president made clear recently when he brought up his former energy secretary's role in his first impeachment. Perry had set up the call between Trump and Ukrainian President Volodymyr Zelensky - a call that House Democrats and a handful of Republicans said amounted to a solicitation of foreign interference in the 2020 election - with Trump asking Zelensky for dirt on Joe Biden and his son Hunter. 'I get indicted for making a phone call,' Trump complained at a meeting with faith leaders last month. 'I told the secretary, who wasn't the smartest bulb, he wasn't ... Rick Perry, you don't want him on your debate team, let me put it that way ... but he's the one that asked me to do it, Rick Perry.' Drive three hours north of Perth, and you'll arrive in Eneabba. This is Western Australia mining territory - the landscape is barren and desolate, just the odd hill in the distance. Buried in this vast terrain is a massive pit, full of what looks like mounds of worthless dirt. But appearances can be deceiving: in fact, this pit is home to a million tonne stockpile containing critical minerals, and Australia's bet on the this year, carmakers and other manufacturers around the world rushed to their war rooms, alarmed that China's tight export controls on rare earth magnets – crucial for making electric vehicles, wind turbines and defence equipment – could cripple was forced to halt production of its popular Explorer SUV for a week at one of its Chicago plants - a bold move for a major automaker already grappling with pressure from Trump's tariffs.A month later, CEO Jim Farley revealed the pause was triggered by a shortage of rare earths, admitting the company was still struggling to secure reliable supply of the critical minerals."It's day to day," Mr Farley told Bloomberg has since agreed to let rare earths minerals and magnets flow to the United States, which eased the without a trade deal between the US and China, the fear is that the rare earths bottleneck could return, creating a massive supply chain triggered a realisation amongst policymakers and manufacturers everywhere: Beijing's control of rare earths has the world in a chokehold."The West dropped the ball – that's the reality. And China was in for the long run – it saw the benefit and was willing to invest in it," says Jacques Eksteen, chair for extractive metallurgy at Curtin University. Why rare earths matter The phrase "rare earths" - referring to 17 elements on the periodic table which are lightweight, super strong and resistant to heat, making them useful in small electric motors - is something of a misnomer."Rare earths are not rare or scarce. Gold is scarce, but it's not a critical material," Professor Eksteen earths are critical, however. Take the average electric vehicle – there might be rare earths-based motors in dozens of components from side mirrors and speakers to windshield wipers and breaking problem is therefore not amount, but the fact "somewhere in the supply chain you've got one or maybe a few countries controlling that bottleneck", Professor Eksteen the 90s, Europe and France in particular had a prominent rare earths industry. Today, almost all these minerals come from China, which has spent decades mining and refining at now accounts for more than half of global rare earth mining, and almost 90% of processing. The US sources 80% of its rare earth imports from China, while the European Union relies on China for about 98% of its supply."China has since very deliberately and overtly sought to control the market for the purposes of supporting their downstream manufacturing and defence industries," says Dan McGrath, head of rare earths for Iluka Resources, in between driving us around the company's vast Eneabba site. But Mr McGrath, and Iluka, are hoping to make a dent in that control - even if it wasn't necessarily in the company's original plan. For decades, Iluka has been mining zircon in Australia - a key ingredient in ceramics, and titanium dioxide used in the pigmentation of paint, plastics and just so happens the byproducts of these mineral sands include dysprosium and terbium - some of the most sought-after rare the years, Iluka has built up the stockpile, and is now worth more than $650m (£440m).This was the easy part, however. The processing or refining is another matter altogether."They're chemically very similar so to try and separate them requires a huge number of stages," Professor Eksteen explained."Also, you've got residues and wastes that you have to deal with out of this industry, and that's problematic. They often produce radioactive materials. It comes at a cost."And that is one of the reasons why the Australian government is loaning Iluka A$1.65bn ($1bn; £798m) to build a refinery to meet demand for rare earths which Iluka sees growing by 50-170% by the end of the decade."We expect to be able to supply a significant proportion of Western demand for rare earths by 2030. Our customers recognise that having an independent, secure and sustainable supply chain outside of China is fundamental for the continuity of their business," says Mr McGrath."This refinery and Iluka's commitment to the rare earth business is an alternative to China." But the refinery will take another two years to build and come online."Without the strategic partnership we have with the Australian government, a rare earths project would not be economically viable," Mr McGrath says. A strategic necessity China's recent willingness to turn supply of rare earths on and off has spurred trading partners to diversify their suppliers. Iluka says because automakers for example plan their production years in advance, it is already fielding requests for when its refinery does come earths are critical to the green transition, electric vehicles, and defence technologies – making their control a pressing national priority."The open international market in critical minerals and rare earths is a mirage. It doesn't exist. And the reason it doesn't exist is because there is one supplier of these materials and they have the wherewithal to change where the market goes, whether that be in pricing or supply," Australia's resources minister Madeleine King sees government intervention as necessary to provide an alternative supply, and help the world rely less on China."We can either sit back and do nothing about that... or we can step up to take on the responsibility to develop a rare earths industry here that competes with that market," Ms King there is something that Australia will have to contend with as it invests and works to expand a rare earths industry – pollution. In China, environmental damage from years of processing rare earths has led to chemicals and radioactive waste seeping into waterways - cities and people bearing the scars of decades of poor rare earths, it's not so much about the mining footprint, rather the processing that is a dirty business – because it involves extraction, leaching, thermal cracking and refining which produce radioactive components."I think there is no metal industry that is completely clean... unfortunately, it's a matter of picking your poison sometimes," Professor Eksteen says."In Australia, we've got mechanisms to handle that. We've got a legal environment and a framework to work with that to at least deal with it responsibly."The EU has in the past accused China of using a "quasi monopoly" on rare earths as a bargaining chip, weaponising it to undermine competitors in key industries. The bloc - which is home to hundreds of auto manufacturers that so desperately need rare earths - said even if China has loosened restrictions on supplies, the threat of supply chain shocks if building a brand new industry will take time, Australia seems to have a lot going for it in the rare earths race, as it tries to be a more reliable and cleaner one that - crucially - is independent of reporting by Jaltson Akkanath Chummar

The Department of Veterans Affairs has lost thousands of healthcare professionals deemed 'core' to the system's ability to function and 'without which mission-critical work cannot be completed', agency records show. The number of medical staff on hand to treat veterans has fallen every month since Donald Trump took office. The VA has experienced a net loss of 2,000 registered nurses since the start of this fiscal year, the data show, along with approximately 1,300 medical assistants, 1,100 nursing assistants and licensed practical nurses, 800 doctors, 500 social workers and 150 psychologists. The numbers are at odds with claims by the VA secretary, Doug Collins, that veterans' healthcare would not be impacted by an agency-wide reduction of 30,000 workers to be completed this year through a combination of attrition, a hiring freeze and deferred resignation program. The reduction in medical staff is also feeding fears that the Trump administration is seeking to transform the VA, which currently operates the largest integrated healthcare system in the United States, into a private voucher program. 'It's a betrayal,' said Manuel Santamaria, 42, a disabled veteran who served as a US army medic and paratrooper in Iraq and Afghanistan. 'It takes away the government's accountability to veterans who have sacrificed for them.' The VA said in a statement to the Guardian that the fear of privatization 'is a far-left canard' and that 'anyone who says VA is cutting health care and benefits is not being honest'. The decline in the number of health workers, documented by the VA's monthly workforce dashboard, comes as the agency has experienced an influx of nearly 500,000 patients with complex medical issues linked to war-time toxic exposure. Bipartisan legislation passed by Congress in 2022 dramatically expanded veterans' eligibility for care for toxic exposure. Patricia Fieldings, a registered nurse who works in the spinal cord injury unit at the VA hospital in Augusta, Georgia, said the departures are creating 'very unsafe' conditions for her patients, many of whom lost the use of their neck or became paraplegic due to war wounds. Because of staffing shortages, nurses are being pulled from other parts of the hospital, who lack adequate training to 'safely care for a spinal cord patient', she said. Veterans and employee unions note that even before these departures the VA was already understaffed. Last August, the VA's inspector general reported that 86% of the agency's 170 medical centers and more than 1,000 clinics reported a 'severe' shortage of doctors, while 82% of facilities reported severe shortages of nurses. The staff reductions reported by VA this year represent an additional 2% decrease in the number of nurses and 3% decline in the number of doctors, exacerbating the existing shortages. In response to a detailed list of questions, the VA press secretary, Peter Kasperowicz, did not dispute that the VA employed fewer doctors and nurses under President Trump, but said 'a nationwide shortage of healthcare workers' had made 'hiring and retention difficult for the last 15 years across the entire healthcare sector, including at some VA locations'. However, agency data shows that under Biden, the VA was able to attract doctors and nurses. The VA added approximately 10,000 'mission-critical' healthcare professionals, between May 2023 and January 2025, agency dashboards show. Kasperowicz said the VA 'has several strategies to navigate shortages while ensuring veterans continue to receive timely, high-quality health care', which include referrals to private healthcare providers and telehealth. He dismissed the 'severe shortages' of doctors and nurses reported by the VA inspector general saying the watchdog's congressionally-mandated report was 'completely subjective, not standardized and unreliable'. Collins has made cutting employee headcount a priority, arguing the agency is bloated and can improve healthcare quality while shedding 30,000 jobs. Collins said his goal is 'reducing bureaucracy' by consolidating 'costly administrative functions'. Documents reviewed by the Guardian, however, show the agency is also losing healthcare providers. So far this year, VA nurses represented by the labor union National Nurses United have filed 116 notices, known as 'assignment despite objection', the union said, to protest situations they believe put patient health and safety at risk. One written by a shift supervisor in the intensive care unit in the Bronx in June, reported three nurses had been tasked with treating three patients each, contrary to established safety standards that stipulate critical care nurses should not care for more than two patients at a time. Six nurses cared for 15 patients at the ICU in the Bronx during the nurse's shift, they wrote: 10 had been placed in isolation, all were listed as 'high risk' of falls. The nurse also reported that equipment was broken and that nurses had to work through breaks and meal times. At the VA in Augusta, Georgia, Fieldings filed five reports alleging conditions that 'posed a serious threat to health and safety of patients' in the spinal cord unit between April and July. On 6 June, she wrote that two registered nurses had been tasked with caring for 10 patients, seven of whom were on ventilators and all of whom were at high risk of falls. On 24 July, she reported three nurses were charged with taking care of nine veteran patients, all of whom had been placed in isolation. 'Staff were not properly trained,' she indicated, and the unit was staffed with 'unqualified' or 'inappropriate personnel'. 'These are some of our most vulnerable patients,' Fieldings said in an interview. 'They have difficulty eating. Many have broken their necks.' She said departures had also led to regular forced overtime for nurses, leaving them exhausted. 'Nurses stay until their moral distress is so great that they can't take it anymore and then they leave,' she said. In its statement, the VA said reports of unsafe conditions in Augusta's spinal cord unit were 'false'and that 'the unit is fully staffed'. The VA did not respond to reports alleging unsafe conditions at the agency's hospital in the Bronx. It said the Guardian was 'cherry picking issues that are mostly routine, limited to a handful of sites and in many cases were worse under the Biden administration'. On Wednesday, the agency said it was terminating labor agreements with unions that represent most of its health workers, citing a March executive order from President Trump that sought to eliminate collective bargaining rights on 'national security' grounds. The termination of union agreements eliminates the VA's obligation to respond to these reports of unsafe working conditions. Contract language requiring nurses be provided professional training has also been voided, along with rules that require management to 'float', or temporarily reassign nurses to cover parts of the hospital experiencing staffing shortages, 'based on qualifications and competencies of the RN'. In statements, the AFL-CIO, National Nurses United and the American Federation of Government Employees, which represents 320,000 VA workers, said the VA is retaliating against workers for speaking up for patients. The agency says the end of collective bargaining means 'staff will spend more time with Veterans' and allow the agency to 'manage its staff according to Veterans' needs, not union demands'. The unions, who say their right to bargain collectively cannot be voided on 'national security' grounds, said they would press their case in the courts. At the VA Northport Medical Center on Long Island, a virtual town hall on 30 July with hospital director Antonio Sanchez devolved; the chat function in Microsoft Teams burst with complaints. 'We are desperate. We are getting burnt out,' a clinician in the facility's inpatient medical unit wrote. 'We are not able to keep pace with our cut in staff.' Others raised concerns about short staffing and patient care across other critical care units at Northport, the Teams chat shows, including the intensive care unit, post-anesthesia care unit and community living centers, the VA's version of nursing home care. Near the end of the meeting, Sanchez responded that he was 'working hard' to bring staff on board as soon as possible. Veterans advocates and employee unions allege the Trump administration is consciously seeking to starve the veterans healthcare system so it can be turned into a private voucher program. The administration's budget proposal for 2026 includes a major re-alignment of priorities – a 50% increase in taxpayer funding for private healthcare for veterans, $11bn, paid for by a corresponding cut to the existing public system. During his Senate confirmation hearing to be the VA's undersecretary for health on 23 July, John Bartrum, a retired air force major general, said it was time to 'revisit the balance' between private and government veterans healthcare. In a statement to the Guardian, the VA rejected the idea that it was seeking to privatize the agency and said it had opened 13 healthcare clinics since January. Advocates said those facilities had been in the works for years. The same day as Bartrum's hearing, the Access Act, a bill that would make it dramatically easier for veterans to seek private healthcare with taxpayer support, cleared a key House committee on a 12-11 party-line vote. A blue-ribbon commission established by the agency last year found veterans received significantly better care at lower cost from the public system. Private providers operated with little oversight, they wrote, and 'are not required to demonstrate competency in diagnosing and treating the complex care needs of veterans nor in understanding military culture, which is often critical to providing quality care for veterans'. Independent studies from the Congressional Budget Office and Rand Corporation have similarly found that veterans in the private system face longer wait times and poorly coordinated healthcare at higher costs to the taxpayers. In Las Vegas, Santamaria bristled at the suggestion the VA was bloated. He said difficulties scheduling appointments have 'gotten much worse' under Trump – and that he now often spends more than two hours on hold. In addition to post traumatic stress disorder, Santamaria suffers from psoriasis, a skin disease that causes a rash with itchy scaly patches that disrupt his sleep and make it hard to concentrate. In February, when his medication ran out, Santamaria sought to switch to a different drug, but was unable to schedule an appointment with the VA until May. By then his entire body was covered by cracked, bleeding welts. Psoriasis is among the conditions presumed to be connected to war-time toxic exposure under the Pact Act, the 2022 law that extended healthcare and benefits eligibility. According to the VA, the wait time for new dermatology patients at the VA hospital in southern Nevada was 137 days as of August 7. In its statement, the VA blamed the delays Santamaria faced on a lack of communication from the private dermatologist he saw through the agency's 'community care' voucher program. Once that doctor filled out the proper paperwork, 'new medication was mailed out just five days later', the agency said. The agency said the average wait time on phone calls placed to the VA in southern Nevada was one minute and 47 seconds. Santamaria said VA workers typically answer the phone relatively quickly, before putting him on hold for two hours or more. The Guardian asked the VA for data in order to compare patient wait times during the Trump and Biden administrations at each VA facility. The VA did not provide it, but shared a chart that indicated wait times for some health services had marginally improved this fiscal year, while others had worsened. The fiscal year covers the final three and-a-half months of the Biden administration and first six months of Trump's second term. The VA's workforce dashboards show the decline in staffing was accelerating. In February, the VA reported a net loss of 223 registered nurses and 94 doctors; in June the VA lost an additional 409 registered nurses and 147 doctors. The VA declined to answer questions on the acceleration in the reduction of 'mission-critical' medical workers employed in agency hospitals and clinics. Those staffing losses do not include 10,000 VA workers who accepted deferred resignation offers early in Trump's second term, whose last day on the books will be 30 September. In response to a Freedom of Information Act request filed by The War Horse, a non-profit news outlet focused on veterans issues, the VA provided a list of occupations held by these workers. In addition to thousands of human resources professionals, management analysts and other government bureaucrats, 214 nurses will also leave the agency, along with dozens of doctors, social workers and psychologists.
