Want to live to a healthy old age? Here's what a top doctor does based on hard science

Who among us wouldn't want to live a good life well into our 90s, especially if we could do it disease-free while still being as witty and spry as Betty White, as eloquent as Toni Morrison or as civically engaged as Jimmy Carter?
But you'd need good genes to live that long, right?
Not necessarily, according to Dr. Eric Topol, founding director of the Scripps Research Translational Institute in La Jolla, California, and former chairman of cardiovascular medicine at the Cleveland Clinic.
For almost 20 years, Topol has been studying a group of 1,400 Super Agers, or people who reached the age of 80 and beyond without developing one of three common, chronic, age-related conditions (cancer, heart disease and neurodegenerative disease), as part of the Wellderly study.
To his surprise, when his team conducted whole-genome sequencing on all the Wellderly study participants, the researchers found nothing genetic, across the board, that conferred an age and health advantage.
Topol's new book, 'Super Agers: An Evidence-Based Approach to Longevity,' explores the other factors at play and the science that backs them up. He examines lifestyle factors such as diet, exercise and sleep. He also looks at new technologies including the following :
polygenic risk scores that sift through a person's DNA to quantify disease risk;
the new science of 'omics' (for example, proteomics to analyze the complete set of proteins expressed by a body) that can be used to produce organ clocks measuring how fast our individual organs are aging; and
artificial intelligence to synthesize all available health data points.
All these technologies show promise in helping more people, if not reach Super Ager status, then achieve a longer health span by either delaying the onset of disease or aggressively treating conditions to prevent progression and complications.
Topol, who will soon turn 71, practices what he preaches.
'I hope to be (a Super Ager),' he told CNN Chief Medical Correspondent Dr. Sanjay Gupta, recently, on his podcast Chasing Life. 'If I can get another 10 or 15 years without an age-related disease, that'd be great. And I've really doubled down on the things that I can do to help get there.'

'I made extensive changes from the research, once we learned that it isn't in your genes … to achieve healthy aging, free of cancer, free of cardiovascular disease and free of neurodegenerative disease,' Topol said.
One of the biggest changes Topol said he made was to his exercise routine. 'I used to only do aerobic exercise, and as a cardiologist, that was my thing,' he said. 'Now I do balance training and strength training (as well). It's an extensive difference. … And so, I'm more strong and fit than I've ever been.'
In fact, exercise is so important that Topol ranks it as No. 1 on the list for anyone who wants to get started on the path of a longer life.
'It's free. And if you're not doing anything, if you're a sedentary, then start with walks, brisk walks if you can, and keep increasing it,' he said. 'Challenge yourself (with) hills and pace … if you are working up a sweat, that's a really good sign.' (Check with your doctor before starting any new exercise routine.)
To get your upper body in shape, he recommends resistance bands, and to improve balance, he said to practice standing on one foot, then the other. 'These are all free or cost minimum,' he said, noting that with a few inexpensive items you can do this at home, no gym membership required. 'So work on that and then other things will start to fall in place. … If you exercise more, you'll sleep better, too.'
Which brings us to the next major change Topol made.
'I realized how important (sleep) is because of 'glymphatics.' That was a new insight in the book: about deep sleep and getting these waste products out of your brain,' he said, referring to the recently discovered glymphatic system, which helps flush out waste products in the brain and central nervous system, much like the lymphatic system does for the rest of the body.
'I was a poor sleeper,' he confessed, so he worked 'extensively' on improving the different factors that could impact it. 'Things like … what do I eat? And when do I eat? Also hydrating, mostly early in the day … to have less interrupted sleep.' He added that for him fasting is 'detrimental' to his sleep, as are both exercising or eating a big meal too late in the evening.
And, do not give short shrift to sleep regularity, Topol advised. 'I used to go to sleep all over the place (in terms of time), and now I'm very regimented, except for maybe one night a week,' he said, noting that this tweak was 'phenomenally effective' in levelling up his sleep game.
As for diet, Topol (who hasn't touched red meat in 40 years but does eat fish) said he wasn't surprised the evidence showed that a plant-based diet with enough lean protein 'prevailed as the best.'
'What was interesting, though, is how toxic (and) pro inflammatory ultraprocessed foods were,' he said, adding that he does everything he can to minimize UPF — which he jokingly calls UFOs, or alien food . 'It's hard to get it to zero, but I've (made a) conscious effort.'
Topol said he also increased his protein intake 'some,' due to his age and because he started strength training, 'but I don't go to toxic levels that are advocated by others, that have no evidence (and) that are pro-inflammatory.'
He said he keeps protein to about 90 grams a day for his 180-pound (82-kilogram) frame, by adding nuts, legumes (including beans) and more fish, even canned tuna in water, to his diet.
'I generally eat a salad every night,' he said. 'Now I'm a little more conscious about having something with it. … But (it's) not the protein craze that's being advocated by some of the extremists.'
What about supplements and vitamins? 'I don't rule out that they could help (if) you have a specific deficiency — if for some reason your vitamin D is very low, or your vitamin B12,' Topol said.
But 'for healthy people who are not symptomatic (and) who take good care of themselves,' he said, 'there's no data to show that these provide any benefit' much less an antiaging one.
'I think they're a waste of money, and some of these supplements have potentially adverse effects,' he said. 'It's just infuriating because they're not regulated, so (marketers) can say anything, and people believe it and buy it.'
When it comes to consuming alcohol, Topol said he keeps his drinking 'light,' to fewer than seven drinks per week, after which the risks start adding up quickly. He said it doesn't seem to affect his own sleep, but it might impact that of others, so he recommends watching out for that.
Another top-of-the-list item: mental health and stress management. Topol mainly turns to exercise to help navigate both of these elements, but he said being out in nature is another often-overlooked route.
'The nature prescription is part of the book. There really is something to this,' he noted, adding that he takes advantage of San Diego's climate and location to do plenty of hiking and walking. 'It's good for mental health, it's good for stress and anxiety, just to be out in our environment.'
It's never too late to start any or all of these lifestyle adjustments, Topol said.
'These diseases take 20 years at least (to develop). So, you get even more (bang) if you start in your 40s and 50s or 60s … but if you start when you're 70 (or) 80, you're still going to derive years of healthy aging, maybe not quite as many, but still,' he said.
And because these common, chronic, age-related conditions — cancer, heart disease and neurodegenerative diseases — do take so long to fully manifest, Topol said we have time to intervene to change their trajectory.
But to fully leverage that latency period before the symptoms of a disease appear, it's important to be aware of personal risk, so people and their doctors (and society more broadly) can focus prevention and early detection efforts on the right people at the right time.
'Knowing one's risk — that's the next part, which is: we're going to prevent these three diseases in the future. We can now, and we're going to even get better at that,' Topol said.
Topol said he learned from his polygenic risk scores that he's at high risk for coronary artery disease, despite not having a family history. So, he has focused on getting his LDL cholesterol down. 'I've gotten it low, and perhaps I got more aggressive about that. I've checked my other lipids, like Lp(a) (lipoprotein[a]), so I feel like I've done everything I can to go into high prevent-mode for that.'
Topol said wants to get his immunome (all the genes and proteins of his immune system) mapped, to measure how well his immune system is functioning. It's a measurement he believes will one day soon be commonplace for everyone. He already has data on 'every virus I've ever been exposed to and every autoantibody that I have.'
He also plans on having his organ clocks measured, because different organs (such as the heart and brain) age at different rates, putting people at higher risk for certain diseases. 'I think it's going to be very helpful, because then I'll know (how) my heart organ clock correlates with my risk of heart disease.
'So those are some of the things I've done and/or I want to do,' he said.
'It's an extraordinary time in medicine, because we have a path to prevention,' Topol said, 'because of the way the science of aging (is developing), with these metrics like organ clocks and protein biomarkers and our genetics — all these layers of data — we are going to be able to prevent these diseases. And that's exciting. We've never done that before.'

A doctor explains what people should know about the ‘cortisol cocktail' that's gone viral on TikTok

(CNN) — If you've spent time on TikTok or Instagram lately, chances are the 'cortisol cocktail' trending as the latest wellness hack has appeared in your feed. This colorful, nonalcoholic drink typically contains a mix of coconut water, citrus juice and certain salts. Also called an 'adrenal cocktail,' it has amassed millions of views, with influencers claiming it can lower stress hormones, boost energy and balance your body's adrenal function. Almost half of Americans (49%) report frequent bouts of stress, and the number of people in the United States who are feeling stressed has been rising for the past two decades, according to a 2024 Gallup survey. In these uncertain times, a stress-relieving tonic feels just right as a soothing balm, but does it actually live up to the health claims? What is cortisol and its relationship to stress? What nutrients are in these drinks and how are they purported to influence cortisol? Do they work? Who should be wary of trying this concoction? To guide us through these questions, I spoke with CNN wellness expert Dr. Leana Wen. Wen is an emergency physician and adjunct associate professor at George Washington University. She previously was Baltimore's health commissioner. CNN: What is cortisol and its relationship to stress? Dr. Leana Wen: Cortisol is a steroid hormone produced by the adrenal glands, which are organs located above the kidneys. The hormone plays a vital role in many bodily functions, including regulating metabolism, blood sugar, blood pressure, sleep-wake cycles and immune responses. Cortisol is sometimes called the 'stress hormone' because its levels naturally rise when our bodies perceive a threat or stressful situation. This reaction is part of the body's 'fight-or-flight' response, helping people stay alert and react quickly when needed. Those temporary increases in cortisol are a helpful evolutionary adaptation. Problems can arise when cortisol levels stay elevated for prolonged periods of time, which can happen when someone is under chronic stress. Persistently elevated cortisol is linked to problems such as poor sleep, anxiety, high blood pressure and even weakened immunity. There are a number of medical conditions that can lead to abnormally high or low cortisol levels. Taking high doses of steroid medications or adrenal gland tumors are examples of conditions that can cause excess cortisol. Addison's disease, or primary adrenal insufficiency, is when the adrenal gland stops its usual function. This condition, and problems with the pituitary gland in the brain, could result in low cortisol levels. CNN: What is in these 'cortisol cocktail' drinks, and how are they purported to influence cortisol? Wen: Recipes vary, but they tend to have a combination of coconut water, juice (usually orange or some other citrus fruit) and some salts (commonly sea salt and/or magnesium). Sparkling water is often added to taste. Contrary to what the name may suggest, it typically does not contain any alcohol. Each of the primary ingredients is supposed to play a role in supporting adrenal function and lowering stress. Coconut water is high in potassium, which helps maintain proper fluid and electrolyte balance. Citrus juice offers vitamin C, which plays a role in adrenal gland function. Salt and magnesium are supposed to replenish minerals depleted by stress. Moreover, some researchers believe that magnesium itself may help reduce feelings of anxiety or stress. Because of this, some versions of the cortisol cocktail include magnesium powder and/or cream of tartar, which has a high concentration of magnesium along with potassium and other salts. CNN: Is there evidence that this drink actually lowers cortisol and reduces stress? Wen: There is no scientific evidence that this drink has the effects it purports. While it's true that nutrients such as potassium, vitamin C and magnesium are important for healthy adrenal function, they are typically obtained in adequate amounts through a balanced diet. Bananas are the classic food high in potassium, but many others are packed with this mineral, too, such as raisins, apricots and tuna. Vitamin C can be found in citrus fruits, strawberries and peppers. And magnesium is also in many foods, including beans, peanut butter and leafy green vegetables. True deficiencies in these nutrients can cause health problems, but such deficiencies are uncommon in otherwise healthy individuals. More importantly, there is no evidence that consuming extra amounts beyond what the body needs will provide additional benefits or that it will meaningfully lower cortisol levels. CNN: What about the claim that the drink can treat a condition that influencers call 'adrenal fatigue'? Wen: The claim that some influencers have promoted on social media is that there is a condition of 'adrenal fatigue' in which stress is overwhelming adrenal glands and people end up feeling tired, anxious and unable to cope with daily life. However, the symptoms could stem from a variety of causes, including poor sleep, mental health conditions, and undiagnosed diseases such as anemia, hypothyroidism and diabetes. Importantly, there is no recognized medical diagnosis of adrenal fatigue. Outside of cases in which there are specific diseases causing adrenal insufficiency, there is no evidence that somehow the adrenal glands get overwhelmed and lose their ability to function. The symptoms often attributed to adrenal fatigue are real, but they are better explained by other, well‑studied causes. One risk I see is that people might rely on this so-called remedy instead of seeking medical evaluation for the real issues behind their fatigue or stress. CNN: What about all the people on the internet who claim that they have had positive health outcomes as a result of this drink? Wen: It may be that some individuals were not getting enough fluids or electrolytes before, and the additional hydration from the drink helped them feel more energized or clearheaded. There is also the placebo effect to consider: When people expect a certain treatment to work, they may genuinely feel better simply because they believe they are doing something beneficial for their health. The ritual of making and drinking this drink could itself reinforce this sense of control and care, which can have real psychological benefits even if the drink's ingredients have no direct impact on cortisol. CNN: Who should be wary of trying this so-called tonic? Wen: People who should be cautious include individuals with chronic medical conditions such as kidney disease, heart disease and diabetes. Certain individuals with kidney disease need to be especially wary of food containing high amounts of potassium. Those with heart failure may be advised to restrict their fluid intake. The sugar in fruit juice may also be problematic for some people with diabetes. Individuals interested in this drink should check with their doctor before adding this or any influencer-promoted wellness product to their diet. They should ask if any of their medications might interact with the drink's ingredients. And if they are experiencing symptoms — such as fatigue, anxiety or trouble sleeping — that prompted them to try this cocktail, they should discuss these concerns with their doctor to determine whether further evaluation is needed.

It can be small, like using roll-on adhesive to help your clothes stay in place or tying a scarf around your waist to go with a plain white T-shirt. Or it can be more striking, like adding a pop of color to an all-black outfit or securing a strapless bra under a beautiful, sleeveless blouse. But perhaps one of the most impactful yet subtle fashion hacks can be achieved by keeping a stash of sticky bras handy. As someone who has gone to countless weddings, formal events and more, I can personally vouch for sticky cover-ups when traditional bras just won't cut it. They've saved the day when wearing bodycon dresses, daring necklines or even delicate fabrics that need a smooth, invisible finish. Lightweight, discreet and surprisingly supportive, they are the true unsung heroes when assembling tricky silhouettes, all while maintaining confidence and comfort. That being said, not all sticky bras are going to do the trick. A good sticky bra must be sweatproof, last through cardio-boosting dance sessions and, ultimately, be comfortable enough to wear for extended periods. That's why we tested a range of sticky bras to find the very best. Here's what we landed on. This bra comes in three shades and ranges in cup sizes from A to E. It features a center-front clip for easy fastening as well. But what really stood out for us was the waterproof material that makes it great for slick summer days. Sticky Boobs Nonadhesive Nipple Covers Small, portable and reusable, our testing panel loved these convenient, nonadhesive sticky nipple covers. Though not as supportive as the Lounge bra, we're fans of the comfort and portability of these palm-sized sticky bras. Even though they're nonadhesive, they stayed put through all sorts of wear. Sticky bras come in many shapes, sizes and forms, and although everyone has different preferences, a good sticky bra should be high coverage, stay put without any sliding or gliding and feel breathable and comfortable. Lounge Underwear's Stick-On Bra won over our panel for these reasons, plus much more. At $40, this Lounge sticky bra felt like a sleek solution for outfits that don't work with strapped bras, like linen ensembles, scarf tops and low necklines. Made with 86% chinlon (a nylon fabric) and 14% spandex (an elastic, stretchy fiber), the lightweight bra features soft foam cups with a center-front clip that makes it easy to fasten and remove. Our testers appreciated the sheer comfort of this bra and that it had no underband or padding, which helps it sit invisibly under clothing. 'It's comfortable to wear and easy to take off,' associate testing writer Gareen Puglia said, noting how thrilled she was to see sizing up to an E. This bra gets the job done without any bells and whistles. 'If you're looking for something with more lift, I'm not sure this does the trick,' Puglia added, 'but this bra [is pretty] great.' And for sweltering days, the waterproof, sweatproof adhesive is designed to hold up in heat and humidity. Testing writer Michelle Rae Uy agreed, adding that it didn't feel uncomfortably sticky, making it ideal for summer heat. It's important to note the stick-on bra comes in three shades: Sand, Mocha and Nutmeg. I appreciate the range of colors, especially when considering lightweight tops for the summer. I also liked the center-front clip, which made everything feel more secure. However, according to our testers, its reusability is one of its most standout features: If it's removed gently, hand-washed with care and stored properly, it can last for multiple wears. Uy preferred this bra over other stick-on styles for its smooth edges that stayed flush against the skin and didn't irritate her sensitive skin. While she noted the adhesive fully covers the cup, including the nipple area, she appreciated that it remained gentle and effective even during sweaty summer days. However, the bra won't add that extra 'oomph' if that's what you're looking for. Puglia, who recommended sizing up, found it comfortable but noted that the bra lacked lift, which may be improved with a proper fit. The Sticky Boobs Nonadhesive Nipple Covers offer an entirely different experience from the Lounge Underwear Stick-On Bras. They use a skin-safe, discreet solution for anyone seeking extra coverage without the irritation of sticky adhesives. Made from medical-grade silicone and available in three shades (Light, Tan and Dark), these covers are designed to blend seamlessly under clothing, whether you're wearing a fitted tank top, strapless dress or bathing suit. 'I liked the nonadhesive Sticky Boobs, which were useful to wear with already tight clothes or even to stick in a bra for extra coverage,' Puglia said, noting just how comfortable the bra felt. The nonadhesive construction was the true icing on the cake. This means they gently cling to skin without sticking, making them ideal for sensitive skin and for layering inside bralettes or unlined bras. Both testers praised its comfort and versatility. Uy, who often experiences irritation from adhesives, noted that these stayed invisible under even thin tops and didn't cause redness or itchiness. 'I love these so much,' she said. 'The lack of adhesive protects my skin since most make my skin itchy and red.' Puglia agreed and gave them high marks, appreciating how well they worked in a homemade halter top and how they stayed secure all night, offering extra coverage without budging. I appreciated the sturdy case that came with the sticky bras, which were great to pack while traveling for weddings or other events. Though they're marketed as reusable and washable, Uy found that after a few weeks of use, they began to lose their shape and grip, raising questions about long-term value. To find sticky bras that meet these standards, we researched our favorite brands, ranging from affordable options like Sticky Boobs to higher-end lines like Lounge. Our panel of testers, ranging in sizes, skin tones and types, needs, and heights, wore each sticky bra for a minimum of two weeks to properly test the look and feel of each one. We wore them in a range of scenarios, like under strapless dresses at weddings and at outdoor concerts. We also considered the following. Support: Arguably, a bra's most important factor is support. Does it adequately support movement during wear? Fit: Can the bra effectively handle busts of all sizes? How does the bra fit your body? Is it snug and supportive without being constrictive? Is it true to size? Versatility: Can the sticky bra be worn with different necklines and silhouettes? Is it reusable? Comfort: How long would you want to wear this bra? All day, or is it something you're ready to take off when the day is over? Re-wear factor: Would you use this bra again? Value: Is the bra worth the price? When shopping for a sticky bra, there are a few key factors to keep in mind to ensure comfort, support and reliability. Adhesive strength and skin safety A good sticky bra should not budge through movement, sweat and long wear — but not at the cost of your skin. Look for medical-grade or hypoallergenic adhesives, especially if you have sensitive skin. Coverage and cup size Coverage can have different meanings across brands. Ensure the bra offers enough coverage for your size and shape, particularly if you're looking for lift or support. Some styles go up to an E cup, while others may run small or lack full coverage. Reusability Many sticky bras are designed for multiple uses, but longevity depends on how well the adhesive holds up over time. That's why we wore our bras for several weeks. We recommend checking if the bra is washable and how many wears it's expected to last. Style Plan ahead and think about what outfits you envision wearing it with. Some sticky bras have plunge necklines or side wings for backless dresses, while others offer more of a lift for strapless looks. The shape should ultimately provide a seamless finish. Comfort and wearability Perhaps one of the most important features of a sticky bra should be its comfort level. Since there are no straps or bands for support, the design itself should feel secure without pinching or pulling. It's smart to consider lightweight materials, smooth edges and a breathable fit, especially in warmer weather. Nood Game Changer Lift and Shape Bra, 4-Pack Perhaps the most discreet sticky bra we tested, these Nood "stickers" offered promising coverage for such a thin cover. They held up during testing and offered a somewhat seamless look. All testers agreed the lift was impressive — but not necessary. Nood's adhesive bras are powerful, to say the least, and we found they deliver impressive lift and support, thanks to a strong acrylic adhesive. That strength, however, means proper use is essential. For safe removal, we recommend using body oil or the brand's own Boob Savior Adhesive Remover, as directed in the instructions. 'It took some trial and error to figure this out,' Puglia said. 'However, this felt like one of the most supportive bras.' This bra, ultimately, got docked in our ranking because it could feel painful to take off. Uy agreed, adding she wasn't a fan of the ultra-lifting capabilities. 'It just feels a bit more inconvenient for me to use,' she said. Commando Top Hats Nipple Concealers Tiny, discreet and travel-friendly, the Commando Top Hats Nipple Concealers were solid and reliable. We found them to be a great option for discreet coverage, all in a cute package to boot. They are, however, pretty pricey for a single pair of covers. I appreciated that the Top Hats are made from hypoallergenic silicone with medical-grade adhesive, meaning my skin wasn't irritated after use. They lay flat under clothing and could be worn with or without a bra. While most retailers only seem to carry one shade, do come in three options: Beige, Caramel and Mocha. Each set includes two boob covers and a storage box, so the $54 price tag seemed worth the splurge — but it is a splurge for one pair of covers — especially considering they're comfortable for all-day wear and stay in place without shifting. One thing to note is that these portable concealers should be hand-washed to maintain their shape and adhesive quality. Fashion Forms Full-Figure Gel Petals These delicate petals offered extra coverage and a smooth, seamless finish under clothing. One major downside? They are limited to just one light nude shade. Like many sticky bras, the gel petals were made with ultra-comfortable silicone, meaning they protect sensitive areas without irritating. They looked virtually invisible under white shirts and even stayed hidden beneath sheer fabrics. Though not revolutionary, these nipple covers offered reliable coverage and a smooth, 'barely there' look. The reusable petals only come in a light nude shade, which is a downside for those with darker skin tones. However, they do come with a travel case for storage, which is a small bonus. We had no issues with stickiness, even after multiple adjustments, but keep in mind that the gel petals should be applied to clean, dry skin before using any moisturizers, powders, deodorants or perfumes. Sticky Boobs Sticky Bra Intimidating at first, the Sticky Boobs Sticky Bra is formidable, sturdy and surprisingly comfortable. The $40 was worth it considering the reusable and washable design, plus "nipple-safe" technology. However, the bra only comes in four sizes — A, B, C and D — so larger busts might struggle with this option. This bra was confusing upon first glance. With its dotted, padded lining, this bra was unlike other smaller sticky bras we tested. However, it's ideal for low-cut or open-back styles, thanks to its discreet design and very reliable support. The front clasp was a nice touch when it came to customizing the level of lift and support too. As someone with a smaller frame, I had no issue with fit or sizing. The smooth, seamless edges stayed hidden under clothing, and the butterfly shape helped prevent any visible lines. But those past a size D might experience spillage, considering the limited sizing. One interesting feature, however, was the 'nipple-safe' technology — an adhesive-free zone in the center, which made it more comfortable for sensitive skin. The zone isn't huge, though, so if they don't fit you perfectly, this feature is sort of a bust (no pun intended). What is the most durable type of sticky bra? What is the most durable type of sticky bra? A sticky bra with strong adhesive will likely be the most durable option, especially if it is made with high-quality silicone adhesive. What type of outfits are sticky bras best for? What type of outfits are sticky bras best for? A sticky bra is the ideal undergarment for backless dresses, tight silhouettes or strapless tops. They can also offer extra support under bathing suits or bralettes. Can you reuse sticky bras? Can you reuse sticky bras? It depends on the specific sticky bra and its materials, but in most cases, yes. With proper care and washing, a sticky bra should last multiple uses. Finding the most comfortable sticky bra is no small undertaking. That's why we asked three CNN Underscored testing writers to wear, style and evaluate a range of bras. Though they had varying style preferences and body types, all shared a common goal of finding a comfortable and durable sticky bra for all occasions. Associate testing writer Carolina Gazal is short and petite, usually reaching for smaller tops. She prefers a moisture-wicking sticky bra for dancing the night away. Associate testing writer Gareen Puglia wore her sticky bras with sleeveless blouses and scarf tops to thoroughly test each one's longevity. Testing writer Michelle Rae Uy wore her sticky bras with a range of blouses to test their endurance. She was critical of how the bras would react with her sensitive skin.
