'We put our baby in their hands - she ended up in unnecessary pain'

The parents of a baby who was given a fraction of the morphine she should have after undergoing open-heart surgery say they are still seeking accountability.
Baby Charlotte Gilchrist is one of seven children who were given a tenth of the pain relief they should have been after undergoing the surgery last summer.
The underdoses affected seven operations over six days within a single theatre between June 13 and 24 last year but were not discovered until nearly a month later, on July 6.
A review by NHS Greater Glasgow and Clyde (NHSGCC) following the incidents found that overworked and fatigued staff at the Royal Hospital for Children in Glasgow missed vital checks.
Charlotte was just 11 weeks old when her parents were told she would need open-heart surgery.
Charlotte's mum Coral told STV News: 'I kept saying there is something not right (with Charlotte) – I didn't want to be right.' STV News
'You automatically think the worst', Charlotte's father, Craig, said.
Everything froze for the couple until after a two-hour-long surgery when they were supposed to be in the clear.
Craig still thinks of the videos he took of Charlotte post-surgery.
'Now, when we look back at videos, I feel dead guilty because, you can see, she's in pain', he explained.
The couple later discovered that Charlotte was given an underdose of crucial pain killers following the procedure. STV News
A review found the seven children, including Charlotte, were administered 1mg/ml of morphine – when they should've received 10mg/ml.
The report also found that the underdose went unnoticed for a month due to staff going on 'assumption' rather than doing 'two-person checks' as procedure requires.
NHSGCC has apologised to the families and said it has begun implementing the recommendations in the report.
But Craig and Coral say that they are still 'heartbroken' and don't think the recommendations are enough.
Coral broke down in tears as she told STV News: 'We handed over our world to somebody else's arms and they didn't look after her the way they should have.'
The Significant Adverse Event Review (SAER) by NHSGGC also found staff were overworked and overtired during shifts.
The review recommendations included staff training and stock management to ensure 'any incident of this nature is prevented from happening again'.
But families affected say that isn't enough. STV News
Coral added: 'There were lots of minor issues that were just careless, and there were a lot of them.
'It was bad enough she had to go through heart surgery, and even though we're thankful, she was still in unnecessary pain.
'We deserve to find out what has actually happened with that. Are there disciplinary hearings going ahead?
'You can give all the training you want, but unless it's implemented in practice, it's not going to make a difference.
'Over a year after her surgery, and we're still having to relive that with every email and every conversation.
'When you properly sit and think about it, it breaks your heart'.
STV News spoke to two other families affected by the incident, who are also disappointed and feel like the review raises more questions than it answers. STV News
Ellie McAdam, mum to three-year-old Theo, said she felt the report was a 'half-hearted acknowledgement of not following policy, but it falls short of real accountability'.
Two-year-old Kai Campbell's mother, Shelby, shared the sentiments, adding that she was 'shocked' as the report 'was just full of so many excuses.'
While Charlotte won't need any more heart surgery, Coral still wants to ensure no parent or child goes through the same ordeal.
Carol said: 'My heart goes out to the families who have gone through this and do need future surgeries.
'I know how anxious it's made me, and I can't imagine how anxious they are knowing that some point in the future they will need more surgery.'
NHSGCC apologised and said families have been invited to meet with members of the review team to discuss their concerns.
'We know the care these patients received fell below the standard expected, and we are sorry,' a spokesperson for the health board said.
'Both human and systems factors played a role in this incident, and we are committed to ensuring that lessons are learned from this extensive review, which was carried out with the involvement of pharmacy, nursing, and medical staff, as well as an individual panel member providing expert opinion.'
The health board said it began implementing the report's five recommendations as soon as the error was identified.
'It includes recommendations around staff training, the physical environment and stock management system, which are designed to ensure any incident of this nature is prevented from happening again,' the board said.

Betsy Williams, the daughter of Christian Williams has finally rung the bell on two and a half years of chemotherapy after being diagnosed with leukaemia Top racehorse trainer Christian Williams and partner Charlotte Smith are celebrating helping the most precious member of their stable across the finish line. And they're marking it by taking off on their first family holiday in two and a half years. ‌ In April 2023 Christian made headlines when star jumper Kitty's Light won the Scottish Grand National and bet365 Gold Cup within the space of seven days. Yet behind the scenes there was a much more important race to be won as a month earlier, he and Charlotte were told their youngest daughter Betsy, then five, had acute lymphoblastic leukaemia, a rare but aggressive type of blood cancer. ‌ The diagnosis emerged after Betsy developed an ear infection. At first her declining health was put down to a virus until, on her third GP visit, she was sent to hospital - where blood tests confirmed her parents' worst fears. ‌ Over a gruelling 27 months, Betsy underwent a bone marrow biopsy, blood transfusions and then started cycles of chemotherapy involving lumbar punctures and steroids, until this month when the now eight-year-old reached a significant landmark when she rang the bell on the Rainbow Ward at Noah's Ark Children's Hospital in Cardiff to signal the end of her treatment. 'We were given the date she would finish right at the start, and we'd been working to May 13 2025, provided there were no complications,' says NHS physio Charlotte, 40. 'She finished on that day and we were so excited. But then two days later she got Covid. It was probably the most poorly she had ever been in the whole two years. 'It was absolutely terrifying. She was in hospital for two weeks, really, really ill. They were convinced there was something else going on. They were running all these tests and scans. I thought she had relapsed already. 'And they came back and said, 'Charlotte, we've looked at everything and everything is negative, apart from this positive Covid swab.' It wasn't the plan we had for the end of the treatment but she is doing really well now, thank goodness.' ‌ Throughout this period, against the odds, Christian managed to keep the winners flowing from his yard at Ogmore-by-Sea on the Glamorgan coast. None was more vital than the success of Kitty's Light at Ayr. 'Betsy was having a particularly tough day but I turned on the TV as I thought it would cheer her up to see her Daddy on TV,' recalls Charlotte. 'To our delight he won. It completely changed our perspective and gave us hope.' ‌ Williams, 42, says: 'We've been lucky as when Betsy was ill and the family were down, we have managed to get a winner somewhere and not all families can look forward to days like that. 'We've just been in survival mode this whole time. Everyone pulled up their sleeves. I'm not sure how we've managed, really.' He adds: 'The drugs take your daughter away from you so now she is coming off the chemo and the steroids we are slowly getting her back.' ‌ Charlotte reflects: 'Christian has been unbelievable. I was fortunate to have sick pay and could take time off work to give my full attention to Betsy and our other daughter Tilly. 'Christian didn't have that option. He had 10 members of staff who relied on their wages, the owners relied on us, too. ‌ 'I couldn't tie my own shoe laces for the first couple of weeks, whereas Christian would be getting up every day, making sure he was with us in hospital, running a stable, liaising with owners and going racing. I don't know how he did it, to be honest. 'He has managed to sustain that, be a support to us and, not just keep the business going, but been really successful, too.' Betsy has won admiration as well for the way she has dealt with serious illness so early in her life. 'The whole time she has never wanted to be seen as ill, never wanted to be different to anybody else,' says Charlotte. ‌ On her sixth birthday, when friends organised a parkrun, Betsy insisted on running the whole 2km course. She had already started chemotherapy. 'How she acted then was the way she acted through the whole two years,' Charlotte adds. 'She still wanted to run, still wanted to go to school, still wanted to do dance. She never used her illness as an excuse not to do things.' There are still challenges ahead but Charlotte says: 'We've rung the bell and are ecstatic that that phase of our life is over. It may take a while for us to transition from that but Betsy is living her normal life.' As a treat the family have taken a rare holiday to Dubai. 'We are going for nine days and we can't wait,' she says. 'Just the four of us. No phones, no work and just enjoy each other. 'We are looking forward to living, making new memories and hopefully this is all going to be a distant memory to Betsy.'

On a quiet Sunday afternoon in June 2023, my Dad, Anthony James Bundy, collapsed whilst out shopping at Costco in Glasgow. He was 53. One moment, he was fine. The next, he couldn't stand. He was vomiting, sweating, his eyes struggling to focus. His brain was shutting down in real time. Advertisement Hide Ad Advertisement Hide Ad At first, stroke wasn't suspected due to the lack of the 'Fast' – face, arms, speech and time – symptoms. An ambulance wasn't initially dispatched. My Dad didn't improve. Finally, an ambulance was sent. Again, stroke was not suspected. Though my family raised concerns, the system, focusing on Fast signs, did not respond as it should have. My Dad spent five-and-a-half hours in Glasgow Royal Infirmary. Then a full-blown stroke struck, this time with clear Fast symptoms. When a scan confirmed a stroke caused by a blood clot, my Dad had to be transferred to the Queen Elizabeth University Hospital. Why? Because, as the Significant Adverse Event Report (Saer) confirmed, thrombolysis at Glasgow Royal is only available between 9am and 5pm, Monday to Friday. It 'dissolves the clot, restoring blood supply to that part of the brain and reduces the amount of damage caused'. The NHS then explains that it 'must be given within four-and-a-half hours of the onset of symptoms, and the sooner it is given, the more effective it is likely to be'. Advertisement Hide Ad Advertisement Hide Ad It's deeply concerning that this vital treatment remains unavailable at weekends in one of Scotland's major hospitals, something I believe can and should be addressed. Thankfully, it is available on a 24/7 basis at the Queen Elizabeth. My Dad finally received thrombolysis, but the blood clot did not fully dissolve. The next step in such a scenario is a thrombectomy: a surgical procedure where doctors physically remove the clot using a tiny device. A 17-hour wait But when my Dad arrived at the Queen Elizabeth, a thrombectomy was not available. The Saer report stated that, on the date he had his stroke, 'thrombectomies were only available to patients on the Hospital 2 site [the Queen Elizabeth], Monday to Friday 9am-5pm'. Advertisement Hide Ad Advertisement Hide Ad My Dad waited 17 hours from the first 999 call to receive a thrombectomy because care wasn't available 24/7. The NHS states that thrombectomy is 'most effective when performed within six hours after stroke symptoms begin'. This, therefore, begs the questions: Why is thrombectomy not available everywhere in Scotland on a 24/7 basis? Why is thrombolysis not available at one of Scotland's major hospitals at the weekend? Sadly, my Dad passed away. We will never know if he would have survived if he got the emergency stroke care on time. Whilst the pain of losing your Dad is always immeasurable, the 'what if' question adds extra weight, like an anchor, to the grief. It's hard to move on when this simple, penetrating question keeps circling your mind. Hard to justify Knowing this pain, I don't want others to go through it. That's why I am campaigning to improve emergency stroke care in Scotland. If my Dad's death can result in Scots having 24/7 access to life-saving treatment, then some good will come from the pain my family continues to experience. Advertisement Hide Ad Advertisement Hide Ad Since my Dad's death, thrombectomy services in Scotland have expanded slightly . In the best-equipped parts of Scotland, thrombectomy is only available between 8am and 8pm, with provision at the weekend still limited. It's hard to justify a situation where this lifesaving, time-sensitive treatment is simply unavailable for 12 hours each day, regardless of need. This doesn't reflect what we expect from an emergency service. Stroke care must be available whenever it's needed, not just part-time. Last year, Scotland's stroke charities highlighted that 'Scotland trails Ukraine in thrombectomy procedures', pointing to data showing that although around 1,000 people in Scotland qualified for a thrombectomy in 2023, just 153 received the treatment, meaning only one-in-seven stroke patients who needed it got it. Let's think about that. Imagine seven people in a waiting room, all needing the same life-saving treatment. Six of them will leave without it. If 1,000 Scots a year had a heart attack and only one in seven received the life-saving treatment in time, the front pages would be screaming. Advertisement Hide Ad Advertisement Hide Ad If 1,000 Scots needed emergency surgery and only one in seven got it, parliament would demand action. So why hasn't this changed yet for stroke patients? The NHS was founded on the principle of care at the point of need. Yet for many stroke victims in Scotland, that principle isn't being met. Cross-party support This campaign isn't about party politics. It's about patients. It's about fairness. It's about life and death. Over the past year, our campaign for 24/7 thrombectomy has gained cross-party support, from Labour, the Conservatives, the Liberal Democrats, and the Greens. There is a rare political consensus. A chance for Scotland to lead, not fall further behind. Now is the time for the Scottish Government to act, and with cross-party backing, I believe real progress is possible. I understand that 24/7 delivery can't happen overnight. It will take more trained specialists, interventional radiologists, and better coordination across services. But the first step is political will. I'm not asking for full delivery tomorrow. I'm asking for a commitment: that by the end of the next parliament, every Scot who needs a thrombectomy can get one: no matter the day or time. Scotland must not lag behind. Your chances of surviving a stroke shouldn't depend on where you live or what time it is.

Doctors warn of 'risk of death' from penis Botox and filler injections

Health professionals have warned of the potentially lethal danger of penis Botox and filler injections amid an increase in men seeking procedures in Scotland. Medics have warned of a 'growing number' of unregulated 'pop-up' clinics offering men treatments for erectile dysfunction or to increase the size of their genitals. The procedures see Botox, which relaxes muscles, or fillers, which can increase size, injected into the patient's penis. Botched cases have resulted in men having their pensies amputated, a doctor has told STV News. Medics warned of pop-up clinics with unlicensed practitioners operating outside the bounds of medical safety, using unapproved treatments and deceptive social media marketing tactics. 'Lifelong problems' Dr Ben Taylor-Davies, who runs the Stockbridge Clinic in Edinburgh and also works as an A&E doctor, said the procedures, even when carried out by an experienced surgeon, carry risks including necrosis or death. 'These are not treatments I'd ever recommend any patient undertake and if they were considering them, seeing an experienced urological surgeon would be vital', he told STV News. 'This is the latest example of why urgent regulation of the surgical and non-surgical aesthetics industry is needed in order to protect the public from harm. 'There are a number of significant risks involved with injections to this region, including infection, allergy, necrosis (dying of tissue) and even death.' Dr Darren McKeown, a plastic surgeon, told STV News that providers on social media platforms such as TikTok are taking advantage of 'common insecurities'. 'Penis size is a concern for many men', he said. 'Research shows that most men underestimate the size of their penis and worry that it is smaller than it is. It's a very common insecurity. 'Unfortunately, there are a growing number of clinics popping up offering quick fixes by injecting fillers into the penis to make it thicker. Some are even injecting Botox to make the penis hang lower when flaccid.' Dr McKeown has warned that while the procedures may seem 'logical and straightforward', they can cause lifelong problems. 'There are many videos on social media, particularly TikTok that make this look like a quick and easy fix, but the reality is very different' Dr Darren McKeown He has even warned of rare cases where men have had to have their genitals amputated. 'They are plagued with problems, including infections, product reactions, and lumps, and many men are left desperately embarrassed and in need of corrective surgery', he added. 'Even after corrective surgery, however, some men can be left with lifelong problems, and in rare cases, some have even needed to have their penis amputated. 'I would strongly urge any man considering cosmetic procedures to see a specialist urological surgeon for advice. 'Most urologists advise against these types of procedures because of the high risk of complications and the poor chances of patient satisfaction. You should not be treated in a pop-up clinic and you should not be treated by someone who is not a specialist, or worse still, someone with no medical qualifications whatsoever. 'There are many videos on social media, particularly TikTok, that make this look like a quick and easy fix, but the reality is very different, and by the time many men are finding this out, it's too late, and they have to live with life-changing injuries.' 'Seek a professional' It is not the first dangerous trend offered by unregulated pop-up clinics offering cosmetic procedures. In May last year, a woman had to be taken to hospital after undergoing a Brazilian Butt Lift in a Glasgow hotel. Thousands of pounds of unlicensed and unprescribed products used in cosmetic injections were seized by authorities from a warehouse in the city in a separate incident. Healthcare professionals previously warned that Scotland was becoming the 'worst country in Europe' for unqualified beauticians injecting customers with cosmetic treatments due to the lack of legislation preventing anyone from advertising on social media offering beauty treatments. Currently in Scotland, under-18s are not banned from receiving these treatments, unlike in England, where stricter regulations have been implemented. Last month, the Scottish Government revealed proposals to crack down on 'cowboy' Botox, fillers and cosmetic surgery procedures. The plans mean dermal fillers and the use of Botox will have to be supervised by a qualified healthcare professional, while 'breast and buttock augmentations' must be carried out by a qualified professional. Improvements in the safety and standards in the non-surgical cosmetic procedures sector have been set out, with a minimum age of 18 to be introduced for all procedures. Healthcare Improvement Scotland has urged anyone seeking cosmetic procedures to put 'their care into the hands of professionals'. 'Private clinics provided by doctors, nurses, pharmacists, dental care professionals, midwives or pharmacy technicians are required to be registered with our organisation,' a spokesperson said. There are over 500 registered and regulated clinics across Scotland, the vast majority providing cosmetic procedures. We do not regulate on the basis of particular treatments. 'Having a cosmetic treatment is a serious undertaking that should only be entered into with due consideration of the very real risks to an individual's health and safety. 'We would urge anyone looking to get cosmetic interventions to think about their health and wellbeing, and put their care into the hands of professionals with appropriate training and experience, and where they can trust them to make their wellbeing a priority. 'People should always check that a clinic is registered with Healthcare Improvement Scotland before undertaking treatments.' The Scottish Government has been contacted for comment.
