A first at KGMU: WLL performed to treat rare lung disease

LUCKNOW The department of respiratory medicine at Lucknow's King George's Medical University (KGMU) successfully performed whole lung lavage (WLL), a rare and complex procedure, to treat a 45-year-old patient diagnosed with Pulmonary Alveolar Proteinosis (PAP), marking a first in the institution's history. Patient Anirudh Maurya, a resident of Basti, showed severe respiratory distress and was later diagnosed with PAP, a rare lung disorder in which proteinaceous material accumulates within the lung alveoli, KGMU authorities in a presser on Thursday.
He was admitted to unit 2 with breathlessness, dry cough and high oxygen requirement of nearly 15 litres per minute under the care of Dr SK Verma, Dr Rajiv Garg and Dr Anand Srivastava of the department. The patient's occupational history revealed several years of exposure to cement and stone-blasting environments, a potential risk factor for certain lung conditions.
The procedure was cashless and covered under the government of India's Ayushman Bharat scheme.
Initial treatment was started with high flow nasal oxygenation (HFNO) and supportive care, while a comprehensive diagnostic workup was initiated. Routine blood investigations and a chest X-ray revealed diffuse bilateral lung involvement. To further investigate, a high-resolution CT (HRCT) chest scan was done, which showed 'ground-glass opacities' with a classic 'crazy-paving' pattern, raising suspicion of PAP, said authorities.
The patient underwent a bronchoalveolar lavage (BAL), which validated the presence of PAP. Given the rarity and severity of the disease, a multidisciplinary team was constituted to perform WLL, currently the only established treatment for this condition. The major challenge for the team was to adequately ventilate the lung during the procedure as both the lungs of the patient were extensively involved with the disease. The procedure was conducted in two stages, right lung lavage on June 13 and left lung lavage on July 7.
The procedure was performed by a joint team comprising, Dr Rajiv Garg and Dr Anand Srivastava, along with their resident doctors. Dr Shefali Gautham from the department of anaesthesia and her team members provided vital support.
The patient responded well to both procedures and was taken off the ventilator within 12 hours during both the settings. The patient is now stable and off oxygen support, marking a remarkable recovery, they added..
Vice-chancellor Soniya Nityanand congratulated the team for their outstanding achievement and commitment to patient care, calling it 'a proud moment for the university and a new chapter in KGMU's excellence in tertiary medical care.'
WHAT IS PAP?
* Pulmonary Alveolar Proteinosis (PAP) is a rare lung disease in which a thick, sticky substance made of proteins and fats builds up inside the tiny air sacs (alveoli) of the lungs.
* This makes it difficult for oxygen to pass into the blood, leading to breathlessness, cough, and low oxygen levels.
* The exact cause is often unknown, but it can sometimes be linked to workplace exposure to dust or chemicals. PAP is uncommon and requires specialized diagnosis and treatment, such as Whole Lung Lavage, a procedure that 'washes out' the lungs to remove the build-up.

New Delhi: The Delhi government is set to inaugurate 34 new Ayushman Aarogya Mandirs across the city by next week, bringing the total number of such centres to 67, officials said on Tuesday. There are presently 33 Ayushman Aarogya Mandirs operational across Delhi aiming to provide holistic, accessible and affordable primary healthcare. The addition of 34 more centres this week will mark the second phase of the rollout. Delhi Health Minister Pankaj Singh told PTI, "We will inaugurate 34 more Ayushman Aarogya Mandirs this week. In the third phase of the rollout, the total number will rise to 80." He emphasised the initiative reflects the government's strong commitment to strengthening the healthcare system and ensuring better services for Delhi residents. The initiative is being positioned as a corrective step toward bridging healthcare gaps that the government claims were neglected by previous administrations. "These are not just buildings. They are centres of primary care and prevention - areas that were lacking in the previous government's approach," Singh said. The district-wise distribution includes around five new centres in central Delhi, four in east Delhi, and several others across north east, north west, south east and west Delhi. Most of these centres are being established by upgrading existing mohalla clinics, dispensaries, and polyclinics. Delhi Chief Minister Rekha Gupta and her Cabinet inaugurated 33 Ayushman Aarogya Mandirs and 17 Jan Aushadhi Kendras across the city last month. The government has set an ambitious target of establishing over 1,100 such clinics across the capital. Ayushman Aarogya Mandirs are a component of the Union government's Ayushman Bharat initiative . After the BJP came to power in Delhi, the state government signed a memorandum of understanding (MoU) with the Centre earlier this month to implement the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB-PMJAY) in the capital -- fulfilling a major poll promise of the party. Additionally, Rs 320 crore has been proposed for the expansion of Health and Wellness Centres and Ayushman Aarogya Mandirs to strengthen primary healthcare services through the establishment of 400 new HWCs and AAMs.

Lucknow: Uttar Pradesh Chief Minister Yogi Adityanath on Monday said that the state's health services must be future-ready to keep pace with global advancements and be able to respond effectively to emerging challenges. He was speaking at the foundation stone laying and inauguration ceremony of various health projects worth Rs 1,000 crore at King George's Medical University (KGMU) in Lucknow. Addressing the event, which was also attended by Deputy Chief Minister and Health Minister Brajesh Pathak and Medical Education Minister Mayankeshwar Sharan Singh, the Yogi said KGMU has consistently set milestones in its 120-year journey and remains a prestigious institution in India. "An institution or an individual's success depends on how well they adapt to the pace of time. Those who prepare in advance progress. Those who fail to do so face decline," Adityanath said. He said KGMU and Sanjay Gandhi Post Graduate Institute of Medical Sciences (SGPGI) have become trusted centres for patients not just from across Uttar Pradesh but also neighbouring states and Nepal. Adityanath emphasised that people have faith in KGMU and SGPGI because they are confident of receiving quality treatment. "Even patients themselves expressed strong belief in their recovery, which shows how positive thinking can improve health outcomes," he said, recounting recent interactions with patients. The CM also highlighted the importance of technology in healthcare, announcing efforts for tie up in a new medical sciences initiative taken by IIT Kanpur with a Centre of Excellence. "We are working towards having a collaboration between the Centre of Excellence in Medical Technology in IIT Kanpur with institutions like KGMU and SGPGI that will be linked to it to prepare for global healthcare challenges," he said. The CM underlined the fast-paced expansion of medical education in the state. "In 1947, we had very few government medical colleges. But last year alone, admissions were made in 17 colleges -- 13 of which were government-run, three PPP-mode, and one private. Today, we are moving towards one medical college in each of UP's 75 districts," he said. Adityanath added that going forward, district medical colleges would conduct initial patient screenings to decongest tertiary centres like KGMU. Only those who genuinely require specialist care would be referred to Lucknow. Reflecting on India's health progress, he said, "From being the 11th largest economy, India is now the fourth. Earlier, there was only one AIIMS in the country. Today, several AIIMS have been established, which also focus on research and development." Calling AIIMS a symbol of excellence not just in healthcare but also innovation, the CM noted that KGMU has similarly carved a new identity by embracing progress, making its 120-year legacy even more glorious. Adityanath also praised the double-engine government model (BJP in Centre and in UP) for accelerating health infrastructure across the state and urged doctors and institutions to treat healthcare expansion as a mission, not just a service. Lucknow (Uttar Pradesh) [India], July 15 (ANI): Uttar Pradesh Chief Minister Yogi Adityanath on Tuesday stated that investment proposals worth Rs 15 crore had come into the state, which would provide new opportunities for the people. Speaking at the inauguration of 'Kaushal Vikas Mela', the CM said that the investments had come into the state due to the improved law and order in the last eight years. 'You must have felt in the last 8 years that when law and order are good, and the government's intentions are right, you see a flood of investments in no time. Investment proposals worth more than Rs 15 lakh crore have come into Uttar Pradesh, and these investment proposals will provide new opportunities for the youth in the state...' the CM said while speaking at the inauguration. He further stated that under the double-engine government led by Prime Minister Narendra Modi, efforts are being made to enhance the skills of the youth of the state. 'Under the double engine government, we have been able to help sharpen the skills of the youth... we have worked in accordance with the demands of the industry and the is neither any shortage of work or human resources in the state...' the CM further stated. Earlier in the day, the CM laid the foundation stone for various health projects at King George Medical University and said that the time has come to provide medical facilities to areas outside the metropolitan facilities. Speaking on the occasion on Monday, CM Yogi said, 'Disease is neutral. Wherever there is a gap, it will attack and engulf you. We used to see the same situation in Uttar Pradesh when people used to get easily affected by the disease. But now, as you have seen, it has been raining for the last month. Yet there is no such situation of any communicable infection anywhere.' 'We had permitted KGMU to set up a satellite centre in Balrampur. They will start a medical college in this or the next session. The time has come to provide medical facilities to areas outside the metropolitan facilities. Today in nursing and paramedical, new Nursing colleges are being set up in UP,' CM Yogi emphasised. (ANI)
