
Doctor shares 1-month plan to support thyroid function through diet and lifestyle

Gluten intolerance may not damage your gut but it could still mess with your brain: What doctors want you to know
Celiac disease, traditionally considered an autoimmune disease largely localised in the gastrointestinal tract, is an underappreciated simultaneous brain disorder. Neurological or psychiatric symptoms may affect about 22% of patients and may occur at the same time as or even precede digestive complaints. In an interview with HT Lifestyle, Dr Rohit Pai, Consultant Neurologist at KMC Hospital in Mangalore's Dr BR Ambedkar Circle, said, 'Gluten ataxia is the most obvious condition and it is thought to be cerebellar through anti-gliadin antibody. It is frequently marked by slurring speech and loss of limb coordination when walking.' He revealed, 'Epilepsy may develop in a minority of cases, usually related to occipital lobe calcification, following visual symptoms. These cognitive brain fog/memory loss and (occasionally) progressive dementia symptoms, when combined with other non-cognitive neurological complications constitute an additional significant burden of celiac disease on the brain.' Bringing his expertise to the same, Dr Amrut SD, Consultant Neurologist at Manipal Hospital in Goa, noted that peripheral neuropathy occurs in about half of celiacs. He shared, 'This tends to be manifest as distal sensory neuropathy, with symptoms of tingling, numbness, or burning in the hands and feet. Gluten sensitivity is an umbrella term encompassing celiac disease, non-celiac gluten sensitivity (NCGS) and wheat allergy.' Dr Amrut SD added, 'In particular, NCGS can 'mimic' multiple neurologic phenomena seen in celiac disease despite lack of autoantibodies or enteropathy. Patients may complain of unexplained chronic fatigue, ataxia, or peripheral neuropathy too, all being related to an altered gut-brain axis. The diagnosis of NCGS is clinical and primarily a diagnosis of exclusion that needs very thorough exclusion of celiac disease, wheat allergy and others.' Dr Meghna Potluri, Consultant - Allergy and Immunology, discussed the immuno-pathophysiology of gluten-related disorders. She highlighted, 'Wheat, barley and rye contain gluten, a protein that may cause an immune response in people who are sensitive to it. In celiac disease, this can also damage the small intestine (but wiring) and can even extend to neural tissue.' The expert informed, 'Specific antibodies to test for include IgA-tTG, EMA and DGP, and genetic testing to determine HLA-DQ2 or DQ8 status is beneficial in confirming the diagnosis. For those patients already on a gluten-free diet, these markers may not be sensitive and upper GI endoscopy with biopsy will need to be considered.' Non-celiac gluten sensitivity, on the other hand, does not include the immune-mediated gut damage that characterises celiac disease. Dr Meghna Potluri said, 'Instead, it can cause symptoms that go on to include nausea, bloating, headache, joint pain and neurocognitive impairment. The diagnosis is established when celiac disease and wheat allergy are excluded, usually by a combination of antibody tests and a clinical assessment.' Celiac disease as well as NCGS is currently treated most effectively with a gluten-free diet. Dr Meghna Potluri said, 'In celiac disease, strict, lifelong adherence is required to avoid complications. NCGS might be associated with a continuum of dietary response related to symptom intensity. Individuals should be encouraged to eat whole, gluten-free foods, including fresh fruits and vegetables, dairy and unprocessed meat and avoid foods made with gluten-containing grain, and if needed, high-FODMAP foods.' The prompt recognition of gluten-related neurological syndromes and an interdisciplinary approach among neurologists, gastroenterologists and immunologists are mandatory. Correct diagnosis, followed by individualised dietary management, markedly improves patient's quality of life. Stress and other surprising causes of bloating: Doctors share gut health tips that could change your life
Feeling bloated or gassy is a frequent cause for complaint and many people think they have overdone it on a meal or suspect they have a touch of indigestion. While these are common suspects —particularly in those that eat a high amount of beans, lentils, cabbage, broccoli, cauliflower, sprouts or dairy —we have to consider a wider and sometimes more serious scope of reasons why you are feeling these symptoms. In an interview with HT Lifestyle, Dr Rajesh Bathini, Consultant – Gastroenterology at Manipal Hospital in Vijayawada, shared that while indigestion is what many people think of when they think of bloating, the problem is really more complicated. He said, 'Some foods (such as high-fiber vegetables and dairy products in those who are lactose intolerant) will provoke bloating, yes. Other gastrointestinal diseases, in particular, peptic ulcer disease, medically caused by Helicobacter pylori infection, GERD and IBS may also be involved.' He added, 'Systemic diseases, such as unstable diabetes, thyroid or medication complications (aspirin or iron supplements) also interfere with the clinical view and conditions such as heart failure or heart attacks may, occasionally, have symptoms that are more related to just abdominal bloating. Symptom onset can rarely be an indicator of underlying gastrointestinal cancers, such as esophageal, gastric or pancreatic, when the presentation is limited to a mere bloated sensation prior to the progression to more specific manifestations.' Bringing his expertise to the same, Dr Gyanaranjan Rout, Consultant – Medical Gastroenterologist in Bhubaneswar, pointed out that while dietary factors such as overeating, chewing gum or drinking carbonated beverages are common triggers, chronic bloating could be a symptom of a larger issue. He revealed, 'Bloating could also be a sign of IBS, lactose intolerance, celiac disease or SIBO. Hormonal imbalances, especially in women, is another reason that may cause one to bloat. Less frequently acknowledged is the contribution of psychological stress and anxiety that can cause distress to affect the gastrointestinal physiology and symptoms.' According to Dr Anurag Shetty, Consultant – Medical Gastroenterology at KMC Hospital in Mangalore's Dr BR Ambedkar Circle, gas and bloating are two issues commonly perceived as nuisances connected to diet and are a result of a complicated web of body mechanics. He explained, 'Gut-brain axis dysfunction in IBS stimulates visceral hypersensitivity, so that normal gas volumes feel inappropriately unpleasant.' Dr Anurag Shetty elaborated, 'Malabsorptive conditions, such as lactose intolerance, can ferment poorly digested food which can give rise to more gas. Furthermore, disrupted gut motility as observed in chronic constipation can lead to fermentation and bloating. Hormonal changes such as those associated with menstruation or menopause very often cause the women to retain water and feel bloated.' Dr Rajesh Bathini advised that any patient with new or worsening symptoms — particularly with warning signs such as unplanned weight loss, fatigue, loss of appetite or anemia — should promptly seek medical care. Dr Gyanaranjan Rout suggested, 'Changes in lifestyle, including keeping a food diary, drinking plenty of water, physical exercise and stress management can identify triggers and offer relief but if they do not help, you may need to consult your doctor to make sure a more serious underlying condition is not responsible for your pain.' Abdominal bloating can be quite harmless but if it persists or you are experiencing more unpleasant symptoms, then don't ignore it. However, the key is knowing when symptoms point to something more serious. This one change in your night routine could dramatically lower your cancer risk
Cancer specialists are increasingly recognising that modern lifestyle habits — especially those affecting sleep — can influence long-term cancer risk. One of the most overlooked threats is the disruption of our body's internal clock due to artificial light, particularly the blue light emitted from digital screens. In an interview with HT Lifestyle, Dr Kiran Kamalasanan, consultant, surgical oncology at Apollo Cancer Centre in Guwahati, explained, 'Exposure to blue light late in the evening tricks the brain into thinking it's still daytime. This delays the release of melatonin — a hormone crucial not just for sleep quality but also for regulating cellular health and immune defense.' Studies suggest that melatonin helps suppress tumour growth and when its levels are reduced consistently, it may create a biological environment where cancer is more likely to develop. Epidemiological evidence links chronic circadian disruption, such as in night shift workers, with higher rates of breast, prostate and colorectal cancers. In fact, The International Agency for Research on Cancer (IARC) has identified night shift work involving circadian rhythm disruption as probably carcinogenic to humans. Dr Kamalasanan revealed, 'Our circadian rhythm is a master regulator—coordinating DNA repair, hormone release, and cell division. Disturbing this rhythm can trigger inflammation, reduce the effectiveness of cellular repair mechanisms and impair the body's ability to detect and eliminate abnormal cells.' Bringing her expertise to the same, Dr Ritika Hinduja, consultant, radiation oncology at PD Hinduja Hospital and Medical Research Centre in Mumbai's Mahim, shared, 'Poor sleep quality is a concerning public health issue. Circadian rhythm or the biological clock is the 24-hour internal clock in our brain that regulates cycles of alertness and sleepiness by responding to light changes in our environment. This is controlled by the hormone melatonin, produced by pineal gland in the brain.' Dr Ritika Hinduja pointed out, 'Disruptions in the body's 'biological clock', which controls sleep and thousands of other functions, may raise the odds of cancers of the breast, colon, ovaries and prostate. Exposure to light while working overnight shifts for several years may reduce levels of melatonin, encouraging cancer to grow.' Sleep deprivation also affects our immune system indicative of body being in inflammation. Dr Hinduja highlighted, 'Although vital for normal health, several decades of research have firmly implicated inflammation in the development and progression of cancer.' The English longitudinal study of ageing has assessed the relation between sleep quality and risk of cancer. Having studied over 10000 individuals, they concluded that poor sleep quality is positively associated with the long-term risk of developing cancer in an elderly cohort. Dr Hinduja concluded, 'Both medical staff and the general public should pay more attention to improving sleep hygiene. Cancer and sleep are intertwined in more than a single way. They emulate a chicken and egg phenomenon. Often, cancer survivors suffer from sleep problems, caused by psychological stress and recently proven, owing to the cancer and its treatment too.' Dr Kamalasanan suggested, 'Promoting healthy sleep habits — like reducing screen exposure at night, dimming lights after sunset, and following a regular sleep schedule — is not just good for mental clarity or energy; it may be a critical component in lowering cancer risk. Good sleep is not optional. It's one of the body's strongest and most natural, lines of defense.'
