
Opioids: Parasitic Worm Discovery Could Lead to Safer Painkillers

The evolutionary trick of parasitic worm that sneakily burrows into the skin of its victims could provide new alternatives to opioid-based pain treatments.
Opioids are currently used to treat moderate to severe pain, as well coughing and diarrhea. Although they can provide effective relief, long-term use can also lead to dependence, with negative physical and mental health consequences.
Researchers discovered that the worm—Schistosoma mansoni—suppresses neurons in the skin to evade detection in a mechanism likely developed to enhance its own survival.
The molecules responsible for this suppression of sensation could help in the development of new painkillers, according to the team at the Tulane School of Medicine.
Schistosomiasis is a parasitic infection caused by helminths, a type of worm known to survive in their mammal hosts for many years. Their larvae can infest freshwater bodies like lakes, ponds and rivers—infecting people who, for example, swim, fish or wash clothes in the water.
"While bacteria often cause painful cutaneous reactions, host skin invasion by the human parasitic helminth Schistosoma mansoni often goes unnoticed," the researchers wrote in their paper.

S. mansoni often evades detection by the immune system, unlike other bacteria or parasites that cause pain, itching or rashes—a feat the team set out to investigate.
After conducting experiments on mice, the researchers found that S. mansoni produces molecules that suppress the activity of TRPV1+, a protein used to send signals that the brain interprets as heat, itching or pain.
As part of the pain-sensing process in sensory neurons, TRPV1+ regulates immune responses in scenarios including allergy, autoimmunity, cancer, infection and even hair growth, the researchers explained.
It is likely the parasitic worm evolved its ability to block TRPV1+ because it enhance its survival, the researchers said.
"If we identify and isolate the molecules used by helminths to block TRPV1+ activation, it may present a novel alternative to current opioid-based treatments for reducing pain," said paper author and immunologist professor De'Broski R. Herbert| in a statement.
"The molecules that block TRPV1+ could also be developed into therapeutics that reduce disease severity for individuals suffering from painful inflammatory conditions."

The study also revealed that TRPV1+ is necessary for initiating host protection against S. mansoni. Activation of the protein leads to rapid mobilization of immune cells, including gd T cells, monocytes and neutrophils, which induce inflammation.
This inflammation, the researchers explained, plays a crucial role in host resistance to the larval entry into the skin.
These findings highlight the importance of neurons that sense pain and itching in successful immune responses.
"Identifying the molecules in S. mansoni that block TRPV1+ could inform preventive treatments for schistosomiasis. We envision a topical agent which activates TRPV1+ to prevent infection from contaminated water for individuals at risk of acquiring S. mansoni," said Herbert.
In the study, mice were infected with S. mansoni and evaluated for their sensitivity to pain as well as the role of TRPV1+ in preventing infection.
"S. mansoni infection significantly reduced thermal pain sensitivity evoked by TRPV1+ neurons. Consistently, isolated skin sensory neurons from infected mice had significantly reduced calcium influx and neuropeptide [chemical messenger] release in response to the TRPV1 agonist capsaicin [an irritant for humans and mammals] compared to neurons from naïve controls," the study authors wrote.
"Using gain-and loss-of-function approaches to test whether TRPV1+ neurons initiate host-protective responses revealed that TRPV1+ neurons limit S. mansoni skin entry and migration into the pulmonary tract."
The researchers are now planning to identify the nature of the secreted or surface-associated helminth molecules that are responsible for blocking TRPV1+ activity and specific gd T cell subsets that are responsible for immune responses. They also seek to further understand the neurons that helminths have evolved to suppress.

"This work highlights how a mother's health prior to pregnancy, not just during gestation, can shape her child's brain development in profound ways," said paper author and epigeneticist professor Alika K. Maunakea in a statement. "We were surprised to find that even without direct maternal contact after conception, these epigenetic imprints from the egg carried enough weight to alter behavior." To isolate the effects of pre-pregnancy obesity away from those during gestation, the time between conception and birth, the researchers used an in vitro fertilization (IVF) and embryo transfer model. This approach allowed them to study early epigenetic programming more precisely. Behavioral assessments of adolescent male mice revealed impaired social interactions and repetitive behaviors. These traits, which mirror characteristics of ASD, were linked to altered gene regulation in the brain, the researchers explained. "By leveraging our institute's expertise in developmental biology, reproductive science and epigenetics we are beginning to understand how early-life programming can ripple through generations," said paper author and reproductive biologist professor Monika Ward in a statement. The researchers say the findings may lead to early interventions, potentially even before conception. However, they hope future studies will explore nutritional or pharmacological therapeutic strategies that may reverse or mitigate these effects. There is no single known 'cause' for autism, but research suggests it could develop from a combination of genetic and environmental factors. "Brain scans show differences in the shape and structure of the brain in people with autism compared to in neurotypical development. Researchers do not know the exact cause of autism but are investigating a number of theories, including the links among heredity, genetics, and medical problems," the Autism Society website outlines. Map Shows States With Most HPV-Related Cancers as 1 in 3 Unaware of Link

People in some U.S. states are more likely to be diagnosed with HPV-associated cancers than in others—with this discrepancy likely linked to awareness of the risk from the virus. Overall, one in three adults nationally are unaware of the connection between HPV and cancer, reveals a study from the MUSC Hollings Cancer Center in South Carolina. HPV, or the human papillomavirus, is spread by skin-to-skin contact and has been linked to the onset of six different types of cancer. It is responsible for almost all cases of cervical cancers, causes the majority of throat or 'oropharyngeal' cancers and can cause anal, penile, vaginal and vulvar cancers. "Awareness of HPV in the general population is an important determinant of preventative care uptake," MUSC researcher Kalyani Sonawane told Newsweek. Such measures, she explained, include the HPV vaccine and cervical cancers screening. "Achieving adequate HPV vaccine coverage is critical for eliminating these cancers." State-level data from the Centers for Disease Control and Prevention (CDC)—see map above—shows geographical differences in the diagnoses of HPV-associated cancers. At joint top of the list, Kentucky and West Virginia recorded 16.9 HPV-associated cancer cases per 100,000 residents in 2022 (the year with the most recent available data.) However, at the bottom of the list, North Dakota recorded just 8.3 HPV-associated cancer cases per 100,000 residents in the same year. According to the new study, around a third of Americans are not aware of either HPV or that a vaccine exists against it—despite this protection having been available for nearly two decades in the U.S. This lack of awareness is heightened in the Midwest and South, the researchers noted. For example, more than 40 percent of people in 13 states—including South Carolina—are unaware there is a vaccine against HPV. "In general, literacy levels (overall and health literacy) are lower in the U.S. South and Midwest, which is likely a reason why we found a lack of HPV awareness concentrated in states in these regions," explained Sonawane. The team used data from the Health Information National Trends Survey (HINTS), a survey conducted by the National Cancer Institute, to inform their research. Sonawane said that the CDC data "further highlights the need for increasing HPV awareness in Southern and Midwestern states"—where cases of HPV-related cancer also tend to be more common. The vaccine is the "first and best defense" against HPV-caused cancers, the researchers said. It is routinely recommended for children aged 11–12 years (and can be started as early as nine years). Catch-up vaccination is recommended through age 26. Additionally, adults aged 27–45 might be able to receive the vaccine after consultation with their healthcare provider. Cervical cancer may show up when people are in their 30s or 40s, but oropharyngeal cancers are mostly diagnosed when people are in their 50s or 60s, the researchers warn. Sonawane noted that the biggest knowledge gap is in relation to oropharyngeal cancer. Across the nation, 70 percent of those who have heard about HPV did not know that it can lead to throat cancer. The vaccine is now approved for boys too, but their vaccination rates remain lower than among girls. "The HPV vaccine was first approved for the prevention of cervical cancer in women and was therefore heavily marketed toward women. This 'feminization' of the HPV vaccine continues to linger—and is likely a reason why most people have heard of HPV and cervical cancer, but not of its link with oral cancers," Sonawane explained. "Boys and men can also get HPV-associated oral, penile, and anal cancers; therefore, getting the HPV vaccine is equally important for males." While the pandemic interrupted measures of preventive care—including the HPV vaccination and screening for breast, cervical and colon cancers—an American Cancer Society report showed screening for breast and colon cancers has since rebounded. However, both cervical cancer screening—which is not a test for cancer, but a test to help prevent cancer—and HPV vaccination rates have not similarly recovered. "Prevention is always better than a cure. We have a safe and effective vaccine to prevent six cancers," Sonawane said. She concluded: "Going forward, we need to ensure that every child and eligible adult is protected from HPV-related cancers by ensuring they are offered the opportunity—i.e., the HPV vaccine—for preventing these cancers."
