
Budget 2025 Fails Rural Health Promises – Hauora Taiwhenua Warns Of Growing Inequities
'For a National-led Government, we would expect the word rural to appear somewhere in the health budget. Instead, rural New Zealand seems to have been sidelined once again,' says Dr Grant Davidson, Chief Executive of Hauora Taiwhenua.
The Government has also failed to act on the flagship rural health policy it entered the last election on and reiterated in its first 100-day plan once elected: the establishment of a third medical school at the University of Waikato focussing on rural general practice. In budget 2025 there is no investment or plan outlined for increasing rural medical placements, either through existing institutions or by establishing a new school. This inaction comes at a time when the rural workforce is in crisis and struggling to meet the needs of growing, ageing, and often underserved populations. We looked forward to congratulating a Government on implementing long-term planning for the rural health workforce but have been sadly let down.
The health budget has increased by 4.8% overall which doesn't cover increases in the cost of health care delivery and population gains. The Budget lacks any specific commitment to address long-standing inequities in rural healthcare. We are waiting to see what capitation uplift will be offered, but early signs are that this is likely to be less than the Health New Zealand calculated increase in the cost of health services of 6.4% which will mean any gap will need to be made up by further increases to patient co-payments, which will need to be well above 6.4%. Any increased fees will only decrease access to general practice to those who most need it and then cause a further decline in already poor health outcomes in rural areas where there are higher populations of low-income, older and Māori populations. The funding for Comprehensive Primary and Community Care (CPCT) Teams, which allowed extra funding for general practices to add allied health and healthcare workers to their teams to improve services, has also been discontinued.
Hopes that Budget 2025 would deliver pay parity for primary care nurses have also been dashed. Instead, legislative changes mean any progress will now be delayed at best. Primary care nurses, especially those working in rural settings, continue to be paid significantly less than their hospital counterparts, worsening recruitment and retention challenges.
Hauora Taiwhenua is also raising concerns about the proposed transfer of mental health crisis response responsibilities from Police to health services. Funding to support the transfer of responsibility of this from Police is included in the budget. This may offer long-term benefits in urban centres, but it raises serious questions for rural areas, where health teams are already stretched and crisis response capacity is limited or non-existent. We are waiting to see if any resourcing or rural contingency planning will be announced to support this shift in the implementation phase.
We acknowledge the pre-budget announcements to enact a Primary Care Action Plan, including funding for urgent and unplanned care initiatives. We are excited by the potential to finally deal with the major issue of providing after-hours and urgent care in rural and remote areas, including surges in demand during holiday times. But there is little detail around these and no ring-fenced funding for implementing them in rural areas.
The Minister's commitment to what he terms 'enhanced capitation funding' is for the delivery of extra services and targets that have not yet been announced, not for the support of already underfunded rural primary care. We understand that Health New Zealand's own analysis shows that delivery of primary care in rural and remote areas costs more than in urban areas, and yet the project to re-weight capitation formulas to provide increased base payments for those in rural areas where increased costs and high co-morbidity demands lie has been put on hold.
There is still some possibility that Health New Zealand may choose to commit some of its increased funding in operational spending to improve the sustainability of under-siege community-owned rural hospitals and interprofessional rural training hubs that are in the Health NZ workforce plan, but we have been holding our breath for over a year already on both of these projects.
'Budget 2025 was an opportunity to demonstrate that rural Aotearoa matters. Instead, it sends a message that the promises made before the election are not being honoured,' says Dr Davidson. 'Without urgent action, we risk further erosion of rural health services and deepening inequities for the communities who already struggle most to access care.'
Hauora Taiwhenua Chair, Dr Fiona Bolden summarises, 'This is a budget which will leave rural health providers and rural communities feeling even more despairing than before when it comes to the sustainability/viability of rural health services and the safety of patients.'
Hauora Taiwhenua will continue to work with the Government, policymakers, Agency staff, rural providers and the community to ensure that the voices of rural New Zealand are heard — and that future decisions reflect their needs and realities.
'We know that delivering healthcare in rural and remote areas comes with unique challenges and opportunities,' says Dr Davidson. 'Our members are ready to work with Government and Health New Zealand to co-design solutions that ensure rural communities receive equitable, high-quality care.'

Dental Advocacy Group Launches Nationwide Roadshow To Overhaul How We Treat Teeth
The dental advocacy group Dental for All today launches a nationwide roadshow to call for an overhaul of our approach to treating teeth. Dental for All - backed by a coalition of dentists and oral health workers, trade unions, and poverty action groups - is pressing for free, universal dental care, delivered consistently with Te Tiriti o Waitangi. The roadshow, beginning in Wellington and Porirua, will see at least 25 events hosted across the country, from Whangārei (and further north) to Invercargill. The first leg of the roadshow between Wellington and Rotorua includes town hall-type meetings in Porirua, Wellington, Palmerston North, Whanganui, Hamilton, and Rotorua. This first leg will see visits to universities, and events in other venues, such as schools and community markets. The second and third legs will also include free dental days. 'This is about the step-change we need in how we treat oral health in this country,' says Hana Pilkinton-Ching, Dental for All campaigner and roadshow organiser. The 2022-2023 New Zealand Health Survey found that nearly half of adults (44%) experienced unmet need for dental care due to cost in the past 12 months. 'It makes no sense that we carve our teeth out from the rest of our body, and it's costing all of us, in lost productivity, downstream health effects, and reduced quality of life,' adds Pilkinton-Ching. Research previously commissioned by Dental for All, produced by FrankAdvice using a Treasury tool, found that excluding dental from the public healthcare system is costing the country $2.5bn in lost productivity and $3.1 billion in reduced quality of life. The cost of free dental for all has been estimated at $1.5bn annually. 'In my clinical work, I see how untreated oral health problems can worsen into serious health problems, and can lead to a loss of self-esteem and confidence, which affects how well people can participate in society,' says Samuel Carrington, an oral health therapist and academic, who is a member of the Dental for All coalition. Dental for All has also produced qualitative research documenting experiences within the oral healthcare system, showing the limitations of existing available dental care for adults, such as the means-tested $1000 Work & Income grant for essential treatment. 'It's time to treat our teeth how we treat the rest of our essential healthcare, and to uphold Te Tiriti o Waitangi in how we do it,' says Carrington. Dental for All is seeking to establish local organising groups to build nationwide pressure for Dental for All. Polling has previously showed more than 7 out of 10 people in New Zealand support making dental free. Dental for All also continues to develop policy work that will be released in early 2026 on Dental for All models. One viable model is to establish an adult community dental service, made up of a network of community clinics with salaried dentists and oral health therapists, alongside Māori-led clinics (such as through iwi and hapū). The roadshow begins with an event with older New Zealanders in Wellington at lunchtime on Monday 11 August, followed by an oral health promotion afternoon event in Porirua and a panel discussion at Tireti Hall at 5.30pm on Monday 11th. An event follows at Te Tai Ohinga, 203 Willis St, at 6pm on Tuesday 12 August. Notes: The first leg of the roadshow begins in Wellington on Monday 11 August and concludes in Rotorua on Tuesday 19 August. The roadshow will also make visits to Palmerston North, Whanganui, New Plymouth, Hamilton, and Rotorua. The second leg of the roadshow begins in Auckland on Sunday 7 September, including a flagship event with Auckland City Missioner Helen Robinson on Thursday 11 September. It ends in Flaxmere on Tuesday 16 September. The third leg of the roadshow begins in Wānaka on Saturday 27 September and moves across the South Island, finishing in Nelson on Sunday 5 October. The Dental for All coalition is led by ActionStation campaigners, and includes the Association of Salaried Medical Specialists, Auckland Action Against Poverty, dentist Hugh Trengrove, Workers First Union, Te Ao Mārama (the Māori Dental Association), Downtown Community Ministry, and the Auckland City Mission.

Tauranga cancer survivor urges bowel screening as almost half of kits not returned
When broken down by ethnicity, fewer than half of the kits sent to Māori, Pasifika and Asian people have been returned – 41.9%, 41.8% and 33.1%. Ferguson said she got upset hearing these statistics because there were 'so many people under the age of 60 that are screaming out to have those kits'. 'They know they have something wrong and they're just not being heard so therefore they're not getting screened because doctors are either saying 'you're too young' or 'it's something else'. 'That's wasted resources.' She said the scary thing with bowel cancer was someone could have it and have no symptoms. This was the case with Tauranga man Paul Blears, who said it was 'a hell of a shock' being diagnosed with bowel cancer after he used his screening kit. 'By doing that test, you are catching things early … which will avoid late-stage diagnosis,' Ferguson said. The National Bowel Screening Programme was introduced in the Health NZ Bay of Plenty region in May 2022. It was the last of the former 20 district health boards to introduce it after the programme started in July 2017. People are invited to join the programme at age 60 then sent a free home test kit every two years until age 74. The Government is gradually lowering the eligibility age to 58, to be in effect nationwide by March. The Health NZ data showed 103 cancers were detected between January 1, 2023, and May 26, 2025, and 78 surgeries were carried out to remove bowel cancer. Health NZ noted this may include people who had surgery who did not have their cancer detected via a screening kit, and that some patients may have had surgery in another district, or privately. Patients who chose chemotherapy treatment would not be recorded in surgery numbers, and others newly diagnosed may be waiting for surgery. 'I feel great, which is the main thing,' said Ferguson, now aged 37. She gave birth to her son George in September 2022. 'He was such a good baby … he's really come into his own kind of personality now.' Ferguson said she had recently completed a reformer Pilates instructor course and would soon start teaching. She had also been doing paid partnerships on social media. With a background in early childhood education, she had done some relief teaching. 'After the diagnosis and also becoming a mum myself, I just felt like I was ready to leave that side of things. 'It's just been trying to find something else.' Bowel cancer is the second-highest cause of cancer death in New Zealand, killing about three Kiwis every day. One in 10 Kiwis diagnosed are under 50. New research by Bowel Cancer New Zealand showed one in four adults were unable to name a single symptom of bowel cancer, which Ferguson said was 'pretty concerning'. Bowel Cancer New Zealand chief executive Peter Huskinson said symptom awareness was the 'strongest line of defence', with screening currently limited to those aged 58 and older. 'Acting early, before symptoms become severe, can mean the difference between a treatable diagnosis at the GP and a late-stage emergency presentation. 'That free test in your mailbox doesn't just detect cancer – it can stop it before it starts,' he said. 'Don't leave it in a drawer. Put it by the loo and get it done.' New Report Reveals The Grim Reality Of Heart Healthcare In New Zealand
A damning new report has exposed deep flaws in New Zealand's heart healthcare system, where access to life-saving treatment often depends on luck, postcode, and ethnicity rather than clinical need. Commissioned by The Heart of Aotearoa - Kia Manawanui Trust and prepared by the University of Otago, the ' Heart disease in Aotearoa: morbidity, mortality, and service delivery' report reveals a system plagued by chronic under-resourcing, critical workforce shortages, and deadly inequities. Trust Chief Executive Ms Letitia Harding says the report lays bare a system that is failing at every level. "Heart care in New Zealand isn't just stretched - it's on the verge of collapse. "We are failing in all aspects, and it's costing New Zealanders their lives." Key findings from the report include: Life-threatening delays: Half of all heart attack patients aren't seen within internationally accepted timeframes. Alarming workforce shortage: New Zealand has only a third of the cardiologists it should have. Systemic inequality: Māori and Pacific people are hospitalised or die from heart disease more than a decade earlier, on average, than other New Zealanders. The financial burden: Heart disease costs the country's health system and economy $13.8 billion per year A postcode lottery for care: Regions with the highest death rates -Tairāwhiti, Lakes, Whanganui, and Taranaki - have the fewest cardiac specialists. The deadliest conditions: Heart disease accounts for 20% of all deaths in New Zealand. The five heart conditions responsible for the majority are: heart failure, atrial fibrillation and flutter (AFF), acute myocardial infarction (AMI), subsequent myocardial infarction and cardiomyopathy. Trust Medical Director Dr Sarah Fairley, who is also a Wellington-based cardiologist, says the findings match what frontline clinicians see every day. "From inside the system, I can tell you that this report reflects what we see every day: a workforce stretched beyond safe limits, patients slipping through the cracks, and no end in sight." Ms Harding says this report should be a wake-up call for the Government. The Heart of Aotearoa - Kia Manawanui Trust is calling for an urgent government response, including immediate investment in public hospital cardiac care infrastructure - beds and equipment - and a national strategy to recruit and retain cardiology staff.
