Does neurodegeneration start when blood vessels are damaged?
Our brain depends on a finely tuned network of neurons, signals, and protective barriers to function seamlessly. This intricate setup underpins every thought, memory, and movement we make. But as we age, or under certain conditions, this system can break down.
Neurodegenerative diseases like Alzheimer's, Parkinson's, and amyotrophic lateral sclerosis (ALS) slowly damage neurons and over time these conditions lead to severe memory loss, confusion, and loss of independence. Despite decades of research, the precise mechanisms driving these diseases have remained elusive.
Shifting away from the traditional neuron-centric view of brain diseases, two studies published recently in Science Advances and Nature Neuroscience, offer a compelling new piece to this puzzle. The teams' research reveals a startling possibility: what if the trouble begins long before neurons die?
The studies suggest that damage to the blood-brain barrier (BBB) may in fact be the first domino to fall in neurodegenerative diseases.
First line of defence
The BBB is one of the brain's most critical protections. It is made up of tightly packed endothelial cells that line blood vessels in the brain. Their job is to gatekeep: letting in vital nutrients while keeping out toxins, pathogens, and harmful immune cells.
'Endothelial cells are the first cells exposed to what we eat, what infections we carry, or even the medications we take,' Ashok Cheemala, lead author of the Science Advances study, said. 'If these cells become inflamed or damaged, the barrier becomes leaky. When that happens, harmful substances can slip into the brain and trigger inflammation.'
This inflammation, in turn, can lead to neuron death, which causes memory loss and cognitive decline — the hallmarks of diseases like Alzheimer's and frontotemporal dementia (FTD).
Helpful and harmful
The TDP-43 protein regulates RNA and ensures proper gene expression inside cells in a process called splicing. Under healthy conditions, it is located in the nucleus of cells. But in people with neurodegenerative diseases, it goes rogue.
'If it accumulates in the cytoplasm, it starts to form toxic aggregates that can spread from one cell to another,' Cheemala said. While these aggregates have primarily been studied in neurons, researchers have been wondering whether endothelial cells that make up the BBB are also affected.
King's College London neurologist Jemeen Sreedharan said, 'TDP-43 is found in virtually every cell in the body, not just in the brain. It's been detected in the skin, liver, kidneys, even reproductive organs. So its presence in endothelial cells isn't surprising. What's interesting is the idea that its dysfunction in these cells could kickstart the disease process.'
Leaky in the barrier
To investigate, the team used genetically modified mice carrying a disease-causing mutation in the Tardbp gene that encodes TDP-43. 'Even a single point mutation in TDP-43 in endothelial cells was enough to cause BBB leakage, brain inflammation, and behavioural changes in mice,' Cheemala said. As they aged, these mice showed increased leakage of molecules from the bloodstream into the brain, evidence of a compromised barrier.
The researchers found that key proteins holding the BBB together, like claudin-5 and VE-cadherin, were lost, allowing molecules from the bloodstream to leak into brain tissue. These mice also displayed memory problems. The team also injected fluorescent dyes and tracked their penetration into the brain, analysing changes in the structure and protein composition of the BBB to verify their findings.
'This mutation is present from early development, even before birth,' Sreedharan said. 'These mice don't develop obvious brain disease but they do have vascular abnormalities. That points to blood vessel dysfunction as a possible early driver of neurodegeneration.'
The human connection
The team also analysed over 130,000 individual brain-cell nuclei from postmortem human brain samples from 92 donors aged 20-98, including both healthy individuals and those with certain neurodegenerative conditions. They profiled the RNA and nuclear proteins at the single-nucleus level and examined molecular changes in various brain cells. 'We specifically looked at TDP-43 levels in the nuclei of endothelial cells. In patient samples, the nuclear TDP-43 was dramatically reduced compared to healthy controls,' Cheemala said.
The findings mirrored those of the mouse model. Loss of TDP-43 caused β-catenin to disintegrate, ramping up inflammatory signalling. The team also identified a specific group of damaged capillary cells that had low TDP-43 and high inflammation, suggesting they'd shifted from maintenance to damage mode.
Still, the human data came with caveats. 'Post-mortem studies are limited by variability in tissue quality and timing,' Sreedharan said. 'But combining those with controlled mouse models makes the case much stronger.'
'It'll be important to see if this endothelial phenotype is specific to neurodegenerative diseases or a more general response to brain injury. Studying non-genetic conditions like multiple sclerosis or traumatic brain injury could help clarify this,' he added.
Early detection opportunity
The findings open a window for early diagnosis and prevention. 'It's compelling to think a disease we've long considered neuron-specific may actually start in the vasculature,' Sreedharan said.
The team is now working on potential blood-based biomarkers, especially proteins that are regulated by TDP-43 and may be secreted into the bloodstream when endothelial cells are affected.
'One candidate is HDGLF2, a protein that changes when TDP-43 function is lost. If we can detect that in blood, we may be able to identify the number of years an at-risk individual has before their symptoms appear, Cheemala said.
The researchers are also exploring whether exosomes — tiny particles released by cells, which may carry distinct protein signatures from damaged blood vessels — could serve as early indicators of disease. This could lead to non-invasive tests for diagnosing neurodegenerative diseases in their silent stages, long before symptoms appear and while interventions may still be effective.

How climate change and rapid development are pushing the Himalayan region to the brink
'The Himalayas are sounding alarm,' Union Environment Minister Bhupender Yadav warned at a climate summit in Kathmandu in May. 'As a consequence of global warming due to human activity, glaciers are retreating, threatening the future of water security for populations downstream.' Yadav's remarks mirror a growing consensus among environmentalists and scientists who warn that unchecked construction, coupled with global warming, is causing severe and potentially irreversible damage to mountain ecosystems. Scientific studies have long shown that Himalayan glaciers are melting at an unprecedented pace due to climate change, threatening water availability, agriculture and livelihoods. According to a 2019 research article Acceleration of ice loss across the Himalayas over the past 40 years, published in the peer-reviewed journal Science Advances, 'glaciers across the Himalayas experienced significant ice loss over the past 40 years, with the average rate of ice loss twice as rapid in the 21st century compared to the end of the 20th century.' 'Due to global warming, Himalayan glaciers are continuously depleting, forming artificial lakes known as GLOFs (Glacial Lake Outburst Floods). When they burst, they wreak havoc. Another catastrophe occurred in Uttarakhand's Chamoli district on February 7, 2021, when an ice-rock avalanche triggered flash floods, killing over 200 people and destroying hydroelectric projects. Scientists and environmentalists warn that such floods are likely to become more frequent in India's hill states as climate change accelerates the melting of glaciers. According to a report Monitoring of Glaciers, Climate, and Runoff in the Hindu Kush-Himalaya Mountains, published in the South Asia Water Initiative, remote sensing data estimates that there are around 7,500 glacier lakes in Himalayan states like Himachal Pradesh, Uttarakhand, Sikkim, and Arunachal Pradesh. Of these, 190 are classified as 'very high' risk for causing a GLOF. Singh also draws attention to riverbed encroachments. 'When massive water flow meets narrowed river channels, it causes large-scale destruction. We saw this during the 2023 Himachal floods when rivers changed course and entered human settlements, leaving hundreds dead,' he says. 'You cannot tame a river.' Infrastructure boom, ecological bust The Himalayan region is witnessing a rapid infrastructure push — highways, tunnels, hydropower plants — posing a significant threat to its delicate ecology, they often ignore local geology and environmental constraints. 'The government's infrastructure projects have penetrated deep into the Himalayas, threatening the region's overall health. Roads are being constructed without considering the fragile geology, using techniques suited for the plains,' says environmentalist Ravi Chopra, who chaired the Supreme Court panel reviewing the Char Dham highway project in 2019. He later resigned when construction continued despite the panel's warnings about severe ecological risks. Chopra also criticised the disregard for the mountains' carrying capacity, an issue echoed by senior environment journalist Hridayesh Joshi. 'Joshimath is one of several places facing land subsidence. The Mishra Committee, back in 1976, had already warned that Joshimath was vulnerable, built on landslide debris, and worsened by blasting activities. Yet, the development model continues to ignore these warnings,' says Joshi. The situation raises urgent questions. Can a seismically active, hydrologically complex, and geologically unstable region bear the pressure of mega projects? 'Hydropower projects in the Himalayas face two constant and serious threats — catastrophic earthquakes and destructive floods triggered by glacial lake outbursts. What happens if these dams fail during such events? What will become of the region? Scientific studies have long warned of a powerful earthquake, exceeding magnitude 8, striking this area. Why, then, are we not prioritising sustainable development for the region?' aks Singh. Meadows turning into dumpyards Plastic pollution is another mounting crisis in the Himalayas. Similar visuals from Parvati Valley depict garbage heaps and polluted rivers. 'Many ecologically sensitive zones are turning into dumping grounds. Waste is often discarded behind hotels and resorts, with little regulatory oversight.' According to reports, Shimla generates approximately 2,800 tonnes of solid waste, while Manali produces over 1,100 tonnes per month. These are the numbers that double during tourist season. In response, the Himachal government in May this year approved a scheme requiring a refundable deposit on non-biodegradable items to incentivise return and recycling. But the problem runs deeper. According to an alliance by Zero Waste Himalaya, an anti-waste collective of NGOs, an organisation based in Gangtok, and the Integrated Mountain Initiative based in Dehradun, single-use food and beverage packaging accounts for 84% of the plastic waste in the region. Alarmingly, 70% of plastic waste collected across the Himalayan belt is non-recyclable and has no market value. 'The authorities are inviting tourists in large numbers without any waste management plan. If you generate revenue, you must manage the waste too,' says Singh, urging tourists to 'carry your garbage back with you. The mountains cannot absorb such massive piles of waste.' The perils of an unregulated tourism Tourism in the Himalayas is booming, and so is the damage, pushing the fragile mountain ecosystem to its limits. According to the Himachal government's 'Economic Survey' report for FY 2024-25, the state recorded its highest tourist footfall in five years, 1.8 crore domestic tourists and 83,000 foreign visitors. Meanwhile, Uttarakhand reported 5.96 crore visitors in 2023, up from 3.68 crore in 2018, according to the state's tourism department. 'Unregulated tourism is stripping this ecologically sensitive region of its natural character,' says Mansi Asher of Himdhara, Environment Research and Action Collective, an autonomous, non-registered environment research and action collective based in Himachal Pradesh. 'Religious tourism is being promoted in an unstructured, unregulated way. Helicopters swarm over sensitive areas like Kedarnath, which is a wildlife sanctuary. Laser shows, ropeway movements, and excessive noise pollution exceed permitted decibel levels,' says Joshi. Bachi Singh Bisht, of Janmaitri Sangathan, adds, 'Helicopters fly from dawn to dusk in Kedarnath Valley, affecting locals, including schoolchildren, some of whom are now partially deaf.' Unregulated tourism is also fueling outmigration. As per 2011 Census, the population in most of the mountain districts of Uttarakhand declined. There was an absolute decline of 17868 persons in the population of Almora and Pauri Garhwal districts between 2001 and 2011. 'Migration is growing, traditional livelihoods are vanishing, and the ecological backbone of the Himalayas is buckling', Bisht says. What can save the Himalayas? Rising temperatures, erratic weather patterns, growing human-wildlife conflict, depleting water sources, and an alarming increase in landslides, floods, heatwaves, forest fires, and droughts are quickly becoming the new normal in the Himalayan region. But what is the cure? Experts say that top-down solutions are failing. 'Multinational consultancies are being brought in to tackle problems that require local understanding, while the knowledge and experience of local communities are being overlooked. This approach must change,' says Mansi. Singh believes the Infrastructure development also needs a major shift. 'We don't need large-scale infrastructure projects. What we need is small-scale, sustainable development. Tourism should be decentralised, focusing on lesser-known destinations rather than building up major hotspots. We must adopt a policy that ties conservation with livelihood generation.' Is it possible to balance development with sustainability while promoting tourism? Joshi thinks so, but not with the current strategy. 'We should revive the traditional style of pilgrimage, which lasted for months, supported local economies, and was deeply harmonious with nature.' The deaths of actor Gene Hackman and his wife Betsy Arakawa made headlines some months ago after their bodies were found at their home in Santa Fe, United States. While Ms. Arakawa was found to have died of hantavirus pulmonary syndrome, a rare rodent-borne disease, Mr. Hackman's autopsy findings indicated severe heart disease as well as advanced stages of Alzheimer's disease. The actor was 95 at the time of his death. What is Alzheimer's disease and what do you need to know about it? Read on, to find out. What is Alzheimer's disease? Alzheimer's disease is the most common form of dementia, which is an umbrella term for several diseases that affect the brain, impairing memory, thinking, reasoning, affecting a person's ability to function well. Dementia is not a normal part of ageing. Alzheimer's disease, the World Health Organization (WHO) states, may contribute to 60–70% of dementia cases. Dementia is currently the seventh-leading cause of death and one of the major causes of disability and dependency among older people globally. India is expected to have about 7.6 million afflicted by 2030, as per the Dementia in India Report, 2020, majority of whom are likely to be diagnosed with Alzheimer's. Alzheimer's is a progressive, neurodegenerative disease, which means that over time, brain cells are damaged and die, leading the brain to shrink. What causes Alzheimer's disease? The exact causes that lead to Alzheimer's disease are still being studied and are not fully understood. Scientists believe the disease may be caused by the abnormal build-up of proteins in the brain. The brain is made up of billions of nerve cells or neurons that are connected in networks. When a protein called beta amyloid builds up and clumps together, it forms plaques that affect the communication between neurons. Another protein, the tau protein, becomes twisted into strands called tangles, which also affect the functioning of the cells, and damages them. When the cells become damaged and lose their connections with each other, they eventually die. The destruction and death of the brain nerve cells cause the symptoms of Alzheimer's. Genetics, environmental and environmental factors are believed to contribute to the cause. Who is at risk for Alzheimer's disease? Age, being 65 years or older, is one of the primary risk factors. Other risk factors include a family history of Alzheimer's, smoking, a traumatic head injury, untreated depression as well as lifestyle conditions associated with heart disease such as obesity, high blood pressure and high blood sugar. The WHO also lists drinking too much alcohol, being physically inactive and being socially isolated as risk factors for dementia. What are the signs and symptoms? One of the first and most common symptoms may be memory loss -- difficulty in remembering recent events or conversations and newly-learned information. This is not the occasional bout of forgetting that most of us may experience -- this form of memory loss is lasting. It may lead, over time, to patients having trouble finding the right words, asking questions over and over, putting familiar objects in unfamiliar places, getting lost in a previously well-known locations and forgetting faces and names. Other symptoms involve thinking and reasoning abilities as well as changes in speech, moods, personality and behaviour. These can include: confusion, difficulties in paying bills and managing finances, difficulties in day-to-day activities such as cooking or driving and poor judgement. Personality changes can include suspicion of others, mood swings, hallucinations, anxiety, depression, social withdrawal and anger or aggression. What is the prognosis? The rate of progression for Alzheimer's disease varies. In the later stages of the disease, a person may lose physical abilities, may need help with all activities, and may lost the ability to communicate. Some people may live several years after a diagnosis, while others may live for fewer years. The stage of the disease at the time of diagnosis and other factors of health may affect life expectancy. How is it treated? It is important to see a doctor if you are concerned about memory or other thinking skills or if you have these concerns about a family member. At present, there is no cure for Alzheimer's., however treatment can help manage symptoms and treat the progression of the disease. Early treatment is optimal to help preserve brain health. Medicines may be prescribed where necessary. This apart, medication may also be given for symptoms such as depression, hallucinations, agitation and mood changes. Opting for a healthy lifestyle including adequate exercise, well-balanced, nutritious meals, keeping up with hobbies and social activities, may all help. Creating a supportive and caring environment at home, one that is calm, structured, is also important to help improve well-being and the ability to function.
