FRU crash: Special task force given 30 days to submit preliminary report
PUTRAJAYA: The special task force formed to investigate the fatal crash that killed nine Federal Reserve Unit (FRU) personnel in Teluk Intan, Perak, has been given 30 days from May 15 to submit a preliminary report to the Transport Ministry, says Anthony Loke Siew Fook.
Transport Minister said the task force commenced its investigation into the May 13 accident last week.
The task force comprises various experts and representatives from key agencies, and is chaired by the ministry's chief air accident inspector, Brigadier General Tan Chee Kee.
Its members include Simpang Renggam MP Datuk Seri Hasni Mohammad; Jerantut MP Khairil Nizam Khirudin; Prof Dr Kulanthayan KC Mani @ Subramaniam of Universiti Putra Malaysia; independent engineering expert Datuk Foong Choy Chye; as well as representatives from the Road Transport Department, the Malaysian Institute of Road Safety Research, the police and the Armed Forces.
"The ministry will ensure that every recommendation and finding submitted by the task force is thoroughly reviewed for appropriate follow-up at both policy and operational levels," he said in a statement on Monday (May 19).
He added that the task force was established to conduct a comprehensive investigation into the root cause of the incident, including identifying any systemic weaknesses in road safety measures.
"They must also reassess existing guidelines, infrastructure and the application of safety technologies to help prevent similar incidents in the future," he said.
He stressed that the primary objective of the investigation is not to assign blame but to identify and address deficiencies in the current system.
"This includes proposing systematic and effective improvement measures to ensure the safety of road users, particularly concerning heavy vehicles," he said.
The nine FRU personnel from Unit 5 Ipoh were killed in the crash on Jalan Chikus-Sungai Lampam at 8.50am, while the convoy was returning from duty during the Chitirai Pournami celebration in Teluk Intan.
Eight died at the scene, while one succumbed to injuries while receiving treatment at Teluk Intan Hospital. – Bernama

Loke said the 30-day period given to the special task force investigating the accident, which began on May 15, was almost over. 'The task force's report will be presented to the Cabinet next week. I will present it to the Cabinet before any further action is taken,' he told a press conference at his ministry today. In the May 13 incident, an FRU truck in a convoy returning from duty during the Chitirai Pournami celebration collided with a gravel-laden lorry along Jalan Chikus-Sungai Lampam. Nine FRU personnel from Unit 5 Ipoh died in the crash. Former Johor menteri besar Hasni Mohammad and PAS MP Khairil Nizam Khirudin are among the members of the special task force investigating the tragedy. The task force itself is chaired by the ministry's chief inspector of air accidents, Tan Chee Kee. Besides probing the cause of the incident, including identifying any systemic weaknesses related to road safety, the task force is also responsible for analysing the effectiveness of current enforcement measures and regulations within the transport industry. 'Install platform safety doors at all LRT stations' Meanwhile, Loke instructed Prasarana Malaysia Bhd to install passenger safety doors at all LRT stations after the death of a Taiwanese man who reportedly fell on the tracks and was hit by a train in Puchong. Serdang police chief Farid Ahmad reportedly said that CCTV footage showed the man walking alone before falling onto the tracks. The body was sent to Sultan Idris Shah Hospital's forensic department for a post-mortem examination, while the case has been classified as sudden death. 'New MRT stations already have safety doors – there's a barrier before the train arrives… this is also the case for the monorail. 'However, for the Ampang LRT line, some of the older station designs still lack safety doors on the platforms,' Loke said.

Last two FRU officers injured in Teluk Intan crash leave ICU
IPOH, May 31 — The last two members of the Federal Reserve Unit (FRU) who were warded at the Intensive Care Unit (ICU) of the Teluk Intan Hospital (HTI) were discharged and allowed to go home yesterday. Hilir Perak police chief ACP Dr Bakri Zainal Abidin said the last two patients who were at HTI, namely Sergeant Mazlan Mat, 50, and Corporal Mohd Izwan Ishak, 39, were discharged at about 6pm yesterday. 'Earlier two others, Corporal Haslizal Mohd Ali, 44, who was receiving treatment at HTI, was discharged on May 28, while Corporal Syed Isfan Syed Mohammad, 32, who was treated at Raja Permaisuri Bainun Hospital (HRPB), was discharged on Friday. 'All patients were discharged yesterday by 6pm. Everyone treated here has been allowed to leave. Thank you all for your support and assistance,' he said in a statement yesterday. Earlier, Bakri mentioned that the last four officers showed significant progress since their critical condition when the accident first occurred. On May 13, an accident at 8.50am claimed the lives of nine FRU personnel and injured nine others when their truck collided with a lorry loaded with gravel stones on Jalan Chikus–Sungai Lampam in Teluk Intan while they were returning to their base in Sungai Senam here. — Bernama
