Special task force gets 30 days for initial FRU crash report

PUTRAJAYA: The special task force formed to investigate the fatal crash that killed nine Federal Reserve Unit (FRU) personnel in Teluk Intan, Perak, has been given 30 days from May 15 to submit a preliminary report to the Ministry of Transport (MOT).
In a statement today, Transport Minister Anthony Loke Siew Fook said the task force commenced its investigation into the May 13 accident last week.
The task force comprises various experts and representatives from key agencies, and is chaired by the MOT's Chief Air Accident Inspector, Brigadier General Tan Chee Kee.
Its members include Simpang Renggam MP Datuk Seri Hasni Mohammad; Jerantut MP Khairil Nizam Khirudin; Prof Dr Kulanthayan K.C. Mani @ Subramaniam of Universiti Putra Malaysia; independent engineering expert Datuk Foong Choy Chye; as well as representatives from the Road Transport Department, the Malaysian Institute of Road Safety Research, the Royal Malaysia Police and the Malaysian Armed Forces.
'MOT will ensure that every recommendation and finding submitted by the task force is thoroughly reviewed for appropriate follow-up at both policy and operational levels,' he said.
He added that the task force was established to conduct a comprehensive investigation into the root cause of the incident, including identifying any systemic weaknesses in road safety measures.
'They must also reassess existing guidelines, infrastructure and the application of safety technologies to help prevent similar incidents in the future,' he said.
He stressed that the primary objective of the investigation is not to assign blame but to identify and address deficiencies in the current system.
'This includes proposing systematic and effective improvement measures to ensure the safety of road users, particularly concerning heavy vehicles,' he said.
The nine FRU personnel from Unit 5 Ipoh were killed in the fatal crash along Jalan Chikus–Sungai Lampam at 8.50 am, while the convoy was returning from duty during the Chitirai Pournami celebration in Teluk Intan.
Eight died at the scene, while one succumbed to injuries while receiving treatment at Teluk Intan Hospital.

IPOH, May 31 — The last two members of the Federal Reserve Unit (FRU) who were warded at the Intensive Care Unit (ICU) of the Teluk Intan Hospital (HTI) were discharged and allowed to go home yesterday. Hilir Perak police chief ACP Dr Bakri Zainal Abidin said the last two patients who were at HTI, namely Sergeant Mazlan Mat, 50, and Corporal Mohd Izwan Ishak, 39, were discharged at about 6pm yesterday. 'Earlier two others, Corporal Haslizal Mohd Ali, 44, who was receiving treatment at HTI, was discharged on May 28, while Corporal Syed Isfan Syed Mohammad, 32, who was treated at Raja Permaisuri Bainun Hospital (HRPB), was discharged on Friday. 'All patients were discharged yesterday by 6pm. Everyone treated here has been allowed to leave. Thank you all for your support and assistance,' he said in a statement yesterday. Earlier, Bakri mentioned that the last four officers showed significant progress since their critical condition when the accident first occurred. On May 13, an accident at 8.50am claimed the lives of nine FRU personnel and injured nine others when their truck collided with a lorry loaded with gravel stones on Jalan Chikus–Sungai Lampam in Teluk Intan while they were returning to their base in Sungai Senam here. — Bernama

(Bernama pic) PETALING JAYA : The owner of the lorry involved in the crash that killed nine Federal Reserve Unit personnel in Teluk Intan on Tuesday says the vehicle passed a Puspakom inspection just last month. The man, who only wished to be known as Ravi, 58, told Utusan Malaysia that the lorry also had all the necessary documents. On Tuesday, deputy prime minister Ahmad Zahid Hamidi said while the preliminary findings into the crash suggested a steering issue, the accident could have been prevented if the vehicle had undergone a Puspakom inspection. 'Last month, I went to Puspakom and the lorry passed inspection. After that, I renewed the road tax,' said Ravi. Ravi added that the driver involved in the crash had only been working for about a month and a half. The driver, Rudi Zulkarnain Mat Radi, 45, was charged with causing death by reckless driving yesterday. The 8.40am crash, which also left two FRU officers critically injured, occurred when a gravel-laden lorry was involved in a collision with a FRU truck transporting personnel back to their base in Ipoh after they had provided security support at a festival in Teluk Intan. Ravi said he was deeply saddened by the incident and its impact on the victims' families. He said he was constantly thinking about the victims' families and hoped to meet them, if possible. 'Of course, I feel heartbroken,' he said. 'On behalf of the company, we apologise to the families of the victims, and I also extend my condolences to them. I would like to meet them to express my sympathies. 'I constantly think about how they must feel. They've lost family members, lost husbands — how will they go on with their lives? When the head of the family is gone, of course their lives will be difficult.'

(Bernama pic) TELUK INTAN : Six Federal Reserve Unit (FRU) members involved in the road crash here last Tuesday are still undergoing treatment while another was discharged yesterday. Perak health director Dr Feisul Idzwan Mustapha said of the six, five are being treated at Teluk Intan Hospital (HTI) and the other at the Raja Permaisuri Bainun Hospital (HRPB), Ipoh. 'Of the five patients currently receiving treatment at HTI, four are in the ICU. The other is in the surgical ward, where his condition is improving and he may be discharged soon,' he said when met by reporters at HTI today. Feisul said those in the ICU are showing good progress, with one of them requiring close observation. Earlier, the Sultan of Perak, Sultan Nazrin Shah, met the family members of the FRU personnel at the hospital. Meanwhile, on the condition of the FRU officer in the ICU of HRPB, Feisul said he is beginning to regain consciousness and is showing good progress, despite suffering serious injuries. In the incident at 8.50am last Tuesday, nine FRU members were killed and nine others were injured after the truck they were in was involved in an accident with a gravel-laden lorry on Jalan Chikus-Sungai Lampam while on the way back from here to the base in Sungai Senam in Ipoh.
