Failures of basic seamanship led to the grounding of a Japanese fishing boat in Auckland's Hauraki Gulf last year, the Transport Accident Investigation Commission has found.

Poor seamanship and a failure to carry proper charts caused a Japanese fishing boat to hit rocks in the Hauraki Gulf last year, an investigation has found.
About 3.40am on 16 April 2024, the Chokyu Maru No.68 was approaching Auckland when it struck The Noises, a group of islands about 25km northeast of the city.
The ship sustained a small hole in its bow and damage to its propeller, but none of the 27 crew on board was injured and there was no oil leak or other environmental damage.
The 48-metre vessel was refloated later that morning and towed to port for repairs.
The grounding sparked an investigation by the Transport Accident Investigation Commission (TAIC), which released its findings late last week.
Acting chief investigator of accidents Louise Cooke said it was a "textbook example" of what could happen when the basics of good seamanship were ignored.
"Basic seamanship is the core issue of this accident. Crew leaders must always plan the voyage, know the hazards, and verify position. This isn't optional, it's basic good seamanship and it's required all over the world," she said.
The TAIC report found the Chokyu Maru No.68 had left Japan without large-scale paper charts - in particular a crucial chart showing the approaches to Auckland - or nautical publications setting out the safest route into Waitematā Harbour.
The longliner also left Japan without a formal voyage plan.
In the absence of detailed paper charts, the master relied on the chart plotter to determine the ship's position.
However, electronic charts for New Zealand had not been installed, so the plotter did not display the small islands of the Hauraki Gulf.
The TAIC investigation also found only one of the vessel's two radar units was in use at the time, because the crew believed the second unit was broken.
It was in fact working, but the settings were unsuitable for identifying hazards.
Without detailed charts or information about navigational risks, the master set a straight-line course to Auckland in the mistaken belief there was safe water all the way.
The TAIC report stated it was "virtually certain" the crew would have identified the hazards and plotted a safe course, had they prepared a voyage plan in advance using proper charts and publications.
The key lesson from the Chokyu Maru No.68 grounding was the "fundamental importance" of a well-researched and documented voyage plan.
Since the accident, Cooke said Maritime NZ had taken steps to improve the safety of foreign-flagged fishing vessels in New Zealand waters.
She said Maritime NZ inspectors now had better tools to identify and address safety issues, thanks to better access to world-wide data and the inclusion of fishing vessels in the international inspection regime.
TAIC investigations aim to establish the cause of transport accidents and prevent similar incidents happening again. They do not seek to assign blame or lead to prosecution.

Photo: RNZ / Luka Forman An Auckland secondary school teacher says he is not surprised by the increasing rate of teenagers leaving high school with no qualifications, and does not anticipate the situation improving. Of last year's school-leavers, 16 percent had no qualifications, the highest figure in a decade . It equates to about 10,600 teenagers, and is 0.4 of a percentage point more than the previous year and about six percentage points higher than the 10-11 percent recorded in the years prior to the start of the pandemic. The percentage of school leavers with no NCEA certificate has been rising since 2020, a trend teachers blamed on the after-effects of Covid-19 lockdowns combined with high employment prompting more young people to leave school earlier than they otherwise would. Papakura High School principal Simon Craggs told Morning Report changes to literacy and numeracy standards were restricting students. "It's not really a surprise to me," he said. "Since they changed the literacy and numeracy requirements that more and more students, particularly from low socio-economic backgrounds, ESOL backgrounds, Maori and Pasifika, weren't going to be able to access qualifications." The Education Ministry's figures showed 13 percent of last year's leavers had not reached the literacy and numeracy benchmark compared to about 10 percent under the previous requirement in pre-pandemic years. The figures showed 81 percent of last year's leavers had stayed at school until the age of 17 or beyond, up from 79 percent the previous year with bigger increases in retention at schools in poorer communities. Craggs said the revised criteria for the co-requisite tests, known as common assessment activities (CAAs), had "restricted students". "There are other factors involved, but I would say it's 90 percent due to the new CAA tests in particular," he said. "The problem I have with that is not that we're trying to strengthen literacy and numeracy, the problem is the test itself is very narrow. "It's not actually measuring functional literacy and numeracy." Māori had the worst results - 28 percent left with no qualification last year, compared with 19 percent of Pacific leavers and 14 percent of European/Pākehā leavers. Papakura High School had scrapped NCEA level 1, instead opting for a two-year level 2 programme. "[Level 1] was too hard for our students to do the co-requisite, get their literacy and numeracy, and get 60 credits," Craggs said. However because students had to stay longer at school to earn a qualification, a higher number of unqualified students would drop out, he said. A tight labour market was also limiting the options for young people. "That's another scary thing," Craggs said. "A lot of them aren't going into quality employment. They are going into employment, but it's casual, reduced hours, take what you get sort of thing. It's very difficult for them to be accessing quality employment. "Maybe three or four years ago they could go into a good full-time job, and build a career from that. They're not able to do that at the moment." Training programmes such as People Potential were proving to be a viable alternative for young people, although spots were tight. Craggs said he did not believe an overhaul of NCEA would lead to declining rates of unqualified school leavers. "If we were looking at making some tweaks to NCEA, making it a better qualification, making the literacy and numeracy requirement more [functional] then we might have some more optimism for the future. "I'm not sure that just dumping NCEA and creating a new qualification which is subject-based is necessarily going to make a big difference to those students." Tsutomu Yamaguchi: The man who survived both atomic bombs

There is only one person who officially survived two. On this day, 80 years ago, young engineer Tsutomu Yamaguchi was telling his boss about the horrors he had seen in the Japanese city of Hiroshima when the room went blindingly white. "I thought the mushroom cloud had followed me from Hiroshima," he told UK Newspaper, The Independent . Yamaguchi was an engineer with Mitsubishi Heavy Industries when the United States dropped atomic bombs on the cities of Hiroshima and Nagasaki. Yamaguchi, then 29, was in Hiroshima for a business trip when the bomb known as 'Little Boy' was deployed, killing tens of thousands in a flash, and leaving scores with burns so severe their skin draped off their bodies. The young engineer was around three kilometres from ground zero and suffered temporary blindness and deafness in one eardrum. After staying in a bomb shelter the first night with other survivors, he quickly made his way back to his hometown of Nagasaki. Then on 9 August, 1945, he went to work and told his colleagues about the horrors he saw. "When they realised that I had returned from Hiroshima, everyone gathered around me and said, 'I'm glad you're alive,' and 'great that you have survived,'" he recounted to Japanese broadcaster NHK. But his boss did not believe him. "He replied, 'You're badly injured, aren't you? Your head must be damaged too. I can't believe what you're saying. How could a single bomb destroy such a vast area like Hiroshima?'" Just at that moment, the United States dropped its second atomic bomb, known as 'Fat Man', killing some 40,000 people instantly. "I immediately recognised it as an atomic bomb," he told NHK. "I hid under a desk right away." The city of Nagasaki will pause today to remember the atomic blast that inflicted so much horror on the unsuspecting city. Within months, 74,000 people were dead after radiation sickness took hold. The bombing of Nagasaki is often overshadowed by the deadlier and earlier attack on Hiroshima, which killed some 140,000 people by year's end. Part of the tragedy of Nagasaki is it was not the original intended target. Two B-29 bombers were sent to destroy the industrial city of Kokura, which was a major hub for ammunition manufacturing. But the city was hidden under cloud cover, so the pilots diverted to their secondary target: Nagasaki. Tsutomu Yamaguchi in 2008. Photo: JIJI PRESS / AFP About 165 people are thought to have survived both atomic blasts, known as nijyuu hibakusha. But Yamaguchi is the only person to be officially recognised by the local governments of Hiroshima and Nagasaki. For decades, he kept his unique story under wraps and worked a blue-collar job. Many atomic bomb survivors, known as hibakusha, feel compelled to speak out, hoping their experiences will spur the world to abandon nuclear weapons. But the family of Yamaguchi feared he looked too healthy, which would undermine the message of survivors. "My entire family opposed it," his daughter Toshiko Yamasaki explained at a peace conference in 2011. "If my father, who had survived two atomic bombings, engaged in peace activities, people might think, 'even after being exposed to radiation twice, he's still healthy, so the atomic bomb isn't scary.'" Photo: Supplied But Yamaguchi did suffer a lifetime of health problems, as is often the case for hibakusha due to radiation exposure. "My father had cataracts, was deaf in one ear, suffered from leukopenia, lost his hair for 15 years after the war, and had after-effects from burns," Toshiko explained. His family endured sickness, too. His wife and son died of cancer. It was only in his final decade that Yamaguchi started to speak more openly, hoping his ordeal would help in the fight against nuclear weapons. "I have walked and crawled through the bottom of hell," he told the ABC in 2009. "I should be dead. But it was my fate to keep on living." Irish journalist David McNeill was one of the last journalists to interview him before his death. "What struck me was how modest he was," he explained. "Like many hibakusha, he really didn't want to discuss his extraordinary life. He had to be pressed into it because he thought he was better off than many of the people who surrounded him, who were getting sick and dying from cancer." Yamaguchi died in 2010, aged 93. The atomic bombings of Hiroshima and Nagasaki were one of the final and most devastating acts of World War II. After the first nuclear attack, Japan would still not surrender, instead deciding to send a fact-finding team to the city after communications went dark. The second attack on Nagasaki was part of the American strategy to make Japan believe it had unlimited supplies of such bombs. Many historians argue the Soviet Union declaring war on Japan was more influential in securing Japan's surrender, as it suddenly exposed its entire unprotected north. Making a single uranium bomb that exploded over Hiroshima was incredibly challenging and chewed up much of the budget and resourcing of the multi-year Manhattan Project. Japan knew how challenging it would be. But the United States had also developed a plutonium bomb - far easier and cheaper than a uranium bomb. This is what detonated over Nagasaki. And the commander of the Manhattan Project boasted the United States could then create two or three atomic bombs a month to assist in the planned land invasion of Japan, scheduled for November 1945. "They had the capacity to make two or three bombs a month by that point," Professor Mordecai Sheftall from Shizuoka explains. "Because the plutonium production facilities in Hanford, Washington State, were going at full tilt." Japan finally surrendered on 15 August, but only after the emperor intervened and broke a deadlock in his war council. The army still wanted to fight on. There are few hibakusha left old enough to remember the blasts of Hiroshima and Nagasaki. But the survivors are still determined to keep telling their stories. After all, those who forget history are doomed to repeat it. "As a double atomic bomb survivor, I experienced the bomb twice," Yamaguchi told The Independent in 2010. "I sincerely hope that there will not be a third." - ABC News
