Trella Health general manager Tyler Rardin said: 'Hospital discharge remains one of the most challenging and disconnected points in the patient journey, with many organisations still relying on paper lists, faxes, and verbal handoffs that create avoidable inefficiencies.

Repisodic, established in 2017, has played a role in enhancing the hospital discharge process through its EHR-integrated automation platform.
This platform aids health systems in improving care transitions and expediting discharges, leading to an increase in referrals to post-acute providers and a reduction in inpatient length of stay.
With the merger, Trella Health's post-acute customers can enjoy new benefits.
By combining Trella's data insights with discharge automation capabilities of Repisodic, the partnership fosters improved communication with hospital case managers and patients.
It ensures real-time visibility into patient acuity, referrals, and provider capabilities, in line with payer contracts, post-acute capacity, and authorisation requirements.
Repisodic CEO Mike Cwalinski said: 'Joining Trella allows us to scale that mission and drive even greater impact across the care continuum. Just this week, we launched our solution with another top-20 health system, connecting more than 1,400 post-acute providers with upstream access to patients and case managers at the point of discharge.

This merger positions Trella Health as a support system for more than 50 health systems and over 1,000 post-acute organisations across the country.
