FAA Warned In 2018 Of Boeing Engine Shutdown Risk But Air India Ignored Fuel Switch Advisory

Did the fuel control switch in the ill-fated Air India 171 Boeing aircraft malfunction because it was disengaged and not locked? The preliminary inquiry report into the incident points to an FAA warning in 2018 that said inadvertent operation of the switch could result in an unintended consequence, such as an in-flight engine shutdown. But the report has stopped short of blaming this as the cause for the crash.
CNN-News18 has accessed the 2018 warning by Federal Aviation Administration (FAA) that Boeing received reports from operators of Model 737 airplanes that the fuel control switches were installed with the locking feature disengaged.
The fuel control switches (or engine start switches) are installed on the control stand in the flight deck and used by the pilot to supply or cut off fuel to the engines. The fuel control switch has a locking feature to prevent inadvertent operation that could result in unintended switch movement between the fuel supply and fuel cut-off positions.
To move the switch from one position to the other under the condition where the locking feature is engaged, it is necessary for the pilot to lift the switch up while transitioning the switch position.
'If the locking feature is disengaged, the switch can be moved between the two positions without lifting the switch during transition, and the switch would be exposed to the potential of inadvertent operation. Inadvertent operation of the switch could result in an unintended consequence, such as an in-flight engine shutdown," the FAA warning said.
'While the airplane is on the ground, check whether the fuel control switch can be moved between the two positions without lifting up the switch. If the switch can be moved without lifting it up, the locking feature has been disengaged and the switch should be replaced at the earliest opportunity," the FAA said.
For Boeing Model 737-700, -700C, -800, and -900ER series airplanes and Boeing Model 737-8 and -9 airplanes delivered with a fuel control switch having P/N 766AT613-3D, it was advised that the fuel control switch be replaced with a switch having P/N 766AT614-3D, which includes an improved locking feature.
Was this Done?
The AAIB report says that the fuel control switch design, including the locking feature, is similar on various Boeing airplane models including part number 4TL837-3D which is fitted in B787-8 aircraft VT-ANB that crashed in Ahmedabad.
'As per the information from Air India, the suggested inspections were not carried out as the FAA directive was advisory and not mandatory. The scrutiny of maintenance records revealed that the throttle control module was replaced on VT-ANB in 2019 and 2023. However, the reason for the replacement was not linked to the fuel control switch. There has been no defect reported pertaining to the fuel control switch since 2023 on VT-ANB," the AAIB report says.
What Happened on AI-171
After the AI-171 aircraft achieved the maximum recorded airspeed of 180 Knots, immediately thereafter, the Engine 1 and Engine 2 fuel cut-off switches transitioned from RUN to CUTOFF position one after another with a time gap of 01 sec, says the AAIB report.
The Engine N1 and N2 began to decrease from their take-off values as the fuel supply to the engines was cut off.
'In the cockpit voice recording, one of the pilots is heard asking the other why did he cutoff. The other pilot responded that he did not do so," the report says. The pilots then tried to restart the engines—but it was too late.

'The vehicle is very much here, as you can see it. Nothing happened to it. Only a little damage," pointed out Sanjeev in a reel posted on August 3. According to him, he took his family out on Saturday and while returning, the car caught fire at Kundalahalli. 'I had a fire extinguisher, which I always keep with me. I parked the vehicle to the side, so that people won't be inconvenienced. I then tried to douse the fire," he said. Meanwhile, many people were making videos of this, he said. 'They had uploaded it on YouTube and other social media platforms," he added. Sanjeev said in his Insta reel that people started trolling him about how he often goes to temples — and makes reels of them — and yet that could not save his car. 'But what do they know? It was God who sent me autorickshaw driver and a cab driver to help me put out the fire," said Sanjeev. A wire had caught fire due to petrol leakage; it has got nothing to do with the engineering of the car, said Sanjeev. 'Try to help people when they are facing a crisis instead of making videos," said the social media influencer. The viral videos, which show flames erupting from the rear of the expensive 4-wheeler where its engine is located, had people, including Managing Director of Raymond Group, Gautam Singhania, demanding the luxury car maker to take responsibility. Singhania posted on X: 'Another day, another Lamborghini in flames. This time in Bengaluru. This isn't a 'rare incident" anymore. It's a pattern." 'Why is Lamborghini silent??? Why are their cars catching fire??? Are their cars safe??? Should they be allowed in India???" The viral clips also show passersby rushing to pour water in an attempt to stop the fire. The excise policy committee will be headed by PWD Minister Parvesh Verma, while the electric vehicle policy committee will be headed by Urban Development Minister Ashish Sood, she said. In a post on X, the chief minister said, 'Keeping social security in focus, the Delhi government has constituted a high-level committee to formulate a transparent excise policy, which will be headed by Cabinet Minister Shri @p_sahibsingh. 'Additionally, a special committee has been formed under the leadership of Cabinet Minister Shri @ashishsood_bjp to develop an effective and public-oriented EV (Electric Vehicle) policy for Delhi. Our government is committed to the holistic development of Delhi and is continuously taking decisions in the public interest. #ViksitDelhi," she said. Both the excise policy and EV policy have been extended multiple times in the absence of new policies. In June this year, the government extended its excise policy for the financial year 2025–26. The current policy was extended with effect from July 1 to March 31, 2026. The extended policy, also known as the old excise policy, came into operation in September 2022 after the then Aam Aadmi Party (AAP) government scrapped its reformative policy (2021–22) that ran into rough weather amid allegations of irregularities in its formulation and implementation. The Delhi government had extended the current Electric Vehicle (EV) Policy until March 31, 2026, as the draft of the new policy will undergo public consultation, which is expected to take time, Transport Minister Pankaj Singh had said last month. The current EV policy, which was first introduced in 2020 during the previous Aam Aadmi Party rule, lapsed in August 2023. The policy has been extended several times since then. Bosch Q1 net profit surges over twofold to Rs 1,115 cr

Agency: Last Updated: New Delhi, Aug 4 (PTI) Auto component maker Bosch Ltd on Monday reported an over twofold jump in its consolidated net profit to Rs 1,115 crore for the first quarter ended June 30, 2025, aided by one-time income due to the transfer of a business vertical. The company posted a net profit of Rs 465 crore for the April-June quarter of the last fiscal. Total income increased to Rs 5,077 crore during the quarter against Rs 4,496 crore in the year-ago period, Bosch Ltd said in a regulatory filing. During the quarter, the company completed the transfer of its 'Video solutions, Access and Intrusions and Communication systems" business, recognising a profit on sale of business for Rs 556 crore, it noted. 'Our performance in the first quarter reflects strong growth, driven by increased revenue, higher demand in passenger cars and a reduction in material costs enabled by favourable product mix," Bosch Ltd MD Guruprasad Mudlapur said. This results from the company's consistent efforts to strengthen core businesses while remaining focused on future-ready technologies, he added. 'Despite global challenges, we remain optimistic about the opportunities ahead," he said. The company is investing decisively in hydrogen, electrification, and digital services – positioning itself at the forefront of sustainable mobility, Mudlapur added. As India moves toward a smarter, cleaner future, Bosch remains a trusted partner in delivering long-term value and innovation-led growth, he said.
