Taupō struggling to attract doctors as residents rally to improve working conditions at town hospital

Taupō residents have vowed to fight to improve the town's hospital, which has faced dire workforce shortages.
Those shortages have been so bad that contingency plans were drawn up last year to move patients to Rotorua Hospital
if Taupō's clinic had to close
.
That would have left the area's 40,000 without easy access to emergency care - a situation those at a packed public meeting on Wednesday night say cannot happen.
Long-time Taupō resident John Davis said locals should be prepared for a rumble in the battle to adequately staff the hospital.
"I'm quite happy to organise, if people are interested. We'll get a committee together and we'll focus on one thing ... that Taupō Hospital doesn't go downhill, it actually goes uphill.
"We'll get people who are prepared to stand up and be counted. I'm happy to stand up and be counted."
Davis said the hospital proved its value to his family when his grandson was rushed there when he was younger and was revived several times before flying to Starship Children's Hospital in Auckland.
The idea for a committee seemed to gain support from the more than 100 people who turned out to Taupō's Hilltop School at a meeting organised by Patient Voice Aotearoa.
Others in the crowd suggested ideas to help find new doctors a place to live and settle in to the area.
Among those attending was local nurse Iretana, who said shortages among healthcare workers in the region were having an effect on people.
"They're getting put back. We can't get into appointments. People are swamping ED [the emergency department] and it's just not viable, so people are getting misdiagnosed or late diagnoses."
RNZ has revealed the existence of the contingency plans should Taupō not be able to get enough doctors to keep the hospital running, although Health NZ says the plan isn't active.
There are just 3.3 full-time equivalent senior doctors employed there out of nine funded positions, meaning expensive locums fill the gaps and
permanent staff face burn out
.
One agency list seen by RNZ shows Taupō Hospital requires locums on seven days between 3 and 18 August, paying $300 an hour for weekday work and $350 an hour for night work.
Robin Chan is a senior doctor at the hospital, and she said roster problems had persisted for some time.
"What I don't see and what I haven't seen is any targeted funding to improve the most pressing issue, which is the medical workforce for rural communities.
"It's not good enough to fund 100 new clinical placements or waive registrar training fees and expect those benefits to end up in rural communities."
Too many health decisions were made through an urban lens that didn't work for smaller areas, Chan said.
In her seven years in Taupō she's seen many colleagues come and go, and she said if doctors were to stay in smaller centres they needed to be valued.
"The greatest predictor of rural practice, of doctors staying rural, is actually being from a rural town. This is where you guys come in," she told the meeting.
"If you have children, and I don't care if they're a toddler or a teenager, or nieces or nephews or grandkids, for god's sake, buy them a stethoscope."
Taupō Hospital's clinical lead Dr Ralston D'Souza said partly because of public pressure health officials were now working with hospital staff to make change.
That includes creating eight more junior doctor positions, of which five were filled from later this year, to help ease the burden.
"It's not uncommon on a shift to see about 15 to 20 patients. Nationally, the average is probably seeing about eight to 10 patients per shift.
"That's the reason why we've struggled to attract doctors as well because they can work in Rotorua ED or other EDs around the country and it's much better working conditions."
Overnight at Taupō just one doctor was rostered on for the whole hospital, which could put off clinicians.
"The other thing that scares the locum doctors to come and work here is we've got a birthing unit right next door and birthing emergencies can happen.
"If a birthing emergency happens in Taupō Hospital, it [could be] just the one doctor and midwife assisting the patient."
Association of Salaried Medical specialists executive director Sarah Dalton said spending money on workforce conditions would help recruitment.
"The single most important issue for our members, senior doctors and dentists, and for all those nurses taking strike action and their colleagues, is they don't have enough colleagues. There aren't enough of them."
Patient Voice Aotearoa's Malcolm Mulholland said staffing was a real issue at Taupō Hospital, and the community had a right to know about the contingency plans and to act "as you see fit".
Health NZ wasn't represented on Wednesday night, but in a statement said its executive regional director Cath Cronin had discussed the meeting with Dr D'Souza.
"They agree that while there have been many challenges with recruiting and retaining permanent senior medical officers (SMOs) and filling vacant shifts, the SMO group feel supported to progress improvements at Taupō Hospital.
"A change being made to the emergency department staffing model is an example of a partnership approach between clinicians and hospital management to support our people, while maintaining access to safe services for the public."
Senior doctors had regular meetings with officials to present their ideas and plans, Cronin said.

RNZ earlier revealed Bellamere Arwyn Duncan died at Starship Hospital on 19 July. The two-month-old was allegedly given an adult dosage of phosphate by a Manawatū pharmacy. A Coroner's preliminary opinion is she died from phosphate toxicity. The revelations have prompted the Ministry of Health and Health New Zealand to "urgently" undertake a joint review into the incident with Medsafe visiting the pharmacy to ensure it was safe to continue operating. The Pharmacy Council, which was also investigating, said it's "clear that an awful error has occurred". Bellamere's parents are calling for a law change that would make it mandatory for medication to be checked by two people before it is dispensed. RNZ can now reveal a pharmacist who works at the Manawatū pharmacy had since been suspended by the Pharmacy Council and was not entitled to practice. Photo: Supplied In a statement to RNZ Pharmacy Council Chief Executive Michael Pead said whenever the Pharmacy Council received a notification of an incident, they began an "initial enquiry" to assess the situation. "At the start of any enquiry, our focus is on ensuring there is no further risk to public safety. There are many ways to achieve this, including suspension of the pharmacist or pharmacists involved or a voluntary agreement that the individual/s will stop working." In order to ensure the inquiry into Bellamere's death was "fair and thorough", and to avoid pre-empting any findings, the council could not provide any further details. "We can confirm that the Pharmacy Council is comfortable that immediate steps have been taken to prevent the risk of further harm while the enquiry is ongoing." The owner of the Manawatū pharmacy that dispensed the medication earlier said in a statement to RNZ the baby's death was "a tragedy". "Our sympathy is with the family and whānau. This is a very difficult time. "We are looking into what has happened to try to understand how this took place. There will also be external reviews which we will work with." RNZ asked the owner how the medication was given at the wrong dosage, whether they disputed the allegations, when the pharmacy became aware the wrong dosage had been given, and what confidence people could have about other medication received from the pharmacy. Photo: 123RF The owner said the pharmacy was "devastated about what has happened and are investigating to find out how this occurred". "It is not appropriate to comment further at this stage." On Thursday, a Pharmacy Council spokesperson said the Council was looking into "what went wrong, how it went wrong, and who was involved". "However, in order to ensure our enquiry is fair and thorough, and to avoid pre-empting any findings, we cannot provide any further details at this stage. "At the end of the process, we will make any recommended changes to ensure as best as possible an event like this does not happen again." While in hospital Bellamere's mum, Tempest Puklowski gave Bellamere her drops for Vitamin D. Nurses also administered her phosphate. When they left hospital they were given some Vitamin D in a little bottle, and a prescription for iron and Vitamin D. The following day Bellamere's father, Tristan Duncan went to a Manawatū pharmacy with the prescriptions. He was given the iron, but said the pharmacy refused to give the Vitamin D as the staff thought the dosage was "too high for her age and her weight". The staff said they would call the neonatal unit and follow-up. A few days later Puklowski received a call from the unit to organise a home care visit. During the call she was asked if she had any concerns, and Puklowski asked if they had been contacted about the Vitamin D. They had not, and said they would follow up and rewrite the prescription along with a prescription for phosphate. A day after the phone call, on July 2, Duncan went to the pharmacy to collect the medication and came home with just the phosphate. Unbeknown to the parents they had allegedly been given an adult dosage of phosphate. The label on the medication directed them to dissolve one 500mg tablet of phosphate twice daily in a glass of water. That evening they gave Bellamere her first dose of the medicine in her formula water. They would give her three bottles in 24 hours as was recommended. The couple noticed in that period that her eating was off, and thought she was "extra gassy," Puklowski said. "She was still feeding fine. She just wasn't maybe going through a whole bottle compared to what she was," she recalled. Then, the day after she got her first dosage Bellamere suddenly stopped breathing. Bellamere was taken to hospital and rushed to the emergency department. Once she was stabilised she was taken to the neonatal unit where she stayed overnight before she was flown to Starship Hospital. The couple had taken a bottle of the medicine with them to Starship Hospital. She gave it to the staff who saw that they had been given an adult dose. The staff then requested the original prescription which confirmed the script had been written with the correct dosage, but somehow the pharmacy had given the wrong dosage Puklowski said. "I keep thinking about how much she ended up having and it just makes me feel sick." Tragically, Bellamere died at Starship Hospital on July 19. On Wednesday, a Ministry of Health spokesperson told RNZ there were a number of investigations underway. "Medsafe has completed an urgent assessment and is comfortable there is no immediate patient safety issue at the pharmacy. Medsafe will continue to work with Health New Zealand and these findings which will inform the information provided to the Coroner. Medsafe is also sharing information with the Pharmacy Council. "Once these reviews are completed, we will be able to look at next steps." Health Minister Simeon Brown told RNZ on Monday he raised the incident with the Director-General of Health as soon as he was made aware. "She assured me that there would be an investigation undertaken by both the Ministry of Health and Health New Zealand. That investigation is underway. "I am advised that this incident has led to Medsafe undertaking an urgent assessment of the pharmacy. A further investigation is being undertaken by the Pharmacy Council, and the death is also the subject of a Coroner's inquest." Health agencies would provide information to the coroner as needed to support the inquest. "It is important that the reviews are undertaken, and that the circumstances that led to this incident are understood. I expect that these investigations may propose recommendations, and that these will be reviewed once reports are complete." Senior doctors say Health NZ offer amounts to 'pay cut'

(File photo) Photo: RNZ Senior doctors have criticised Health NZ's latest pay offer saying it amounts to a pay cut that won't cover inflation. Health NZ's lead for industrial relations, Robyn Shearer, said the new offer - which covered a 21-month term - included "salary increases between 3.5 percent and 11 percent depending on experience and funding for incentives to help retain senior doctors in hard to staff areas and specialities". "We value our doctors and want to do the best we can for them. We are committed to reaching a settlement with ASMS [the Association of Salaried Medical Specialists] and we believe the offer made today is a fair and reasonable one," she said. According to Health NZ the new offer means: Health NZ's offer also included a ring-fenced $34m fund to help retain specialists in hard-to-staff areas. "We have listened to the union on their concerns regarding growing and retaining our senior doctor workforce," Shearer said. Thousands of operations and specialists appointments had to be cancelled during the unprecedented 24-hour strike by 5500 members of the ASMS in May. ASMS executive director Sarah Dalton was doubtful the union's 5500 members would find the new offer acceptable. Health NZ had "misrepresented" what the offer would mean for first and second-year specialists, she said. "When you annualise that they are between 2.1 and 3.2 percent annually. Ninety percent of our members under this offer would receive about 1.16 percent per year. "So yet another under CPI [consumer price index] real term pay cut for senior doctors and dentists." Health NZ had refused to discuss details of how the proposed incentive fund would be applied, nor was the agency prepared to "cement it" in the collective agreement, she said. "A capped sum of $17m a year for recruitment and retention doesn't really stack up compared with the $384m they're currently paying contingent staff. "There's quite a lot to say about Health NZ as an incompetent employer, which is driving people out of salaried work into locum work and temporary gigs." With no clear indication of how the payments would be disbursed, there was a risk that Health NZ would want to scrap other allowances already in place, she said. "We've seen this shambolic approach to the ED shift allowance that we won in the last bargaining round and which is still not properly applied across the country. "So I guess our trust in the competence of Health NZ to put these things in place is very low." Man considered dangerous had escaped from Hillmorton Hospital

Photo: Canterbury DHB Mental Health Minister Matt Doocey has reiterated to Health New Zealand that patient and public safety are of "paramount importance", after a man absconded from a mental health facility. Canterbury police released a statement about 9pm on Thursday appealing for information about the 35-year-old who had absconded from the care of another agency. Police had been notified about 6pm that the man was missing. They said he was dangerous and shouldn't be approached. The man was found about 10pm. Do you know more? A spokesperson for Mental Health Minister Matt Doocey told RNZ his office was advised on Thursday night about a patient leaving Hillmorton. "The Minister was advised by Health New Zealand that safety of the individual and community was at the forefront in how they responded with police." HNZ staff "responded immediately" and worked with Police to safely locate the person and return them to Hillmorton last night. "Health New Zealand is taking the incident seriously and there will be a rapid review and will take appropriate action as required. "The Minister reiterated to HNZ that patient and public safety are of paramount importance, and Health New Zealand have assured the Minister that they are taking all the necessary steps to ensure both." The incident comes three years after Hillmorton forensic mental health patient Zakariye Mohamed Hussein murdered Laisa Waka Tunidau as she walked home from work. Hussein was on community leave at the time of the killing. Two reviews were ordered, one into Hussein's care, and another looking at Canterbury District Mental Health Services. In June another Hillmorton mental health patient, Elliot Cameron was sentenced in the High Court at Christchurch to life imprisonment with a minimum term of 10 years for murdering 83-year-old Frances Anne Phelps, known as Faye in October last year. Following his sentencing a suppression order was lifted allowing RNZ to report Cameron killed his brother Jeffrey Cameron in 1975. A jury found him not guilty of murder by reason of insanity and he was detained as a special patient. Cameron was made a voluntary patient at Hillmorton Hospital in 2016, and then in October last year murdered Phelps, striking her with an axe. RNZ exclusively obtained emails from Cameron to his cousin Alan Cameron sent over more than a decade, detailing his concerns that he might kill again . In response to the revelations, Chief Victims Advisor Ruth Money said it was hard to see Phelps' death as "anything other than preventable". RNZ earlier revealed another case involving a man who was made a special patient under the Mental Health Act after his first killing was recently found not guilty of murder by reason of insanity for a second time, after killing someone he believed was possessed. After that article, Money called for a Royal Commission of Inquiry into forensic mental health facilities. After Cameron's first killing was revealed, Money said she stood by her recommendation. "The public deserves an inquiry that can give actionable expert recommendations, as opposed to multiple Coroners inquests and recommendations that do not have the same binding influence. The patients themselves, and the public will be best served by an independent inquiry, not another internal review that changes nothing."
