Living longer is a goal many people share — but as we grow older, it becomes clear that how we age matters just as much as how long. Increasingly, the focus has shifted from simply adding years to your life to enhancing the quality of the years you enjoy. Whether you're already into your golden years or you're in your 30s or 40s and planning ahead, healthy aging is worth paying attention to. The stats confirm it: The global market for complementary and alternative medicine aimed at anti-aging and longevity is projected to soar from $51.87 billion in 2024 to $146.29 billion by 2030. A major player in that space? Supplements. From creatine and collagen to omega-3s and resveratrol, the list of popular anti-aging supplements keeps growing. One you've likely heard of is CoQ10. Coenzyme Q10 (CoQ10) is a naturally occurring molecule that's found in every cell in your body. It plays an essential role in energy production and also acts as an antioxidant, helping to protect your cells from harmful free radicals. CoQ10 levels naturally decline with age, and having low levels of CoQ10 may play a role in the development or progression of certain diseases. "Lower CoQ10 levels are linked with reduced mitochondrial efficiency and increased oxidative damage. Because mitochondrial dysfunction and oxidative stress are major contributors to age‑related decline, [some] researchers and clinicians see CoQ10 as a potential way to support cellular longevity and overall health as we age," says Dr. Pooja Gidwani, a physician focused on longevity and health optimization. Luckily, CoQ10 is found in a variety of foods, as well as supplements. The question is, how beneficial is it to take a CoQ10 supplement? As a registered dietitian whose job is to review and stay up to date on supplements, I was curious to find out. I consulted two medical doctors and two fellow RDs to better understand what CoQ10 is, what the evidence says about its benefits and who might benefit the most from taking it as a supplement. "Coenzyme Q10 is naturally produced in the body and is inside the mitochondria. Most people may remember learning about CoQ10 in biology as a part of the electron transport chain," says Dr. Austin Shuxiao, an internist. Shuxiao goes on to explain that CoQ10 plays a role in generating adenosine triphosphate (ATP) — a molecule that carries energy to all of the body's cells. In addition to helping produce energy, Shuxiao says that CoQ10 also acts as an antioxidant, helping protect your body from damage caused by free radicals — unstable molecules that contribute to aging and chronic diseases. (back to top) CoQ10 and aging Clearly, CoQ10 is kind of a big deal. When it comes to healthy aging, Shuxiao notes that it's clear CoQ10 plays a key role, but there's still a lot we don't know. "Research for CoQ10 and aging is ongoing, and studies show mixed results or inconclusive results. To be honest, for most supplements related to aging, getting objective data is hard because the studies need to be stretched over decades to get real results," he says. Both Gidwani and Shuxiao highlight that the strongest evidence supports CoQ10's potential role in heart health. "In one randomized controlled trial involving older adults, CoQ10 combined with selenium in patients with selenium deficiency significantly reduced cardiovascular mortality and improved markers of heart function," says Gidwani. Additionally, several studies have demonstrated that CoQ10 supplementation may enhance endothelial function, which is essential for heart health, and may offer benefits for older adults with cardiovascular disease. A 2022 review published in the Journal of Nutrition found that adding CoQ10 to standard treatments helped improve physical performance and the ability to carry out daily tasks in individuals with heart failure. Other areas of healthy aging that CoQ10 may play a role in include: Brain health and neurodegenerative disorders: While more research in humans is needed, early animal and test tube studies suggest that CoQ10 may play a role in the prevention or slowing of Alzheimer's and vascular dementia. Additionally, a small study found a link between low levels of COQ10 and a decline in cognitive functioning in older adults. Exercise performance: Some studies suggest that CoQ10 might help reduce exercise-related muscle damage and oxidative stress, although researchers emphasize that more and longer-duration studies across different demographics are essential for bolstering these findings. CoQ10 may also help enhance exercise performance in older adults who are taking statins. Diabetes management: CoQ10 might be especially helpful for people with type 2 diabetes. One review found that taking 100–200 mg per day was linked to better insulin sensitivity and improved blood sugar control. Skin health and appearance: Research on CoQ10's role in skin aging is limited. That said, one review found that topical creams containing CoQ10 were linked to a noticeable reduction in wrinkle depth. The enzyme might also help protect against sun damage, though most studies have been done on mice. "No human study has demonstrated CoQ10 supplements make people live longer or age slower," says Shuxiao. Whether or not to take a CoQ10 supplement ultimately depends on your individual diet and health status, so it's best to talk with your doctor or registered dietitian before deciding. That said, Gidwani says CoQ10 can be especially helpful for older adults since their levels are naturally declining. It might also be worth considering if you are taking statins (which can lower CoQ10 levels) or have: heart failure multiple risk factors for heart disease type 2 diabetes a fatigue syndrome, like multiple sclerosis Additionally, Shuxiao recommends that those with a family history of Alzheimer's or vascular dementia talk to their doctor about possibly adding CoQ10 to their supplement routine. It's important to remember that there isn't a magic bullet when it comes to living a longer, healthier life. "CoQ10 is best thought of as a supportive tool for healthy aging. It works synergistically with healthy lifestyle foundations like balanced nutrition, movement, stress management and adequate sleep. Supplements can support mitochondrial health, but they are most effective when layered on top of these fundamentals," says Gidwani. CoQ10 supplements aren't for everyone.
