Health providers and the public have been urged to be on the alert for mpox cases after particles were detected in several sewage samples in North Carolina.
The risk to the public is low at this time, the North Carolina Department of Health and Human Services (NCDHHS) said in a news release on Tuesday.
Why It Matters
Mpox, previously called monkeypox, is a rare disease. Part of the same family as the virus that causes smallpox, it is caused by a type of virus typically found in West and Central Africa. It is "primarily spread by prolonged close contact, typically skin-to-skin, often during sexual activity," according to the NCDHHS.
Early symptoms include fever, tiredness, headaches and sometimes a sore throat or cough. Those who contract the virus often develop a rash that changes over time (from flat spots to raised bumps, blisters and scabs). The process usually takes between two and three weeks, and those who are infected are contagious until all scabs heal and new skin forms.
What To Know
North Carolina's detections were found in wastewater samples collected on March 25, March 28 and April 8 from a treatment plant in Greenville.
They were all clade I—one of the two genetic types of the virus, the other being clade II. To date, only four clade I cases have been reported in the U.S.
"These detections mean there was possibly at least one person with an undiagnosed or unreported clade I mpox infection present or traveling through the Greenville area around the time of these detections," the NCDHHS said.
The results were found by the North Carolina Wastewater Monitoring Network, which was launched in 2021 to help improve information about the spread of certain viruses.
Samples are collected routinely from 35 wastewater treatment plants across the state and tested for specific viruses, including SARS-CoV-2 (the virus that causes COVID-19), influenza and respiratory syncytial virus.
This year has already seen two mpox cases identified in North Carolina. Both were clade II.

It was kind of like seeing a pregnant octogenarian in a nursing home. This is just the second time scientists have observed such a disk around a star at the end of its life. And though they can't actually see baby planets in their nebula images, they are now wondering if these space environments could trigger a second generation of planets, long after the original brood of worlds formed around the star. The discovery, published in The Astrophysical Journal, is "raising questions about their nature, formation, longevity, and potentially a second phase of planet formation," the authors wrote. Unlike giant stars that explode into a supernova and collapse into a black hole when their time is up, a medium star gradually runs out of nuclear fuel and suffers a more prolonged death. These stars, like our midsize sun in perhaps 5 billion years, decline into so-called planetary nebulas, a confusing misnomer for the phenomenon because they have more to do with aging stars than planets. Scientists have discovered a few thousand planetary nebulas in the Milky Way. Those include the Ring Nebula, aka NGC 6720 and Messier 57. With Webb, the researchers could see a compact dust cloud around the central white dwarf creating it. "These are first seen as the gas-and-dust-rich planet-forming disks in young stellar objects, and are an integral part of the star formation process itself," the authors wrote. "Remarkably, dusty disks or disk-like structures manifest themselves again as these stars reach the ends of their lives." Such a rare sighting is believed to have happened once before, when astronomers got a look at the Southern Ring Nebula with Webb, at wavelengths far beyond what people can see with their eyes. In the first images, astronomers were amazed to spot the true source of the nebula, said Karl Gordon, an astronomer at the Space Telescope Science Institute in Baltimore. "We knew this was a binary star (beforehand), but we effectively didn't really see much of the actual star that produced the nebula," he said during a 2022 news conference. These particles are miniscule, perhaps less than one-thousandth the thickness of a human hair. The cloud itself stretches thousands of times wider than the distance between the Earth and the sun. The researchers noticed something else intriguing at the center of the nebula. The white dwarf's brightness keeps changing. That might be a clue that another star is lurking in the shadows nearby, perhaps a small-but-feisty red dwarf star. While scientists haven't directly spotted the buddy yet, they can infer it's there from patterns in the nebula. The study could help confirm earlier findings that suggested the star had two companions — one far in the outskirts of the system and another nearby. A close star could explain the strange arcs and rings seen around the nebula.

Last year, South Dakota reported its first measles case in nine years. The state Department of Health recently reported the state's first case this year in Meade County in western South Dakota. Last week, on Friday, a second case was reported in Rapid City. People who visited Sam's Club in that city on June 1, or Dakota Premier Medical Center the following day were urged by the department to monitor themselves for symptoms for 21 days. Measles is a highly contagious viral disease that spreads through the air. Those who lack immunity from vaccination or past infection are highly likely to catch it from an infected person. As surrounding states report more cases, Health Department Secretary Melissa Magstadt said the state's 'Wellness on Wheels' clinics can help encourage vaccinations. 'I question it myself': South Dakota vaccination rates fall amid mistrust and misinformation The fleet boasts five vehicles equipped to provide immunizations, test for sexually transmitted diseases such as syphilis, as well as provide screenings, prenatal care and other support. The effort fills in gaps to public health care access across the state, especially in rural and tribal communities, Magstadt said. 'It's about how we can actively use these tools to reach underserved populations,' Magstadt said. 'It's not something I would have thought about looking to leverage for something like measles vaccinations before.' The department hasn't decided where to send their fleet. Counties with the fewest kindergarteners vaccinated per capita for measles, mumps and rubella include Faulk, Jones and Hutchinson, state data shows. South Dakota counties that share tribal land and rural counties in south-central areas of the state rank the worst for clinical care use and access in the state, according to the University of Wisconsin Population Health Institute's 2023 report. Federal COVID relief funds paid for Wellness on Wheels. 'Because of the pandemic, public health infrastructure was found to be wanting,' Magstadt said. That infrastructure missed 'critical pieces' that hadn't been invested in, she said, such as health care access in rural areas. More than a hundred rural hospitals in the U.S. have closed in the last decade. The program, launched in April of last year, cost about $800,000 in federal funding. The state's public health COVID funding was also used to support a community health worker program, update emergency medical service equipment and telemedicine access, analyze the state of emergency medical services in South Dakota, and build a Public Health Lab and department training center. Magstadt said staff working with the Women, Infants and Children (WIC) program requested the mobile units. WIC is a federal-state program that provides healthy food, nutrition education and health care referrals to low-income women and their young children. So far, Wellness on Wheels staff have mainly driven to events. Magstadt said the department has focused on increasing awareness of the program and building trust in communities and among tribal leaders. She plans to have staff drive the buses to rural communities more regularly to increase exposure and encourage use. 'Being consistently at a facility or place every other week will help people find it,' Magstadt said. 'We talk about the importance of STI testing, for example, but if you don't know where to get tested then that's another barrier.' Magstadt plans to have the department park one of the vehicles at a homeless shelter in Rapid City this summer as well to encourage underserved urban communities to seek services. 'We like people to be connected to primary care services, but there are unique situations where it's harder to get to health care facilities,' Magstadt said. She compared the mobile clinics as a return to home visits by doctors. That practice largely ceased in the 1960s due to cost efficiencies. 'It's a part of this menu of health care access and options no longer requiring people to come to a clinic or health care system,' Magstadt said, 'but health care being taken to patients and families who need it.' Mobile health care clinics are available in every state. They range from public entities like South Dakota's Wellness on Wheels, to specialized, private care. "I had full trust in the universe and what life was going to look like for me," she said. "That was until my life drastically changed within a matter of days." Gwendolyn was what Spearing called her "pleasant surprise." While her pregnancy was unplanned, she said she quickly embraced it. "I remember daydreaming what my child would look like; who they would grow up to be," Spearing said. "For me, my pregnancy was just the beginning of something amazing, and I was so, so excited to be a mama." At 28 weeks and five days, Spearing went into premature labor—a terrifying experience that unfolded while she was visiting Gwendolyn's father, who had recently been admitted to the ICU after a serious accident. "I didn't want to believe it as I was hardly into my third trimester," she added. Spearing walked herself to the labor and delivery floor, where she was met by a team of specialists. "I faintly remember looking over to the bedside nurse, asking her what I should expect and begging and pleading for a safe arrival of my baby," Spearing said. "I told the nurse, 'That wasn't my birth plan. I don't even have a nursery at home.' "She looked at me and said, 'Hunny, there is no such thing as a birth plan'," Spearing said. Within two hours and just three pushes, Gwendolyn was born at 2 pounds, 10 ounces. What followed were months of medical hurdles. Gwendolyn was diagnosed with Esophageal Atresia with Tracheoesophageal Fistula (EA/TEF)—a rare congenital condition where the esophagus doesn't connect to the stomach. Gwendolyn's prematurity brought additional complications: brain bleeds, underdeveloped lungs, multiple surgeries, code blues (where a patient requires resuscitation or is in need of immediate medical attention) and long hospital stays. Overall, Gwendolyn spent 170 days in hospital before being cleared to go home. Against the odds, she was thriving and hitting every milestone, according to her mom. "Appointments were further and farther between; she was attending playgroups and making friends; she was learning and discovering as a child should be at her age," Spearing said. "For the first time in her life, she was able to be just a kid." Then, on April 5, their world came crashing down. "For us, it was a normal day," Spearing added. "Little did I know this day would change my life forever." That evening, Gwendolyn's heart stopped. The autopsy provided no definitive cause. Some specialists suspect a catastrophic seizure or undetected heart failure. Spearing explored genetic counseling in the aftermath but was overwhelmed by the lack of answers. "I may never know why my daughter passed, and although I know now sometimes things just aren't meant to have an explanation, I still lay awake at nights contemplating the days leading up to her death and wondering if there was something I missed and if there was something apparent; maybe I could have saved her," Spearing said. Now, three years later, grief remains a constant companion. Spearing said that she barely remembers anything after Gwendolyn passed. "I was very angry. I lost trust in the universe and I questioned my beliefs," she added. "I wanted nothing more than to wake up from this nightmare that I was living in. I couldn't understand why a beautiful, innocent soul, who fought so hard to be here, only to be taken so abruptly." Out of her grief, Spearing created The Gwen Effect, a foundation honoring her daughter's legacy. It supports premature infants and children with complex medical conditions, funds research, and offers community support for grieving families. "I found a way of turning my pain into a purpose," Spearing said. "I continue to speak on grief and life after loss as I truly believe it is something that needs to be talked about more." The foundation's mission is clear: raise awareness, provide resources and create spaces where both grief and hope can coexist. Through it, Gwendolyn continues to touch lives. "Gwen was a ray of sunshine," Spearing said. "She was born tiny but mighty. Her passion for life while in and out of hospital showed us and many others how precious our time is. She was strong and brave."
