A council does not know how many of its staff who require criminal record checks have had one completed, it has emerged.
Norwich City Council has been told to urgently find out who has undergone the Disclosure and Barring Service (DBS) process and who needs one.
There was missing data for 41 out of 54 members of staff whose job required an enhanced DBS check.
The local authority said: "There are no examples of the council putting anyone at risk because of a lack of DBS checks when they have been needed."
Council bosses faced pointed questions at a meeting of its audit committee on Tuesday night.
A report presented to the committee said the council was "not able to identify whether all staff in posts requiring a DBS check have had one".
It described a process in which the data was manually compiled on to a spreadsheet but was incomplete.
A private company has been brought in to complete the process.
The problem was identified in July 2023 following an audit into the council's safeguarding arrangement, councillors were told.
But the audit report said since then, there had been no progress reports and the issues had not been escalated.
Alex Catt, the Green group leader, described the situation as "shocking".
"It has been known to the council for at least two years with seemingly nothing done about it," he told the BBC.
"The council works with lots of vulnerable people and we need to know the background of the people working with them."
When asked how long this situation had been going on for at the meeting, executive director of human resources Dawn Bradshaw replied: "I don't have the answer to that."
Emily Yule, the council's executive director of resources, said: "This is the first DBS audit we've had in the 20 years I have been here."
The checks are undertaken by the DBS agency, previously known as the Criminal Records Bureau.
According to the Home Office, employers can request one in order "to make safer recruitment decisions" particularly when employing people working with children and vulnerable adults.
Basic DBS checks reveal spent and unspent convictions, cautions and reprimands held on the Police National Computer.
Enhanced checks include "additional information held by local police that is considered relevant to a specific role".
A spokesperson for Norwich City Council said DBS checks were a "serious matter to get right" and claimed that "all of our people who needed a DBS check, have had one".
"There are no examples of the council putting anyone at risk because of a lack of DBS checks when they have been needed," the statement continued.
"We fully accept we were slow in providing information to audit colleagues about DBS checks and that our record keeping needed to be improved.
"Both of these matters have been picked up and are successfully being addressed."

SINGAPORE – A total of 22 people, aged between 22 and 56, were arrested for their suspected involvement in the recent spate of government official impersonation scam cases. In a news release on May 28, police said the 17 men and five women were nabbed following islandwide anti-scam operations conducted between March 13 and May 22. Another four men and five women, aged between 22 and 77, are assisting in the investigations. Police said they have received several reports of scam cases involving the impersonation of banks and government officials since early March. The victim would first receive an unsolicited call from a scammer impersonating a bank officer, typically from DBS, OCBC, UOB or Standard Chartered Bank, or an employee of China-based services such as Union Pay and WeChat, police said. The scammer would tell the victim that he or she was suspected of being involved in a criminal matter, then connect them to another scammer posing as a police officer. This 'police officer' would convince the victim to surrender valuables such as cash, jewellery and bank cards to an 'agent' for investigation. In some cases, the victims would be instructed to buy gold bars, which they would then give up to an 'agent'. Preliminary investigations showed that the 31 people had allegedly facilitated such scam cases by collecting the valuables from the victims. These mules would meet the victims in public places to collect the items from them. They would later hand the items over to another mule, or leave them unattended at a public place to be picked up later. 'Victims would only realise that they had been scammed when the scammers become uncontactable or when they sought verification with the banks or SPF,' police said. Police investigations are ongoing. Those convicted of assisting another to retain benefits from a scam could face a jail term of up to three years, a fine, or both. 'The police take a serious stance against any person who may be involved in scams, and perpetrators will be dealt with in accordance with the law,' they added. 'To avoid being an accomplice to crimes, members of the public should always reject requests by others to use your bank account or mobile lines as you will be held accountable if these are linked to crimes.' Government officials will never, over a phone call, ask them to transfer money, disclose bank log-in details, install mobile apps from unofficial app stores or transfer their call to the police, they said. Police also encouraged members of the public to adopt crime prevention measures, such as installing the ScamShield app, setting security features and checking for scam signs with official sources. In 2024, victims lost a total of $1.1 billion to scams – a record high for a single year. A central court could be added to a city's renowned marketplace as part of plans for a major revamp. Smaller squares and a covered pathway through the middle are also options being put forward by Norwich City Council. Carli Harper, the Labour councillor responsible for major projects, said the authority wanted to make it "one of the most magical places you can visit in Norwich or the region". The council has now launched a survey asking for the public's views on its proposals and said it hoped to have final plans drawn up by this autumn. Earlier this year, the cabinet of the Labour-run authority agreed to spend £740,000 to commission proposals for potential new designs. Should changes go ahead, it would be the first major redevelopment of the market since 2006 – when ageing, wooden stalls were replaced with metal structures with roofs. The council said a central court or smaller squares could be used for "special events" and pop-up stalls, whilst a covered pathway would provide "a clear entrance point". Dominic Burke, co-owner of the Sir Toby's Beers stall, said some "exciting ideas" had been proposed. He added that traders had been meeting with the council to discuss plans and it was important for the wider public to comment. "The market belongs to everyone," he said. Parents fined nearly £1m over school absences

Derbyshire County Council has generated nearly £1m from fines issued for unauthorised school absences, a Freedom of Information Act request has revealed. The Local Democracy Reporting Service (LDRS) obtained figures which show the authority issued 20,487 penalties since 2021, totalling £961,320, over the three academic years since 2021-22. The county council said the fines had been issued at the request of head teachers and according to the law. However, mother-of-two Natalie, from Marehay near Ripley, has started a national campaign, called Fight School Fines, to get the law changed. She has started a petition, so far signed by 161,000 people, calling for parents to be entitled to 10 days of term-time leave without a fine. "A lot of these fines that are being issued and prosecutions for unauthorised absences aren't due to just going on holiday because they want to get it cheaper, which is the media narrative," Natalie said. "They are marking things down that should fall under exceptional circumstances. "For example, we are seeing loads of stories from parents where they have asked for authorised leave to visit family members who are poorly, or they have gone to a funeral overseas and they are not marking it as authorised, they are refusing it." The number of fines issued in Derbyshire in 2021 was 3,175 and that rose to 10,505 in 2023-24. Fine amounts stand at £160 per parent, per child, with this fee reducing to £80 if paid within 21 days. The council said 16,064 fines were for first-time offenders and 1,567 for second offences, with parents only able to accrue two penalties in a three-year rolling period before being prosecuted in court. Council figures show 1,016 Derbyshire parents have been prosecuted in the past three complete academic years , with six cases being withdrawn. Court fines do qualify as a criminal conviction and are attached to a criminal record, which would need to be added to a DBS check, the LDRS said. "I am not surprised to see it going up," Natalie said. "They say it will act as a deterrent, but clearly it is not doing because people still need to take leave during term-time. "Even if it is a cost thing, they are doing it to save money, we don't know the reason why they can't afford to go in the holidays. "It is not so black and white as people make out. Some people work seasonal jobs or run their own businesses, they can't just shut up shop. "It doesn't act as a deterrent. "It is criminalising parents. "Many parents don't know that what comes with that court fine is a criminal record. "Most of the time, parents are doing it because it is the best decision for them and their children. They are our children, they are not the state's. "Having a criminal record for this is one step too far. "I know families need to access term-time absence and that will never stop. "This isn't just about being on holiday and getting a cheaper holiday." The LDRS said it asked the county council if it felt the penalties were serving as a deterrent, if they could respond to the perception that the penalties were criminalising parents, and if parents were saying they were willing to accept the fines to gain term-time holiday savings. A council spokesperson said: "Headteachers ask us to issue the fines, so this is one for schools and not the council. "We respond to headteacher requests, we do not make the policies that schools make that lead to fines being issued. "The government sets the penalty fines amounts. "This is the law and not the council's decision," the spokesperson added. The Department for Education, responding to Natalie's petition in December, said: "We sympathise with parents who, for a variety of reasons, face barriers to securing their children's school attendance. "This government is taking a new approach to tackling absence based on responsibility, partnership and belonging." It said that involved working with schools and councils to remove barriers to attendance. The spokesperson said: "The government takes seriously its responsibility to ensure that schools are equipped to meet children's needs and help them succeed, but that is matched by parents' legal responsibility to send their children to school every day that they can. The department said it acknowledged some children had long-term physical or mental health conditions or special educational needs and disabilities (SEND) which affected attendance.
