Health insurance is fundamentally broken. Here's how to fix it from the ground up

It's no secret that when it comes to simplicity and convenience, insurance has lagged behind modern businesses across most industries. While many legacy products have been overturned by newer, intuitive solutions or adapted to meet today's consumers' needs, insurance offerings have remained a complex anomaly—built more for business and regulatory needs rather than real people.
We already know that healthcare in America is too expensive, and far too many people simply can't afford to get sick. Health insurance deductibles are at an all-time high, while denied and delayed claims payments persist. Insurance companies have continued making money while a staggering number of people are putting off care because of the cost, or pulling money out of their 401(k) plans early to pay for medical expenses.
We also know that there is no shortage of people who are fighting for healthcare policy changes, more accountability around business practices, and broader reform. But consumer patience is running out. And with 100 million American adults in medical debt, incremental regulatory changes won't be quick enough to take on the challenges that so many people face today in affording basic, and sometimes unexpected, costs related to their healthcare.

It's time we ask the question: Can we start making progress by rethinking how health insurance products are fundamentally designed? In order to do this, we have to accept some consumer truths about health insurance today.
First, more than 50% of Americans don't understand their health insurance. That's because, unlike most other industries that have embraced simplicity and ease, insurance remains complex, filled with exclusions, jargon, and unnecessary paperwork. So even with the best of educational tools, we're looking at a large literacy gap.
Second, consumer animosity toward health insurance companies is almost as old as the business itself. Even traditional supplemental health insurance offerings like Accident Insurance and Critical Illness that are intended to provide extra support were not designed to cover very much. And the amount of money that goes back to the insured at the end of the day is low for every dollar of premium paid. So it's not surprising that people are tired of frequent claim denials and delays, and don't trust that their coverage will minimize their out-of-pocket exposure as much as they need it to.
It's obvious that we have to start with addressing what's covered, and how we pay claims, which is core to any insurance product. We can't just slap technology onto insufficient coverage and hope that it'll improve consumer confidence and trust. But there are some things we can do today to help health insurance catch up with other consumer industries through human-centered design. Here are three:
1. We can set up insurance products like subscriptions
Digital simplification across industries has been happening since the dawn of Amazon and Uber, and has taught consumers to have certain expectations around their apps and online experiences. Just for the purposes of this exercise, let's think about insurance in terms of the direct-to-consumer fulfillment model—and treat insurance like a subscription where care is the purchase and benefits are refunds.
Thinking of benefits as refunds enables us to align insurance innovation with the digital simplicity today's consumers expect. By removing the complexity wrapped around benefits, we can make claims intuitive and the delivery of benefits seamless. We have to eliminate the common limitations and restrictions as well as the excessive paperwork and evidence required for claim approval to enable quicker benefit decisions. People should be able to track the status of their claim in real time, so they can stop chasing their insurance companies for answers. Payment should be electronic, and take days, not weeks. In doing this, we can start to replicate the standards of efficiency and transparency people today are accustomed to when they buy and return products, and build a modern insurance experience that meets digital consumers' expectations and needs.
Treating health coverage like a subscription also provides clear opportunities for engagement. And insurance companies across sectors are just starting to take note. Beam Technologies tracks teeth-brushing habits through a Wi-Fi-connected toothbrush to tailor dental insurance discounts, while Discovery offers its Vitality Program that rewards customers with points for preventative checkups. By encouraging consumers to engage with their coverage, motivating them to use it by offering rewards and discounts like other consumer businesses, these companies drive utilization and deliver value as a result.
2. We can take an (active) back seat
Another school of thought tells us that the best insurance companies are the ones that people don't think about at all. Unlike the previous approach, which assumes consumers will use their coverage like they do their subscriptions, this strategy doesn't expect that the modern consumer wants to interact with their insurance company at all. Instead, it offers a simple promise of value without any participation required.
This approach provides obvious opportunities for leveraging data and automation to set up a product experience where the claim end-to-end is managed by the health insurance company, with no necessary intervention from the insured at all.
Innovators in the space are just getting started automating several, if not all, aspects of the claims process. Current players like ClaimsMinder, Human API, and Claritev are just beginning to unlock the power of data to streamline the notification, filing, processing, and payment of a claim so that employees can get more out of their benefits. Ansel Health offers 'medical claims integration,' which enables them to determine when individuals have a covered condition and are eligible for a benefit, and pays them directly without requiring them to ever file a claim.
In doing this, we eliminate coverage disputes, delayed claims payments, or even just make someone's day a little bit easier—and maybe then begin to build some trust.
3. We can become a logical extension of the modern care-delivery revolution
When people get sick, they don't see their preventative care or treatment and their health insurance as two separate entities, but rather one experience.
Unlike insurance, modern care delivery has evolved at a rapid pace over the past 25 years. Digital and in-person clinics like Wally, Maven, Tia, Omada, Parsley, and One Medical are setting new standards for the way people experience care, and even legacy care providers are mimicking their practices with apps and more. Notably, most of these providers have chosen to offer subscription-based or up-front payment models and to 'cut out the insurance middleman.' Why? Because it just doesn't align with their promise of simplicity and empathy through the provider experience.
By applying human-centered design, we can bridge this gap—creating insurance brands that look and feel like a natural extension of the modern care delivery experience. Companies like Oscar, Rightway, and Sana are already proving this is possible, offering an integrated experience that resonates with today's consumers.
It's a fair question to ask, but why will business stakeholders care?
A part of it will be about finding a set of believers and innovators who are both mission-aligned and consumer-focused. But it's also important to acknowledge that fixing the flaws in the health insurance experience doesn't just address the financial crisis around healthcare, it aligns with business needs too.
In fact, there's a clear business case for creating simpler health insurance products.
According to the Integrated Benefits Institute, serious illnesses, when not treated, result in an average of 1.5 billion lost work days per year, costing employers $575 billion annually. When employees don't delay or skip care because they're confident in their health coverage, they're less likely to require sick time, workers' compensation, disability, or family and medical leave. Employers see higher retention, insurance brokers and agencies build deeper trust with their clients, and insurance companies see a higher rate of renewals. Established companies can participate too through partnerships with modern solutions, allowing them to stay competitive in an evolving marketplace, benefiting new, innovative players seeking broader distribution.
Our goal is simple: In 10 years' time, we want people to be talking about how complex health insurance used to be, and finally build some real trust.

Older Adults Are Using More Cannabis

Cannabis use in older adults is up, according to researchers who used a national survey database to study emergent patterns in older adult use of the substance. Their results were published online in JAMA Internal Medicine . After noting an upward trend in seniors using cannabis — from 1.0% in 2005 to 4.2% in 2018— a group of investigators led by Benjamin H. Han, MD, from the University of California San Diego School of Medicine, conducted a cross-sectional analysis of the 2021-2023 cohorts from the National Survey on Drug Use and Health. Their sample included 15,689 adults aged 65 years or older. More than half of the sample (54%) were women. Han and colleagues found that when compared with prior years, past-month cannabis use increased significantly across the cohorts, from 4.8% (95% CI, 3.9%-5.9%) in 2021 to 7.0% (95% CI, 6.2%-8.0%) in 2023. In a subanalysis, the investigators found that increased prevalence in past month cannabis use was associated with multiple demographic factors. Both women and men showed increased trends, with an odds ratio (OR) of 1.32 (1.10-1.59) in women and an OR of 1.18 (0.97-1.43) in men. Increased past month cannabis use was also observed in older adults with a minimum $75,000 annual income, those with college or postgraduate degrees, those who were married, and those living in a state with legalized medical cannabis. Further subanalysis of the survey respondents by race showed that persons who identified as 'other' sustained the highest OR of increased use, OR = 2.26 (1.12-4.59). Older adults with the highest incomes previously were found to have the lowest prevalence of cannabis use compared with other income levels, but by 2023, they had the highest usage increase. The study authors suggested this could be due to their ability to afford medical cannabis. Although an increase in past-month cannabis use was associated with a number of medical conditions, the researchers wrote that they were unable to cleanly sort recreational from medical use. People with chronic diseases, especially those with multiple conditions, saw a spike in cannabis use. The most common medical condition recorded was chronic obstructive pulmonary disease with heart conditions, diabetes, hypertension, cancer, and two or more chronic conditions also noted to have increased prevalence (6.4%-13.5%). 'The substantial increased prevalence in states with legalized medical cannabis highlights the importance of structural educational support for patients and clinicians in those states,' Han and colleagues wrote. 'The use of cannabis products, especially with psychoactive properties, may complicate chronic disease management among older adults.' A cannabis researcher not involved in Han and colleagues' study agreed. 'One of the most important findings was that those in residence in a state where medical cannabis is legal at the time of interview also showed greater increases in cannabis use,' said Elise Weerts, PhD, a professor in the Department of Psychiatry and Behavioral Sciences at Johns Hopkins Medical School, in Baltimore, in an interview with Medscape Medical News . Weerts is also a researcher at the Cannabis Science Laboratory at Johns Hopkins. Meanwhile, in an accompanying editorial, experts pointed to how, despite a lack of consistent data on cannabis use in older age, its use is growing as it is increasingly legalized. 'Existing therapeutic evidence for medical cannabis in older adults has been inconsistent across several conditions, with many studies suggesting possible benefits, while others finding limited benefit,' the authors of the editorial wrote. 'The potential harms of cannabis use in older adults are apparent, with increased risks of cardiovascular, respiratory, and gastrointestinal conditions, stroke, sedation, cognitive impairment, falls, motor vehicle injuries, drug-drug interactions, and psychiatric disorders,' they wrote. The editorial authors stated concerns about scant evidence and a lack of standards around administering cannabis in this patient population. 'Existing therapeutic evidence for medical cannabis in older adults has been inconsistent across several conditions, with many studies suggesting possible benefits, while others finding limited benefit,' they wrote. 'Much of the evidence for benefit derives from a single or a small number of studies with nonrandomized designs, and very few studies evaluated harms, making the benefit to risk ratio unclear.' The editorialists also pointed to the frustration older adult users of cannabis feel around 'the lack of awareness and education about age-related issues' at cannabis dispensaries and even in healthcare workers. The study's findings did not surprise Weerts, who said she has been tracking similar data elsewhere. She said she agreed with the editorial writers that the gap between cannabis use and the understanding of its potential adverse effects or contraindications is widening. 'I am concerned that older adults using cannabis may not be aware of the risks, and that data supporting its efficacy for medical purposes are still not available,' Weerts said. 'We need randomized placebo-controlled trials to demonstrate any potential benefits and also track any adverse effects and potential harms.'

Guardians' Triston McKenzie Struggles In Arizona Complex League

Command and control continue to haunt Cleveland Guardians right-hander, Triston McKenzie. McKenzie, 27, struggled mightily in his brief appearances with the Guardians this season. To put McKenzie's background in context, he was a former Cleveland Indians 2015 first round draft pick out of Royal Palm Beach Community High School in Florida. McKenzie made just four appearances for the parent Guardians in April. His ERA of 11.12, and 2.47 WHIP came in 5.2 innings on the mound. In his appearances, McKenzie walked seven, struck out four, and yielded one home run among the seven hits he allowed. The Guardians designated McKenzie for assignment on April 21.. McKenzie was not claimed by another organization. He cleared waivers, and accepted an outright assignment to the team's Triple-A Columbus club. Prior to reaching Columbus, McKenzie was initially sent to the Guardians spring training complex in Goodyear, Arizona, to work on his mechanics. McKenzie, 6-5, 165 pounds, had been dealing with shoulder/elbow and arm related issues for the past few seasons. McKenzie decided not to have any type of corrective surgery, opting instead to rest, rehabilitate his throwing arm, and try to recapture the volume of work he enjoyed in 2022, his best season with Cleveland. McKenzie, nicknamed 'Dr. Sticks,' because of his long, lanky frame, won 11 games for Cleveland in 2022. He threw 191.1 inning in 30 starts. McKenzie flashed 'electric stuff' that year, and the Guardians hoped they had developed a reliable starter for years to come. It never materialized. McKenzie was sent to the Arizona complex to work with Cleveland's highly regarded pitching coaches and tutors. Columbus would have to wait. Then, on Friday, May 30, McKenzie entered an Arizona Complex League game for the Guardians team. He started the second inning of the game. Here are his statistics from that Rookie League appearance in the Arizona desert: 1.1 innings pitched 1 hit 2 runs, both earned 4 walks 3 strikeouts 1 home run 1 Wild pitch 1 batter hit by pitch Of the 45 pitches McKenzie threw, only 21 were strikes. He faced a total of 10 batters. The results of his first appearance with the Rookie League team were discouraging, to say the least. Once again, command and control escaped McKenzie in the outing. The Arizona Complex League is populated by young, mostly first-year professionals who are playing for their first professional team. It is a time to learn the ropes of becoming a professional baseball player. Many of the players were international selections, and are stateside for the first time. There is an occasional veteran who shows up on a rehabilitation assignment. That was the case with Shane Bieber, who made a start in the Arizona Complex League as he works his way back to the Cleveland major league mound. When healthy, McKenzie flashed excellent life on his fastball, and a good 'feel' for his secondary pitches. Sadly, his shoulder/elbow/arm issues were never treated with a surgical procedure, and the impact of those injuries may still linger when he throws. McKenzie is working on a one-year, $1.95 million contract. While that isn't a great deal of money in today's baseball landscape, it is a Guardians financial investment with no immediate return. Baseball fans everywhere are hoping McKenzie can find the command, control, and pitching mechanics that have eluded him for quite some time. Perhaps his next appearance in the Arizona Complex League will be better than his first.

Deadly toxic algae bloom off Southern California coast ends

The toxic algae bloom off the Southern California coast that's been killing and sickening so many sea lions and other marine mammals has finally ended. "Our demoic acid toxic hostess event is over, for the time being," biologist Dave Bader said. Bader, who works for San Pedro's Marine Mammal Care Center, said recent samples of the ocean water do not show signs of the algae that create the toxic domoic acid bloom that has been afflicting sea lions, dolphins, pelicans and even whales over the past three months. "It's been rough," Bader said. "We've seen so many animals." For those three months, Bader and his team have triaged and treated thousands of animals that were seizing, dying or dead on beaches from Ventura to Orange County. Rescue centers shared the burden and pain during the toxic algae bloom. "It's not just the volume of animals," Bader said. "The females were pregnant, almost all of them, they all aborted their young ... Also, there were so many dolphins this time around as well, which is also tragic. So to see the volume of animals, the types of animals that are coming in, it was really hard on everybody." Typically, the algae blooms begin in June. However, this round of domoic acid spread began much earlier and has the possibility of returning later this year, according to Bader. "We still have another several months when a bloom of this type could reoccur," he said. Bader said humans created this crisis, and can ultimately solve it. "We need to do a better job of mitigating against the future effects of climate change, making it not as bad as it could be. That's step number one," he said. "It's a big step that we have to take. The ocean is telling us it's not well, it's not healthy, and we have to take those big steps to solve problems like this."
