
Sudden Intense Chest Pain Unlike Prior Pneumothorax

Background and Initial Presentation
A 35-year-old man with a history of two prior spontaneous pneumothoraxes — both managed conservatively — presents to the emergency department with new-onset chest pain and lightheadedness. The pain is described as sudden, intense, and exacerbated by deep breathing. He indicates that it is located in the center of the chest. He denies dyspnea or leg swelling and has no other complaints. When asked, he says this pain is different from his pneumothorax pain, mainly because it is in the center of his chest.
Physical Examination and Workup
Vital signs are within normal limits, except for a pulse > 120 bpm. Physical examination reveals diminished breath sounds on the left hemithorax. There are no rales or wheezes and no leg edema.
Discussion
Central pleuritic chest pain in a patient with a history of pneumothorax suggests a pulmonary etiology. A chest x-ray is the most appropriate investigation to promptly assess for recurrent pneumothorax, pleural effusion, structural abnormalities, or other intrathoracic pathology.
ECG or cardiac biomarkers such as troponins are indicated primarily if myocardial ischemia or infarction is suspected. This suspicion would be raised if the patient described his chest pain as squeezing or pressure-like sensation radiating to the neck, jaw, or left arm. The patient's young age and lack of history of coronary artery disease also make acute coronary syndrome less likely.
A CBC may provide supplementary information about signs of infection or inflammation, especially if the chest x-ray appears normal, but a CBC alone is unlikely to determine the cause of the patient's acute pleuritic chest pain.
On initial testing, chest x-ray, metabolic panel, and troponin level were normal. CBC showed an elevated white blood cell count (16,200/μL). An ECG was also performed (Figure 1).

Despite the central location of symptoms, the patient's young age, history of pneumothorax, and presence of pleuritic chest pain would have placed pneumothorax high on the initial differential diagnosis. After pneumothorax was ruled out with chest x-ray, PE should have become the leading consideration given the pleuritic pain and the patient's age group, in which PE is far more common than coronary artery disease. Although anxiety is also common in this age group — as it is in others — it should remain a diagnosis of exclusion, considered only after more serious conditions have been reasonably ruled out.
While the ECG is not diagnostic of PE, it raises suspicion by demonstrating three supportive findings: tachycardia, incomplete right bundle branch block, and nonspecific ST-segment changes.[1] A subsequent D-dimer test was positive, and chest CT angiography showed extensive bilateral pulmonary emboli, more pronounced on the left side.
PE typically presents as either unilateral pleuritic chest pain or as dyspnea with or without chest pain.[1] However, PE can present without the typical symptom of chest pain, sometimes being asymptomatic or discovered incidentally during diagnostic workup for other conditions.[1,2,3] Other symptoms of large pulmonary emboli may include syncope, diaphoresis, and cardiac arrest. Other symptoms of smaller emboli may include minor hemoptysis or cough.[1,3]
Although most patients with PE have at least one identifiable risk factor, up to 20% of patients present without any known risk factor, so the absence of risk factors should not exclude the diagnosis.[1]
Pain in patients with PE is believed to result from pulmonary infarction, which typically occurs when small to medium emboli lodge distally in the peripheral pulmonary arteries — areas with limited collateral circulation — making them more susceptible to infarction. The absence of chest pain does not exclude PE and may contribute to missed diagnoses, increasing the risk of patient morbidity and mortality.
PE classically presents with pleuritic chest pain and dyspnea associated with known risk factors, tachycardia, and clear lungs both on auscultation and chest radiography. However, most patients with PE present with one or more atypical features, which may include the absence of pain or any known risk factors and/or normal or nonspecific ECG findings.[1] About 40% of patients with PE have tachycardia.[1]
Scoring systems such as the PE Rule-out Criteria (PERC) can be useful in evaluating patients with suspected PE, but clinicians must be familiar with both the inclusion and exclusion criteria and should recognize that applying PERC requires a low pretest probability based on clinical judgment and the presence of a more likely alternate diagnosis with adequate supporting evidence.
When PE cannot be excluded based on clinical assessment, diagnostic testing is warranted, typically beginning with a D-dimer assay. If the D-dimer is positive, imaging with CT pulmonary angiography or a or ventilation-perfusion scan should follow.[2,3] D-dimer should not be ordered reflexively or 'just in case,' as this often leads to unnecessary imaging. As Greg Henry advises, 'In medicine and life, don't ask questions you don't really want to know the answer to.'
PE is typically treated with anticoagulants unless they are absolutely contraindicated, in which case a vena cava interruption filter may be used.[2,3,4] The treatment setting and choice of anticoagulant depend on various factors, including PE severity, comorbidities, and bleeding risk.[3] Most patients are admitted for treatment initiation, but some low-risk patients may be discharged with oral anticoagulants.[2] Patients with hypotension or right ventricular strain often require ICU admission for close monitoring and may be treated with thrombolytic therapy or, in some cases, surgical intervention.[1,2,3]
The absolute contraindication to thrombolytic therapy is a history of intracranial hemorrhage, due to a significantly increased risk of catastrophic bleeding.[1,2,4] Thrombolytic agents can dissolve blood clots, but they also impair hemostasis.
A history of pneumothorax episodes is not considered an absolute contraindication to thrombolytics in this patient.[1] Anemia is an important clinical factor that significantly increases the risk of bleeding during anticoagulation, but it does not preclude thrombolysis, if not caused by active bleeding or associated with a significant coagulopathy.[1,3,5]
Hemodynamic instability is not a contraindication but rather an indication for thrombolytic therapy in patients with massive PE. The benefits of restoring circulation outweigh the bleeding risk associated with thrombolysis.[1,2,4]
Hospital admission on intravenous heparin is reasonable. The patient could deteriorate if additional thrombi embolize. ICU admission is typically reserved for patients who remain unstable or require intravenous fibrinolytics. Discharge may be appropriate for stable patients who meet discharge criteria. For patients who are stable but do not qualify for discharge and have a low risk of decompensation, admission to a general medical floor may be considered.
Because this patient's CT angiography showed extensive PE and his vital signs were concerning, thrombolytics were considered. However, after heparin was initiated, his vital signs normalized within a few hours, so he was able to be admitted to a telemetry bed.
Although most patients with PE meet criteria for outpatient treatment,[3,4] a minority of eligible patients are actually discharged from the emergency department despite having an estimated mortality risk of less than 3%.[1,2] Risk stratification tools such as the Pulmonary Embolism Severity Index and the Hestia criteria can help identify candidates for outpatient treatment. Clinicians should also consider using an online calculator (eg, MDcalc.com). In addition to PE severity, clinicians should evaluate the patient's bleeding risk on anticoagulation when making disposition decisions.[1,2,3,4]

Rare condition made mom feel like she was "walking on a hot bed of Legos"
When Amber Meade twisted her ankle and broke a bone in her foot, she expected recovery to be easy. She needed surgery to shave down the damaged bone and make sure the nerves and tendons in her foot were where they should be. The procedure seemed to go smoothly, but in the weeks that followed, she began to feel debilitating pain. A second surgery found that a screw placed in her foot had been pressing on the head of one of her nerves. That screw was moved, and a nerve decompression was performed but the pain didn't stop. "It would just turn black and blue, swell up really big, and I just was unable to put any weight on it," 41-year-old Meade said. The pain upended everything. She left her job as a surgical assistant since she could no longer stand for long periods. She was unable to take part in family activities with her two young sons. Even light breezes could trigger the pain. Resting under a blanket was unbearable, making sleep difficult. Meade compared the sensation to "walking on a hot bed of Legos constantly, while getting stabbed with a pencil." Meade spent months seeing different specialists. Finally, after nearly a year, a doctor suggested Meade might have something called chronic regional pain syndrome. It was the first time Meade had heard of it. It would take even longer before she could find a treatment that helped her pain. What is chronic regional pain syndrome? Chronic regional pain syndrome, or CRPS, is a chronic disorder with no cure, said Dr. Rohan Jotwani, an interventional pain specialist and anesthesiologist at NewYork-Presbyterian and Weill Cornell Medicine. Jotwani was not involved in Meade's care. CRPS can occur after surgery, like Meade's, or from other injuries to the nerve. "For some patients, they'll have an injury to a nerve and it will get better with time. For some patients, that injury will actually develop into its own chronic pain disorder," Jotwani said. Diagnosing the condition is difficult, Jotwani said. No test or scan can definitively identify it, and because it is a rare condition, many doctors may not be familiar with it, he added. Physicians have to rely on clinical criteria. Telltale hallmarks for the condition are extreme levels of pain at the source, even when nothing should be causing it - like when a bedsheet causes pain, as Meade experienced. CRPS patients may also experience swelling, changes in temperature and skin color or texture changes. They may even begin to lose function in the affected area. Diagnosing chronic regional pain syndrome is only the start, Jotwani said. Actually treating the condition requires a "multi-pronged approach" that can vary from person to person. Treatment typically starts with physical therapy. Patients may also take medication to decrease nerve signals in the area, so they feel less pain, or try interventional techniques like a nerve block. Meade tried multiple options, but nothing was improving her pain. "Doing regular therapies was not working. I even heard, at 33, that I was getting older and that 'sometimes we just hurt more.' I thought that was comical," Meade said. Treating chronic regional pain syndrome In 2022, long after Meade's pain started, a doctor suggested she see physical therapist Dr. Anita Davis, who specializes in treating the condition and leads the comprehensive pain rehabilitation program at Brooks Rehabilitation in Florida. The pair worked to develop a physical therapy routine that would work for Meade. "Before me, she had all of the traditional exercises ... and those things were just not possible with that much pain in her foot," Davis said. "As healthcare providers, we ask patients to rate the pain from zero to 10. These folks are typically going to be at the top of the scale, even on a good day. When I ask her to do something that's painful on top of her existing pain, it's just crazy to think about doing that." They also worked on relaxation and emotional exercises to cope with the mental load of the constant pain. "Pain management for Amber ... isn't just the physical aspects," Davis explained. "Anything that enhances relaxing the nervous system is valuable." The protocol led to some improvements for Meade, but while CRPS patients can have "symptoms subside for a length of time," it is "a lifelong condition," Davis said. "There's not a cure for it," Davis said. Working towards a less painful future Meade said she still feels pain and discomfort, and is at risk of flare-ups when the weather changes or if she steps the wrong way. In addition to working with Davis, she is taking medication and receiving ketamine infusions every few months. She said the combination of ketamine treatment and physical therapy allows her to "do a lot more than I have been able to." The effects of the infusion usually last around four weeks. Meade uses that period to work on strengthening her body. "I try to make some headway in my physical health before that starts going down, because I know it's going to," Meade said. "The pain is a lot less when my physical health is better." Meade said even without the effects of the infusions, she is dealing with less pain than before her diagnosis and treatment. She can focus on her day-to-day activities. She sleeps better and can walk on the beach, one of her favorite activities. She can't do too much running around with her kids, but she can play catch and help with homework most days. Meade said she hopes to improve her functionality and eventually go back to work and be more active, but for now, she is grateful for what she has gained. "I want to try to get to that point where I have less and less flares, and get up to doing what everybody enjoys doing: Working, coming home, having something to work towards, going on vacations," Meade said. Summer heat, outdoor fun ... and cold and flu symptoms? The three may not go together in many people's minds: partly owing to common myths about germs and partly because many viruses really do have lower activity levels in the summer. But it is possible to get the sniffles — or worse — in the summer. Federal data released Friday, for example, shows COVID-19 is trending up in many parts of the country, with emergency department visits up among people of all ages. Here's what to know about summer viruses. How much are colds and flu circulating right now? The number of people seeking medical care for three key illnesses — COVID-19, flu and respiratory syncytial virus, or RSV — is currently low, according to data from the Centers for Disease Control and Prevention. Flu is trending down and RSV was steady this week. But COVID-19 is trending up in many mid-Atlantic, southeast, Southern and West Coast states. The expectation is that COVID-19 will eventually settle into a winter seasonal pattern like other coronaviruses, but the past few years have brought a late summer surge, said Dr. Dean Blumberg, chief of pediatric infectious diseases at University of California Davis Children's Hospital. Other viruses circulating this time of year include the one that causes 'hand, foot and mouth' disease — which has symptoms similar to a cold, plus sores and rashes — and norovirus, sometimes called the stomach flu. Do viruses spread less in the summer? Many viruses circulate seasonally, picking up as the weather cools in the fall and winter. So it's true that fewer people get stuffy noses and coughs in the summer — but cold weather itself does not cause colds. It's not just about seasonality. The other factor is our behavior, experts say. Nice weather means people are opening windows and gathering outside where it's harder for germs to spread. But respiratory viruses are still around. When the weather gets too hot and everyone heads inside for the air conditioning, doctors say they start seeing more sickness. In places where it gets really hot for a long time, summer can be cold season in its own right. 'I grew up on the East Coast and everybody gets sick in the winter,' said Dr. Frank LoVecchio, an emergency room doctor and Arizona State University researcher. 'A lot of people get sick in the summer here. Why is that? Because you spend more time indoors.' Should you get another COVID-19 booster now? For people who are otherwise healthy, timing is a key consideration to getting any vaccine. You want to get it a few weeks before that big trip or wedding, if that's the reason for getting boosted, doctors say. But, for most people, it may be worth waiting until the fall in anticipation of winter cases of COVID-19 really tick up. 'You want to be fully protected at the time that it's most important for you,' said Dr. Costi Sifri, of the University of Virginia Health System.
