Eight babies have been born free of a disease that can lead to terrible suffering and early death, thanks to pioneering scientists in the UK employing a form of genetic engineering that is banned in some countries, including the US and France. Ten years ago, when the government and regulators were considering whether to allow mitochondrial transfer technology, critics warned of 'Frankenstein meddling' that would lead to three-parent children. It's hard now to justify such hostility in the face of the painstaking work carried out by the scientific and medical teams at Newcastle, resulting in these healthy babies and ecstatic families.
Mitochondria, like tiny battery packs, supply energy to every cell of the body. Their DNA is handed down in the egg from mother to child. In rare instances, there are genetic mutations, which means the baby may develop mitochondrial disease. About one in 5,000 people is affected by it, making it one of the most common inherited disorders. As the cell batteries fail in various organs, the child can experience a range of symptoms, from muscle weakness to epilepsy, encephalopathy, blindness, hearing loss and diabetes. In severe cases, they die young.
There is no cure yet, so the aim is prevention. Women who have some damaged and some healthy mitochondria can have IVF and pre-implantation genetic testing (PGT) to select embryos that are clear of mutations or only slightly affected. The options for women with 100% mutated mitochondria used to be limited to donated eggs or adoption – until parliament changed the rules to allow the technology in 2015 and the Newcastle Fertility Centre was granted a licence by the Human Fertilisation and Embryology Authority to use it in 2017.
The process does indeed involve three people. The would-be mother's egg and a donor egg are both fertilised by the man's sperm. The nucleus of the donated egg is removed and replaced by the nucleus of the woman's egg, but its healthy mitochondria remain. This composite egg is inserted into the woman's uterus. The resulting baby's DNA will be 99.9% from the parents and only 0.1% from the donor. Hardly a three-parent child.
Yet there are controversies. Some countries will not permit use of the technology because of concerns over human germline genetic modification. The lab-mixed DNA will be passed to future generations, with who knows what consequences. And a question hangs over something called reversal, or reversion. The results of the Newcastle research published in the New England Journal of Medicine show that some of the embryos with healthy donated mitochondria developed mutations somewhere along the line. Mutations formed in 12% of one baby's mitochondria and 16% in another's. That was not enough to affect the babies, who were healthy, but previous work by other scientists has suggested that mutations can increase with time, and nobody yet understands why.
The Newcastle scientists and medics have been highly praised for their slow and methodical work. They have brought joy to some families and hope to others. But this is still experimental technology and caution is absolutely valid. And inevitably there are cost issues. People who can afford it will no doubt pay, but the NHS is unlikely to be able to help the rest. Nonetheless, this groundbreaking research must surely be allowed to continue, albeit only in the same careful fashion.

The 67-page document was written from Sophia's perspective as it was felt she had not been "seen or heard for such a long time".In it, Sophia said she believed she would be "happier" and "taller" if her life had been different. Renner-Thompson, who is the council's cabinet member for children and young people, said he wanted to make sure the local authority had done everything it could to make sure a similar situation did not happen said one of the problems found in the report was, in some cases, there was little cross-checking between different agencies."There was one example where some police officers visited the home and they left saying everything is fine," he said. "A GP commented later that the fact the child was so small and in a wheelchair, despite being able to talk and being quite happy and bubbly, should have raised alarm bells."Renner-Thompson said the council's introduction of family hubs was a solution to that problem, as it allowed families with issues to be identified and for partner organisations to be brought together "in the same room". He said the report did not blame a single organisation and all process in place had been followed he admitted that while those processes worked 99% of the time, they were found to be lacking in "very extreme cases"."I think the conclusions of the review would say that if there were different processes in place, it could have been prevented," he said."This is why the processes involved have to be changed and have been changed, and have to be recognised as have being changed." The NHS in England could learn important lessons from Scotland after a measles death in Liverpool last week, experts say. Comparative data shows that vaccine uptake in Scotland is considerably higher than in England, with nearly 90 per cent of children fully protected against measles by age five. The difference, say experts, is most likely to be explained by the fact that access to GPs and other primary health care facilities is much better north of the border 'Everybody talks about [vaccine] hesitancy, but the main thing that's stopping uptake is access,' Dr Helen Bradford, Professor of Children's Health at University College London, told The Telegraph. 'That includes knowing what vaccine is due and when, being able to make an appointment, and actually getting to that appointment – all of which can be really difficult for some families.' News of the death of a child in Liverpool of measles comes as Britain's vaccination rates have fallen to the lowest of any G7 country, including the US. The problem appears to be being driven by the performance of the NHS in England. At least 89.2 per cent of Scottish children are fully vaccinated with the measles, mumps, and rubella (MMR) vaccine by age five, compared to just 83.9 per cent in England. At the same time, Scotland has much better access to primary care – with 76 GP's per 100,000 people, compared to just 58 per 100,000 in England. 'General practice [in England] has continually had resources withdrawn, but at the same time delivers most of the childhood vaccinations,' said Dr David Elliman, an honorary senior associate professor and medical doctor at UCL. Access, he added, was the most important issue. 'In a GP practice, most parents are happy to have their children immunised. Some may have questions, but as long as they receive clear and satisfactory answers, they're usually comfortable proceeding,' he added. During the pandemic – a time where nearly all health systems saw a noticeable decrease in routine vaccinations – Scotland's coverage of routine childhood jabs actually increased. Researchers at the University of Edinburgh found a 14.3 per cent increase in children receiving the second dose of the MMR vaccine on time during the first lockdown in Scotland compared with vaccination levels in 2019. England saw a net fall in the same period of 1.93 per cent. The researchers linked the boost to flexible working for parents – which meant they could attend vaccination appointments – and better access to mobile vaccination centres. In 2018, the Scottish Government took the decision to transfer the responsibility for delivering vaccinations from GP practices to regional health boards as part of a plan to boost uptake. While most vaccines are still administered by GPs, the shift gave health boards the flexibility to tailor services for harder-to-reach communities. Unlike in England – where families often struggle with rigid booking systems, the so-called '8am scramble' for same day appointments, and limited availability – Scotland offers vaccinations in several different settings like schools, A&E departments, and at home visits. 'We need to get away from being too rigid in what sort of settings we give the vaccine in. Traditionally it's either been a community clinic or general practice but we ought to be doing it more in the places that parents go to, and particularly children who are at high risk,' said Dr Elliman. 'That means more immunisations in hospital settings and in neighbourhoods,' he added. Scotland's better vaccination rates appear to be positively influencing outcomes. So far this year, England has recorded 529 measles cases, while Scotland has had just 27. In 2024, those numbers were even more dramatic: 2,911 cases were recorded in England, compared to just 24 in Scotland. While population size partly explains the disparity – England's population is more than eleven times larger than Scotland's – it doesn't account for an almost 120-fold difference in infection rates, say experts. Only last week, the health secretary Wes Streeting acknowledged the need for better access to primary health care in England. Under the government's new NHS 10-year plan, he committed to rolling out 'neighbourhood health services' to try and 'end the 8am scramble and make it easier to see your GP,' referencing the country's maddening and archaic GP booking system. The document also includes a plan to boost vaccine uptake by allowing nurses and midwives that carry out home visits to administer vaccines in England. Yet it is unclear when the new measures will come into force: England currently faces a 5,000 shortfall in the number of health visitors needed to provide the mandated four at-home visits required for every child in the country under school age. Scotland outcompetes England in this area, too. About 90 per cent of Scottish children receive at least their first four scheduled reviews, compared to just short of 65 per cent in parts of England, including London. 'These are the public health nurses that have contact with all young families with young children, often the first port of call, the first place that parents will ask questions about immunisation. The numbers [in England] have gone down dramatically, which for those people who have got questions means there isn't always somebody obvious to ask,' said Dr Bradford.

I'm a dementia professor, but the solution to my brain fog surprised me

Aimee Spector, 50, is professor of clinical psychology of ageing and director of the Menopause Mind Lab at University College London (UCL). She has two children of 18 and 20 and lives with her husband, Nick, 52, in Hertfordshire. I'd always prided myself on being a 'names person'. My job as a professor at University College London involves chairing meetings, introducing people, giving lectures and leading teams – I'd remember everyone. But in 2017, aged 42, that changed quite suddenly. I'd recently taken a new consultancy role and was being shown around. Just as I went to introduce a very senior colleague, someone I'd worked with for 15 years and talked to daily, his name just… vanished. Gone. Awkwardly, feeling my face redden, I ended up helplessly pointing at the name on his door. I went home confused, shocked and frightened about what was happening. More mortifying still was only last month, when I was working with the World Health Organisation updating dementia guidelines. During a high-level meeting about HRT and dementia risk, I forgot the word 'oestrogen'. I paused, tried to joke, 'it's my morning brain fog ' and people smiled sympathetically, but I felt paranoid they thought I was losing it. A scary black fog had overtaken me After that first time forgetting my colleague's name and dreading it happening again, I'd walk into meetings feeling panicked about forgetting names, which of course made it even more likely to happen. It wasn't just at work. At home I'd ask my children – then both in their early teens – questions about a party for example. They'd eye-roll, 'Mum, you just asked that and we told you already.' I'd watch a film with my husband, Nick, and have no recollection the next morning. I don't mean I couldn't recall the name of the movie, I couldn't remember the entire storyline, or even the genre. It was like a scary black fog had erased my memories. Another time I drove off with my handbag still on top of the car, scattering its contents over the road. My fear of dementia was real – and informed Until then, I'd been a successful psychologist specialising in dementia research, developing cognitive stimulation therapy, to help people with dementia improve their memory and thinking skills, which is now used across the NHS and in 42 countries worldwide. As my career had been dedicated to studying dementia, my memory loss took on a sinister edge. In my early 40s, mine was now failing me and fearing I had early-onset dementia felt like my whole world unravelling. I told my GP about these frightening memory blanks. She reassured me it probably wasn't dementia, because I was still able to work with numbers efficiently (essential as a scientist) and neither did I get lost in familiar places, or struggle recognising familiar objects. I had no problem dressing or feeding myself or parenting my teens. I never forgot faces. It was just that my ability to recall stories and my verbal skills seemed to be replaced with blackness. The GP never suggested menopause as a possibility, and because I still had regular periods and no hot flushes, it never crossed my mind, either. Back then, brain fog wasn't discussed as a menopause symptom. She and I agreed to keep an eye on it and over the next two years we spoke several times. Blessedly, my symptoms didn't worsen, which would not have been the case with a disease. Talking myself out of a career I spoke to my husband, parents and close friends about my fears. 'Don't worry,' they said. 'You're still functioning at a high level at work – no one else notices.' Classically, with early-stage dementia it's not just the person suffering symptoms who realises it, it's other people who notice it. 'You're fine,' everyone reassured. But inside, I seriously considered quietly leaving my job. I'd tell myself: 'You've had a good career, made a difference. Maybe it's time to slip away before everyone notices you're losing it.' Brain fog destroyed my confidence, my sense of competence and my identity. I've since learnt that around one in 10 women in menopause quit work because they feel embarrassed about their abilities. Even more step back from important meetings (the kind necessary to further careers) or they reduce hours or pass up promotions. Careers women have grafted for, over decades, go up in smoke. It's frustrating to witness yet completely understandable – I was nearly one of them. Finding the real cause It wasn't until 2021, at the age of 47, nearly five years after my first symptoms, when a friend asked me 'could it be hormonal?'. So I paid to see a private menopause specialist. Hormonal blood tests are notoriously unreliable, yet mine indeed showed rock-bottom oestrogen levels, far lower than expected at any point in the menstrual cycle. My progesterone and testosterone were also low and the specialist put me on HRT: oestrogen gel combined with progesterone tablets. Within six weeks, it was as if someone had flicked a light switch in my brain. My memory returned, as did my confidence. Finally I had confirmation my brain fog was hormonal, not degenerative like Alzheimer's. When my health took a step back But just nine months later I had to give it up when, in 2022, I was diagnosed with stage one breast cancer. I'd found a lump and it was caught early so I needed surgery but no chemotherapy. I didn't blame HRT, I hadn't been on it long enough, and had a strong family history of breast cancer. However, because my cancer was hormone-receptive (meaning oestrogen was feeding the cancer cells) I had to immediately stop using it. That devastated me as much as the diagnosis itself, as I dreaded the return of brain fog. But interestingly, it didn't come back as badly as before, and I believe that's because now I understood it wasn't dementia – that fear had gone and it was easier to manage. Why perimenopause can cause brain fog I wanted to investigate why perimenopause causes these cognitive changes – and what can be done. Especially for women who don't want, or can't take, HRT. I found it helpful to acknowledge what was happening in my brain, understanding that my drop in oestrogen was likely to be affecting the neurotransmitters which affected memory and words. It's this fluctuation of hormones during perimenopause when things like brain fog, or hot flushes or poor sleep, are triggered. The good news is that once the hormones re-stabilise post-menopause, these symptoms go away. Putting my experience to good use After what happened to me I wanted to see what the research said, and discovered there was little out there. That had to change. I wanted other women to feel more supported. So in 2023, alongside my work in dementia, I established UCL's Menopause Mind Lab, a research hub focusing on the impact of menopause on cognition and mental health. I'm proud of what we've achieved so far. But there is more to do. Campaign group Menopause Mandate's survey last year (of nearly 20,000 people) identified brain fog as the single most troubling and prevalent symptom for 76 per cent of women – not hot flushes, which surprised many. That's why we're joining forces for this year's survey; we're determined to find better treatments. This is why this year we want more women – 30,000 – to take part, from as many different backgrounds and ethnicities as possible, including those with no symptoms at all: it's all helpful. These results will inform academic research on menopause and brain health and power lobbying efforts for better workplace policies. We will present our findings to MPs later this year. Currently at UCL we're developing a non-drug-based treatment including attention training, cognitive strategies and CBT (cognitive behaviour therapy) to support women who cannot take HRT. The strategies I use to help me At 50 now, I still have regular periods, and I still forget words, but I no longer spiral into panic now I've developed strategies to cope (see below). I want other women to know they're not losing their mind. Brain fog is a manageable symptom of menopause transition. With understanding, support and the right strategies, we can continue to thrive at work and at home. I almost gave up on myself – and I'm so glad I didn't. Please don't lose hope.
