The "One Big Beautiful Bill Act" that is making its way through Congress may put millions of people at risk of losing access to Medicaid.
One reform that may prompt that loss of coverage — the implementation of new work requirements — was included in both the Senate and House versions of the bill.
In 2034, about 4.8 million additional people would be uninsured based on work requirements in the House Republican budget bill, according to the Congressional Budget Office, a nonpartisan legislative scorekeeper.

"There's no question that millions of people will be harmed," said Allison Orris, senior fellow and director of Medicaid policy at the Center on Budget and Policy Priorities.
Overall, 16 million individuals may be uninsured by 2034, according to CBO, based on the combined effects of the House Republicans' budget bill on Medicaid and Affordable Care Act coverage; the expiration of enhanced premium tax credits tied to the ACA; and a separate rule proposed by the Trump administration targeting ACA marketplaces.
The House and Senate versions of the "big beautiful" bill would impose federal work requirements on Medicaid for the first time.
Per the House and Senate proposals, individuals ages 19 to 64 who apply for Medicaid or who are enrolled through Affordable Care Act expansion group would need to would need to work or participate in qualifying activities for 80 hours per month.
Adults may be exempt if they have dependent children or have qualifying circumstances such as medical conditions; however, "exemptions don't always work, and people could lose coverage anyway," Orris said.
Medicaid work requirements proposed in the House bill would cut federal spending by $344 billion over 10 years, representing the legislation's largest source of Medicaid savings, according to KFF, a nonprofit provider of health policy research.
Current law prohibits basing Medicaid eligibility on work requirements or work reporting requirements, according to KFF.
"Many people on Medicaid, if they're able to, are already working," said Robin Rudowitz, director of the program on Medicaid and the uninsured at KFF.
However, some states may implement work requirements if they receive approval through waivers. Georgia is currently the only state with a Medicaid work requirement. "Several" other states have recently submitted waiver requests to put such requirements in place, according to KFF.
Arkansas previously implemented Medicaid work requirements. However, estimates have shown while more people became uninsured because of that policy, there were not meaningful increases in employment, according to Rudowitz.
The Senate version of the bill introduced a "harsher" take on the work requirements that would apply to some parents, Orris said. The Senate calls for limiting parental exemptions to those with children ages 14 and under, rather than all parents of dependent children as the House proposed.
Individuals who apply for Medicaid coverage would need to meet work and other requirements for one or more consecutive months before they apply. Eligibility redeterminations would be conducted at least twice per year to ensure enrollees still meet those requirements.
The Senate version proposes capping the look-back period for showing compliance with work requirements to three months, which on net may be helpful to people, Orris said.
If an individual is denied coverage or disenrolled because they do not meet the Medicaid work requirements, they would be ineligible for subsidized marketplace coverage.
The Senate bill also allows for a longer timeline for states to comply with the Medicaid work requirements. The chamber's bill would give states the ability to ask for a good faith waiver that would give them an additional two years to come into compliance with the provision, or until the end of 2028, rather than the end of 2026 in the House version.

WASHINGTON (AP) — The budget bill championed by President Donald Trump could complicate next year's tax filing season after the IRS lost one-quarter of its employees through staffing cuts, an independent watchdog reported Wednesday. The IRS workforce has fallen from 102,113 workers to 75,702 over the past year, according to the latest National Taxpayer Advocate report to Congress. The report Wednesday offered the first official numbers on the IRS job losses associated with Elon Musk's Department of Government Efficiency. Some of the findings from the report: Taxpayers will likely see effects of staffing reductions The Trump administration's efforts to shrink the size of the federal bureaucracy to a mass exodus of probationary employees who had not yet gained civil service protections and were offered buyouts through a 'deferred resignation program.' More than 17,500 IRS workers took that route. The biggest cuts were in taxpayer services, the small business/self-employed office and information technology. The report noted that the Republican administration's proposed budget includes a 20% reduction in IRS funding next year. That's a 37% reduction when taking into account the supplemental funding in the Biden-era Inflation Reduction Act that Republicans previously stripped away. 'A reduction of that magnitude is likely to impact taxpayers and potentially the revenue collected," wrote Erin M. Collins, who leads the organization assigned to protect taxpayers' rights. The 2026 tax season could be precarious Collins said the 2025 filing season was 'one of the most successful filing seasons in recent memory,' though she warned that the 2026 season could be rocky. 'With the IRS workforce reduced by 26% and significant tax law changes on the horizon, there are risks to next year's filing season,' Collins wrote. "It is critical that the IRS begin to take steps now to prepare.' She said that, halfway through the year, there were concerns that the IRS had not yet undertaken key preparation steps, including hiring and training seasonal and permanent employees. Trump's package could add new layer of problems The report warned about the possibility of understaffing to manage new provisions from Trump's legislative package if it's enacted. 'Several provisions will retroactively affect the 2025 tax year, thus impacting millions of taxpayers and requiring the IRS to quickly update tax year 2025 tax forms and programming for the 2026 filing season," the report said. Specifically, the House bill retroactively prohibits the IRS from allowing or making payment of Employee Retention Credit claims filed after Jan. 31, 2024. The report also said the IRS historically receives more calls in years following significant changes in tax law, so it may need additional employees and improved digital tools to maintain its level of service. Identity theft cases are still piling up The IRS is dealing with delays in resolving self-reported identity theft victim assistance cases — taking up to 20 months to resolve, the report said. As of the end of the 2025 filing season, the IRS was handling about 387,000 of these cases. That is a slight improvement from the more than 22 months it took to resolve identity theft cases, as noted in last year's report, which outlined roughly 500,000 unresolved cases in its inventory. 'The cycle time remains unacceptably long," Collins said. 'I continue to urge the agency to focus on dramatically shortening the time it takes' to resolved identity theft cases, "so it does not force victims, particularly those dependent on their tax refunds, to wait nearly two years to receive their money.'

WASHINGTON (AP) — President Donald Trump insisted Wednesday that U.S. strikes delivered a crushing blow to Iran's nuclear program despite a preliminary American intelligence assessment suggesting that the assault inflicted only a marginal setback. 'This was a devastating attack, and it knocked them for a loop,' Trump said as his administration deployed a phalanx of top officials to defend his claims that Iran's nuclear program was 'completely and fully obliterated.' Defense Secretary Pete Hegseth said the leaked intelligence assessment, which said Iran suffered a delay of only a few months, was 'preliminary' and 'low confidence.' Secretary of State Marco Rubio said the officials who disclosed the findings are 'professional stabbers.' The White House pointed to a statement from the Israel Atomic Energy Commission that said Iran faced a setback of 'many years.' Drawing reliable conclusions about the impact of the U.S. strikes is difficult, making the issue a breeding ground for competing claims that could determine how American voters view Trump's risky decision to join Israel's attacks on Iran. Also at stake are Trump's next steps in the Middle East, where diplomatic efforts could be required to prevent Iran from rebuilding its nuclear program. Iran maintains that its atomic ambitions are for peaceful purposes, while U.S. and Israeli leaders have described the country's nuclear program as the precursor to obtaining a nuclear weapon. One of the targets of the U.S. attack was Fordo, where nuclear infrastructure is buried deep underground. Wisconsin Supreme Court sides with Republican Legislature in fight with governor

MADISON, Wis. (AP) — The Wisconsin Supreme Court handed a victory to the Republican-controlled Legislature on Wednesday in a power struggle with Democratic Gov. Tony Evers. The court, in a unanimous ruling where the four liberal justices joined with three conservatives, struck down Evers' partial veto of a Republican bill in a case that tested both the limits of his broad veto powers and the Legislature's ability to exert influence by controlling funding. The court also ruled that the Legislature can put money for certain state programs into an emergency fund under the control of its budget committee. Evers had argued such a move was unconstitutional. The ruling against Evers comes after the court earlier this year upheld Evers' partial veto that locked in a school funding increase for 400 years. The court last year issued a ruling that reined in some powers of the Legislature's budget committee, while this ruling went the other way. Evers clashes with Legislature Evers, in his seventh year as governor, has frequently clashed with the Legislature and often used his broad veto powers to kill their proposals. Republican lawmakers have tried to take control away from the governor's office by placing money to fund certain programs and state agencies in an emergency fund controlled by the Legislature's budget committee. That gives the Legislature significant influence over that funding and the implementation of certain programs within the executive branch. Evers argued that the Legislature is trying to limit his partial veto power and illegally control how the executive branch spends money. It ruled that Evers improperly used his partial veto on a bill that detailed the plan for spending on new literacy programs designed to improve K-12 students' reading performance. The court also sided with the Legislature and said the budget committee can legally put money into an emergency fund to be distributed later. That is what it has done with the $50 million for the literacy program. Evers and Republican lawmakers did not immediately return messages seeking comment. Fight over literacy funding In 2023, Evers signed into law a bill that created an early literacy coaching program within the state Department of Public Instruction. The bill also created grants for schools that adopt approved reading curricula to pay for changing their programs and to train teachers on the new practices. However, Republicans put the $50 million to pay for the new initiative in a separate emergency fund controlled by the Legislature's budget committee. That money remains in limbo amid disagreements about how the money would be used and who would decide how to spend it. Evers argued that the Legislature didn't have the power to withhold the money and the court should order it to be released to the education department. The Legislature has been increasing the amount of money it puts in the emergency fund that it can release at its discretion, but it remains a small percentage of the total state budget. In the last budget, about $230 million was in the fund, or about half of a percentage point of the entire budget. Republicans sue to stop veto Evers used his partial veto power on another bill that created the mechanism for spending the $50 million for the new program. He argued that his changes would simplify the process and give DPI more flexibility. Evers also eliminated grants for private voucher and charter schools. State law allows only for a partial veto of bills that spend money. For all other bills, the governor must either sign or veto them in their entirety. Because the bill Evers partially vetoed was a framework for spending, but didn't actually allocate any money, his partial vetoes were unconstitutional, lawmakers argued. Evers argued for a liberal interpretation of his veto powers. He said that by challenging it, the Legislature was trying to weaken his powers. A Dane County judge sided with Evers, determining that the bill in question qualified as an appropriations bill subject to partial vetoes. But in a win for the Legislature, he did not find fault with the Legislature's budget committee putting funding for the program under its control.
