De Bruyne, who recently joined as a free agent from Manchester City, was handed his first start by manager Antonio Conte.
Minnow side Arezzo struck twice, once late in each half, to secure a remarkable pre-season win over the champions of Italy.
The opener came through Emiliano Pattarello's covered penalty just before halftime, catching Napoli off guard after an otherwise comfortable 45 minutes.
Then, deep into stoppage time, Muhamed Varela doubled Arezzo's lead to seal a famous victory at Napoli's own training camp venue.

But despite the result, classy De Bruyne took time to stand for photos after being asked by a string of Arezzo players on Tuesday evening.
The Belgian great, 34, was spotted standing in the middle of the pitch, with his boots off and looking red faced while smiling into the camera.
De Bruyne was subbed off at half-time for defensive midfielder Stanislav Lobotka, while former Prem star Romelu Lukaku got a full second half.
De Bruyne will wear the No.11 shirt in the coming season but, as a welcome gesture from the club, he has been training wearing Diego Maradona's retired No10 on his shorts.

The pair have reportedly been managing minor strains caused by the intense physical demands of Conte's pre-season sessions.
Arezzo's goalkeeper, Luca Trombini, was the standout performer, denying Napoli repeatedly with a string of big saves.
Napoli's pre-season continues with another friendly scheduled against Serie B outfit Catanzaro on July 26.
Their title defence begins with a trip to Sassuolo on August 23.

Video of Morgan Gibbs-White transfer U-turn gets same response from ALL football fans

Tottenham were seemingly closing in on the signing of Morgan Gibbs-White earlier this month but the Nottingham Forest star has now signed a record contract to stay put Morgan Gibbs-White has officially committed his future to Nottingham Forest - just weeks after the attacking midfielder looked all but certain to join Tottenham. ‌ News broke earlier this month that Spurs had triggered a £60million release clause in the 25-year-old's contract. However, Forest were left angered by what they considered to be an illegal approach from their Premier League rivals for one of their most influential stars. ‌ Gibbs-White has since played for Forest in pre-season as all talk of finalising said move to Spurs went quiet. ‌ And the club have now confirmed that Gibbs-White has signed a club-record three-year deal, putting an end to speculation over the midfielder's future. A video showing Gibbs-White and Evangelos Marinakis, the club's owner, was posted online to announce the news. But some mischievous fans have quipped that the Forest star looked like a 'hostage' in said video. One social media user joked: "Nothing like a hostage video with your captor standing beside you so you don't cry for help! #FreeGibbsWhite". Another poster quipped: "Gibbs-White fake smiling with his owner after being held hostage at Forest just like Olise at Palace." A third football fan said cheekily: "A lot of interesting moments throughout this transfer window. Obviously United's chase of players, Liverpool splashing huge cash, Arsenal always in the news, the Isak situation & a lot more. But I didn't expect a Morgan Gibbs-White hostage video. Never a dull moment". Despite the jokes from fans, Gibbs-White said that he was 'excited' for what's next at Forest after the club qualified for Europe last season. ‌ Speaking to the club's official website, he said: "I've felt at home at Forest from the moment I arrived. The support from the fans, my teammates, and everyone around the club has been unbelievable. "I believe in what we're building here – and with the backing of Mr Marinakis and the enormous ambition he has, I want to be part of making something special. I'm excited for what's to come." Forest owner Marinakis, who posed with Gibbs-White in photos celebrating the new contract, said: "Morgan is a special player – not just in terms of talent, but character and mentality. He represents everything we want this football club to be: he is a winner, talented, ambitious, fearless, and proud. "There was significant interest from various clubs, but we were determined to build our future with Morgan at the heart of it. I promised our fans we would not only compete, but grow stronger and stronger every season. Today is another big step in that journey." Luis Diaz set for 75m euro move to Bayern Munich as Liverpool accept latest bid

Liverpool have agreed a 75 million euro deal with Bayern Munich for Reds forward Luis Diaz, the PA news agency understands. The agreement comes after the Bundesliga champions were understood to have reopened dialogue regarding a move after having an initial offer of 67.5 million euros (£59m) dismissed last week. Liverpool are currently on their pre-season tour in Asia and Diaz was still with the team when they arrived in Tokyo on Sunday, but now has permission to leave. The 28-year-old has two years left on his original contract and it is understood the Reds accepted the bid for what they consider to be his market value. Diaz joined Liverpool in January 2022 and made 50 appearances in all competitions last season – scoring 17 goals – as he helped the team clinch the Premier League title. His time on Merseyside also saw him lift the FA Cup and two Carabao Cups. Diaz was left out of Liverpool's 4-2 friendly defeat by AC Milan in Hong Kong on Saturday as speculation surrounding his future continued to mount. Speaking post-match, head coach Arne Slot said: 'In Lucho's (Diaz's) situation, there's a lot of rumours around him lately and I cannot go into that. 'He's training really well with us but we have decided for now not to play him yet. 'I'm used to talking to most of my players on a regular basis. I cannot comment much more about that.' Liverpool have been busy in the transfer window so far and have bolstered their attacking options with Florian Wirtz – who made his first start as a false nine on Saturday – and Hugo Ekitike joining the club. There have also been further arrivals on Merseyside, with Jeremie Frimpong, left-back Milos Kerkez and goalkeeper Giorgi Mamardashvili all signing and the trio played in the second half of Saturday's friendly.
