A chilling risk of recreational nitrous oxide use: Frostbite

Recreational use of nitrous oxide — often referred to as whippets or laughing gas — left a 23-year-old with frostbite in his mouth and throat, according to a report of the man's case published Wednesday in the New England Journal of Medicine.
Recreational nitrous oxide use has been on the rise in recent years: Data from America's Poison Centers shows a nearly 60% increase in reports of intentional exposure to the drug from 2023 to 2024. The Food and Drug Administration has issued several warnings advising people against inhaling nitrous oxide products, which are often marketed to be used to make whipped cream.
Frostbite occurs when ice crystals form in the skin and deeper tissues, damaging cells and disrupting blood flow. Dr. Michael Patrizio, associate director of acute care at the University of Virginia, said getting frostbite in the mouth and throat from inhaling nitrous oxide is rare, but not unheard of.
The patient told Patrizio that his painful swallowing and hoarseness started immediately after he inhaled nitrous oxide directly from a canister two days prior. White blotches — the frostbitten tissue — swelled on the roof of his mouth, uvula and throat.
'Had he not been forthcoming with that information, that probably wouldn't have been something that crossed my mind,' said Patrizio, who saw the man at the campus's outpatient clinic and was a co-author of the case report. 'I would have thought a viral infection — mono, strep throat — or an STI in the throat.'
When a compressed gas expands, it can cool to as low as minus 40 degrees Fahrenheit — this is why compressed gas is used to cool refrigerators. Canisters of compressed nitrous oxide work in the same way. When the gas is released, it cools, sometimes drastically.
'This has the potential to cause frostbite,' Dr. Cara Borelli, an addiction medicine physician at Yale University, said in an email. Borelli wasn't involved with the patient's case.
Surgeons use compressed nitrous oxide to perform ablations, a procedure that uses extreme cold or heat to remove tissue.
'If you have someone using drugs in an uncontrolled setting, you may certainly be doing damage to tissue that you don't want to be doing damage to,' said Dr. Jeffrey Ruwe, an emergency medicine physician at Cleveland Clinic who wasn't involved with the man's case.
Overall, cases of frostbite from nitrous oxide canisters are rare, though they are likely underreported, Patrizio said. More commonly than in the mouth and throat, illicit nitrous oxide use leads to frostbite burns on the thighs and hands, since people hold chilled canisters and fill balloons with the gas, which allows the gas to warm before people inhale it.
Frostbite can be serious — at least several people have required skin grafts and surgery due to nitrous oxide burns on their legs and hands — however 'that's not really the risk people think of,' Patrizio said.
The more common risks of recreational nitrous oxide use include a ruptured lung, from inhaling the gas too deeply, and a lack of oxygen in the brain, he said. It can also cause vitamin deficiencies that can lead to potentially fatal neurological problems.
'Nitrous oxide inactivates vitamin B12, meaning that the body's B12 does not function as it is supposed to,' Borelli said.
The body uses vitamin B12 to synthesize myelin, a substance made from fats and proteins that surrounds and insulates nerves. If this sheath breaks down, it can cause serious neurological issues. After chronic use, nitrous oxide can cause nervous system and brain changes that can progress from weakness and unsteadiness to paralysis.
'That's very real,' Ruwe said.
Patrizio told his frostbitten patient to use over-the-counter pain medication, lidocaine and a steroid paste to reduce inflammation and swelling while his frostbite healed.
Ruwe said this may not be the case for everyone.
'Frostbite is very, very rare, but if inhaled directly from a higher-pressure tank, or if it causes airway swelling, it could be time-sensitive and potentially deadly,' he said.

Here's what you'll learn when you read this story: The first babies with three biological parents were born out of a new technique to prevent mitochondrial disease. The nucleus of an egg fertilized in vitro was transferred into a donor egg without a nucleus, but with viable mitochondria. Eight healthy babies, including a set of twins, were born with low to undetectable levels of mitochondrial mutations. The only creatures known to conceive offspring from more than two parents are salamanders. Females from the genus Ambystoma (which are notoriously promiscuous) mate with up to three different males, and that DNA is then incorporated into what is known as a triploid genome in their offspring. Now a version of this has become possible in humans. It seems limb regeneration isn't the only way medical intervention can put humans on salamanders' level. Being born with three genomes is not a phenomenon that occurs naturally in Homo sapiens, but in an attempt to prevent certain genetic conditions caused by mutations in the mitochondria, scientists have found a way. Mitochondrial DNA or mtDNA is exclusively passed down from the maternal side. Dysfunction in the mitochondria can lead to metabolic diseases characterized by symptoms such as seizures, developmental delays, blindness, and loss of muscular function. Some can even be fatal. Mitochondrial diseases occur in about 1 in every 5,000 people. They were previously only preventable by using a donor egg or foregoing the conception of biological children altogether. This is why pediatric neurologist Bobby McFarland, of Newcastle University in the UK, led an experimental study that would reduce and potentially eliminate the risk of mitochondrial disease with a new method of in vitro fertilization. McFarland and his research team wanted see if removing the nucleus of an egg and placing it in a donor egg with viable mitochondria would result in healthy offspring. 'We found that pronuclear transfer, a form of mitochondrial donation, was effective in reducing the level of pathogenic mtDNA variant to substantially below the threshold for clinical disease in the offspring of women with homoplasmic (or high heteroplasmic) levels,' he said in a study recently published in the New England Journal of Medicine. When mitochondria are homoplasmic, all copies produced by cell division have mutations. Mutation levels vary in heteroplasmic mitochondria. Preimplantation genetic testing (PGT) can screen for these abonormalities, and women with homoplasmy or high levels of heteroplasmy can benefit from what is now known as pronuclear transfer. This involves eggs from both the mother and donor being fertilized with the father's sperm in vitro. Nuclei are then removed from both eggs after ten hours. Since the nucleus carries most genetic material and has no connection to mitochondrial disease, the mother's nucleus is implanted into the donor egg to take advantage of its mitochondria. While there is a chance that a few of the mother's mitochondria may end up in the embryo, it is unlikely to cause a debilitating disease. Levels of defective mitochondria in offspring conceived via pronuclear transfer were low enough to escape that fate. Eight pregnancies (including a set of twins) resulted from the experiment, and while there were a few minor health problems in the newborns, these were either treatable or corrected themselves. Not only were levels of heteroplasty low for the babies, but undetectable in five of them. Developmental progress also turned out to be normal. Though one baby had a form of infant epilepsy, and another had heart arrhythmia and hyperlipidemia, or high levels of fats and lipids in the blood, both of these conditions were treated and resolved. Whether the hyperlipidemia was even caused by mtDNA is uncertain, especially because the mother also had severe hyperlipidemia during her pregnancy. Though there was a chance that any of the mothers with pathogenic mtDNA had a higher risk of complications during pregnancy, which could possibly cause their children to have health issues, there is no proof for now. 'We are assessing, over the long term, the health and extent of heteroplasmy (if detectable) of the offspring,' McFarland and his team said. 'Indeed, the role of mitochondrial donation as a choice for women with a heritable pathogenic mtDNA variant will only be established with the availability of additional data.' Nicholas Clark, M.D., an orthopedic spine surgeon for HSS at NCH, says that while many guys prioritize building their chest, arms, or legs, they often ignore the muscles in the neck and upper back—leaving them prone to pain and limited movement. "The neck and trapezius muscles are your body's unsung heroes, stabilizing your head and shoulders during every lift, run or even desk session. Neglecting them can lead to chronic pain, reduced mobility and even spinal issues down the line," Clark tells Men's Fitness. In his work as a spine surgeon, he's treated countless men in their 40s and 50s dealing with chronic neck and trap pain—issues that could often be prevented or improved with the right exercises. Below, Clark shares four underrated exercises—ones you've probably never done—that target the neck and traps to help reduce pain, improve posture, and keep you strong and mobile well into middle and Trapezius Exercises for Pain Relief Directions "Start with two to three sessions per week, and pair them with your existing routine. Over time, you'll notice less stiffness, better posture and a stronger, more balanced upper body," Clark says. Cervical Retraction with Resistance Band Sets: 3 Reps: 12-15 How to do it: Anchor a light resistance band to a sturdy object at chest height. Hold the band with both hands, standing about 2 feet away, and keep your arms relaxed. Gently tuck your chin toward your neck (think of making a double chin) while pulling the band slightly toward your face, keeping your head level. Hold for 3 seconds, then release. Tip: Focus on keeping your shoulders relaxed to avoid tensing the traps. If you feel strain, use a lighter band or no band at all until you master the movement. By 40, many men develop forward flex head posture from the impacts of daily life—like prolonged sitting or smartphone use. In turn, this strains the cervical spine and trapezius. "This exercise strengthens the deep neck flexors and posterior neck muscles, counteracting that slouch while relieving tension in the upper traps," Clark says. Scapular Wall Slides Sets: 2-3 Reps: 10-12 How to do it: Stand with your back against a wall, feet about 6 inches away. Press your lower back, upper back, and head lightly against the wall. Bend your elbows to 90 degrees, with your forearms and hands touching the wall (like a 'W' shape). Slowly slide your arms upward to form a 'Y,' keeping your elbows, forearms, and hands in contact with the wall. Lower back to the starting position. Tip: If your arms don't reach the 'Y' position without losing wall contact, don't force it. Work within your range and progress as mobility improves. "Tight traps often stem from poor scapular mobility, which is common in men who lift weights or sit for long periods. This exercise improves scapular movement and strengthens the lower trapezius, reducing upper trap dominance and neck strain," he says. Prone Trapezius Raise Sets: 3 Reps: 12-15 How to do it: Lie facedown on a bench or the floor with a light dumbbell (2–5 pounds) in each hand. Let your arms hang straight down, palms facing each other. Keeping your arms straight, lift them out to the sides to form a 'T,' squeezing your shoulder blades together. Your thumbs should point upward at the top of the movement. Lower slowly. Tip: If you feel your upper traps taking over, reduce the weight or do the exercise without weights. Focus on initiating the movement from your mid-back. "The middle and lower trapezius muscles are critical for stabilizing the shoulder blades and supporting the neck, but they're often neglected in favor of upper trap-focused exercises like shrugs. This move isolates the mid and lower traps, reducing neck stiffness and improving posture," Clark explains. Isometric Neck Bridge Sets: 2 Reps: 10-12 How to do it: Lie on your back with your knees bent and feet flat on the floor. Place a folded towel (about 2 inches thick) under your head for slight elevation. Gently press the back of your head into the towel, engaging your neck muscles without lifting your head. Hold for 5 to 10 seconds, then relax. Tip: To target the traps more, slightly shrug your shoulders toward your ears during the hold, but keep the movement subtle to avoid strain.
