
Heart disease: I'm a cardiologist. Here are 10 science-based ways to prevent it

This is considered the single most effective medical intervention to protect against atherosclerosis and promote healthy ageing. Physical activity lowers inflammation in the body. Evidence has shown that both aerobic and strength training forms of exercise are important. But only one in four Americans meet the two activity guidelines from the American Heart Association: aerobic exercise of 150 minutes per week of at least moderate physical activity, such as walking, bicycling on level ground, dancing or gardening, and strength training for at least two sessions per week, which typically translates to 60 minutes weekly.
The protective benefit of exercise is seen with even relatively low levels of activity, such as around 2500 steps per day (via sustained physical activity, not starting and stopping), and generally increases proportionately with more activity. It used to be thought that people who exercise only on the weekend – known as 'weekend warriors' – put themselves in danger, but recent data shows the benefits of exercise can be derived from weekend-only workouts, too.
2. Follow an anti-inflammatory diet

A predominantly plant-based diet – high in fibre and rich in vegetables, fruits and whole grains, as seen with the Mediterranean diet – has considerable evidence from large-scale observational and randomised trials for reducing body-wide inflammation and improving cardiovascular outcomes.
Foods rich in omega-3 fatty acids, such as salmon, also form part of a diet that suppresses inflammation. On the other hand, red meat and ultra-processed foods are pro-inflammatory, and you should limit your consumption. High protein intake of more than 1.4g per kg of body weight per day – around 95g for someone who is 68kg – has also been linked to promoting inflammation and to atherosclerosis in experimental models. That is particularly related to animal-based proteins and the role of leucine, an essential amino acid that is obtained only by diet.
3. Maintain a healthy weight
A lean body weight helps reduce heart disease risk.
Being overweight or obese indicates an excess of white adipose tissue. This kind of tissue can increase the risk of heart disease because it stores fat cells, known as adipocytes, which release substances that contribute to inflammation.
In studies, we've seen that glucagon-like peptide (GLP-1) drugs can reduce inflammation with weight loss, and a significant reduction of heart attacks and strokes among high-risk patients treated for obesity. Lean body weight also helps protect against atrial fibrillation, the most common heart rhythm abnormality.
4. Know and avoid metabolic syndrome and prediabetes

Tied into obesity, in part, is the problem of insulin resistance and metabolic syndrome. Two out of three people with obesity have this syndrome, which is defined as having three out of five features: high fasting blood glucose, high fasting triglycerides, high blood pressure, low high-density lipoprotein (HDL) and central adiposity (waist circumference of more than 40 inches in men, 35 inches in women).
Metabolic syndrome is also present in a high proportion of people without obesity, about 50 million Americans. Prediabetes often overlaps with it. Prediabetes is defined as a haemoglobin A1c (a measure of how much glucose is stuck to your red blood cells) between 5.7 and 6.4%, or a fasting glucose between 100 and 125 milligrams per decilitre.
Both metabolic syndrome and prediabetes carry an increased risk of heart disease and can be prevented – and countered – by weight loss, exercise and an optimal diet.
As the glucagon-like peptide drug family moves to pills and less expense in the future, these medications may prove helpful for reducing risk in people with metabolic syndrome and prediabetes. For those with Type 2 diabetes, the goal is optimising glucose management and maximal attention to lifestyle factors.
5. Keep your blood pressure in a healthy range

Hypertension is an important risk factor for heart disease and is exceptionally common as we age. The optimal blood pressure is 120/80mm Hg or lower. But with ageing, there is often an elevation of systolic blood pressure to about 130mm Hg, related to stiffening of arteries. While common, it is still considered elevated.
Ideally, everyone should monitor their blood pressure with a home device to make sure they haven't developed hypertension. A mild abnormality of blood pressure will typically improve with lifestyle changes, but more substantial elevations will probably require medications.
6. Find out your genetic risk

We now have the means of determining your genetic risk of coronary artery disease with what is known as a polygenic risk score, derived from a gene chip. The term polygenic refers to hundreds of DNA variants in the genome that are linked to risk of heart disease. This is very different from a family history, because we're a product of both our mother's and father's genomes, and the way the DNA variants come together in each of us can vary considerably for combinations of variants.
That means you could have high or low risk for heart disease that is different from your familial pattern. People with a high polygenic risk score benefit the most from medications to lower cholesterol, such as statins. A polygenic risk score can be obtained from a number of commercial companies, though it isn't typically covered by insurance.
I don't recommend getting a calcium score of your coronary arteries via a computed tomography (CT) scan. This test is overused and often induces overwhelming anxiety in patients with a high calcium score but without symptoms or bona fide risk. If you have symptoms suggestive of coronary artery disease, such as chest discomfort with exercise, then a CT angiogram may be helpful to map the coronary arteries. It is much more informative than a calcium score.
7. Check your blood lipids

The main lipid abnormality that requires attention is low-density cholesterol (LDL), which is often high and for people with increased risk of heart disease should certainly be addressed. While lifestyle improvements can help, significant elevation typically requires medications such as a statin; ezetimibe; bempedoic acid; or injectables such as evolocumab (Repatha), alirocumab (Praluent) or inclisiran (Leqvio). The higher the risk, the more aggressive LDL lowering may be considered.
It should be noted that the use of potent statins, such as rosuvastatin or atorvastatin, especially at high doses, is linked to inducing glucose intolerance and risk of Type 2 diabetes. While this is not a common side effect, it requires attention since it is often missed from lack of awareness.
A low high-density lipoprotein (HDL) cholesterol often responds to weight loss and exercise. We used to think that high HDL was indicative of 'good cholesterol', but more recent evidence suggests that is not the case and it may reflect increased risk when very high.
To get a comprehensive assessment of risk via your blood lipids, it's important to get the apolipoprotein B (apoB) test at least once because about 20% of people have abnormal LDL and a high apoB.
Like low HDL, high fasting triglycerides may indicate insulin resistance as part of the metabolic syndrome and will often respond to lifestyle factors.
The lipoprotein known as Lp(a) should also be assessed at least once because it indicates risk when elevated. The good news is scientists are on the cusp of finally having medications to lower it, with five different drugs in late-stage clinical trials.
8. Reduce exposure to environmental pollutants

In recent years, we've learned a lot about the substantial pro-inflammatory effects of air pollution, microplastics and forever chemicals, all of which have been linked to a higher risk of heart disease. In one study, microplastics or nanoplastics in the artery wall were found in about 60% of more than 300 people. Researchers found a vicious inflammatory response around the plastics, and a four- to fivefold risk of heart attacks or strokes during three years of follow-up.
While we need policy changes to address these toxic substances in the environment, risk can be reduced by paying attention to air and water quality using filtration or purification devices, less use of plastic water bottles and plastic storage, and, in general, being much more aware and wary of our pervasive use of plastics.
9. Don't smoke

By this point, it should be well known that cigarette smoking is a potent risk factor for coronary artery disease and should be completely avoided.
10. Get good sleep

Although we tend to connect sleep health with brain and cognitive function, there's evidence that sleep regularity and quality are associated with less risk of heart disease. Regularity means adhering to a routine schedule as much as possible, and its benefit may be because of our body's preference for maintaining its circadian rhythm. Sleep quality – meaning with fewer interruptions – and maximal deep sleep can be tracked with smartwatches, fitness bands, rings or mattress sensors.
Sleep apnea, when breathing stops and starts during sleep, is fairly common and often unsuspected. So if you're having trouble sleeping or you snore loudly, talk to your doctor about ruling out the condition. Testing for sleep apnea can involve checking for good oxygen saturation throughout one's sleep. That can be done through a sleep study or at home using rings or smartwatches that include oxygen saturation in their sensors and body movement algorithms that pick up disturbed breathing.

What's it like living with sarcoidosis

Here, Donna Thomas, 58, tells Paulette Crowley about living with sarcoidosis, a rare inflammatory and autoimmune condition. I was planting some trees in my garden one day and put some compost down. I didn't put a mask on while I was doing this, which was really silly. I remember leaning over the compost bag and noticing the tiniest puff of dust going up my nostril. That was in December 2021. After that, I had some trouble breathing – shortness of breath – and didn't feel well. I went to the doctor's a few times, maybe. Then in April, I had some chest pain. My daughter drove me down to the hospital, where I ended up having a CT scan. That's when they saw a granuloma – kind of a lump of infection – on my lung. They had to remove the upper right lobe of my lung to biopsy it. That procedure didn't go smoothly, as my ribs dislocated during the surgery. It's called a subluxation and the surgeon was kind of blown away because it's unusual. But when I look back from where I've landed now, I must have been in a highly inflamed state. I was finally diagnosed with sarcoidosis, an autoinflammatory disease, in March 2022 by a respiratory physician. I learnt that well over 90% of sarcoidosis cases start in the lungs, but can go on to affect different parts of the body. About 5% of people will end up with it affecting their neurology – it's called neurosarcoidosis. That's what I have ended up with. Donna Thomas (left) before she was diagnosed with sarcoidosis. The specialist who diagnosed me said most GPs would never see a case in their medical lifetimes. They call it a snowflake disease because everyone who has sarcoidosis presents so differently, meaning that much like snowflakes, no two cases are alike. This is a really big challenge for medical people, and for me as well. I had a consult with a new neurologist recently. When I asked him if he knew about sarcoidosis, he just didn't say anything. We look to medical professionals to treat us and if they don't know what's going on, it's really scary. Some of the ways that sarcoidosis affects me is that my legs, hands and feet ache. This can wake me at night. I also get cramping and burning in my muscles and I've got numbness and tingling in my fingers and toes. My cognition is also affected – I find it hard to remember things – and I get very fatigued. Sarcoidosis accelerates many conditions. I've had significant surgery around my mouth to get basal cells removed and I have cataracts that are coming on hard and fast. I've also just found out I need a hip replacement. I now live a different life to what I used to. I was a trauma specialist and managed a team of 70 people before I was medically retired. I was working 60 hours a week, like lots of people do. Back then, I could spin a lot of plates, you know, just throw anything at me and I'd just shuffle it around. And it always makes me want to cry because I miss my career, but I can't do that anymore. Now, it's not about trying to be who I was but trying to function in a way that I'm not lying in bed all day. Obviously, I can't work, but I might get in two productive hours a day, doing housework and tidying up. I have to take things slowly. If I've got medical appointments, the next day has to be a rest day because my brain and body are just tired. There's a massive spectrum with sarcoidosis – depending on the type of disease, some people can be fine, and lots will go into remission. I don't know if I'd say my disease is at the top of the chain, because I have refractory sarcoidosis, which is chronic and progressive. Frontline treatments haven't worked to put me into remission. Now, we're at the next level of treatment and the last thing we've got to try, at the moment, is a medication called infliximab. The hope is that it will bottom out my immunity and my inflammation levels will drop. I am so, so far away from the person I used to be and have a lot of grief still around that. But you know, you've just got to kind of get on with things. I can either lay down and die or stand up and live the best life I can with this. Grief just takes away time from family and it's made me realise that work's not the most important thing in the world. I've got five kids and my ninth grandchild on the way, so now, it's more family time for me. And more and more I just want to do the things I enjoy, not the things I have to do. Sarcoidosis usually affects people's lungs, although it can affect every system in the body. It mainly occurs in people, especially women, between 25 and 45 years of age. Some studies have suggested there might be a link between sarcoidosis and environmental factors, including potting mix. Some people will have no symptoms from sarcoidosis or have mild, treatable symptoms that clear within a few years. For others, symptoms can become widespread, chronic and hard to treat. Ozempic and other weight-loss drugs linked to rare but serious eye conditions
Drugs such as Ozempic, Wegovy and Mounjaro (known as semaglutide and tirzepatide) have changed the way clinicians manage diabetes and obesity around the world, say researchers Flora Hui and Pete A Williams. Collectively known as GLP-1 agonists, these drugs mimic the hormone GLP-1. This limits both hunger and interest in food, helping users lose weight, and helps control blood sugar levels. But two new studies published today show that people taking these drugs may have a small increased risk of serious eye conditions and vision loss. Here's what you need to know if you're taking or considering these medications. What damage can occur? ADVERTISEMENT Non-arteritic anterior ischaemic optic neuropathy, or NAION, is a rare but devastating eye condition that occurs when blood flow to the optic nerve is suddenly reduced or blocked. It's also called an 'eye stroke'. The exact cause of NAION remains unclear and there are no current treatments available. People with diabetes are at increased risk of developing NAION. Unlike other eye conditions that develop gradually, NAION causes a sudden, painless loss of vision. Patients typically notice the condition when they wake up and discover they've lost vision in one eye. Vision tends to worsen over a couple of weeks and slowly stabilises. Recovery of vision is variable, but around 70% of people do not experience improvement in their vision. What has previous research shown? A previous study from 2024 found participants prescribed semaglutide for diabetes were four times more likely to develop NAION. For those taking it for weight loss, the risk was almost eight times higher. In June, the European Medicines Agency concluded NAION represented a 'very rare' side effect of semaglutide medications: a one in 10,000 chance. In a first for medicines regulators, the agency now requires product labels to include NAION as a documented risk. ADVERTISEMENT However the recent studies suggest the risks may be lower than we first thought. In addition to NAION, there is also evidence to suggest GLP-1 drugs can worsen diabetic eye disease, also known as diabetic retinopathy. This occurs when high blood sugar levels damage the small blood vessels in the retina, which can lead to vision loss. It may sound counter-intuitive, but rapid blood sugar reductions can also destabilise the fragile blood vessels in the retina and lead to bleeding. What do the new studies say? Two newly published studies investigated people with type 2 diabetes living in the United States over two years. The studies looked at the medical records of 159,000 to 185,000 people. One study found semaglutide or tirzepatide was associated with a more modest risk of developing NAION than previously thought. Of 159,000 people with type 2 diabetes who were taking these drugs, 35 people (0.04%) developed NAION, compared with 19 patients (0.02%) in the comparison group. The researchers also found an increased risk of developing 'other optic nerve disorders'. However, it's unclear what kind of optic nerve disorders this includes, as the medical record codes used didn't specify. ADVERTISEMENT Counter to this, the second study did not find an increased risk of NAION among those taking GLP-1 drugs. However, the researchers found a small increase in the number of people developing diabetic retinopathy in those prescribed GLP-1 drugs. But overall, participants on GLP-1 drugs experienced fewer sight-threatening complications related to their diabetic retinopathy and required less invasive eye treatments compared to the group taking other diabetes medications. Further studies are still needed to understand how GLP-1 drugs can lead to eye complications. A current, five-year clinical trial is studying the long-term effects of semaglutides and diabetic eye disease in 1,500 people, which should tell us more about the ocular risks in the future. What does this mean for people taking GLP-1 drugs? NAION is a serious condition. But we need to strike a balance between these (and other) risks and the benefits of GLP-1 medications in diabetes care, obesity treatment, reducing heart attack risks and extending lives. The key lies in informed decision-making and identifying different levels of risk. ADVERTISEMENT People with multiple NAION risk factors – such as sleep apnoea, high blood pressure and diabetes – should undergo careful consideration with their treating doctor before starting these medications. 'Crowded' optic nerve heads are also a risk factor for NAION. This is an anatomical feature where blood vessels at the optic nerve head are tightly packed together. People with crowded optic nerve heads should also undergo careful consideration before starting GLP-1 medications. Although NAION can strike without warning, regular comprehensive eye examinations with your optometrist or ophthalmologist still serve important purposes. They can detect other drug-related eye problems, including worsening diabetic retinopathy, and can identify patients with crowded optic nerve heads. It's also important to tell them if you are taking GLP-1 medications so they can keep a close watch on your eye health. Emerging research also suggests that improving your heart health might help reduce risks of developing NAION. This includes proper management of high blood pressure, diabetes and cholesterol – all conditions that compromise the small blood vessels feeding the optic nerve. Studies also show patients with heart conditions who better adhere to their medication prescriptions have lower risks of NAION than those who don't. Doctors should discuss NAION risks during prescribing decisions and work with eye care providers to monitor regularly for diabetic eye disease. Patients need clear instructions to seek immediate medical attention for sudden vision loss and the need for regular eye examinations. Aggressive treatment of sleep apnoea and other heart conditions may also help reduce NAION risks. But for now, there remains an ongoing need for more research to understand how GLP-1 medications can affect the eye.
