Major study on autism uncovers four distinct subtypes, 'transforming' diagnosis and treatment

Researchers have uncovered four distinct types of autism, each with its own genetic fingerprint, which could unlock the mystery of what causes the condition and how it can be treated.
A team at Princeton University and the Simons Foundation analyzed 5,000 children in SPARK, the most extensive autism research study in the country. They recorded their traits and separated the children into groups based on their behaviors.
They examined more than 230 behavioral and developmental traits—from social interactions to repetitive behaviors—and created four subtypes of autism: Social and Behavioral Challenges; Mixed Autism Spectrum Disorder (ASD) with Developmental Delay; Moderate Challenges; and Broadly Affected.
Then, they compared the subtypes with each child's genetic profile, comparing patterns between the categories. Researchers looked for mutations that were inherited from parents, damaged mutations that were not inherited, and gene activity patterns.
Scientists once believed autism's genetic roots were prenatal, but the study found DNA glitches can determine subtypes before or after a child is born, and autism subtypes differ in the timing of genetic switches, which guide distinct stages of development.
Aviya Litman, a PhD student at Princeton University and co-lead author, said: 'These findings are powerful because the classes represent different clinical presentations and outcomes, and critically, we were able to connect them to distinct underlying biology.'
Researchers have determined that common gene variants account for only around 20 percent of ASD diagnoses, leaving the remaining 80 percent to a complex interplay of environmental factors, modifications to DNA that regulate gene expression, and gene-environment interactions that scientists still don't fully understand.
Their findings unlock opportunities for parents to better understand their children and guide them through milestones with the proper treatment, such as occupational therapy. They could also usher in new targeted gene therapies for people severely disabled by autism.
To analyze data from the 5,000 children, researchers used a computational model to group children together based on a combination of over 230 different traits an individual could have, such as repetitive behaviors, sound sensitivity, gross motor delays, and aversion to certain textures.
This resulted in the creation of four subtypes, which the researchers linked to unique genetic profiles and developmental trajectories.
Natalie Sauerwald, associate research scientist at the Flatiron Institute and co-lead author, said: 'What we're seeing is not just one biological story of autism, but multiple distinct narratives.
'This helps explain why past genetic studies often fell short — it was like trying to solve a jigsaw puzzle without realizing we were actually looking at multiple different puzzles mixed together.'
Children in the Social and Behavioral Challenges group (37 percent of the children in the study) display classic autism traits, including social difficulties and repetitive behaviors, yet still meet developmental milestones on time.
Their high rate of co-occurring mental health conditions sets this subtype apart. ADHD, anxiety, depression, or OCD frequently appear alongside their ASD diagnosis.
These unseen challenges often dominate children's needs. The fact that they're still able to hit developmental milestones on schedule may explain why so many children with this subtype are not diagnosed until they begin to have more social demands in school age.
Those in the Mixed ASD with Developmental Delay group (19 percent) tend to lag behind their neurotypical peers in hitting key milestones, including first words and walking. Still, they do not show signs of anxiety, depression, or disruptive behavior that are more common in other subtypes.
The 'mixed' label reflects their variability in core autism traits—some have severe social challenges but mild repetitive behaviors, while others show the opposite pattern.
Only the Mixed ASD with Developmental Delay group was more likely to carry rare inherited genetic variants.
Children with the Moderate Challenges subtype (34 percent) represent those with the mildest form of the disorder and exhibit more subdued core traits of autism while hitting milestones generally on time.
Importantly, these children lack the psychological difficulties that children in the first subtype exhibit, which suggests that they may need less long-term care, such as medication and therapy.
The Broadly Affected group is the most severely impacted by the disorder and represents 10 percent of the children in the study.
They face a myriad of challenges, including developmental delays, social and communication difficulties, repetitive behaviors, and co-occurring psychiatric conditions like anxiety, depression, and mood dysregulation.
They're also more likely to carry damaging gene mutations that they did not inherit from their parents. Other types had different genetic underpinnings.
'These findings point to specific hypotheses linking various pathways to different presentations of autism,' Dr Litman said, referring to differences in biology between children with different autism subtypes.
An estimated 2.3 million children and 7million adults in the US have ASD.
Diagnoses are becoming more common by the year. According to the CDC, around one in 150 children were diagnosed with ASD in 2000. By 2020, that figure ballooned to one in 36.
A 2024 analysis of electronic health records for more than 12.2 million Americans found that diagnoses increased by approximately 175 percent over 11 years.
Public health experts, pediatricians, and child psychologists posit that the spike in diagnoses is due to broader acceptance of autistic and neurodivergent people and better screening in children.
Health and Human Services Secretary Robert F Kennedy Jr has made identifying the cause of autism and reasons for its increased prevalence a hallmark initiative.
He cites chemicals, diet and environmental toxins as possible causes of the increase.
Senior study author Olga Troyanskaya, director of the Princeton Precision Health and Lewis-Sigler Institute for Integrative Genomics, said: 'Understanding the genetics of autism is essential for revealing the biological mechanisms that contribute to the condition, enabling earlier and more accurate diagnosis, and guiding personalized care.'

Women's health topics are more than three times more likely to be blocked on social media than men's, according to a study. Posts are less likely to be viewed or engaged with due to algorithms, but the opposite is true about men's health. 1 Influencer Aly Boghici [ allmumstalk ] and her husband Justin [ alldadstalk ] shared content about male and female health to see the reaction from followers. Aly shared content about menopause, pain during sex and vaginal dryness - these types of posts were viewed by 66 per cent fewer non-followers than her typical Instagram activity. By comparison, when Justin posted about erectile dysfunction, leaking after sex and testosterone imbalance, he only noticed a mild drop in non-follower views, by 16 per cent, compared to his normal content. It suggests women's health and sexual wellness content is being routinely restricted on social media platforms as outdated algorithms detect words like 'vagina' or 'period' and block non-followers from viewing. Engagement rates between posts were significantly different, with Aly receiving a massive drop in comments by 69 per cent, whereas Justin received an uptick by 76 per cent as both followers and non-followers expressed interest in men's health topics. Hygiene and health company Essity, which produces menstrual health brands Bodyform and Modibodi, commissioned the experiment. It is campaigning to end the practice of social media companies banning medically and anatomically correct terminology related to women's health. Aly Boghici said: "It is clear my content is being censored and I can see clearly that the censorship of women's health related content is an issue. 'I have identified these following topics results in my posts receiving less engagement: menstrual health, charities supporting menstrual health, sexual health or charities supporting these important conversations, and the list can get longer. Millions of women live life according to their menstrual cycle, study shows 'I might be reluctant to post about these important issues moving forward as the message doesn't seem to be supported unless more support from the algorithms will be given, and people to be given a chance to voice their personal experiences, otherwise we cannot make a change and support women going through different health scenarios… we cannot break taboos if we keep silent .' Additional posts during the experiment by Aly focussed on periods, heavy bleeding and endometriosis - which saw 25 per cent less views. Taboo topics such as thrush and discharge had 14 per cent less views than normal content. Justin posted about taboo topics such as pain in the testicles, weird discharge and thrush. It doesn't feel fair that one gender appears to be disproportionately censored when it comes to sharing information about health. His posts illustrating learnings about penile, testicular and prostate cancers experienced an increase in non-follower views by four per cent. Justin Boghici said: 'After one week of comparing notes, I noticed no notable change in engagement on my posts on male health issues, if anything, they've actually increased, especially on the reel message. 'It was quite interesting to make this men's vs women's health comparison. 'It doesn't feel fair that one gender appears to be disproportionately censored when it comes to sharing information about health.' The experiment comes as research of 4,000 adults found 47 per cent of those who use social media feel it is an important platform to educate themselves about health and wellbeing. And 45 per cent don't believe any restrictions should be placed on posts containing the correct medical or anatomical words and phrases to describe the human body. Some 44 per cent think anyone should be able to publish content related to women's health, for example, menstrual health or breast cancer. Just under eight in 10 adults (77 per cent) said words like 'vagina' or 'periods' should not be restricted on social media when used in an educational context. Essity is working with campaign group CensHERship to raise awareness about the negative impact of women's health censorship on social media. Spokesperson Kate Prince said: 'Our Bodyform brand is regularly censored when it launches new campaigns on social media that are designed to improve education around menstrual health. 'Women and girls must be able to access information and advice about critical health topics on social media and they should be able to hear about and engage with the experiences of others. 'We can fully understand the need to monitor certain words and phrases, but when anatomically and medically correct language is censored then we think it's going too far. 'We're asking the social media giants to use their expertise and offer a solution that would help millions of women and girls gain access to important content and information.'

A hospital in Minnesota was left in disbelief after a nine-year-old boy survived an accidental fall that sent an arrow through his brain and nearly out the back of his skull. Abby Deterding recently told the Minneapolis news station KARE 'it's a miracle' that the freak mishap did not kill her son, Gus. 'We're just thankful, so grateful,' she said. As his parents recount it, Gus was helping his father, Dave, pack up his truck for a hunting trip in March when he slipped on the icy driveway outside their home in Alexandria, about 130 miles north-west of Minneapolis. A youth bow and arrow were among the items Gus was carrying at the time he slipped. And he fell face-first on the arrow, which penetrated his head just to the right of his nose. Gus pulled the arrow out of his head by himself and immediately went to see his mother, who was vacuuming inside, KARE reported. He evidently was not bleeding much but was panicked. 'He kept saying, 'Mom, am I dying? Am I going to leave you? I don't want to leave you yet,'' Abby Deterding recalled her son saying. She said she told Gus he would be OK and they were just going to the hospital to get some stitches. A cautious emergency room doctor had Gus flown by helicopter to Children's Minnesota hospital in Minneapolis, where staffers observed him a few hours before releasing him home. But, after getting home, Gus began vomiting, and his parents brought him back to the hospital, which took head scans of the boy. The scans contained a startling revelation, as KARE noted: the arrow had pierced through the middle of Gus's brain and came within less than an inch of exiting his skull. 'It literally took my breath away, and I felt sick to my stomach,' Abby Deterting said to the outlet about what it felt like for her to see those scans. 'I couldn't believe it.' She said she remembered thinking: 'No, that far, what?'' Adding to the confusion was the fact that Gus seemed to be fine while in a hospital bed nearby. The medical director of the Children's Minnesota's pediatric intensive care unit, Dr Ken Maslonka, reportedly said to KARE that he had 'never seen anything like this' in his 28-year career. 'He looked too normal,' Maslonka said. Maslonka told the outlet that he had created a model meant to show the trajectory of the arrow that went into Gus's head. Had he seen his model without already knowing the outcome, Maslonka said he would have assumed the injury depicted was fatal. Gus, though, was showing no ill effects from the wound a few months after it had occurred, prompting Maslonka to attribute that to either 'fortune' or 'God controlling the direction of that arrow'. Abby and Dave Deterding said it was their opinion that Gus's survival was miraculous rather than lucky. 'We know,' Dave Deterding said, 'it didn't just happen.'
