Cognitive Therapy Offers Sustained Relief of Low Back Pain

A personalized protocol involving self-managed cognitive functional therapy (CFT) was associated with sustained symptom improvement at 3 years in patients with chronic low back pain in a phase 3 follow-up study. The three-armed RESTORE trial assessed adult patients in Australia with chronic disabling low back pain, showing effectiveness for CFT with or without motion sensor biofeedback compared with usual care for up to 1 year. In this new extension analysis of RESTORE, both CFT treatments were still more effective than usual care at 3 years, with greater reductions in both pain intensity and activity limitation. Interestingly, no differences in outcomes were found between the CFT-only group and the CFT plus biofeedback group. 'It shows that if we empower patients with knowledge and skills to manage back pain, including future flare-ups that are almost inevitable, then we can markedly reduce the massive personal and societal burden of back pain,' lead author Mark Hancock, PhD, Spinal Pain Research Center, Macquarie University, Sydney, Australia, told Medscape Medical News. 'We hoped to see these results, given the intervention design, but we were still somewhat surprised how well the effects lasted over time,' he added. The findings were published online on August 5 in The Lancet Rheumatology . Building on 'Unusual' Initial Findings Hancock noted that most treatments for low back pain produce small, short-lasting benefits. 'The CFT intervention is specifically designed to have long-term effects, as it gives people the skills, knowledge, and confidence to self-manage. The results we previously published showed the effects were sustained at 1 year, which is unusual, and we wanted to see if it remained effective at 3 years,' he said. In the original RESTORE trial, the investigators recruited adult patients who had low back pain lasting more than 3 months and randomly assigned them to receive one of three treatments: CFT only (n = 164), CFT plus biofeedback (n = 163), or usual care (n = 165). The CFT groups received up to seven treatments over 12 weeks plus a booster session at 26 weeks, all of which were delivered by trained physiotherapists. Investigators noted that CFT targets biopsychosocial barriers to recovery and has three components that are based on an individual patient's goals. These components include 'making sense of pain,' exposure with control, and lifestyle change. Both CFT groups wore movement sensors during their sessions, but only the CFT plus biofeedback group and their physiotherapists had access to the data. Usual care involved treatment that was chosen by the patient themselves and/or recommended by their healthcare provider. The new 3-year follow-up included 312 of the original participants (mean age, 48 years; 60% women) and between 60% and 65% of each of the original treatment groups. The extension's primary outcome was self-reported pain-related physical activity limitation, as measured on the Roland-Morris Disability Questionnaire. The secondary outcome was pain intensity, as measured by the numeric pain rating scale. Long-Term Pain Reduction Results showed that CFT with biofeedback was more effective in reducing activity limitation at the 3-year mark vs usual care (mean difference, -4.1; P < .0001), as was CFT alone (mean difference, -3.5; P < .0001). Additionally, CFT with or without biofeedback was more effective in reducing pain intensity vs usual care (mean differences, -1.5 and -1.0, respectively; P < .0001 for both). For both measures, the differences between the two CFT groups were not significant. Sensitivity analyses showed similar, although slightly smaller, effects. In addition, 49% of the CFT plus biofeedback and 43% of the CFT-only groups maintained recovery level scores between the 1-year and 3-year follow-ups compared with 17% of the usual care group. 'These long-term effects are novel and provide the opportunity to markedly reduce the effect of chronic back pain if the intervention can be widely implemented,' the investigators wrote. They noted that this would include an increase in clinician training and replication studies in other populations. 'This type of biopsychosocial intervention is widely recommended in guidelines, but the evidence [for it] is now becoming much stronger,' Hancock said. 'Many clinicians aim to deliver these interventions, but they often find it hard, especially if they were trained in a more pure biomedical way of thinking. There are now more resources to assist clinicians in upskilling in these approaches,' he added. 'Cautious Optimism' In an accompanying editorial, Dimitrios Lytras, PhD, Department of Physiotherapy, International Hellenic University, Thessaloniki, Greece, applauded how the study was conducted. 'Methodologically, RESTORE is exemplary: a pragmatic, multicenter trial embedded in routine care [and] supported by thorough therapist training,' Lytras wrote. He added that the results 'offer cautious optimism' and a shift from passive care models to ones that are more patient-centered. The addition of biofeedback not leading to added benefit could be explained by the fact that CFT 'already incorporates rich feedback mechanisms, making supplementary sensor input redundant,' he noted. Lytras wrote that the intervention is also low risk and high value. Durable Functional Gains Commenting for Medscape Medical News, Sean Mackey, MD, PhD, Redlich professor and chief of the Division of Pain Medicine at Stanford Medicine, Palo Alto, California, said the study showed durable functional gains and modest pain changes. The dose of seven visits over 12 weeks plus a 26-week booster was 'lean,' but the effects held, which was 'pleasantly surprising,' said Mackey, who was not involved with the research. However, he did note a few caveats, including that only 63% of the randomized cohort contributed to the 3-year data. In addition, 'those followed were less severe at baseline and did better at 1 year, though loss to follow-up was nondifferential by arm,' he said. In reflecting on the study design, Mackey noted that an 'attention-matched active comparator to blunt performance and expectancy bias' would have been helpful. He pointed out that the researchers themselves acknowledged that usual care was not contact-matched. He added that it would also have been valuable to include data on 3-year adverse events, healthcare utilization, and costs. If future research confirms its generalizability to other countries, Mackey said he would recommend clinicians 'adopt the CFT package' of graded exposure with a control and lifestyle coaching, schedule 6-8 visits plus a 6-month booster, measure function as the primary outcome, and set expectations for about a one-point reduction in pain. He recommended against adding sensor biofeedback because 'it didn't help.'

Toronto, Ontario--(Newsfile Corp. - August 12, 2025) - Northwest Healthcare Properties Real Estate Investment Trust (TSX: (the "REIT" or "Northwest"), a global investor and operator of healthcare infrastructure assets in North America, Australasia, Brazil, and Europe, announces results for the three and six months ended June 30, 2025. In addition, the REIT announces the suspension of its distribution reinvestment plan (the "DRIP") and the appointment of Zachary Vaughan to the Board of Trustees (the "Board"). Zachary Vaughan, CEO of Northwest, commented, "Having just joined Northwest in July, I am excited by the caliber of the portfolio and the strength of the team. The assets we own and operate are critical components of healthcare infrastructure. Our assets have long-term demand drivers, recession-resistant cash flows and very low obsolescence risk. Our cash flows are backed by outstanding underlying credit, often government supported, with long-term indexation. Given Northwest's unique position as a leader in healthcare infrastructure, we are well positioned for future growth. "Our Q2 2025 results reflect continued operational momentum, with improvements in AFFO per unit, a lower payout ratio, and continued execution of our capital recycling strategy with over $282 million in non-core asset sales completed year to date. "Looking ahead, we're focused on disciplined capital allocation and unlocking value through strategic asset management and selective dispositions. This will provide us with flexibility to further reduce debt, reinvest in our core portfolio, and pursue accretive growth opportunities that drive long-term value for unitholders." Q2 2025 Highlights Highlights for Q2 2025 and events subsequent to the quarter are set out below: Revenue from investment properties was $99.0 million for Q2 2025, a decrease of 16.9% from Q2 2024 driven by the disposition of non-core assets during 2024 and 2025 to date, partially offset by solid same property revenue growth; Same Property Net Operating Income ("SPNOI") increased by 2.8% to $73.2 million for Q2 2025, over Q2 2024, reflecting steady growth across all regions (see Exhibit 1); General and administrative expenses, excluding unit-based compensation and employee termination benefits and associated costs, were $12.6 million for Q2 2025, a decrease of $0.5 million from Q2 2024, primarily due to headcount reduction and simplification of the REIT's business; Net income for Q2 2025 was $32.6 million compared to net loss of $127.2 million in Q2 2024, primarily due to a decrease in mortgage and loan interest expense, positive fair value adjustment on investment properties, and unrealized foreign exchange gains, partially offset by fair value loss on revaluation of financial instruments and by lower net operating income as a result of disposition activity; Adjusted funds from operations ("AFFO") was $0.10 per unit in Q2 2025 compared to $0.10 per unit in Q1 2025 and $0.09 per unit in Q2 2024, resulting in an AFFO payout ratio of 88% in Q2 2025 compared to 92% in Q1 2025 and 105% in Q2 2024 (see Exhibit 2); During Q2 2025, the REIT recorded fair value gains on investment properties of $13.6 million, compared to fair value losses of $172.4 million in Q2 2024. The fair value movements were mainly attributable to changes in valuation parameters, incorporating market evidence when available and rent reviews. The REIT's portfolio cap rate as at June 30, 2025 is 6.3%; The REIT's leverage was 48.5% at the end of Q2 2025, as compared to 50.0% as at December 31, 2024, driven by proactive debt repayment from dispositions; and Strong operating performance in Q2 2025 was supported by a stable, long-term lease maturity profile with a weighted-average lease expiry ("WALE") of 13.5 years and a global portfolio occupancy rate of 97%. Operations and Leasing The REIT's consolidated SPNOI increased by 2.8% in Q2 2025 compared to the prior year period, driven mainly by inflationary rent adjustments, rentalised capital spend, and improved recoveries reflecting steady growth in the REIT's underlying lease income. Regionally, SPNOI increased by 2.0% in North America, 4.6% in Brazil, 2.3% in Europe, and 2.6% in Australasia (see Exhibit 1). In Q2 2025, the REIT completed 298,000 square feet of new, renewal and early leasing, achieving a strong renewal rate of 89%. As previously disclosed, in March 2025, following sector margin pressures exacerbated by its capital structure, Healthscope Pty Ltd ("HSO"), the REIT's second largest tenant, occupying 12 properties and accounting for 6.1% of the REIT's proportionate revenues, requested temporary relief from its lender syndicate and landlords, including Northwest. In May 2025, HSO's parent entities entered receivership, with its lenders appointing McGrathNicol Restructuring to work with HSO management to complete an orderly sale of the business. All hospitals continue to operate as usual, and the company has received an additional A$100m facility to support operations. In July 2025, (i) the HSO receiver repaid 50% of the previously announced rent deferral accrued to June 30, 2025, and (ii) Northwest agreed to an updated rent deferral arrangement with the HSO receiver starting July 1st whereby 10% of ongoing rent will be deferred. All deferrals are expected to be repaid by March 31, 2026 or upon completion of sale of material assets. The deferred rent has priority over senior debt. The receiver-led sale process commenced in late July, and it is anticipated that a new owner will be confirmed in Q4 2025, with the transition process and timing of regulatory approvals to be determined. Northwest maintains constructive dialogue with the receiver and ongoing discussions with prospective operators participating in the process. As of today, all rent owing to the REIT from HSO (other than the rent subject to rent deferral arrangements) has been paid and HSO continues to meet all lease obligations. Disposition Activity and Assets Held for Sale The REIT's capital recycling during the quarter, including subsequent events, has generated over $231 million of proceeds as detailed below. During Q2 2025, the REIT sold its shares in Assura through two on-market transactions for total proceeds of $209.3 million (£114.7 million) resulting in a full divestment of the shares. The REIT's investment in Assura was acquired as partial consideration for the REIT's disposition of its UK portfolio in August 2024 at a value of $177 million (£100 million), resulting in a gain on disposition of approximately $32.3 million (£14.6 million). Proceeds were used to repay related debt and corporate facilities. In Q2 2025, the REIT completed the sale of one income producing property in North America, one development property in Europe, and one development property in Australasia, classified as assets held for sale at their fair value of $22.1 million. The proceeds were used to repay directly attributable debt and outstanding balances on credit facilities. As at June 30, 2025, the REIT held one income producing property with a value of $34.3 million as classified as held for sale. Financing Activity During the quarter, the REIT also extended or refinanced several debt facilities including Canadian mortgages and Australasian term loans. On July 29, 2025, the REIT amended the terms of its revolving credit facility. The amendment extends the maturity date of the facility to July 2027 and moves the REIT to grid pricing based on its credit rating, reducing the REIT's cost of borrowing by 65 basis points. The amendment also provides a $100 million accordion commitment, subject to incremental security and lender approval, that provides the REIT flexibility with mortgage maturities as it transitions away from amortizing debt. Subsequent to quarter end, the REIT repaid Canadian mortgages totaling $40.8 million, with a weighted average interest rate of 2.67%, using capacity on its revolving credit facility. As of today, the REIT has $20.9 million of Canadian mortgages with 2025 maturities that will be repaid with capacity from the revolving credit facility and $11.7 million of European mortgages that will be renewed in the ordinary course. Of the REIT's proportionate 2026 debt maturities totaling $424 million, over 50% or approximately $226 million are maturing in the fourth quarter of 2026, with the remainder relating to mortgages. The REIT currently has approximately $230 million of available liquidity, consisting of cash and the unused portion of its credit facilities. Commencing with the September 2025 distribution (payable on or about October 15, 2025), unitholders enrolled in the DRIP will receive distribution payments in cash. If the REIT elects to reinstate the DRIP in the future, unitholders that were enrolled in the DRIP at the time of its suspension and remain enrolled at the time of its reinstatement will automatically resume participation in the DRIP. As Miami-Dade faces budget crunch, fight over healthcare heads for a showdown

Managing healthcare bills for Miami-Dade's roughly 31,000 county employees is big business, and that's prompting a big fight at the County Commission. At issue is a proposal by Mayor Daniella Levine Cava to drop the county's longtime healthcare provider, AvMed, for a national competitor, Aetna — something the mayor claims would save the county about $40 million as it faces a major budget crunch. The fight to win the county's healthcare contract has gotten messy enough that commissioners are holding a special meeting Wednesday dedicated solely to picking a winner. AvMed, based in Doral, argues Levine Cava is rushing the effort ahead of a fall open-enrollment period that's bound to be chaotic if a new provider takes over. AvMed also points to high customer-service rankings as a selling point — particularly with management based in the Miami area. But Levine Cava points to a pair of consultant studies showing that Aetna's national purchasing power would bring Miami-Dade cheaper healthcare costs in a year of budget strains. AvMed, based in Doral, and Aetna, a Connecticut-based subsidiary of the CVS drugstore chain, each want to manage the crucial billing process — including reimbursement rates for healthcare providers. Unions representing county employees are divided. The police union backs keeping AvMed, while the transit union wants to switch to Aetna. In a recent Miami Herald op-ed, Levine Cava said Aetna will bring both savings and expanded options for county employees. 'Partnering with Aetna means not only financial savings for the county but better healthcare access and quality for our workforce,' she wrote. AvMed, which has contracts across Florida, rejects the notion that Aetna will be cheaper for Miami-Dade and points to a recent J.D. Power ranking giving it the top scores for customer service in Florida. AvMed first won the county contract in 2016 and is encouraging Miami-Dade to stick with a healthcare company that has proven itself. 'By continuing its partnership with AvMed, Miami-Dade County can ensure uninterrupted access to the #1 ranked health plan in Florida,' the company said in a statement, referencing the J.D. Power ranking. If the county switches providers, Aetna would earn administrative fees from Miami-Dade worth about $15 million a year. AvMed's fees are lower, at $10 million. But the fight comes over the much larger county expense involved in the agreement: what Miami-Dade pays providers for medical care and supplies, including prescriptions. Like most large governments, Miami-Dade uses its own cash to pay medical expenses, rather than relying on an insurance company to reimburse providers. Under the 'self-insured' model, Miami-Dade can save money if reimbursement rates drop for doctors, pharmacies and other providers used by county employees. Miami-Dade's medical expenses currently hover around $600 million a year, according to county estimates. The county's insurance company negotiates the rates behind those expenses, and Aetna claims it can lower Miami-Dade's healthcare costs over the life of the seven-year contract. Two healthcare consultants hired by Miami-Dade back up that contention, estimating yearly savings of around $40 million when calculating how much the county would pay under each competitor's reimbursement rates. Commissioners have devoted a rare special meeting to deciding the contract award, placing board members in the position of refereeing between competing claims in the complicated realm of healthcare finances and reimbursement rates. 'I'm not for Aetna. I'm not for AvMed,' Commissioner Roberto Gonzalez said during a July 16 discussion of the healthcare contract. 'I'm for whatever is best for the employees of Miami-Dade County.'
