
University makes 'ground-breaking' discovery offering hope for chronic pain sufferers
Scientists in Aberdeen have made a ground-breaking discovery that could revolutionise treatment for chronic pain.
Researchers from the University of Aberdeen, Academia Sinica in Taiwan and a group of international experts have discovered that chronic and acute pain are physiologically different.
It is hoped the breakthrough will lead to new treatments for conditions such as fibromyalgia and rheumatoid arthritis.
Marlene Lowe knows only too well what it's like living with debilitating pain.
The 35-year-old suffers from both chronic fatigue and fibromyalgia, which causes widespread pain.
Often dismissed by the healthcare system, she says these conditions can feel like an invisible burden. STV News Marlene describes her experience of living with the chronic pain condition fibromyalgia
She told STV News: 'One day you can be perfectly fine and you can go for a jog and the next day it is difficult to get out of bed. It's hard to balance those things out, so when you are in that state, people don't see you.
'They see you when you are feeling better and out and about. It's incredibly demoralising to say 'look, I'm in agonising pain constantly' and someone says 'But you are still working, you must be alright'.'
Marlene is one of millions who suffer from persistent chronic pain which, unlike acute pain – typically caused by injury – is very hard to treat.
Until now, Western cultures have not differentiated between the two.
But scientists at Aberdeen University, together with experts from Taiwan, have discovered that in the nervous system, chronic pain is processed differently from the pain that comes from an injury or over exertion.
Crucially, they found a new and distinct separate physiological pathway for this chronic type of pain, which means it can now be a target for future therapies.
Dr Guy Bewick is a senior lecturer in biomedical science at the University of Aberdeen.
'The GP cannot give these patients anything to relieve the pain. It doesn't respond to painkillers,' he said.
'They have to give them moral support and advise them on things they should or shouldn't do, which will make it worse or better.
'But we think we have found the pathway. We have a model and we have found out how that gets turned on by two specific set of nerves, which are talking to each other.' STV News Dr Guy Bewick
He explained that Eastern cultures have differentiated pain for centuries, with chronic pain being called 'sng' or 'suan tong' (sour pain) in Mandarin.
'The stinging pain from sharp objects and surgery can usually be treated effectively with common painkillers, but chronic pain often cannot,' he added.
'New treatments require an identifiably different drug target. This study has found that target. Specifically, we discovered the mechanism of this pain we call 'sng'.'
Dr Bewick's team discovered that a molecule called glutamate is released in muscles to activate a highly unusual receptor. This sparked a collaboration with Professor Chen's team in Taiwan who found that too much glutamate release activated pain nerves nearby making them permanently active and not switch off as they normally would.
Crucially, they then discovered that blocking the newly discovered, highly unusual, glutamate receptor entirely stopped the chronic pain being triggered. STV News Chronic pain has been shown to be physiologically different from acute pain – and now scientists have the roadmap for how to target it
Scientists hope their findings will help develop new treatments for conditions such as fibromyalgia and rheumatoid arthritis.
'It's this ache which can become chronic and very distressing to some people,' Dr Bewick said.
'The acute pain responds well to painkillers; whereas the second pathway, people didn't know what it was.
'This has the potential to help the many people whose pain is currently inadequately treated.'
The wider research was led by Professor Chih-Cheng Chen from Academia Sinica, supported by National Science and Technology Council's Brain Technology Project and an Investigator Award of Academia Sinica.
They were able to differentiate between the two types of pain by genetically silencing neuronal pathways in a mouse model. At the Taipei Medical University Hospital in Taiwan, the theory was then tested in practice in a patient with a spinal cord injury that blocked 'standard' pain but spared the newly discovered pathway.
Professor Chen said: 'Fundamentally, we found that sng persists even in people who have lost other pain sensation, for example, a patient with spinal cord damage did not notice when he had broken a toe but could still perceive 'sng' and position in the same leg.
'Clearly, therefore, sng is a separate pathway.
'This finding could lead to new pain relief treatments for such conditions as fibromyalgia, exercise-induced muscle pain (DOMS), rheumatoid arthritis, and chronic pain after spinal surgery.
'It is a truly ground-breaking discovery in pain research.'

The celebrated medical specialist sadly died of natural causes in Greece. The television star, 67, was renowned for his expertise in nutrition and diet plans, helping countless people lose weight while bolstering their long-term health. One famous dietary programme was the 5:2 diet, which he developed with his wife, Dr Clare Bailey Mosley, in an effort to simplify dieting for everyone. Despite Dr Mosley's tragic death, the tremendous impact of his work will live on, with his family now establishing a research fund in his name. But what exactly is the 5:2 diet, and how can it help you? If it's something you'd be keen to explore, look no further. The Mirror has compiled everything you need to know about the popularised regimen. What is the 5:2 diet? The 5:2 diet plan is designed to support anyone looking for an effective but gradual way to lose weight. It involves eating normally for five days and reducing consumption for two, often called 'fasting' days. During these fasting days, dieters are advised to consume between 500 and 800 'high-quality' calories while eating within an eight, 12, or 14-hour window. It doesn't matter what days of the week are fasting days, though it is advised to separate them with one 'normal' day. Prioritising Mediterranean-style food is also encouraged across all days, as emerging research increasingly shows it offers greater health benefits than typical 'Western' staples. Specialists at Healthline have explained: "One common way of planning the week is to fast on Mondays and Thursdays, with two or three small meals, then eat normally for the rest of the week. "It's important to emphasise that eating 'normally' does not mean you can eat anything. If you eat too much junk food, then you probably won't lose any weight, and you may even gain weight." Does it work? While limited research has been conducted on the 5:2 diet specifically, academic studies have acknowledged the benefits of intermittent fasting. This goes beyond weight control, with scientists highlighting its potential role in chronic illness prevention, too. Following a study in 2021, Dr Benjamin Horne at the Intermountain Healthcare Heart Institute, explained: "Inflammation is associated with a higher risk of developing multiple chronic diseases, including diabetes and heart disease. We're encouraged to see evidence that intermittent fasting is prompting the body to fight inflammation and lowering those risks." He later added: "The best routine is one that patients can stick to over the long term, and this study shows that even occasional fasting can have positive health effects." It's also worth noting that Mediterranean-focused diets other than the 5:2 have garnered notable attention for their possible health perks. These benefits similarly extend beyond weight loss. For instance, last year, the Autonomous University of Barcelona listed garlic, saffron, olives, rosemary, and grapes among the Mediterranean foods with 'active components' that may be advantageous for heart health. Meanwhile, another study from Tulane University even found that Mediterranean diets may be associated with a slashed dementia risk. "Our findings suggest that the Mediterranean diet or its biological effects could be harnessed to improve scholastic performance in adolescents, or work performance in young adults," corresponding author Dr Demetrius Maraganore at Tulane University said at the time. "While these findings are based on animal models, they echo human studies linking the Mediterranean diet to improved memory and reduced dementia risk." Is it for everyone? Despite its potential advantages, the 5:2 diet is not for everyone. Kerry Torrens, a nutritionist at the BBC's Good Food, warned that it isn't safe for pregnant or breastfeeding mothers or anyone using specific medications. It's also unsafe for children and teenagers who need more crucial nutrients for growth. Therefore, anyone considering the 5:2 diet is encouraged to seek medical guidance first. Kerry said: "Much of the emphasis given to the 5:2 is on the 'fasting' days, but to be safe, effective and healthy, the food consumed during the other five days of the week needs to be of high nutritional value. "For this reason, you should aim to include essential fats from oily fish, nuts and seeds, lean sources of protein, wholegrains and starchy carbs with plenty of fruit and vegetables to supply the necessary fibre, vitamins and minerals that you need."

Mum left 'helpless' after baby son diagnosed with rare life-limiting disease
An Aberdeen mum has called for more support to be made available for those with rare conditions after her son was born with a life-limiting disease. On August 30 last year, Bobbie Hardingham and Callum Booth welcomed Tommy Booth into the world. He was born four weeks prematurely and kept in Aberdeen Royal Infirmary's neonatal unit due to his low body temperature. After undergoing numerous tests, medical teams were unable to identify the issue. Less than one month after welcoming their boy into the world, the couple returned to the hospital as Tommy was having difficulty breathing and his body temperature dropped to 28C. Tommy was airlifted to Edinburgh for treatment and further tests and scans. Doctors still had difficulty identifying the route of the youngster's problem. STV News Tommy Booth was diagnosed with Menkes disease. Finally, a genetic test was carried out, which revealed that Tommy had an adrenal deficiency affecting his kidneys. The family believed Tommy would be on steroids for the rest of his life however on April 2, the couple were told it was 'not good news' by doctors. Tommy was diagnosed with Menkes disease, a rare, inherited X-linked recessive disorder that affects copper metabolism. Symptoms include kinky and sparse hair, hypothermia, developmental delays, and seizures. According to the National Organization for Rare Disorders, the condition affects one in 35,000 live male births. A majority of children born with Menkes disease do not live past the age of three. Bobbie says she felt 'helpless' after being told her son had an incurable condition. The mum told STV News: 'I just broke down, I couldn't believe it. 'I didn't understand it fully at the time. When the doctors said there was nothing they could do, you just felt helpless and felt you'd done something wrong. STV News Tommy Booth received the diagnosis following a genetic test. 'But of course, we haven't done anything wrong, this just happened. There aren't any words to describe how you feel at that time. You just feel numb. 'There were no leaflets about support, it just felt isolating.' The family believe the ordeal their nine-month-old son is facing has completely shifted their perspective on life. Bobbie added: 'There were things I'd moan about in the morning that I wouldn't complain about anymore. 'When you get hit with something like that, it changes you. To be honest, I don't know how we are coping. 'Our family and friends have been encouraging us and supporting us. If we didn't have them around us, I don't know how we'd continue.' Tommy's condition has deteriorated over time, meaning he is unable to sit up, grab things or smile. He has also started to have seizures, and his parents believed at one point, he would no longer be able to eat. The 36-year-old mum hopes sharing her son's story will help other families across the country notice the signs of the condition. Bobbie also believes that more help should be made available for those living or caring for someone with a rare condition. Bobbie said: 'We want to raise awareness of this awful disease. 'We need to make Tommy as happy as we can. As a family, we can make as many memories and show Tommy as much love as we can. 'We need to make sure that Tommy is as comfortable as possible. For children with rare conditions, there isn't that support. STV News Tommy's mum believes more should be done to support those with rare conditions. 'There's hundreds and thousands of children with conditions that don't have the support they need.' The Scottish Government says it remains 'committed' to raising awareness of rare conditions and ensuring support is available. A Scottish Government spokesperson said: 'We are very sorry to hear about this family's experience, and our thoughts are with them as they navigate the challenges of this extremely rare and life-limiting condition. 'We recognise that living with a rare condition can have a significant impact not only on the individual but also on their families and carers. 'That is why we are committed to improving care and support through our Rare Disease Action Plan for Scotland, which focuses on faster diagnosis, better coordination of care, increased awareness among healthcare professionals, and improved access to specialist services.' A fundraiser has been set up to support Tommy's medical needs.
