
Nonsurgical Options Show Modest Benefit in Chronic Back Pain
METHODOLOGY:
Researchers conducted a systematic review and meta-analysis of 75 randomized or cluster randomized controlled trials involving 15,395 participants aged 16 years or older (mean age, 45.7 years; median percentage of women, 61%) with chronic low back pain lasting at least 12 weeks.
They assessed the effectiveness of nonsurgical interventions for chronic low back pain (including 91% trials on nonspecific chronic low back pain) with a focus on long-term (1-2 years) and very long-term (≥ 2 years) outcomes.
Psychological, physical, and combination nonsurgical interventions, including cognitive-behavioral therapy, mindfulness, and multidisciplinary care, were assessed, with exercise being the most frequently assessed intervention.
Interventions were compared with placebo or sham, adjuvant interventions, no intervention, or usual care.
Pain intensity and disability were assessed using scales like the Numerical Pain Rating Scale and Oswestry Disability Index, respectively, along with other measures, and the certainty of evidence was assessed.
TAKEAWAY:
At long-term follow-up, small-to-moderate reductions in pain intensity and disability were possibly seen with cognitive-behavioral therapy (mean differences, -7.2 and -5.7, respectively) and mindfulness (mean differences, -10.0 and -9.3, respectively) in individuals with nonspecific chronic low back pain. The evidence was of moderate certainty.
Goal setting and needling interventions probably resulted in small reductions in disability, with mean differences of -8.3 and -4.8, respectively.
Multicomponent biopsychosocial care and behavioral therapy might have led to moderate and small reductions, respectively, in pain intensity in those with nonspecific chronic low back pain (mean differences, -10.2 and -7.4, respectively; low-certainty evidence). Multidisciplinary care could have led to small reductions in disability (mean difference, -8.3).
At very long-term follow-up, multidisciplinary care may have led to a moderate reduction in pain intensity (mean difference, -10.1), whereas exercise may have resulted in a moderate decrease in disability (mean difference, -10.2) in those with nonspecific chronic low back pain. The evidence was mostly of low certainty.
IN PRACTICE:
'Although we identified some interventions with long-term effects for people with chronic low back pain, the clinical importance of the effects is uncertain,' the study authors wrote. 'Greater attention is needed on developing and testing interventions with long-term effects for chronic low back pain.'

You have got bigger problems than groceries, if she won't even contribute to food costs," one person observed. Covid is rising. New vaccines may not be ready until mid-September.
Coronavirus infections are climbing again, marking another summer wave as children go back to school. But this uptick arrives with an added layer of uncertainty because it's unclear when and which Americans can receive updated vaccines this fall. Kuritzkes, chief of the division of infectious diseases at Brigham and Women's Hospital in Boston, said the current rise looks similar to seasonal bumps in previous years and is not driving a surge in severe illness. This is the new rhythm of covid waves. Many people are getting sick - some feeling lousy - especially after returning from vacation and conferences. Most probably won't even know it's covid because the symptoms can be indistinguishable from a cold or other respiratory bug. Yet hospitals are no longer flooded with patients because immune systems are much better trained to fight the virus five years after it arrived. But Kuritzkes and other medical and public health experts worry that delays and confusion surrounding the vaccines will limit access for those who need them the most: Adults ages 65 and older and people who have chronic conditions, weakened immune systems or are pregnant. The federal committee that would recommend the updated coronavirus vaccine is not expected to meet until at least mid-September, according to industry employees and Centers for Disease Control and Prevention officials who spoke on the condition of anonymity to share sensitive information. By that point, the summer covid wave could be over, but Americans could still get a boost of immunity ahead of an expected winter wave. Here's what to know about the latest with covid and how to protect yourself: - - - What does the data show? Several metrics tracked by the CDC show a midsummer uptick of coronavirus underway in the United States. Viral levels in wastewater started rising in late June and are considered high in the West, South and Midwest and headed that direction in the Northeast, according to Marlene Wolfe, principal investigator and co-program director of WastewaterSCAN, a private initiative that tracks municipal sewage data. The levels of virus are roughly one-third of the levels recorded a year ago, when the country was experiencing an unusually large summer covid wave. Emergency room data suggests infections are disproportionately affecting children. As of Aug. 7, coronavirus diagnoses accounted for 2.18 percent of emergency room visits among children up to age 11, the largest jump of any age group. Still, some experts caution that numbers are still very small. Aaron Milstone, a pediatric infectious-disease specialist at Johns Hopkins University School of Medicine, said most children now have some level of hybrid immunity from prior infection and vaccination. The current predominant variant is XFG, according to CDC wastewater tracking. XFG is an offshoot of the JN.1 subvariant of omicron that became dominant in the winter of 2023-2024. The Food and Drug Administration has advised vaccine manufacturers to update their formulas to target the JN.1 lineage, specifically using the LP.8.1 strain that was dominant in the spring and is closely related to XFG. The latest variants do not appear to cause worse symptoms or have significant differences compared with its predecessors, according to Scott Roberts, an assistant professor of infectious diseases at Yale School of Medicine. - - - What's the latest with vaccines? For the coming fall season, the FDA has not yet approved an updated coronavirus vaccine. In recent years, the agency has typically signed off on an updated version by late August or early September in order for pharmacies and doctors' offices to place orders. But the agency is expected to narrow its approval for the vaccine to those 65 and older, as well as others considered at high risk for severe disease. The new approach marks a significant shift from past years when the shots have been broadly available, including to children and generally healthy adults. Once the FDA gives the greenlight, the CDC's Advisory Committee on Immunization Practices decides who should get the shots. Most insurers must pay for ACIP-recommended vaccines. The American College of Obstetricians and Gynecologists has strongly criticized that move, noting research has shown the virus can be dangerous for expecting mothers and cause birth complications. - - - How can I protect myself and others? Public health experts say the best way to protect yourself from covid is to stay up-to-date on vaccines. The 2024-2025 vaccine formula is still available. But it might be harder to find because some doctors and pharmacies may have decided not to restock their supplies while they wait for a new version to arrive. The CDC recommends two doses of the vaccine for people who are 65 and older or are immunocompromised. Jessica Justman, an infectious disease specialist at Columbia University, said if someone in those groups had not yet received their second dose, they should go ahead and get it now, especially if they anticipate increased exposure such as attending a large indoor gathering. The primary purpose of vaccination is to reduce the risk of severe disease, hospitalization and death. For those wanting to avoid infections completely, experts advise wearing surgical or KN95/N95 masks, avoiding crowded indoor settings and ventilating rooms. 'Sleeping giant' fault beneath Canada could unleash a major earthquake, research suggests

A major fault in the Yukon, Canada, that has been quiet for at least 12,000 years may be capable of giving off earthquakes of at least magnitude 7.5, new research suggests. Based on the amount of strain the Tintina fault has accumulated over the past 2.6 million years, it is now under an amount of stress that could lead to a large quake within a human lifespan, researchers reported July 15 in the journal Geophysical Research Letters. The finding may require experts to rethink the earthquake danger in the region, the study authors said. An magnitude 7.5 earthquake would threaten a few small communities within the remote Yukon. But the finding that the Tintina fault may be capable of such a large quake is notable because the fault has been quiet since before the last ice age ended. "Major ancient faults like that can remain as weak zones in the Earth's crust and then focus ongoing tectonic strain," Theron Finley, a geoscientist who conducted the research while earning his doctorate at the University of Victoria in Canada, told Live Science. The Tintina fault is over 620 miles (1,000 kilometers) long and stretches from northeast British Columbia through the Yukon and into Alaska. On its southern end, it connects to the Rocky Mountain Trench fault, which creates a huge valley through southern Canada and northern Montana. Forty million years ago, during the Eocene epoch, one side of the Tintina fault slid 267 miles (430 km) against the other at a rate of about half an inch (13 millimeters) each year. Today, the fault seems quiet, with only occasional small earthquakes of magnitude 3 to 4 in some sections. However, "there has always been a question of whether it's still a little bit active or still accumulating strain at a slower rate," Finley said. To find out, Finley and his colleagues used high-resolution satellite data and lidar imagery of the Yukon. Lidar is a type of laser measurement that allows for precise imaging of topography while ignoring vegetation — an important tool for an area blanketed with forest. With this imagery, the researchers looked for signs on the surface of ancient earthquakes, such as fault "scarps," where the ground moved sharply upward on one side of the fault. "Those features can be hundreds of kilometers long in some cases, but they're only on the order of a couple meters high or wide, so we need the really high-resolution topographic data," Finley said. The researchers determined the dates of each rumple of the landscape by using traces left by incursions of glaciers, which occurred at known intervals 12,000 years ago, 132,000 years ago, and 2.6 million years ago. They found that over 2.6 million years, the fault's sides moved relative to each other by about 3,300 feet (1,000 m). Over the past 136,000 years, the opposing sides of the fault moved about 250 feet (75 m). It probably took hundreds of earthquakes to accumulate all that movement, Finley said, which translates to between 0.008 and 0.03 inches (0.2 to 0.8 mm) per year. The fault has not had a large earthquake that ruptured the ground surface for at least 12,000 years, according to the study. The researchers estimate that in that period, the fault has accumulated about 20 feet (6 m) of built-up strain — movement that hasn't yet been released in an earthquake. The fault probably breaks at between 3 and 33 feet (1 to 10 m) of strain, Finley said, so it's in the range where it might normally fracture. "It could still be many thousands of years before it reaches the threshold where it ruptures, but we don't know that and it's very hard to predict that," Finley said. Because the fault is active in its Alaska portion, it's not surprising to learn that the Tintina fault could be a sleeping giant, said Peter Haeussler, a geologist emeritus at the U.S. Geological Survey in Alaska. He said he was glad to see the evidence emerge."Somebody's finally found evidence for activity on the Tintina fault in the Yukon," Haeussler told Live Science. The fault runs near Dawson City, Canada, Finley said, which has a population of about 1,600 and would be most threatened by a large quake. There are also mining facilities in the area, as well as a risk of quake-triggered landslides. To better understand the risk, geoscientists will need to excavate trenches in the fault to look for rock layers that show past earthquakes and how often they occurred. "Right now, we just know that many have occurred, but we don't have a sense of how frequently," Finley said. "Is 6 meters a lot of strain, or is it more likely there's a long way to go before another rupture?"
