
Why the big consulting firms are bad for healthcare
Every year, American healthcare systems spend hundreds of millions of dollars on consulting firms that deliver PDFs instead of solutions. While patients suffer and clinicians burn out, these legacy firms collect their checks and move on—leaving implementation challenges to healthcare institutions they've diagnosed but failed to treat.
Proponents of the 'Big Five' consulting model would argue that sclerotic institutions need an untainted outsider to parachute in. Someone who isn't married to the nuances of an existing system. But theorizing is easy to do when you're not tethered to the results. In healthcare, it's not just financial results the consultants are off the hook for, it's people's lives.
What do providers get with a legacy consulting firm? Well, a sizable stack of documents. With massive fees to match. While a 300-page PDF may impress at first blush, it won't lead to actionable, sustainable solutions. It certainly won't allow the health system to rapidly test and refine new models of care delivery, proving which ideas do and don't work in practice.
Time pressure and fitting solutions in boxes
At Cactus, the design firm I cofounded, one of our clients was let go from a major health system as part of a big consulting cost-cutting round. He had been working to reduce staffing shortages and developed a novel method for care teams to work together more efficiently, freeing up precious time for overworked nurses. Despite having proven results, he was let go because the consultants couldn't fit his already-implemented, already-proven solution into their model. He has now founded a business to sell that same method to health systems as a SaaS business.
Traditional consulting firms also worsen a key challenge in healthcare: time pressure. Most health systems plan year-to-year based on government reimbursements. With revenue cycles already complex, consultants often default to short-term cost cutting—an easier sell than long-term change. The result? Innovation stalls, patient experience suffers, and the cycle repeats.
If it's so difficult, why not just leave healthcare alone?
Because clearly, patients want better services. And by and large, those closest to the patient aren't the problem. There is an abundance of clinical excellence in the United States but this doesn't always translate to the best outcomes or patient experiences. The disconnect lies in how we approach system-level change.
While working with a leading cancer center, my team found a big pain point for doctors: low compliance—in other words patients weren't following instructions. Research revealed that while treatment plans (housed in large binders) were technically sound, they weren't tailored to patients. As a result, patients would 'underperform.' We found that clearer communication, organized around actionable items and digestible content, could drive meaningful improvement. This 'user experience–first' investigation, which centered on the needs of both doctors (frustrated by low compliance) and patients (overwhelmed by information), is rarely prioritized in traditional consulting models but is core to a more modern, design-led approach.
The house renovation problem
Imagine you're renovating a Victorian mansion with a funky layout. The big consulting approach would be to tell the construction team to spend the allotted budget on turning the largest bedroom into two bedrooms, thereby increasing the home value. Maybe they'd advise a fresh coat of light grey paint, chosen to be least likely to offend potential buyers. But what if the person buying the home doesn't have people to fill those bedrooms? What if they would be happier with a bolder color? What if in the process of splitting the bedroom, the floorboards are found to have mold? Well, the suit-clad consultants are already gone. You're on your own, kid.
Now imagine a design-led approach. In this scenario, the firm leading strategy is also the one implementing the changes. They spend the budget shifting the plumbing, dealing with mold issues as they arise. They don't add another bedroom because it's not needed, even if it would theoretically increase value. They try out a few paint swatches and see market appetite for bolder colors. Buyers are happier—they're willing to pay more! Everyone wins.
From slide decks to solutions
Apply this rubric to healthcare. A design-led approach can balance strategic and business goals with the realities of user experience and complexities of implementation. Consultants that also build can pilot innovations faster, see results faster, reducing overall risk and cost. This type of team can rapidly experiment and improve in a virtuous cycle like the best startups do.
Design-led consulting firms that implement their own changes also have a higher stake in outcomes. They create working prototypes before prescribing final solutions. They iterate based on real-world feedback. This way mistakes are found fast and plans are adjusted before scaling, saving cost and allowing faster and more thorough implementation.
A call to healthcare leaders
To healthcare leaders, I ask: What could you ship in the next 90 days with a design-led approach? What might still be sitting in a binder three years from now with traditional consulting? If the answer is something that could change people's lives, and I suspect it is, it might be time to ditch the big guys.
Design-led firms offer a fundamentally different relationship: partners who share your risk, commit to real-world results, and aren't afraid to get their hands dirty implementing solutions. They bring technical talent alongside strategic thinking. They work in weeks, not quarters. Most importantly, they are judged by what they build, not what they recommend.
The question isn't whether you can afford this approach. The question is whether you can afford not to try it. Because while big consulting firms continue collecting their checks for delivering their slide decks, your patients and workforce are waiting for something better. They deserve it. And with the right partners, you can finally deliver it.

The following three stocks are great opportunities for those looking for companies with excellent growth potential still trading at a fair price. 1. Datadog Datadog (NASDAQ: DDOG) provides real-time monitoring for IT systems with a particular focus on cloud computing. The company is in a great position to benefit from continued growth in AI spending in two ways. First, as a leading cloud observability solution, it benefits from growing cloud spending for artificial intelligence (AI) training and inference. All three of the biggest public cloud platforms have noted that demand for their AI services continues to outpace their ability to supply it. Since Datadog uses consumption-based pricing, growing cloud spending also means growing revenue for Datadog. Additionally, Datadog is releasing AI tools of its own. It's released several AI agents that autonomously investigate and fix code before bugs become an issue. It's also released its own large language models for use on its observability platform. As a result, Datadog saw revenue growth accelerate to 28% last quarter. It also saw customers using more of its products with 52% using four or more and a 120% dollar-based net retention rate. On the other hand, heavy AI spending has weighed on profitability, with adjusted operating margin contracting 4 percentage points last quarter. But accelerating revenue growth more than justifies the upfront costs involved with capturing the massive opportunity with AI. Shares of Datadog currently trade for around $128 as of this writing. That price is about 12 times sales expectations for the next year, which is appealing for a company growing revenue in the high-20% rate. While AI spending is weighing on profits, the path toward bigger profits is a lot easier with a much bigger revenue base. So, it's worth picking up a couple of shares with your $250 right now. 2. Atlassian Atlassian (NASDAQ: TEAM) is an enterprise software provider focused on improving collaboration among workforces. Its flagship Jira and Confluence platforms boast over 300,000 customers, a growing number of which are large enterprises. Management said it doubled the number of deals worth more than $1 million last quarter. Unlike most enterprise software companies, Atlassian doesn't have a big sales team driving revenue. Instead the focus is on improving the product and charging consumption-based pricing. As such, great product improvements will help drive revenue through more usage and higher pricing tiers. That's evident in Atlassian's move to include its AI capabilities in its premium and enterprise subscriptions. The newest effort is its AI agent Rovo, which can automate tasks across its software suite. There's a clear demand for AI capabilities, with premium and enterprise recurring revenue growth of 40% year over year last quarter. That's well ahead of its overall revenue growth of 22%. As Atlassian grows its revenue, it should see strong operating leverage thanks to its small sales team. Indeed, last quarter's adjusted operating margin of 24% increased 5 percentage points. Management expects fiscal 2026 operating margin in line with that number, expanding to 25% in 2027. Despite strong sales growth, shares of Atlassian have fallen to about $167. That puts its price-to-sales multiple at just 8.4, and just 4.4 on a forward looking basis. That's an extremely attractive price as the company looks to have a lot of operating leverage and plenty of revenue growth ahead of it. The software stock could be a great buy with your $250 right now. 3. The Trade Desk The Trade Desk (NASDAQ: TTD) has seen its stock take a wild ride over the last year. Shares fell earlier this year after operational challenges weighed on its fourth-quarter revenue. Specifically, the company was slow to transition customers to its new AI-powered Kokai ad-buying platform. As a result, the company missed its own revenue outlook. The stock recovered, but it dropped again following the release of its second-quarter results. The culprit this time was its outlook for the third quarter coming in well below expectations. That said, the second-quarter results were strong, and investors may have been overreacting to management's muted outlook. Management's forecast for just 14% revenue growth in the third quarter is a marked slowdown from the 19% growth it just posted. Part of the blame is on the political advertising landscape in a non-election year, which makes the comparable quarter tough. Management also blamed macroeconomic factors like tariffs, which could weigh on overall ad spending. Still, analysts were expecting stronger numbers after seeing positive outlooks from big advertising companies like Meta Platforms, Alphabet, and Amazon. The Trade Desk separates itself from those so-called walled gardens by offering ad placement across a variety of sources ranging from connected TV to podcasts to retail sites and more. It gathers data from multiple partners, which helps improve its programmatic ad placements to ensure high-quality targeting and measurement. As a result, it's slowly gained share of digital ad spend over the last decade. That trend doesn't seem to be slowing down despite the weak outlook for the third quarter. And with its Kokai transition finally on the right track (75% of customers were using it as of the end of the second quarter), it should be able to cater to more customers' needs. It may even push into the small and medium-sized business segment dominated by Meta, Alphabet, and Amazon. Moreover, as a dominant force in connected-TV advertising, The Trade Desk still has a lot to gain in more ad spend transitioning to streaming from linear television, catching up with actual view time. With shares trading at just $54 after the post-earnings sell-off, shares look extremely attractive. Investors can pay less 8.5 times forward sales estimates for the company. I burned out working at consulting firms like KPMG and became a stay-at-home dad. I feel some guilt, but I'm glad I left.
Nicholas Gilpin and his wife decided he would leave his full-time job to care for their first child. A second kid and three years later, Gilpin has felt guilt for not providing as much financially. He said the reward of raising his sons outweighs those feelings, and he's glad to be there for them. This as-told-to essay is based on a conversation with Nicholas Gilpin, a 38-year-old stay-at-home dad and entrepreneur from Fair Oaks, California. The following has been edited for length and clarity. I never planned to be a stay-at-home dad. When I was an information technology consultant at KPMG, there were some weeks when I worked almost 100 hours across all seven days. Now, I spend those hours with my sons — a very different environment. Almost three years since becoming a full-time stay-at-home dad, I'm glad to invest this time I can't get back with my sons. We didn't take our sons' births for granted At KPMG, the burnout was relentless. I managed million-dollar technology implementations and worked nonstop. In late 2021, I joined Crossfuze, a consulting company, as a project manager and worked there for a little over a year. In 2022, my wife and I were expecting our first child, and her pregnancy needed more attention from us both. She was also working full-time as a case manager at a health insurance company, but needed to take time off before the birth to keep up with doctor's visits. Supporting my wife and keeping up with client demands became too much. We felt it made sense for me to take a break from working, so two weeks before my first son was born, I clocked out of the corporate world and clocked in as a full-time dad. We'd spent years trying to have children, and the months of visits to the doctor's offices took an immense emotional and mental toll on both my wife and me. So, when our first son was born through IVF, and our second two years later naturally, it was a gift we didn't want to take for granted. Having a parent at home is worth more than a second paycheck At Crossfuze, I was making $135,000 a year before my son was born. That loss of salary didn't play a large part in my transition to being a full-time dad, though. We'd always planned for one of us to stay home with the kids. Ultimately, we saw having a parent home as more valuable than maintaining two full-time incomes. We didn't want to outsource parenting. We don't know what a day care or babysitter might be teaching our kids, and the time spent driving them to and from those programs may negate the benefit of sending them there in the first place. I'm with my boys every hour of the day, and sometimes, we sleep in the same room throughout the night. My first son is almost 3 years old now, so he requires much more attention and play time, while my other son, almost 6 months old now, spends more time with his mom. I take care of 70 to 80% of the daytime responsibilities, and my wife takes care of our family when she's not working. She's the most active with our boys in the evenings and weekends. I feel guilty for not providing as much Thankfully, my wife and I worked hard early in our careers, and she can support us through her private mental health counseling practice. We also bought a duplex and rented out half of it to help cover our mortgage. We've really had to get intentional about what matters most. Personally, I don't buy anything unless it's essential — supplements, a gym membership, and basic toiletries. We rarely go out to eat, and I cook nearly all of our meals at home. My mom has also helped provide essentials for our boys, and my wife's parents have contributed financially since our youngest was born. We're making it work — but every dollar has a job. A part of me wants to rebuild my career to support my family more financially. Society's expectation for fathers has always hinged on them being providers. While this era of parents is evolving away from stereotypical parenting roles, I still feel a sense of guilt for not providing as much while our sons grow up. I have to remind myself that I'm doing enough for my family through the hours of spending time with them, potty training, going on walks around the neighborhood, playing in the backyard, and making them breakfast. This has been my biggest challenge when trying to have it all as a dad. Being a stay-at-home dad can sometimes feel isolating I spent most of my adult life connecting with clients, working on projects, and advancing my career. I've invested heavily in my skills throughout my career and obtained my MBA. When it's just my sons and me during the week, I lack the adult conversations I was used to. It can feel isolating, and like I'm regressing in my career or missing out on the latest tech advancements. A month after my first son was born, ChatGPT was released to the public. Seeing the way AI has been implemented to outsource work, code, and handle menial tasks around the office makes me feel like I'm missing out on a major tech advancement in my field. But staying home has also given me the space to build my own business and work on the book I'm writing, "Thriving Dad," during my sons' naps or after their bedtimes. AI models have leveled the playing field for entrepreneurs like me as I try to rebuild my career a couple of hours at a time, since 100% of my day is invested in my boys. I use ChatGPT as a personal assistant to help manage all the tasks and coding I want to get done during the day, and chip away at the business I want to sculpt. Balancing my time between childcare, spending time with my wife, and rebuilding my career is a challenge. I'm grateful to be able to give my sons what I didn't have I find spending time with my sons incredibly rewarding. I grew up without a father in my life, and I consider the time I spend with my sons to be unfulfilled time I didn't have with my father. In a way, I'm kind of winging it. Time is invaluable when children are young, and I want to pour this experience into my sons while I stay home. While it's difficult to pinpoint the most rewarding experience of being a dad, recently, my oldest said, "I love you, daddy," for the first time. That hit me hard. Seeing these milestones for both of them and seeing parts of myself in them, I am immensely grateful to give them what I couldn't have and what I needed as a child. Trump Widens Metal Tariffs to Target Baby Gear and Motorcycles
(Bloomberg) -- President Donald Trump stunned the logistics industry on Friday by widening his steel and aluminum tariffs to include more than 400 consumer items that contain the metals, such as motorcycles and tableware. Customs brokers and importers in the US were given little notice to account for the change, which went into effect Monday and did not exclude goods in transit. Official guidance has been muddled, especially for goods already on their way to the US, and it's unclear whether the metals levies stack on top of country-by-country tariffs. Having weathered six months of Trump's trade war and a pandemic that triggered mass supply disruptions, it's hard to rattle the freight carriers, cargo owners and middlemen that keep cross-border commerce moving. But the scope and implementation speed of this latest notice took many by surprise. 'We've had a lot of these 11th-hour implementations throughout 2025, this one in particular impacts every single client I have to an enormous degree,' Michigan-based customs broker Shannon Bryant said in an interview. 'Earlier announcements at least had some in-transit exemptions so at least importers could make reasonable buying decisions,' said Bryant, president of trade compliance advisory service, Trade IQ. 'This one was unique in that way — it's very much a 'gotcha.'' The new list includes auto parts, chemicals, plastics and furniture components — demonstrating the reach of Trump's authority to use sectoral tariffs. That is separate from the executive power he invoked for his so-called reciprocal tariffs. 'Basically, if it's shiny, metallic, or remotely related to steel or aluminum, it's probably on the list,' Brian Baldwin, a vice president of customs in the US at logistics giant Kuehne + Nagel International AG, wrote in a post on LinkedIn. 'This isn't just another tariff — it's a strategic shift in how steel and aluminum derivatives are regulated.' Compliance Costs The difficulty with applying tariffs to derivative products lies in determining what percentage of an item is made from the targeted materials. Flexport, a digital freight forwarder, said in a blog post that 'for many brands, this means chasing suppliers for detailed data: aluminum weight, percentage of customs value, and country of cast/smelt.' The compliance burden, Flexport said, 'is significant.' This tranche of tariffs is also particularly expansive, including items such as motorcycles, cargo handling equipment, baby booster seats, tableware and personal care products that come in metal containers or packaging. Jason Miller, a professor of supply chain management at Michigan State University, conservatively estimates that the metals tariffs now cover about $328 billion worth of goods, based on 2024 import data. That's six times greater than in 2018 and a big jump from the $191 billion worth of goods covered prior to the change, he said in an email to Bloomberg News. Broker's Plea Bryant, whose clients include cosmetics and commercial cookware importers, sent a letter to her elected officials in Washington on Monday warning that the complexity of overlapping tariffs is becoming unworkable even for professionals. 'For small importers,' she wrote, 'it's impossible.' 'I'm trying to think of a client that's not impacted,' Bryant said. 'These are American companies that employ American people that are being ambushed by their own government.' Trump first imposed steel and aluminum tariffs in 2018 with the goal of boosting US output by making it more expensive for Americans to buy foreign material. But several major suppliers including Canada, Mexico and the European Union were ultimately exempted, and US industries have said they're still struggling to compete with imports. Big Steel Applauds In June, Trump fulfilled a campaign promise by doubling the levy on steel and aluminum to 50% and also sought feedback from industry on how to broaden it further. Lourenco Goncalves, chief executive officer of US steelmaker Cleveland-Cliffs Inc., applauded the expanded tariff list in a statement on Monday, thanking the Trump administration for 'taking decisive and concrete action that will deter tariff circumvention occurring in plain sight with stainless and electrical steel derivative products.' There's very likely more to come. At the end of July, the Trump administration imposed a 50% duty on semi-finished copper imports valued at more than $15 billion and ordered officials to come up with a plan to slap tariffs on an array of other copper-intensive goods. 'This isn't over,' said Pete Mento, DSV's global customs director, in a social media post on Monday. 'The next list will surely be for copper and I expect that to be equally as miserable.'
