She had been up for two nights as her six-month-old ran a fever and cried nonstop. She had thought about going to the ER but worried about the long wait, especially without childcare for her two-year-old as her husband was out of town. Now she was in the pediatrics office with an air of despair. 'Please help me. I don't know what's wrong with my child".
Parenting Can Be Stressful
Being a parent is a journey, even in the best of times. Whether it's a sick infant, a clumsy toddler with a broken arm, a 9-year old starting the soccer season with a concussion, or a teen struggling with anxiety at school, the potential challenges can be endless. Unfortunately, the weight of parenting and supporting kids through this myriad of issues often falls disproportionately on moms. Almost 25% of women leave their jobs in the first year after having kids. After five years, only about 30 percent of these moms have returned to work. Over three-quarters of moms report they are the ones responsible for managing their children's healthcare. It is not surprising to see that JAMA recently published a study about the stress and decline in mental health that moms are experiencing. This seems to be relevant to many professions. In a Healio survey of physicians, 64 percent of women reported that they had caregiving responsibilities, and greater than 50% reported that this had a significant impact on their stress levels.
Mental Health is Often Impacted
Maternal stress and mental health can have rippling effects and impact kids' own mental health, affect their social skills, educational success, and even their weight. These consequences may begin even before birth – infants of mothers dealing with depression, anxiety, or stress while they were pregnant have been shown to struggle more with social and emotional development. Parental mental illness is one of the adverse childhood experiences that the American Academy of Pediatrics flags as high risk for leading to poor health as adults.
Community is Key
Stress and mental illness do not exist in a vacuum - a multitude of external factors exert stress on a parent's day-to-day life, from financial instability and food insecurity, to lack of childcare and mental health services. Policies that protect and expand social services for families would help to alleviate many stressors that parents may face, including protections for SNAP, Medicaid, and legislation to support affordable childcare. Guaranteed robust and paid maternity leave policies would allow new moms to heal, rest, and adapt to this next chapter. In Europe, for example, many countries offer up to a year of parental leave, with much of it paid.
Online community support can be helpful and some resources do exist. The National Maternal Mental Health Hotline is available at 1-833-TLC-MAMA (1-833-852-6262) via text or call as a 24/7 service for pregnant people or moms to talk to counselors who can provide support and refer them to local resources.
Self Compassion Can Buffer Stress
But an essential change to help our moms, often the 'Chief Health Officers' of the family, is a culture shift. Moms and caregivers should be encouraged to get off the treadmill, pause, take care of themselves and support systems should be in place to allow this to happen. This concept of 'self compassion' has been studied extensively and has made a difference in teachers, healthcare professionals and parents. The 'reset' includes breathing techniques, visualization, guided meditation and mindset shifts. To allow for these pauses, our essential caregivers must also have an inner team of family and friends that they can hand off to, or lean on, so they can take time to reset and breathe.
Imagine this - the young mother calls her neighbor, who watches her toddler while she takes her baby to urgent care. The doctor diagnoses the child with an ear infection, prescribes antibiotics and shows her how to suction her baby's nose so he (and she) can sleep soundly. Everyone wakes up the next day rested and calm.
Self compassion, connection and a village are essential parts of helping our most significant caregivers- moms. When we uplift our moms and other caregivers, there is a rippling effect to the health of the children now and longterm.

What to know about COVID variant NB.1.8.1 causing 'razor blade throat'
A newer COVID-19 variant may be causing a severe sore throat in some people who contract the infection. The variant, known as NB.1.8.1, has been nicknamed by some as "razor blade throat" due to the painful symptom. MORE: Why are more than 300 people in the US still dying from COVID every week? Data from the open global genome sequencing database GISAID shows the new variant has been detected in several states, including New York, Illinois, Texas and California. Public health experts told ABC News there is no cause for serious concern yet because the virus does not appear to be more severe than previous variants and there are steps that can be taken to protect yourself. NB.1.8.1 derives from the recombinant variant XVD.1.5.1, which is a descendant of the omicron variant. The first sample of NB.1.8.1 was collected on Jan. 22, according to the World Health Organization (WHO). It was first detected in China and other parts of Asia before spreading to Europe. It was designated as a "variant under monitoring" by the WHO, meaning it may require prioritized attention and monitoring but is not as serious as a "variant of interest" or a "variant of concern." As of the week ending June 7, NB.1.8.1 is the second most dominant variant in the U.S., accounting for an estimated 37% of cases, according to the Centers for Disease Control and Prevention (CDC). The virus appears to be more transmissible because there appear to be changes to the spike protein, which is what the virus uses to attach to and infect cells, said Dr. Peter Chin-Hong, a professor of medicine and infectious disease specialist at the University of California, San Francisco. It also seems to attach more easily to ACE2 receptors, which are proteins found on the surface of cells and how the virus that causes COVID enters cells, he told ABC News. NB.1.8.1 doesn't yet appear to be causing increases in cases or in hospitalizations with rates remaining "stable" so far, according to Chin-Hong. The variant has also been called "Nimbus," which appears to have been coined on X by T. Ryan Gregory, a Canadian professor of evolutionary biology. "Nimbus is a catchy, quick name, and it also includes an 'N' and a 'B' from the lineage, which is NB.1.8.1. So it's easier for people to be able to say these monikers for COVID, rather than remember the actual lineage," said Dr. Alok Potel, a pediatrician at Stanford Children's Health and an ABC News contributor. "But I think it's important also because it keeps people paying attention to new COVID variants that can be different in terms of infectivity and in terms of spread," he added. It's also unclear if the "razor blade throat" is more common in those who are more up to date on vaccination compared to those who are not up to date. "I think it's certainly amongst the spectrum of symptoms that you can get, and we know that sore throat is reported by about 70% of patients now with COVID, so it's not unusual, and like with everything in medicine, there's always a spectrum," Chin-Hong said. There are no other symptoms outlined that are specific to NB.1.8.1 by the CDC. Symptoms listed by the health agency still include sore throat, cough, fever, chills, shortness of breath, difficulty breathing, congestion, runny nose, loss of taste or smell, fatigue, muscle aches, body aches, headache, nausea, vomiting or diarrhea. The experts recommend that high-risk Americans receive a vaccine twice a year as recommended by the CDC. The CDC also currently recommends most adults aged 18 and older receive an updated 2024-2025 vaccine and that parents of children between ages 6 months to 17 years discuss the benefits of vaccination with a health care provider. COVID-19 also tends to spike in the late summer and early fall, so people should consider opening windows to increase ventilation, wearing a mask in certain situations and avoiding crowded areas, Schaffner said. "Time to stream a movie, as I like to say, rather than going to the movies," he added. Patel said it's important to practice good hygiene such as proper hand-washing and covering your mouth when coughing or sneezing. He also recommended that people test if they are symptomatic and said over-the-counter rapid at-home tests work. "Getting infected with COVID and other infectious diseases is not necessarily life or death, but it can still be very debilitating." Patel said. "It can cause people to miss work, to spread the virus, to get people who are higher risk sick and there's still an untold amount of people who have long COVID." He added, "So, we don't want to look at COVID as just another common cold, because there's still so much we're learning about it, and there's still so many possibilities with this virus continuing to mutate, if we let it."

What The Senate Budget Bill Would Mean For Older Adults

It would scrap a Biden-era minimum staffing rule for nursing homes. And, at the same time, it drops a House proposal to increase tax-free savings that higher-income households could use to buy long-term care insurance or pay caregiving costs. The overall measure, approved by the Senate Finance Committee and likely to reach the Senate floor sometime next week, would cut taxes by trillions of dollars over the next decade and cut spending, though by significantly less. The Congressional Budget Office has not yet calculated the costs of the package. It also is possible Senate GOP leaders will revise the bill before it reaches the Senate floor. However, the current version would make substantial changes to programs affecting older adults, especially those on Medicaid. While Medicaid is widely considered to be a program for poor families, more than half of its benefits go to older adults and younger people with disabilities. About 7.2 million seniors and 4.8 million younger people with disabilities are enrolled in both Medicaid and Medicare. The Senate bill mimics the House measure by limiting the ability of states to tax Medicaid providers, such as hospitals and nursing homes, but it is even more aggressive. Those complex provider taxes make it possible for states to pay providers more and effectively bill most of the additional costs to the federal government, which pays for about 70 percent of the program on average. The House bill would cap these taxes at current levels. The Senate plan would require states to lower their taxes for most providers, though they still could tax nursing homes at the higher levels. The provider tax limits would apply only to the 40 states and the District of Columbia that expanded Medicaid under the Affordable Care Act. Critics call provider taxes a financial gimmick that allows states to boost federal payments for the program. However, the real-world impact would be less federal funding for state Medicaid programs. And that would force states to either cut Medicaid benefits, limit program eligibility, or raise taxes to fill the federal hole. It also likely would result in states scaling back their home and community-based benefits, which are optional under the law, and shift more people to nursing home care, which is required. Like the House version, the Senate bill also would require Medicaid recipients to work. It exempts older adults and people with disabilities from having to work. But it could require some family members who are unable to work because of their caregiving responsibilities to choose between assisting a loved one and losing their own Medicaid benefits. The House bill appears to exempt family members caring for children and younger people with disabilities, but it is not clear whether it protects those staying home to assist frail parents or spouses. The Senate bill is ambiguous in a different way. It would exempt people from the Medicaid work requirement if they are a 'parent, guardian, caretaker relative, or family care giver (as defined in section 2 of the RAISE Family Caregivers Act) of a dependent child 14 years of age and under or a disabled individual.' And what does the RAISE Act say? 'The term 'family caregiver' means an adult family member or other individual who has a significant relationship with, and who provides a broad range of assistance to, an individual with a chronic or other health condition, disability, or functional limitation.' What does that mean? Well, nobody really knows. What does 'significant relationship' mean? What is a 'broad range of assistance?' What happens if two Medicaid recipients are caring for a parent? Which is exempt from the work requirement? Presumably, somebody at the Department of Health and Human Services eventually would have to write regulations to clarify it all. But that could take months, at the very least, especially since the Trump Administration fired so many HHS staffers earlier this year. The Senate bill also adopts many of the House's additional paperwork requirements for those applying for Medicaid or trying to keep benefits. It would block a Biden Administration rule that makes it easier to enroll in Medicare Savings Programs, which allow Medicaid to cover Medicare premiums and cost sharing. Without MSPs, low-income Medicare beneficiaries would have to pick up those costs themselves or buy costly Medicare Supplement (Medigap) insurance. The Senate bill also would effectively scrap Biden rules that would require nursing homes to maintain a minimum level of staff, including aides and Registered Nurses. Nursing homes are fighting those rules in court but a congressional repeal of the regulations would make the legal battle unnecessary. Finally, the Senate bill excludes a change in the House bill that could benefit higher-income people who want to put money away for long-term care. The House version would double the maximum contributions to employer-based Health Savings Accounts, but the Senate measure currently has no provision. The House and Senate are operating on a self-imposed July 4 deadline for passing this huge fiscal bill. It will be important to keep an eye on what happens over the next couple of weeks.
