
How to banish toxic positivity at work
In my experience as an emotional intelligence and human behavior specialist, our workplaces are becoming more focused on employee wellbeing, but it's an easy way to compel us to fake optimism, regardless of the real circumstances at hand. In workplace cultures, toxic positivity compels people to remain optimistic or think positively regardless of the real circumstances—say, key clients lost, budgets and bonuses frozens, or team-wide layoffs. And it's pervasive: one survey by workplace blog Science of People finds that almost 68% of people had experienced toxic positivity in the last week.
The fundamental basics of relationships between people is based on the ability to trust. Trust is created through being honest and transparent, being accountable and creditable, and being empathetic and vulnerable. It takes being real—and fake positivity isn't real. If I can see that your optimism is a put-on, how can I trust the other things you say or do? Do I feel safe to be real, or do I, too, need to fake positivity?
When this occurs, it impacts every part of our workplace—from our culture, to our performance and our mental health. Ultimately it impacts the overall success of each person and in turn, their organization. Here are five steps to shut down toxic positivity in a workplace.
Own the reality of the situation. The world is not perfect; we are not perfect. Things will go wrong, and we will get it wrong at times. The only thing we have control over in this world is how we choose to respond—and our response should be authentic and genuine. Respond appropriately to the workplace situation, at the right intensity, without the need for forced toxic positivity.
Face emotions head-on. There is no such thing as a 'good' or 'bad' emotion, and while we tend to think positivity is the former, that isn't the case. We should be focusing on whether an emotion is appropriate for the situation, and whether the intensity that we are feeling the emotion is appropriate. We feel emotions for a reason. Acknowledge and understand what is driving an emotion so that it can be processed before we move on.
Understand how the people around us are feeling. Our emotions are influential to the people around us, but people can spot an insincere emotion from far away. Faking an emotion is setting a standard in workplace environments of what is acceptable and what is not. Trust and respect won't be created when people are not being authentic or genuine.
Ask the right questions, and answer questions asked. Communication is always key to the workplace environment, and the ability to communicate effectively directly influences our culture. When we are feeling any form of intensive emotion, we have something to say. Ask the right questions to better understand what is driving another person's emotions. Answer the questions they have, and provide the information they require to be able to move forward.
Drive emotional intelligence. Realistically, we know that a great culture in a workplace is when all emotions are being displayed appropriately. In some situations, it may be optimism—and other times it might be sadness, anger, disappointment, fear, or frustration. Let's not judge someone else's emotion. Assess it, and do what it takes to ensure they are felt and processed before moving forward.

Basically speaking, clinicians can ask questions to a chatbot called ChatEHR and automate charting, as well as promoting enhanced diagnostics. Some of what people like about the software is that it's secure, direct and seamless. Stanford chief data science officer Nigam Shah points out that for AI to be effective, it should be embedded in their workflows and have a high degree of accuracy. 'ChatEHR is secure; it's pulling directly from relevant medical data; and it's built into the electronic medical record system, making it easy and accurate for clinical use,' Shah said. Why it Matters Is this software valuable to clinicians? You have to look at the current context, and what healthcare professionals need. Too many clinicians spend a lot of their time dealing with chart notes. Take this testimony from one frustrated medical worker posted on Reddit: 'Hello fellow docs - third year Family Medicine Resident here. Recently ramped up to 20 patients per day in clinic at my FQHC and I feel like I'm drowning in notes. I spent all of this weekend finishing notes. I know parts of the problem are: 1) using NextGen, 2) having to write extra to prove to attendings you can think.' The poster talks about current methodology: 'I've started using the computer as much as possible in the room to get all the HPI down. I type out the whole plan before the patient leaves. I am having patients come back more instead of addressing a lot each visit. But I STILL am spending all my time doing notes it seems. This is not sustainable. I feel like I will never have a life. A scribe isn't in my near future as usually attendings have to work a few years to get one.' Then this person asks for help from the forum. 'Any tips? How do you guys feel about charting taking over your free time?' These new technologies have enormous potential to help with exactly this kind of workload. And from reading this person's feedback, the workload seems heavy indeed. Feedback from Anurang Revri At a recent IIA event, I sat in on a lecture with a panel, including Anurang Revri, Chief Enterprise Architect for Stanford Medicine. Here are a couple of quotes that Revri brought up around the idea of automating tasks like this: 'We support research education as well as clinical care … so it's an interesting sort of place to be, where we have this kind of amazing research that we can translate into clinical workflows or external vendors and solutions. So that's sort of our focus, to implement the responsible AI life cycle.' 'When you start combining all those pieces of data and building models, multimodal models, around that, then it becomes a really powerful thing in clinical care.' Use Cases for the Technology It's expected that the ChatEHR technology will lead to various types of advances in automated healthcare. A resource from Nelson Advisors puts this into five categories, so I'll outline them with bullet points: Enhanced clinical decision support and diagnostics Streamlined administrative and clinical workflows Enhanced patient engagement and personalized care Global health applications and bridging access gaps Advanced data utilization and research Some of these are very interesting. Under decision support, you have advances in treatment. With administrative issues, you see how this works with the patient life cycle. Under the fifth column of 'advanced data utilization and research,' you have the idea of digital twins, in order to house a person's health data in a certain digital entity – and I think that wearable devices will come in handy here. All of it is likely to help us make medicine more effective, and do earlier interventions for patients. And that's a big deal. Look for this type of technology to blossom in the healthcare industry as a whole.

Behavioral Therapy as First-Line for Bladder Control in PD?
TOPLINE: Structured behavioral therapy focused on pelvic floor muscle exercise was noninferior to treatment with solifenacin for alleviating overactive bladder symptoms in patients with Parkinson's disease (PD), new research showed. Because it was also associated with fewer side effects, especially falls, researchers recommended the behavioral therapy as a first-line treatment option. METHODOLOGY: This 12-week randomized noninferiority trial was conducted between 2018 and 2023 within four Veterans Affairs healthcare systems in the US. Overall, 77 patients diagnosed with PD and overactive bladder symptoms, as measured with an International Consultation on Incontinence Questionnaire overactive bladder module (ICIQ-OAB) symptom score ≥ 7, were included. All participants (mean age, 71 years; 84% men; 90% White individuals) were randomly assigned to receive either pelvic floor muscle exercise-based behavioral therapy with urge suppression training (n = 36) or 5 mg solifenacin daily with need-based titration up to 10 mg daily (n = 41). The primary outcome was ICIQ-OAB symptom score at 12 weeks. Secondary outcomes were ICIQ-OAB bother and quality of life scores and drug-related adverse events. TAKEAWAY: At 12 weeks, clinically significant improvement in ICIQ-OAB scores was observed across both treatment groups within a noninferiority margin of 15% (drug vs behavioral therapy: mean ICIQ-OAB score, 5.8 vs 5.5; P = .02). Both groups also had a reduction in symptom frequency, which was associated with a reduction in bother scores and an improvement in overactive bladder-related quality of life. Compared with the behavioral therapy group, the solifenacin group had a greater number of adverse events, including dry mouth (P = .002) and pain or burning during urination (P = .03). They also had a greater number of falls (6 vs 0). IN PRACTICE: 'The finding of increased falls in the solifenacin therapy group reinforces the need to carefully consider the risk-benefit ratio of medications for urinary symptoms, especially given the increased risk of falls among persons with PD. Results of this study suggest that behavioral therapy is an effective treatment option for persons with PD,' the investigators wrote. SOURCE: The study was led by Camille P. Vaughan, MD, Division of Geriatrics and Gerontology in the Department of Medicine, Emory University, Atlanta. It was published online on July 14 in JAMA Neurology. LIMITATIONS: The follow-up duration was limited to 12 weeks, restricting insight into longer-term outcomes. The cohort predominantly had male participants, reducing the generalizability of the findings, and the study relied on patient-reported data. Additionally, a higher dropout in the drug group led to nonrandom missing data, potentially biasing comparisons. DISCLOSURES: The study was funded by a grant from Veterans Affairs Rehabilitation Research and Development.
