Ambulance union raises response time concerns after man's death at Sellicks Hill
The Ambulance Employees Association (AEA) said it took 17 minutes for an ambulance crew to arrive at the scene of the category one case — which in metropolitan areas would require a response within eight minutes in 60 per cent of cases.
Health Minister Chris Picton said the patient was "outside the Adelaide metropolitan area".
South Australia's Ambulance Service (SAAS) chief executive Rob Elliott said the patient was seen as soon as possible "on our most urgent priority", with multiple resources sent to the scene
He said it was "not very sensible" to aim for an eight minute response time in regional areas where towns could be sparsely located and that SAAS instead endeavoured to "make sure that we have good regional coverage, good access to resources".
He extended his sympathy to the man's family, and praised the "extraordinary" efforts of the bystanders and paramedics who attended the scene.
AEA general secretary Paul Ekkelboom said the case was initially triaged as a category two but within two minutes was upgraded to a category one case where "every minute counts".
He said the "number one thing" affecting ambulance response times was available crews.
"When we don't have crews sitting on station or actively free to respond then it's going to be a delay to get a crew to respond to a patient," he said.
"When they're stuck on a ramp, then obviously we can't free them up to respond to the community.
"The real effect is obviously when we can't get to our patients in time to administer life-saving care."
When asked if the man in this case would have survived if the ambulance response time was faster, Mr Elkkelboom said he was "unsure" but added that he did not have all the patient's medical history.
"What we do know is if we can get there in a timely fashion, oxygenate the brain, and keep that blood pumping and give them the life-saving care that [is] needed, there's a good opportunity that we might be able to get a favourable outcome," he said.
He said bystanders at the scene did their best to help while awaiting the arrival of paramedics.
While he said demand on the health system was high, Mr Ekkelboom acknowledged the government had planned to open more beds later in the year.
"I don't want to see another patient in the community experience what unfortunately happened to that gentleman over the weekend," he said.
Mr Picton said at the time, the ambulance from the closest station "was at another job in the community, assisting someone else".
"Our crews did everything they could to get to that case as soon as possible," he said.
Mr Picton said that because the case occurred outside the metropolitan area, the eight minute response time goal applicable to metropolitan areas did not apply.
When asked, he said ramping did not play a role in this case "based on the regional location".
Last month's ramping numbers were the second highest on record, with ambulances spending 5,387 hours waiting.
Mr Picton said the state's health system — including the ambulance service — was currently dealing with the highest number of influenza cases in the state for the past decade.
He said while 2,000 influenza cases had been recorded in the past week, the numbers were "vastly underestimated" because not everyone with the flu had a PCR test.
There were 164 hospital admissions due to the flu.
"Our health system is under pressure, we know that the flu numbers are up, we know that admissions to hospitals are significantly up, but our doctors, nurses and ambos are working as hard as they can — particularly to see the most urgent cases — to make sure that people can get the care that they need," he said.

Neurosurgeon Greg Malham terminated from Warringal Private Hospital after Four Corners investigation
Controversial neurosurgeon Greg Malham has been terminated from working at a Melbourne private hospital following a Four Corners investigation into his behaviour in the workplace. Ramsay Health Care chief executive officer Maree Mendola wrote to staff working in Australia's largest private hospital operator on Tuesday to inform them of the decision to terminate Mr Malham's temporary credentialing at Warringal Private Hospital following the story. "The allegations raised in the media are serious and do not align with Ramsay's values," Ms Mendola wrote. "I want to reaffirm Ramsay's commitment to maintaining a respectful, inclusive and safe workplace for all our people, patients and practitioners. Before the Four Corners story was published, Ramsay Health Care said in a statement that Mr Malham held temporary credentialing and that his application for full credentialing was progressing. But Mr Malham will no longer have that option. Warringal is the second hospital to part company with Mr Malham in recent months. He resigned from Melbourne's Epworth Private Hospital after it set up an external investigation following a video published in April of him during the federal election campaign. The footage, leaked to The Age newspaper, showed Mr Malham tearing down independent MP Monique Ryan's election corflute and talking about burying the body under concrete. This prompted Four Corners to investigate his background, and it found multiple stories of sexist and unprofessional behaviour: uncomfortable nurses, crying radiographers, patients who thought he was egotistical and lacked care and compassion, and a devastated, grieving family of a young nurse who left a suicide note blaming Mr Malham for her decision to end her life. Greg Malham's former patients, Annie Sargood and Frédérick Le Guen, welcomed the news of his termination by Ramsay Health Care. The Royal Australasian College of Surgeons also wrote to its members following the Four Corners program, saying that it took the matters raised in the story "extremely seriously". "As a college, we want to be clear: behaviour that undermines patient safety or fosters discrimination or harassment has no place in surgery," president Owen Ung wrote. "While the vast majority of surgeons are dedicated professionals who make a substantial contribution to patients and their communities, we recognise that unacceptable behaviour by a minority can have serious consequences for individuals and for trust in the profession."

VitalTrace fetal monitoring device touted as 'absolute revolution' in childbirth
For the first time ever, health professionals will be able to accurately monitor oxygen levels in babies during childbirth to prevent major health issues and unnecessary emergency procedures. Experts have described the "breakthrough technology", developed in Western Australia, as a world-first and the biggest development in fetal monitoring in 50 years. The VitalTrace device attaches to the baby's head and transmits small, continuous electrodes to measure the baby's heart rate and level of lactate, which can detect fetal distress. University of WA professor Jane Pillow described the device as "the first breakthrough in fetal monitoring that we've had since the 1960s". She said the current method of fetal monitoring was not a "good predictor" of how well oxygen was being delivered to the baby during labour, she said. "The problem with that is … sometimes we miss babies that are not getting oxygen all that well," she said. "And at other times the diagnosis of when they aren't getting oxygen might be significantly delayed, and the consequence of that for babies is really devastating." A lack of oxygen could result in death but often it's the cause of significant health issues like cerebral palsy, brain damage and organ function complications. Emily Stone said it could provide re-assurance to mothers. "There were times during the birth of my daughter, when her heart rate couldn't be detected," she said. "It can be quite frightening in what's already an intense experience. "If this device was available, it just would have given me greater piece of mind that she was healthy and safe." Obstetrician and associate professor and the University of WA Scott White said the new development had the potential to halve unnecessary emergency caesareans, describing it as "an absolute revolution in childbirth". "It allows us to better identify those babies who are compromised," Ms Scott said. "But equally as important, it allows us to reduce the interventions we unnecessarily apply to women, which is a real profound improved health outcome." Mr Scott explained one of the well-known side effects of traditional heart rate monitoring is its inaccuracy. "At the moment, we look at changes in the babies heart rate pattern, that requires a lot of training, skill and interpretation and therefore errors can happen," Mr Scott said. "Whenever you use an indirect measure, you get false positive results and false negative results … [and] it tells us babies are unhappy when frequently they're not. "But this device is much more accurate which allows us to safely leave babies in when they're safe and to more accurately recognise those babies that truly do need to be delivered early." The device has taken about seven years to develop and the technology is similar to what people with diabetes use to monitor their glucose levels. VitalTrace co-founder and chief executive Arjun Kaushik said the response to the device had been overwhelming. "The enthusiasm to adopt this device cannot be overstated, I think this is an area of medicine that has long been under-served in technology," he said. Both the state and federal government have backed the device, with the state government contributing about $5 million to the technology's development. Early trials of the technology involved 20 mothers who participated nationwide. But professionals are hoping another larger trial will start next year to test the devices efficacy and dependability with the aim of getting regulatory approval in 2027.

The toll of living with someone who is frequently moody
Being around someone when they are in a bad mood isn't fun. But what about when you live with that person — and they're frequently moody? Carly Dober, a psychologist and policy coordinator at the Australian Association of Psychologists, says that situation can be unpredictable and stressful. "It becomes quite uncomfortable to be living with someone who you feel you are walking on eggshells around, or you don't know what mood they will be in day to day or hour to hour." There are many reasons someone can be moody, bad-tempered, or sullen, but regardless of the reason, Ms Dober says regular occurrences can lead to disconnect and resentment. "It can impact the quality of the relationship [when] people stop spending so much time together, or communicating." Relationship counsellor Jill Dzadey says dealing with someone who is often moody can be exhausting "if you are the person always adjusting to someone's mood". It can also lead to feelings of loneliness and rejection, she says. If there are children in the home, Ms Dzadey says they may learn that frequent moodiness and dysregulation is "normal" and acceptable. "Some people's dysregulation is quite intense, and it's quite powerful and can be felt by a lot of people around them." There are many reasons people may experience mood fluctuations. We've tried to capture most of them here with the help of Ms Dober and Ms Dzadey. Ms Dober says some people use moodiness to try and communicate they are upset. She says the expectation is people will "read their mind, which is impossible". Gendered stereotypes about men and women's moodiness may also play into how we express our emotions. "It's somewhat socially acceptable for men to brood or be moody or angry," Ms Dober says as an example. Moodiness is a form of coercive control when it's used to manipulate another person's behaviour or punish them, says Ms Dzadey. "This could look like never really knowing what version of this person you are getting, and you might feel like you need to adjust your behaviour. Domestic violence victim-survivor and advocate Deborah Thomson says her ex was "the king of moody". "His moods were unpredictable but always used on his part with purpose and need to control all scenarios," the 63-year-old from Tasmania says. "The unpredictability left me feeling as if I was living in a war zone, constantly super vigilant, waiting for the next — metaphorically speaking — bomb to drop." Deborah says his moods were a form of psychological abuse and manipulation, where she was "assigned responsibility" to make him happy. "To dress as he suggested, and to converse on topics he was interested in only, to such a degree I quickly lost sense of reality and the ability to discern acceptable from non-acceptable behaviour." Ms Dzadey says moodiness is often a sign someone is struggling. Encouraging them to get support in a non-shaming way can be helpful. "It's important to name the pattern of what you're seeing, name how it's making you feel, but do it without blame." Ms Dober says to provide specific feedback about what you have observed. "Maybe you have noticed they haven't been hanging out with friends as much, or going for their usual runs, or they seem more angry than usual. "Ask, 'Is there anything going on for you?'" She says these insights can be helpful in creating an action plan. "What can they do? Can they go see their GP to get a blood test, or get on a mental health care plan? "If there are big life stresses … [maybe] it's a chance to redistribute household chores if things are too much for them," she says for example. The conversation should also be an opportunity for the person to hear the impact of the moodiness is having on the household or relationship, Ms Dober says. She says the impact of frequent moodiness on the wellbeing of the person observing it "can't be underestimated", and it may be necessary to seek some support. "Talk to your friends and family about the stress it might be putting you under." You may also consider speaking to your GP, a counsellor, or psychologist. Ms Dzadey says it is important to set boundaries to create your own emotional safety and remember you are not responsible for someone else's moods. "They need to own their behaviour and actions." "The moody person needs to know although you can be empathetic and compassionate, you need to protect yourself with your boundaries and make it clear what those are."
